you need stock rom, no modifications made, and stock kernel for it to work
Download and extract the original 4.2 and fastboot flash system system.img and it'll restore all missing apps.. I had to do it earlier to allow the system to install the update

update manually.
So I got my update but it keeps failing during install. I'm rooted and every time the update starts it kicks over to CWM recovery and stops with a message something like, "do you want to install this untrusted zip file" I say yes but it still fails. I have even tried to manually install the update through CWM but that fails too. My N7 is rooted but otherwise stock, I have not removed any stock apps or flashed roms or kernels so WTF must I do???
Did you not read the posts above yours? CWM recovery is not stock.

I don't know where, but I saw a few posts about working updates to 4.2.1 with CWM recovery.
However, I'm not getting it to work.
How do I get rid of the recovery without loosing the root?
If you are on Windows I find the Nexus 4 Toolkit to be super easy to use. I flashed a stock recovery so I could get the update. Unfortunately, I forgot I had made changes to the stock weather widget so I can't apply the update anymore because of a missing geniewidget.odex file.
You will need to be fully stock to get the update to take as posted earlier in this thread. This is a well known requirement when doing OTA

Successfully installed OTA here while rooted with CWM recovery, though everything else is stock.

Hey,
got an Error over here with stock recovery and root.
However, tengen says he made it with CWM and root - maybe it's because I'm using stuff like Busybox or the Pimp my Rom APK?
EDIT: I think its because of the %-BatteryMod I'm using. I'll try to flash the custom rom.

Unlocked Bootloader.
CWM.
Root.
Fail in updating, with message something like assert fail apply_update_check status 7.
Revert to Stock Recovery and Unroot (bootloader is still unlock), update again, result in error with the android robot and red exclamation.
Happen to my Nexus 7 and Galaxy Nexus as well (same unlock bootloader cwm root, same revert method, same error).
Has anyone face this and any solution to make OTA works?
Get back to stock, then try again

flashing these 2 files WILL NOT CAUSE application to be formatted (like what I thought at the first time).
It will simply remove any tweak done to system such as ads blocking (adaway, etc), battery icon mod, etc.
Then, you can update just fine.

Sorry, but I searched and searched but I still don't understand. I removed all my mods.
I wanted to update to 4.2.2 but it says apply_patch_check error about Currents.
How do I flash that boot.img and system.img? Trough custom recovery or stock? And where can I find those files? Thanks.

boot.img and system.img can be founded in the STOCK ROM, here is the link for STOCK 4.2.1 for Nexus 4:

Extract the file, you can find boot.img and system.img inside it.
To flash these 2 files, since I am not familiar with fastboot, I am using Nexus 4 Toolkit to help with it.

Copy boot.img and system.img to Nexus 4 Toolkit folder of "put_img_files_to_flash_here"
then using "11. Boot or Flash .img Files to Device option", flash the file to the correct partition
(when flashing boot.img, to boot partition | when flashing system.img, to system partition).
So I got the OTA update, but after downloading it, the install failed (dead android says: error), then the Nexus 7 reboots to 4.2.2.
I already did a factory reset, but it didn't help either...
Anyone with the same issue?

I got this problem...
I download this OTA...Then it say reboot to install but it enter TWRP.
Then I put the zip to internal storage and flash but I got error.

looks like your SystemUI.apk has been modified not in original. I would recommend to get copy of systemUI.apk from stock 4.2.2 and copy it to your tablet and using file explorer, replace it. and give a try.

my OTA failed too, different error. this usually always the case if /system/ folder integrity check failure (error 7), means files in /system/ folder are not original or modified. Yours does not give any details, but I bet it is. to see what error, try to flash it thru CWM or any other custom recovery you use.

I guess I'll download the Nexus Toolkit, click on the flash stock + unroot button, so it flashes the stock 4.2.2 and then I'll try to OTA update again.
I already unrooted, but I think it's not a problem.
Seems like it is not fixable!
I used fast boot to push my missing file.
I used es file explorer.
I used titanium back up to make the app, system...
No way, nothing. Not the slightest luck.
(My missing app is xt9ime.apk)
I haaaaaate this situation.
the worst thing is that xt9ime.apk is asus keyboard, and it is disabled by google on nexus 7.
why should not having an app that they don't want to use be a problem. X(

Are you guys with errors using SuperSU? If so what version?
It seems like that the last couple versions touch a file used by OTAs. The Dev recommends using the "Full Unroot" option in the SuperSU app prior to accepting the OTA.

I did full unroot, and I also did a factory reset, but it still fails to install.
Using Wug's Toolkit to flash stock 4.2.2 is a good idea?
Then try to OTA update 4.3 again?

Fastboot is a utility used to control the base fastboot firmware on many Android devices. It lets you do low-level actions such as flashing a custom recovery, ROM, or kernel. Toolkits run fastboot behind the scene, but automate the commands for newer users.
I had exactly the same problem and after trying a few things, I just found a working solution for my Nexus 7.
I used the same tool I used to unlock and root a few months ago: Nexus Root Toolkit 1.6.3.
I made a backup of my files first, my idea being to reflash the 4.2.2 rom
I downloaded the system file here: https://developers.google.com/android/nexus/images#occamjwr66v
Then, in the Nexus Root Kit, launch " flash stock + unroot" and select the downloaded system file.
The flash lasts 5 minutes, and the system will reboot.
Then you need to reactivate the usb debugging .
The next action is to relock the Nexus, with the "OEM Lock" in Nexus Root Toolkit.
The system will reboot after flash.
Then, the 4.3 update will be available automaticaly but this time the installation won't crash.
I hope this could help.

Thanks for the idea. I used a similar approach with different steps and got 4.3 and root back.
I had stock 4.2.2, stock recovery, rooted (SuperSU).
Backup data.

I wanted to save my data and wasn't sure if going directly to 4.3 would be safe. Was just being cautious and letting the OTA do the update.

I'm a complete noob when it comes to rooting
I've been searching online on how to root a nexus 4 using a mac.. every single guide is different, some skip the unlocking bootloader part, some skip other parts, and most guides are just outdated.
I'm running on absolute stock, 4.4.2.
What's the best (and most current) guide out there? I'm interested in rooting my phone mainly so I can get LTE access.. do no care too much for non stock ROMS (not yet at least)

You don't have to install another ROM. You can just download the recovery (CMW for mako touch or non touch) in the post. Also download the su zip tot get root acces.

ah, so the touch column is all the 'stock' roms. how do I know that it's the latest 4.4.2 stock rom? will be able to do OTA updates after I flash the mako touch?
If you're device is rooted I think you won't be able to install OTA updates from google anymore. But, you can flash these stock roms easily. I'm sure people make flashable zips of every new version of Android. About the columns: ROMs=you can download custom roms (you don't need that); Recovery and recovery touch is basically the same thing. The difference is one recovery can be controlled with the touchscreen and the over with the volume and power button. It's just what you like best.

What is the definition of CWM (clockwork mod), what exactly does it do?
What is a factory then? (when someone talks about flashing a factory image, I thought that's what CWM is, what's the difference?)

Cwm(clockworkmod recovery) is a side load which helps u to flash different ROMs and zip files. It also helps u to wipe battery stats, partition sd card(not with case of n4)
Factory image is the stock ROM that Google provide that u r having in your nexus 4

And regarding your previous question of OTA updates, You'll have to flash the stock recovery if you're on CWM or TWRP (Team Win Recovery Project - Many people recommend this one, I use it myself) and you shouldn't have made any changes to the system files of your stock ROM.
You will receive the OTA if you have modified anything in system, you just won't be able to flash it. And you can also flash the OTA with TWRP instead of stock recovery. Root does not matter, although it will be lost after you update. Before updating, also make sure you're not using a custom kernel. Kernel: The "thing" that android runs on. It's the heart of the OS, you could say. No kernel, no boot.

So let me make sure I'm following this..(sorry, I'm sure you saw such questions countless times before)
CWM (or TWRP) is a custom recovery instead of the stock 'recovery' option that's found in the bootloader. If I choose to go with either CWM or TWRP, how much of an issue is it to flash back to stock recovery? would it be done through ADB the same way the custom recovery was flashed in the first place?
As I understand.. the first thing I should do after unlocking the bootloader (already done) and flashing a custom recovery (not done yet) is to create a backup through the custom recovery. Does that back up the radios as well? If I decide to flash the all in one LTE zip and it doesn't work or what not.. will using that recovery flash back the old radios?
Now to the actual rooting..
so flashing super su is what actually does the root? How would I go about returning to complete stock if I ever need to? (stock recovery, remove root, lock bootloader)
When flashing a different ROM, how come I need to make a backup of my apps? what if I want to start completely fresh? Would I not be able to go to the playstore after flashing a custom rom and downloading everything I need? or there is not going to be a play store after flashing? (that's why GAPPS are needed?)
so I rooted and flashed the hybrid radio, works like charm!
I answered most of my own questions in the post above just by going through the process.
What happens if I decide to factory reset through settings? will CWM stay? will the root stay? will the lte fix stay?

I want to update to latest 4.4.2 and save all my data and customization (I'm using Xposed framework)
To my understanding, I cannot use OTA update due to TWRP, so how can I manually to this? what is the best method?
My intuition says flash "image-occam-kot49h.zip" using TWRP, but the I will lose the custom kernel and maybe root?

There are many ways but I would recommend that you flash stock kernel before doing any of these below.
1. Download OTA package from a thread in General Section here (use search tool) and flash it via twrp.
2. Download a flashable 4.4.2 stock rom (or any custom 4.4.2 rom) from a thread in Android Development Section (use search tool) and flash it via twrp. Then download the new .98 baseband (optional, from General Section) and flash it in twrp.
3. Download 4.4.2 factory image (search in google) and flash it via ADB. You don't need to flash the whole package. Just system.img and radio.img should be enough.
Note
1. The one you said image-occam-xxx.zip that is not flashable via twrp. You need to extract the .img files and flash it via ADB.
2. Yes all the above option will make you lose root. Not a big deal, just flash back SuperSu after.
3. The above note (no 2) also applies for kernel. Unless, you use the third option to upgrade.

Then, I flashed stock kernel, stock recovery, uninstalled Xposed framework, and tried to apply OTA (not manually) and it failed
I don't want to install a fresh 4.4.2 because I want to save all my customization, how can I do it?

You're not going to be able to. The customizations are what are letting you from getting the ota.

I've uninstalled Xposed framework, and all the customization are now gone (but saved), according to other threads here this is enough to be able to get OTA update. all other customization are not related to ROM features.

To get the ota flash the system and boot images from the factory image with fastboot.

Then just use option 3. Just flash system.img and radio.img (radio is optional though). It will rewrite your system partition. Therefore root and anything you installed as system apps will be lost. But just flash SuperSu, and reinstall and update xposed. Then activate back all your customization.
