I think this is a problem not related to the Nexus 4 but related to Android 4.2
My problem is. During voice calls the screen goes off. During Skype calls the screen is always on. Do you think you had something like this?
The issue is, I used to have my screen protector all the time and it didn't cause a problem. I can't remove it now because I am at a bit of a rough environment now and I need to know if this is something related to Skype or related to the new 4.2 Android..
Update: Skype got updated and with this update the proximity issue was fixed. I read about this and someone told that something that is related to proximity sensor detection during calls got its name changed. After the last Skype update everything is going just fine. Too bad I am selling my Galaxy Nexus as I've already got my Nexus 4

I am having the same problem. Does not seem to ever turn off. No screen protector or case. Just normal incoming and outgoing phone calls.
I got the 4.2.1 update yesterday and am not having the problem anymore. I don't know if that was supposed to fix it or if it was coincidence

So I also dropped mine a few weeks ago and had to replace the screen. Upon replacing the old components into the new screen and frame, I *thought* I had been careless and mangled my audio jack ribbon, so I ordered a new one off Ebay. When IT didn't work, I knew something had to be up, so I started testing vs options in this thread and others. I hung the sensor loose and verified that it did indeed work unplugging and replugging it in and using "Proximity Sensor" (Google Play app) .. finally found a position it worked best in, also took the T5 screwdriver, ran it through the holes (ran it through is an incorrect term, I simply removed the purple haze I could see looking through it towards a light) .. even with this modification, something was pushing it out of the way and actually disconnecting it when I put the plastic cover back on (which I fortunately tested before completely reassembling the phone!! ) ..my overall solution was to cut the corner piece off the back plastic cover, making sure not to lose the metal piece, and it's working fine. I tested about 10 times as I'm reassembling and putting the phone all back together, and it now works properly.
So you might try carefully cutting a plastic corner off if software fixes are not working for you.
Where did you cut? Do you have a picture? I just replace my front glass and my proximity sensor is bugged... It thinks I'm close to it all the time.. Out of the casing it works fine but the second I install it, it stops working.
Did you leave the small rubber part in there?
I had exact same prob.
Dropped, replaced digitizer, agonized over prox sensor, bought new one, scrapped off purple tint.

There is an incredibly easy fix for this!! Everyone on every site is either saying its unfixable or you have to like mess with the hardware. Completely unnecessary. This is how I fixed it.
1. Download ScreenOff Lite in Google play store.
2. Open app, go into settings, then all settings
3. Make sure only these options have a check next to it:
-disable in landscape mode
-enable during call only
-Locks phone on screen on
4. Go back to the apps main menu and press "Start Service"
Wallahh!! You are done! Your phone's messed up proximity sensor will not turn off now during calls.

Your approach was quite similar to mine so I figured I'd post some images to help others who are attempting the same fix. My light sensor wasn't working either so this process also fixed that.

I used a small safety pin and held up the LCD/digitizer/frame to a light source so that I could see what I was doing a little better.
Location of the plastic tabs to cut off the mid frame:
On mine, when the sensor was pressed in too far into the recess I'd get zero light reading and the proximity sensor would activate; indicating that something was in the way. The rectangular plastic part on the mid frame pushes down on the flex ribbon that the proximity sensor is attached to. I also needed to remove one of the strips which sits alongside the headphone jack as it was also pushing down on the sensor too much. Using a sharp craft knife I cut off the plastic flush with the screw holes .That was what my particular sensor needed so that it worked after everything was screwed and clipped back together but YMMV so I'd suggest experimenting with what works best for you.
The flex cable sits pretty snugly in the recess without much support so I think it'd take a decent drop for it to move again. Tested plugging and unplugging my headphones a few times and everything still works so I'm crossing my fingers that everything will be fine from this point forward.

All of these fixes are good options but there are many factors that could contribute to a faulty proximity sensor. Recently a friend of mine replaced his screen and afterwords his proximity sensor would always register as having something near it. He tried many software fixes but in the end he opened his phone back up to find a bunch of dust collected on the sensor. The fix was as simple as gently blowing off the sensor and putting everything back together.

BEWARE this suggestion. With nothing more than the install I was not even able to turn on the screen with the power button (Nexus 4). Had to wait for the battery to drain and factory reset from recovery screen...
Hi, i allso have a strange problem with the proximity sensor.
During a call the screen turns off but if i touch the screen it's actually active so many times i found many things messed up, the status bar dragged and so on...
I haev to mention that the screen + frame was changed. If anyone else faced this problem or know a way to slove it..

just replaced mine today. Screen digitizer/frame had the same problem. Most people that replace their screen will have this problem, First off install a proximity sensor app from the play store and check if the sensor is working or not. There is many reasons this sensor is a pain and many ways to fix it. Google is your friend, in my case it was as simple as reversing the rubber with the 2 small holes that is attached to the ear phone jack (also part of the proximity sensor) one hole is smaller then the other but it wont matter. I look at it like flipping a tap washer and you get a bran new seal worked for me. Try the easy fix first then work your way up if need be.
can confirm this works as i just repaired a second n4 with the same problem and the sensor works great

Similar story here. I dropped my phone after having it less than a month. Broke the screen. I ordered a new screen assembly from LG ($140!) and fixed it myself. Only later did I discover that my proximity sensor no longer worked. I did not want to take it apart again. So I suffered with no proximity sensor for six months. I set the phone to hang up calls using the power button. And I plugged in a headset when I listened to voicemail. For some reason, the screen stayed on when a headset was plugged in.
Anyway, today after reading other stories similar to mine, I was emboldened to take the phone apart once more and try to fix this thing. I thought it was out of alignment or something. What I discovered instead was that it seemed to work perfectly when the phone was taken apart. It was only after final assembly that it failed. I took a closer look at my old screen assembly (yes, I had saved it all this time) and discovered a tiny missing part. It's a little rubber rectangular holder for the proximity sensor. It looks like this:
Those of you that still have your old screen, look for this part and install it in the new screen. It's easily missed. It fixed it once and for all for me.

Then remove the back cover, the top section of the phone including the battery, the sim card and the memory card.
Next, lift off the the top section which contains the power button.
Then use the safe pry tool, release the clips and remove the button piece of the phone.
Next, release the pop ribbon connector of the LCD screen as shown at figure 2.

Then use safe pry tool to separate the LCD screen and the glass screen away from the phone. Gently pry and lift off as there is still a connector at the top of the screen.
Remove the 2 screws using Small Phillips screw driver. Then use safe pry tool to pop off the earpiece.

Gently lift off the metal cover to release the screen digitizer cable. Once the ribbon cable is disconnected, you can now totally separate the screen from the phone.

To separate the LCD screen, you will need to use your hot air gun or hair dryer to heat up the glue that holds the glass in place, then you can gently pry it off using your open safe pry tool.
Now you will be able to replace any parts on your HTC Aria. Simply replace the damaged parts with the new ones and reverse the order to put your phone back together again.
Where can I get an adhesive tape for the digitizer, mine has lost it's stickiness and there a gap is starteing to appear between the digitizer and the screen. I've found only strange sites that sell this, without an option to pay in paypal.
I'm also looking for the adhesive tape that it is on the bottom part of the phone. as you can see there are two stripes on both sides of the trackball:

I wish you had the digitizer adhesive "square" pre made, like there are for other devices.
I will just order 2mm tape for the digitizer and 4mm tape for the bottom part.
btw, any useful tip on how to peel the paper from the tape after you adhere the tape on the one side to the phone? would be very helpful.

Strips are very easy and simple to cut.
You would actually need to cut any strips as you need to avoid blocking the sensor hole etc.
Just place the strips in place. Once ready just remove the backing then install. We actually have an video showing the adhesive process on YouTube if you want to check it out.

I checked the LCD connector and digitizer connector several times but still same problem. Is it possible that the LCD got damaged during disassembly?

My first thought would be to check the connector for the LCD and make sure it is fully inserted and is secure making a full connection.
If its connected fully and still not functioning and the phone is powering normally then you may of damaged it and will need a replacement however it's most likely a connection problem.

I ordered a replacement digitizer from RepairsUniverse. The video helped a lot for disassembly, and I was grateful that the "tape strips" I ordered ended up being about 8 square inches of tape!
My tips:
After plugging the digitizer connector back in, replace the two tiny screws and attach the screen to the phone (not to the digitizer). This will make it easier to clean off and attach new double-sided tape to the screen. If you leave the white stuff on the tape, you can fold the digitizer back to its normal place and power up the phone to make sure everything works. Then peel off the white stuff and attach the digitizer permanently.
The new digitizer didn't come with the white plastic "diffuser" taped to the back, underneath the four menu buttons (Back, Home, etc..) I didn't realize that 'til I put it back together. The menu buttons work fine, but without the diffuser and the black tape below it, the menu area looks "cheap" when it's lit up. It's not a big deal to me.
For me, putting the earpiece back in was the most time-consuming part. It wanted to keep popping back out on its own. I ended up using a tiny bit of double-sided tape between the earpiece and the screen, which held it in place.
Everything seems to work perfectly, except for the ambient light sensor. I cut plenty of clearance for it and the proximity sensor beside it, but I somehow lost the auto-brightness feature. The proximity sensor works fine. Maybe some light from the screen is leaking in and saturating the sensor.
It's nice not having an AT&T logo on the phone any more.

Glad you were able to repair your phone successfully.
You can transfer the white diffuser over to the new screen and this will sort out the light issue.

thank you for sharing. has video so it could be more detailed, I find the majority of screen htc glued so sure, so the replacement and repair is very difficult
So I dropped my nexus 4 cracking the screen which required me to replace the LCD assembly. After putting it back together and firing it up I noticed that the Auto Rotation (Yes the setting is checked in the settings) and the proximity sensor are not working. I installed Android sensor box and it is getting no reading from the proximity sensor or the gyro sensor. I have taken the phone apart multiple times to ensure everything is connect and everything looks correct. I have also unlocked the phone and have re flashed the stock rom with it not fixing the problem. Any help would be much appreciated.
My wife destroyed her display. Did same but gyro works fine but proxy is the problem. Work around so far is to set power button to end calls becasue screen just goes black when making/receiving calls kind of makes phone useless if you can't see screen.
I too have taken it apart several times and still nothing. Next plan is to find a new sensor unit and replace it.
From what I have been able to see in all my searching the proxy is extremely sensitive any deviation in placement, smuge on unit or display, minor deviation on color of display, screen protector, any interference from skins can affect it so check all those also.
In one of the threads on the Galaxy Nexus there is a patch to turn off sensor and there is mention of app in market that does the same, haven't tried yet have to get phone from wife to try, could be possible solutions also.
Will post back with findings as I get to try them.
Found great app in market Sensor Kinetic Pro shuts off proximity sensor or any other sensor no more problem with black screen after making calls. Still going to try replacing sensor however found it on Amazon less then $5.00.

Somewhat comforted to see others are also having the same problem... I recently replaced my nexus 4 screen only to have my proximity sensor fail on me too. Interestingly enough, i disassembled my phone multiple times to test if the sensor still works, turns out it does, but it is super sensitive. In my first experiment I had the sensor dangling out of the socket to see if it was still receptive to light, it worked perfectly - I used my hand as a 'face-simulator' to test for sensitivity. I then tried to place it back in the rubber grommet with the grommet seated firmly in the socket and what I found was that it worked, so long as there was only limited pressure on the enclosure. For example, it only works when there are no adjacent screws (for the plastic motherboard cover) applying pressure in the nearby area. I thought I could live with that, but then I tried putting the rear plastic cover back on and,... back to square one, dead screen on-call. I figure it has something to do with either pressure sensitivity or perhaps the pressure from the screws or the rear cover pushes the sensor too close to the glass?
Anyway I'd be keen to see if replacing the audio jack will actually fix the problem, any of you guys had any success with that? Meanwhile i'm going to just use the power button for ending calls. Any tips for how to use the speakerphone during a call?

New sensor arrived yesterday changed out my wife's non functioning sensor and new one working great. Old sensor was bad !!!! She had dropped the phone several times and last accident she unintentionally threw it. Suggest no one do that

I put a case on mine and it started working. It always would work if i put a bit of pressure on different parts of the phone. So im not sure if bad contact on sensor or if more some type of alignment issue. But it does work now. speakerphone is terrible since cracked screen tho..
So i recently upgraded to Lollipop but my proximity sensor is broken. I used an app called Proximity Screen off lite but it is not working on latest android lollipop. So now when i make a call my screen goes dark until the call is disconnected from other party. same is the case with incoming call. Please help

Another option that would've worked pre-5.0 was to use an xposed mod to disable the proximity sensor, but of course that doesn't work in 5.0 now. Hopefully someone else can chime in with a software solution that I couldn't figure out.

I replaced the original screen with an after-market one as well, and that's what caused my proximity sensor issue. There is a good chance your proximity sensor is actually working perfectly, but the after-market screen is blocking it from reading a "far" value. If you're comfortable with taking your phone apart, you could fix it without replacing anything. There were two things that I did to fix mine, but I'm not sure which of the two actually did the trick. 1) I repositioned the sensor so that it sits further away from the glass, and 2) I scratched off the black paint on the back side of the glass around the two clear sensor holes. Obviously you should attempt option 1 first, because the second option is permanent and will damage the aesthetics of your phone. Others have reported success with option 1, by gluing a thin spacer with two holes to the front of the sensor, but I did not have any luck with it when I tried initially. You will find that it helps a lot to do this while the phone is on, and you can see the results of your changes immediately with the Proximity Sensor Finder app.
Edit: Before you attempt any of the fixes, you should verify that your sensor is in fact working by pointing it away from the phone after opening up the back.
I had the same problem, I gave up on a software fix and I ended up taking the phone apart and fixing it outright. It's not too difficult, all you need is a plastic tool to open the case and a T5 and a 00 screwdriver.
There's a good youtube video (I can't link - see /watch?v=AZDAIgwbXk4) showing how to do it. Skip the battery section, you can keep that in place. At 9:00 he indicates where the proximity sensor and light sensor are. Take the small rubber gasket that covers the two sensors, and flip it around so that the larger hole is over the proximity sensor. Put it back together and turn it on, it should work perfectly.
Also, in the interim, you may wish to have your phone end calls with the power button, it can be found under accessibility settings. At least that way you won't be stuck waiting for the other party to pick up or hang up.

I had the same problem, solved by a thin plastic sheat that I drilled two holes in and then cut to the right size.
I placed the plastic part in the bottom of the hole, where to fit the sensor. I also shaved of aboute 1 mm on the part that presses the sensor from the back.
( I hade already taken the blackish film away in a own attempt to fix the problem, at least the light sensor seems to meassure more accurat)
Hi all, I've similar problem on my LG G2 D802. I have tempered glass with covered prox. On KK everything was OK, but unfortunately Lollipop has much more intensivity on prox
and I had the same issue as you, I mean - turned screen while I maked call and vice versa. Moreover, I can't get in to the service menu on LP (AOSP 5.1.1).
I bought new tempered glass with uncovered prox but fortunately Xposed has working old module: Disable proximity sensor.
I don't know about what module you guys talked about but this module works very well.

I've recently changed the LCD panel on my Nexus 4, as well as the headphone jack (which includes the proximity sensor) - the screen was broken, and the headphone jack had very noisy output.
After putting it back together, I noticed that the proximity sensor wasn't working - during calls, it would constantly blank out the screen, and there was no way to unblank it (i.e. it seems to permanently think there's something there).
I took it apart again and noticed that the rubber gasket that fits behind the sensor is missing - it must have come off during the repair and gotten lost.
I tried putting some Blu-Tack behind it as a temporary fix, but no change. Now, I'm not so sure the new proximity sensor is working either.
Anyhow, my first question is, where can I buy a replacement rubber gasket? I've done a search online, and I can't seem to find it for sale anywhere?
Is it available as part of another spare part for the Nexus 4?

I had to replace the screen on my n4 too and fixing the proximity sensor was a pain in the ass. I had the rubber gasket but it was only working when the front panel was not attached as soon as i attached the front panel it stops working i.e. when in call the screen goes off but it doesn't come back on. So what the guy did was to scratch off the two holes on the top left from the inside of the panel. its looks a bit nasty but its working fine now. you might wana try to scratch off a bit neatly. like in a round shape.
I dont know if the gasket is the cause, but I know a case where this exact problem happens : its when u don't push the sensor well enough to fit in its place, cuz it should be near enough, or it'd blank the screen during calls. Try using a flat screw driver, and push the orange part where the sensors are well enough, that should do the work ( don't push haaard or I'll Damage it). I hope that'd help, cuz finding this gasket is hard. And this applies also for the guy who commented, cuz this is the case for him

just yesterday I got my phone back from repair (I cracked my screen, nothing was working then).
Here is a photo of my old screen, which had a hole right below the upper loud speaker:
I think it might be that the drop broke the sensor, and not the repair itself?!?
Using an app like this: https://play.google.com/store/apps/details?id=com.tigermonster.proxfinder shows that my sensor is constantly detecting something.
I'm not using any screen protectors or anything
For now I rooted my device and disabled the sensor by installing the xposed framework and then using http://repo.xposed.info/module/com.mrchandler.disableprox
Strangely enough, yesterday, like 3h after I got my phone back I called someone and everything, including the sensor was working perfectly!
Can anyone show me a picture of such a proximity sensor? Do you guys think a fix like in this video shown: https://www.youtube.com/watch?v=HPHsianYnqo, putting a piece of paper behind the proximity sensor could work?
You never ended up showing any images of the phone before/after the repair but if you have a screen protector fitted to the screen and it doesn't have a little cutout on the right just below the top speaker then the protector is constantly tripping the proximity sensor.

Thanks for the info about the position of the sensor - I'll definitely do that when I'm going to disassemble my phone (which is gonna take a while since I'm waiting to receive a new rear housing from a seller in the US).
I'm planning to disassemble as little as possible, so I'm hoping it's easy to access the proximity sensor (but guessing by the pictures it should be).

Good luck. Hope you can fix it by cleaning it. Make sure to have some thermal compound on hand to replace the old stuff when you disassemble.
Hey one more quick question:
Do you think it's possible to remove the motherboard without removing the battery?
I guess that the problem is that the proximity sensor is either too close to the screen, or too far away (like there's some small gap between the proximity sensor and the screen). I will probably have to remove the motherboard if it's the former (if it's the latter I can just put something like a pice of paper at the back of the rubber).

You won't need to remove the battery to remove the main board. You'll just have to unplug it.

I don't know which type of proximity sensor this is, IR, led, electromagnetic etc. But all proximity sensors I know of have two "eyes" One eye emits ir, led, or electromagnetic field, the other eye looks for a change in the return field. I think you'll find a scratched, smashed return eye. Or debris lodged in the sensor housing. Use a magnifying glass and excellent lighting.
If you shine a bright flashlight at the sensor through the screen can you see anything blocking the eyes? Doesn't matter I guess, you'd have to disassemble to get it out of the way anyways.

It looks like the prox sensor comes covered in a rubber cover. In this pic the rubber is there:

I did what you asked: There is no light coming out on the back of the phone if that's what you meant.
Anyway, I'll have to wait until I receive my replacement back housing and then I'll try my best to repair the sensor. I hope the rubber is just misaligned.
Alright - so I did have time today to disassemble my phone (got the replacement housing last week).
I noticed that there was no rubber part on top of the light and proximity sensor, as well as on the device above the two (i don't know what that is).

Everything is working well again (there were some lints on the display right below where the proximity sensor is located, that's probably why it stopped working from one day to another).
I guess the main function of the rubber is to protect the sensor against dust and lints.
I also am very mad at myself for having forgotten to take out the NFC antenna from the old housing before replacing it, so I had to open my phone again after I did already close it (and now there are slight traces of an opening - again )
But apart from that everything's fine now and I hope it'll stay this way.

Glad you got it fixed! And yeah, unfortunately it's nearly impossible to remove the back cover without leaving dings at the injection site. When I put on my new top and bottom rear covers I removed most of the glue. So next time I can remove them without much trouble.
I am having similar problems. I just traded for a nexus 6P and everything works great except for the proximity sensor. The 6p was just OTAed to 7.0 and I only unlocked it and installed TWRP. No other changes and no root (yet).
There is no screen protector on the device.
When I make a call, the screen stays on. The screen turns off as soon as I cover the proximity sensor. The screen then does not come back on until the call is completed. I downloaded a bunch of proximity apps, including Proximity Sensor Finder. Proximity Sensor Finder (and others) report that sensor detects Far (5cm or more) and Near (value of 0 cm when the sensor is covered). The sensor doesn't seem to identify intermiate values. The odd thing is that the sensor doesn't seem to reset. If I cover the sensor (value of 0 cm) and remove my hand, the sensor continues to read 0 cm.
Does this sound like a hardware or software issue? The device appears to detect both the near and far states, but doesn't return to far after the sensor is uncovered. Thanks for any help.
Solved the problem. I downgraded to MM and the proximity sensor problem persisted, so I figured it was a hardware issue.
I took apart the 6P and the there were signs that repair had been performed. The screen was probably replaced previously. The little rubber gasket covering the proximity sensor was missing, just as the OP described. I cut a blue-colored straw and placed it over the proximity/light sensor. Works perfect!
I need to bump this thread cause a few days ago I started having the same problem that nickfuzz describes.
Now I've got my opened phone in front of me and tried putting a pice of a black straw around the two sensors, but that does not help. It reports 5.0 cm and if I cover the sensor it stays at 0.0 cm
Can anyone give me an advice about what to do now?
Update:
The light sensor is also not working properly: It is reporting way too less lightness, and tends to report 0 lux when I'm not directly holding the phone under a lamp (for instance).
After 5 hours of trying I somehow managed to at least get the sensor to always report 5.0 cm. That's better than constantly reporting 0.0 cm.
I put a piece of a wide straw (there are different sizes, you know) around the hole board on which the two sensors (light & proximity) sit on. I guess this is a bad idea.
I am now trying to get this rubber gasket from somewhere, but I guess I'll have bad luck since I'd have to buy a defective nexus 6p from which I could take it from
UPDATE:
Can anyone tell me why the screen is painted black on the inside, right where the proximity sensor is located? I scraped off the paint and painted it over with a black pen. That must be the reason why the proximity sensor is always reporting 5.0 cm (which doesn't make much sense, because at some point it would have to detect something if it's not broken).

Can you tell me if you put the straw around both sensors and their "chassis" (that black thing they sit on), or did you put the straw only around the sensors (i.e. on top of the chassis)?
I still need to find a way to fix my sensors
I tried to use some electric tape and put it on top of the sensors with two small circles cut in, but eventually I figured that the problem actually lies in the black paint on the inside of the screen.
I removed that paint a few weeks ago and I couldn't paint a new black paint on it that is not too thick, so I am stuck with a sensor consequently reporting 5cm.
I did try to use an Edding.

Everything was still good after uninstalling that app too.
