The high percentage started before I even add a mp3 file.
This is frustrating as hell.
I listen to music about 2 hrs a day.

Sorry for the lack of translation...
Mediaserver is used for many things, including apps syncing, location report, anything that use a file on storage (not only mp3 but any type of file, really, playing games will increase media server usage for example).
What I see from you screen shot is that you listen to music using Google Music and it uses the media server service.

I tried using another player but Mediaserver keeps working hard. Any advice on how to contain it?

It would be the same because mediaserver is used when files are used, whether local or cloud, whether uplink or downlink.
If you're willing to unlock bootloader and root, you can flash a custom kernel such as franco.kernel, the tweaks will reduce medua server usage and increase battery life sunstantially. But.... if you listen to music or play games all day, dont expect any change.

right now I get roughly 3hrs of screen time while listening to music for 2 hours a day, a few minutes of gaming and a lot of 3g (reading aricles / news ).
I use Juicedefender on custom settings (15m).

You might get another hour, depends on signal quality. 2 hours of music explains the media server usage. If you find a ROM/kernel that reduces the media server, the Play Music % will shoot up, because it is what actually uses the battery.

I can't believe how bad the battery life is compared to rooted roms. I can't go 7 hours without running out of battery.
I use music streaming from Google music for about 1 hour, internet probably 20 minutes for every hour on and texting periodically. No calls no pictures. Sync is set to manual and don't have any apps that I know of working in the background that are ruining my battery life.

well the streaming from google music will take up a lot of battery considering its using data to stream and your speaker to play music (even if your using headphones...it'll use battery) internet takes up battery too....so no wonder you have crappy battery life
a lot of rooted ROMs have custom kernels that allow the user to over/underclock their cpu's and undervolt their batteries therefore giving them better battery life.

music is like one of the main uses of my phone when im out im pretty much always plugged in and i none of previous i felt that i've had such high battery drain , Screen of and music with ear phones plugged in the drain used to be very very less even after a few hours of music , but on n4 the drop is significant i dont have any screen shots right now because i just charged the phone but i will update it as i get some .
Does anyone else feel the same way ? and any idea whats the reason for it ?

I listen to podcasts all day at work on a Bluetooth a2dp headset and I can Agree with you. the battery drain is higher on this device compared to my galaxy exhibit 4g. I'm going to try another app and see if that helps. I am using pocket casts right now. I will get some screen caps tomorrow so we can compare.

Have the same issue with podcasts, drain rate is up to 15% per hour, even when podcasts are locally stored. This is almost as muc power as the screen uses. Whenever I listen to a podcast 'Mediaserver' is at the top of my battery list.
Sounds like you have the #1 most dreaded N4 battery drain issue. This is the one and only reason I would ever RMA. Absolutely terrible.
No drain here... I stream on DC-HSPA and have normal drain.... On Wi-Fi... It drains very slow.. Almost less than 10% / hour
But off of stored music and podcasts the battery goes and goes. I use Beyond Pod

i would'nt call it battrey drain as such , But all im saying its almost double or more than that compared to every other phone i've used when it comes purely to listening to music .
other devices it used to be only the music playing app which drains battrey here higher or equal amounts are drained by media server so if out of 40 % drained 50 % is from these 2 for about 1.5 hours to 2 hours of music listening , Whihc imo is very bad for liek 20 + % battrey drain for 1.5 to 2 hours of pure music listening with ear phones .

Do you use an equalizer? I suggest to stop using it if you are using one and see if there is any difference.

I use the inbuild DSP manager as i use apollo music player but then again i've used them on all my phones anyidea how that makes any difference ?
like i said i like music and its one of my main uses thats why i have a klipsch s4i which is about to die on and have a shure 215 on the way already , is there any reasoning behind the EQ thing ?

hope this question wasn't asked already, I searched for it and found nothing, anyway.
So, I'd like to know, when you're using your phone normally (browsing, texting), how many minutes does it take for 1% to drop? of course, I mean while screen is on.
I would appreciate if you also wrote which rom and kernel you're using. Also, what governor is being used, whether sync, Google now And location are on.
I'm currently using Slim + Semaphore (always switching governors), sync, GN, location and WIFI and under those conditions, 1% drops after 2-3 minutes.
Many people report hanging a great battery life, with 4-6 SOT and I just don't understand how it's possible. For comparison, I'm currently on 44% with 1 hour and 18 minutes SOT.
Please no flaming and such, I really want others to contribute to this discussion rather than hear something like "RMA your phone" and such.

It depends how you use your phone. I don't use facebook, i dont sync twitter or g+. I rarely use location services. The only things cause wake locks are whatsapp and hangouts for me. Then, I get 20 hours with 4 hours sot on wifi 2.5 to 3 hour on 3g. If i don't play any games i can get 5 to 6 hours sot (i don't know what can i do for 5 hours without playing games). It's not important in how many minutes 1% drain occurs. Sometimes 2% drain in 2 minutes sometimes no drain in 10 minutes. If you minimize your idle drain for your usage like get rid of unnecessary apps and greenify the apps have unnecessary notifications you can have 4 hours sot with browsing, twitter, xda, messaging and playing not heavy games.
I use latest nightlies of pa and franco all the time.

I get 6 hours of SOT when strictly web browsing. From what I have seen, this is about par for being stock. I don't use any battery saving apps.

Browsing the net seems to be what drains the battery the fastest.
I do deliveries and use GPS HEAVILY. So 6-7 hours generally if I've been working. It's hard to tell with me because I'm generally charging when possible, but probably 12 on a normal day.
Way better than the Galaxy Nexus, that's all I care about.

10 hours and it's usually around 10-15%. Rarely browse the web and rarely use GPS/NAV. Mostly IM, Twitter, email and a few other apps to a lesser extent...and phone. The screen is on a good bit but I have good charging practices. Had the thing since right after Christmas.
Either others "heavy usage" isn't heavy at all or I'm calling BS on 16-28 hours. Battery life is worse than my last Blackberry and that's saying something.

battery life is what you make of it.... for me, I can go 24 hours easily as long as I keep my on-screen time under 3 hours...

I have 40h with 3,5h on-screen time, but without WiFi (only mobile data) on 2G GSM Kyivstar (Ukraine)

I get 10-14 hrs of use depending on what I am doing, I stream music (either Google Music or TuneIn) all day and I use JD "aggressive" am running Eclipse on mine.
Only Netflix ...max brightness... max volume.. I thought it may give you a better idea... plugged in at around 25%

My nexus 4 runs with ~3,5 hours screen on time through the day. Heavily listen to music, chatting, browsing news and internet but without google now and gps off.

I need google now and gps, i guess thats what sucks the most battery.

google now disabled , nfc disabled , location disabled , brightness set to about 30 % and using hells doctor kernel on default settings i get about 4 hours of SOT which involves playing san andreas , listening to music , reading news and reviews on chrome browser some sms texts here and there, so if i just listen to music and use chrome i can squeeze about 5 and half hours of SOT i.e using wifi , and my nexus 4 is more than year old now so its still going strong as before i think oh yeah even if i used google now it had little to no impact on battery life for me
Everything turned on, full power screen, about 3 hours SOT. I'm using Hells Doctor kernel and an untouched phone drains less than 1% power hour.

GPS sucks a lot of batterie. Do you use location service in batterie save mode aka high accuracy?

Unless you disable it, GPS will kick on from time to time if you have apps running in the background, like weather widgets.

When I bought the nexus 4 I would get around 3-4 hours of sot(screen on time). Its by this number people judge their battery life.
However I use paranoid android with their stock kernel/ Franco kernel which boosted my screen on time to 5-6 hours.
If you want a better battery life, my opinion is to flash a new Rom.
Another great app that did wonders for my battery was greenify. If you could get the donation package combined with xposed installer, your phones battery life would be extended by a large amount.
