Download the stock image file and extract the boot.img from there and place it in kerne_backup folder.

If you are trying to perform an upgrade to 4.2.2, then you would need the boot image from the version of the ROM you are upgrading from.
If you already have 4.2.2 (see your sig), why would you be trying to install an update package?
If you already have 4.2.2, but a custom kernel, you can flash the boot.img file from the Google distro a bunch of different ways, here's 3:
1) fastboot (unlocked bootloader):
- fastboot flash boot boot.img
2) ADB (with custom recovery running)
- adb push boot.img /sdcard/boot.img
- adb shell dd if=/sdcard/boot.img of=/dev/block/platform/sdhci-tegra.3/by-name/LNX
3) restore only the boot image from a prior Nandroid backup (twrp)
If you want decent help, you really ought to state:
- is your bootloader unlocked?
- do you have a custom recovery installed?
- if Yes (to the above question), did you make any Nandroid backups prior to the custom kernel install?

You are going to run into a huge amount of trouble if you don't understand the difference between bootloader and boot partition.
Perhaps it's just a language thing, but understanding the difference means the difference between a tablet which survives and one that becomes a brick.

Finally I using nexus toolkit and install fresh android 4.2.2 using image from Google dev web site and re unlock and root my tablet. After this everything working like before

So I am trying to adb sideload 4.2 and its failing at the update, specifically "verifying current system....assert failed: apply_patch_check("/system/app/MusicFX.apk") and a bunch of numbers then Installation aborted. Im running stock rom 4.1.2 and the update originally failed after the OTA so I decided to try manually and same thing. Anyone have any ideas?
Download the full 4.2 factory image from Google's Nexus factory images page and fasboot flash that

Just flash the system image only, via "fastboot flash system system.img" (extract the ZIP in the download), no wipe then.

I recently unrooted my nexus 7 device because I really want to upgrade to 4.2.1. I tried every possible way. My deice is unrooted, so I tried to update my it from settings, but it goes into custom recovery and saya...

So I said ok, it doesn't work, so I found the most recent update for the nexus 7 and downloaded it, went to custom recovery, found teh file and tried to update it manually.... This is what is said..

Just download the firmware direct from Google and flash it using fastboot. Easiest and most reliable method, though you do need an unlocked bootloader.

From Google meaning their website? or from settings on the Nexus 7? and what do you meant by a bootloader? which bootloader should I use?
I'm not sure but i think if you want to take ota it might need stock recovery to install and to update manually from custom recovery I think you need the BIG update from 4.1.2 JO??? to 4.2 JO40C as the update from JO40C to 4.2.1 JOP40D is incremental if I'm not mistaken....also you may need to update clockwork mod

I've got the same issue.
CWMRecovery says the same thing when I want to update by OTA.
I tried to install manually the ZIP file (/cache) by copying it in my /sdcard with CWMR but I had the same script.
I'm rooted, in 4.2 (logic) with CWMR 6.0.1.9 no touch.
Do I have to restore stock recovery or a sideload flash (or fastboot) would work ?

I am using clockwork recovery. How can I change the recovery?

Follow the above link. It walks you through downloading a factory image from google and using fastboot to install it, regardless of what you may have done to your device. Also, if you are doing this, you can go ahead and use the latest image if you want, which puts you at 4.2.

I have flashed my nexus with Clockwork Mod Recovery. Any places were I can get this rom and how I can install it?

That's because you are using a custom recovery. You can either flash a stock recovery or edit some files in the zip so you can flash it in the custom recovery.

Possibly, but probably not. In most of the reports that have been coming in - when the users actually report the full error message rather than saying something ambiguous like "I got a Status 7 error" - the problems revolve around altered/removed files in /system, or having a custom kernel (boot or LNX partition) installed. That suggests that the recovery version is not causing assert() failures.
The other thing to rememder is that the program that is interpreting the edify updater-script is the "updater-binary" program which is shipped inside the OTA - so it is virtually identical independent of the choice of recovery (other than perhaps kernel behavior differences).

Even though what you are saying here does only require being able to operate a plain-text editor and a zip tool, it's way above the heads of most folks who think that OTAs should work on non-stock ROMs. And I'm not sure how these folks would be better off with partially-patched ROMs. In any event, if this course is taken indeed a custom recovery would need to be used for this, as modifying the zip contents will break the OTA package signing, so the stock recovery couldn't be used.
The OP should just
- make a full nandroid backup of what they have now
- download the factory 4.2.2 images from Google
- extract the system.img file and boot.img images
( hint for windoze users: 7-zip understands .tgz files)
fastboot flash boot boot.img
fastboot flash system system.img
- reflash a basic SuperSU or Superuser root kit
Sort of crude, but it's a decent hack for the lazy or poorly informed. It won't "erase my data", but presumably it will break whatever it was that altered the stock system and/or boot partitions - the very things that are causing the OTA to fail on them with their rooted & modified ROMs.
If they don't like the result, they can simply restore their nandroid backup.
OK, just for background info, my Nexus 4 is currently rooted, running the hybrid radio firmware (so I've got LTE, that's working), and my current firmware is build JWR66V.
I'm trying to upgrade to the newer firmware (the newest 4.3), which supposedly has some minor security updates to the PRNG & such, but I'm getting an error when I try the OTA update.
When the phone boots up to the bootloader (Team Win 2.6.0.0), I get the following messages in its terminal:

You will need to flash back the 0.84 radio, and replace the build.prop file with the original. (You must have flashed something that modified it.)
Where would I find an original radio file and build.prop?

What I would do if I were you is get the JWR66Y factory image from Google's developer page, and flash the system.img, boot.img and modem.img files (only those three) using fastboot. Have a look at the return to stock link in my signature. (You will need to flash a superuser zip again afterwards if you want root.)
Is the boot.img file the stock bootloader? I may just flash the system and modem images if that's the case, to make re-rooting easier.

No it is not. The boot.img file contains the kernel and ramdisk. In any case, you can flash the bootloader.img file as well the 3 mentioned above. It doesn't affect your ability to get root...

Got it. I'll flash all three, though since I've got the Team Win bootloader, I'll probably keep that.
Next time I have some quality time to tinker with my phone, I'll try the updates you suggested.

That would be the TeamWin recovery.img, not bootloader. Recovery is not a bootloader.

Looks like it's all working! Flashed the three .img files you suggested using fastboot, wiped the Dalvik cache, rebooted, now the setting show I'm running JWR66Y.

Want to OTA update my N7 3G (2012) to 4.4.2, but failure to update in TWRP.
Since OTA update not allowed in custom revovery, it any way to update it ?? Or I must wipe and fresh install 4.4.2 ??
Otherwise, is OTA update alloed in rooted device ?? (keeping stock recovery)

OTA updates are possible with both, standard and custom recovery, but if you have done modifications to kernel / system most likely the OTA will fail. Or, if it doesn't fail after the update you will loose the custom recovery.
The easiest way to update is flashing boot.img and system.img (extracted from the appropriate Google factory image) by fastboot.

You can find a description how to do it on my blog, even if it explains haow to install Android 4.2, the steps are the same.
OTA's are crap. If you're rooted w/custom recovery there is never (IMHO) any good reason to take an OTA.
