SOLVED: MAKE SURE THAT THE ROM YOU'RE FLASHING IS FOR YOUR DEVICE!
Hello,
I can't seem to be able to flash any rom on top of my current CM10.2Nightly.
I'm trying to flash ParanoidAndroid3.99 onto my Nexus 4 and it won't let me.
It says that the ROM and GAPPS are installed successfully but when I boot
the CyanogenMod boot animation starts and CM10.2 boots.
Why?
I did the following(Using TWRP):
- FACTORY RESET
- INSTALL ROM and PA GAPPS
- WIPE CACHE & DALVIK CACHE
- REBOOT
Also, my phone connects to my computer using debug, but once it gets to the bootloader it loses connection and can't install anything.(Using Nexus Root Toolkit.)
Any help would be greatly appreciated!
Odd. Try going into advanced wipe within twrp, select system and wipe it. Then flash PA.

Try reflashing twrp. Also redownload PA and make sure the md5 sum is correct.

First off.. Don't panic ... It wasn't your fault it failed ... I know because it happened to me . don't get it from "downloads" or goomanager .. Get it from the second post I. That thread on the latest date
Second . I don't think otg works in twrp so you will have to use adb to load the ROM

OTG does work in TWRP. I have found that it doesn't like some flash drives - no idea why. I've tried quite a few, some of mine work all the time, other just won't work.

Ok went to twrp and used the ADB sideload feature but it said OTG cable fail to mount.
FIXED
WOOO FIXED IT NVM! Mod please delete thanks for help guys
Someone with more experience correct me if I'm wrong, but shouldn't we be avoiding wiping system in TWRP prior to flashing ROMs? I believe I read a thread (probably on RootzWiki) that said wiping system messes things up as you lose the ability to flash a ROM in the "traditional" manner. I wipe cache, dalvik, then click factory reset. I leave system alone and I've never had a problem with flashing ROMs.
So i got my nexus 4 yesterday and Ive always wanted to root an android phone and install a custom rom. So i unlocked the bootloader and rooted it pretty easy. I want to flash AOKP rom. i downloaded the rom file and google apps from their website on the phone. I went into CWM recovery and installed those files from the sd card, and it says its complete. I reboot the device and it just sits on the nexus logo screen. I dont know what i did wrong. Anybody got any ideas
It's normal for it to sit on that screen for about a minute or so.. How long did you give it?

make sure you have the correct files for your N4 or "Mako" device and that you did the steps properly.
wipe data/factory reset in recovery
flash ROM
flash Gapps
reboot

yeah im pretty sure i did that all correctly, but ill try again. Also for some reason when i try to boot into recovery sometimes, instead of going into CWM, it shows the android robot open with an exclamation mark on him. Any idea what that is

I believe that the stock recovery...
When you first boot cmw, it does not stay permanently afterwards...
You can flash it again by downloading ROM manager??
Or somthing else..
Maybe use adb+fastboot to flash ROM..
Just to try if the files aren't broken.

You can't flash a ROM in fastboot, you can only flash images.
You can flash the recovery image through fastboot.
I would let it sit for more than just 3-4 minutes as well.

Maybe I'm wrong, but I think you have to format /system and format /boot when switching from another rom, otherwise it bootlops.

I flashed argghhh's kernel and am having bluetooth stutters (with apt-x headphones). I am not sure it's because of the kernel though and want to test a bit.
Does anyone know how to restore the previous kernel without re-flashing the stock rom?

Just dirty flash the stock ROM, without wiping anything. Well, you could wipe cache/dalvik just to be safe. If you don't wipe data, dirty flashing the ROM shouldn't affect anything.

As sloosecannon made clear a simple solo restore of the boot partition / img is replacing the kernel.
9.8.1Q-78-1 Stock Kernel
Thanks everyone!
And thanks Attack!
Update: Oh wow, that was it. No skips at all now. 2/3 of my audio programs would skip before when starting a song (again, only when using an Apt-X compatible bluetooth headset). Or they'd skip when opening a menu while playing a song, or turning off the screen. Skips would continue for 5-10 seconds and then it would even out.
Not even once on the stock kernel. I guess I'll report this to arrghh

Hmm, I tried this and it installed and all, but my wifi is broken. I wonder if it is because I was on a deodexed stock rom. Will try a dirty flash of the whole thing now.

Yeah, no idea. A dirty flash got it all back and working.
If I pressed the wifi toggle it would just turn back to off. After a few times I got the 10 fast vibrates and still nothing. Wiped caches a few time, still no.
But I'm back up and running
I was having an issues with my WiFi when using arggghhss.
Flashed the stock kernel from this thread and now I have the same problem with my WiFi not turning on. A dirty flashed fixed this? What's a dirty flash and where can I find a stock ROM?

Dirty flashing is just flashing without wiping anything. Just dirty flash whatever ROM you're using and it will reload the original kernel that came with that ROM. You can wipe cache/dalvik to be safe, but don't worry about wiping data when you're dirty flashing.

it should still work via CWM - but I think most folks are using TWRP.
what are your steps in the process?
mine would be:
copy zip file to the root folder of the sdcard (mnt/external1)
reboot recovery
mount system
wipe cache
wipe dalvik
wipe data/factory reset
wipe system
reboot recovery (not sure if this in an option in CWM, if not, then just make sure that system is still mounted)
mount system
install ROM
reboot

Well I got it flashed. I was doing a dirty flash so I didn't wipe data or system. But I guess I didn't realize that you had to Mount system. I assumed that the recovery did that automatically.
After mounting system the stock rom flashed just fine and my WiFi is golden. Thanks to everyone who helped.

yeah, I think what happened was that you were installing a ROM with an incompatible kernel (existing one) or vice versa... in either case, glad you got it working. i almost always mount when tinkering....

So I decided to flash slim 4.4 on my d800 and it failed using twrp 2.6.3.2(?). I had the options for home, reboot system and something else. I elected to reflash the ROM that I'd been using, as I was hesitant to reboot system knowing that a flash had just failed. My question is, if I had chosen to just reboot system, would it have rebooted to the ROM I had been using prior to attempting to flash? Noob question for sure, but this was my first failed attempt at flashing. Thanks.

Best thing to always do in that situation is to just restore your BU and try redownloading the ROM. Or wipe and flash something else(working obv lol). Sounds like a bad DL to me

Don't think the md5 part matters. Sounds to me like you're not wiping completely (/system specifically). Follow these steps and you should be good to go..
Wipe dalvik cache
/system
/data
cache.
DO NOT WIPE INTERNAL STORAGE
