Just thought i'd post an inquiry after having it drive me nuts now. I'll try to describe it but the subject should be descriptive enough!
Basically what happens, you go to dial a number (or dial from contacts) and the phone calls out then hangs up (hangup tone) then quickly dials your intended number. When you look at the call log you see 2 calls, one being 0min 0sec then the next one right after with the amount of time you spent on said call.
I saw this in another thread and decided to make a thread about the topic instead of clouding up that one:

I have directdial feature on my SIM which allows me to dial international numbers directly. My AT&T MVNO includes international calls - H2O Wireless but you normally needs a access number, their new SIMs got direct dialling now and I think that's what's causing the double dial thing.
When i dial an international number it sends me to the access number then my phone pops up a toast that says "Sending" which then sends the actual number i'm calling. It then connects after some information about the call. I think it does this for all calls on the N4 but then drops the first call to the access number and dials the number straight to the local number instead.
What's weird however is i don't have that issue with my SGS2 and the NEW SIM cards so it's something with the N4.

Seems to be going on there too with the double dialling issue but obviously with a different provider.

You're absolutely right, I had noticed the same issue but never really thought of checking the call log. After reading your post I checked my call log and every single outgoing call had been logged twice. The first had a zero second duration and the other had the actual call duration.

Theres a couple of use having this issue on the Solavei network! i think it might be a bug with 4.2 and mvno's. we tried reporting this to our carrier but they have no idea what can be causing this! I have replaced sim card, phone, and deleted the google voice app and none have solve this issue! i think we have to contact google and let them know about this issue.
I've had a similar issue. I'll type in the number hit send, then after i put the phone to my ear I hear it dial the number and I get nothing until I end the call. In the call log, it shows 0min 0sec but it's not duplicated. Strange thing is that it only happens when I dial a toll free 877 number. It's never happened on local or long distance call or on 866 numbers. Haven't tried an 800 number yet.
about the double dial... i noticed a different issue few days ago...
i dont get a double dial for call log... but what happens is
after i finish a call... the phone automatically dials the second last person from the call log...and disconnects the call automatically...
first i got scared of whats happening and but call never went through... it got disconnected after a while
I have the exact same problem as shotta35. I'm in Canada, so on a different network most likely (rogers).
I've tried the factory reset, didn't fix anything. Tried switching to EDGE mode, doesn't do anything. enabled/disabled bluetooth (in case it was some kind of bluetooth dialing thing). Incoming calls don't seem affected in any way.
However, this hasn't happened every time to me. there were 3 or 4 calls where it did work the first time, just after a reboot typically.
Anyways, still looking into it and hoping someone can find the solution because this is annoying!
Any updates on this? Quick question for the ones with the problem. Is your Sim cut or micro? Is it rooted?

My Sim is not cut, and yes I am rooted on stock ROM!
Ok I'm on solavei with a cut Sim card. Rooted using mmuzy ROM. So the problem could be root. I asked about the Sim because last week I was on straight talk with micro Sim and had no problems. What service are you on?

I doubt root would be the cause of this but I guess anything is possible. I had the same carrier with an older SIM (manually cut) when I first got my N4 then upgraded to the microSIM. With the original SIM I didn't have the double dial thing but I rooted around the same time I switched SIMs so I don't remember when it started.
I have this too. If you use Google voice, even as VM only as I do, it double dials on outbound calls. Only seems to happen with solavei if you have voice installed on N4. Doesn't happen with any other phones with solavei AFAIK.
I just don't care I'm so pleased w solavei otherwise. I tried porting my phone # to Google voice previously, and I hated how text messages would not be left alone with the stock app. Then I tried T-Mobile prepaid and I found out it doesn't let you use Google voice as VM. So given that I have Google voice voicemail and SMS working properly finally at least, this bothers me very little.
I found that if you return a call directly from Google voice voicemail it only dials once..

I have Solavei and have the exact same issue.
double dialing fixed
I had the same issue, and I finally figured it out. I`m on Rogers Canada, so I switched my sim with my wife`s (on Rogers as well) and my Nexus 4 had to restart. When booted up the problem was gone. Next day I went to a Rogers store and bought a new sim, activated it and I`ve been using it for 24 hrs now and the problem is completely gone.
Another issue that got fixed was using the 2 second pause or hard wait while dialing. I guess when the phone was double dialing it dropped the pause/wait in the second dial. but now it works perfectly.
By the way new sim is LTE enabled, and I have LTE enabled on the phone as well and it's working fine.

I am also on Rogers.... I had a new uncut micro-SIM, the LTE one from Rogers. I may try to get a new SIM to see if it fixes the problem.
I don't know if I have direct dial or not... how can I tell?

I'm in one of those areas AT&T got when Alltel got sold. I've got an Aria with 2.1 on it, and other than the fact I'm already running out of internal storage (no app2sd) I really dig it.
I like the Sense UI, I've used z4root and a freezer app to quiet the bloatware on it, and a battery lasts me at least a day with moderate use as a wifi+gps device.
I'm looking at either installing CM7 or Gene's Liberated 2.2.2 ROM, but I've got one question:
Currently AT&T hasn't actually extended service to my area even though they shipped the phone... I get "Simcard is not MM provisioned" (or similar) messages, and no service. Since I'm on campus most of the time with Wifi, I've been using the Aria as a wifi+gps device only. In late April or early May my current phone will supposedly stop getting service, and the Aria will automagically start working. Jokes about the competence of AT&T aside, what are the chances of this actually happening if I install CM or Liberated?
They communicate with your sim card not your phone.when att finishes the merge they'll authorise your sim card on the network.if you switch to cm7 make sure you write down your voicemail # first.its not in the rom like the liberated series.

Voicemail number is visible from Settings > Call settings > Call forwarding, just copy that number to voicemail number.
the answer is absolutely yes. They just activated mine. (just in case anyone stumbles across this).
It did take them nearly an hour from shutoff to activation, but oh well.
Yeah I live in south Dakota and was part of the transition too. Of course I tore into my phone before it was active by rooting and installing CM7.
When they finally activated it I didn't have any problems.
If you are tired of waiting for the transition to take place and would like to get your phone activated earlier then send me a message and I will let you know what I did to get mine working. Mine wasn't suppose to be active until the 19th of this month.

Yeah, that works too. It redirects to the actual voicemail number when you call yourself. I think the downside of putting your own number as your voicemail number is that when you dial voicemail your call log will say your own name instead of "Voicemail". Not a big deal though.
I have a very strange issue on ATT, no one else seems to have:
first, I got a new micro SIM and I'm on the correct 4g plan (I generated an IMEI for an ATRIX 4g to give to ATT).
Problem:
About 2 to 10 minutes into a call the person the other end looses the ability to hear me. but I can hear them. so I hang up. at this point ANY calls in or outbound are silent in both directions. Bluetooth, Speakerphone or headset, same results. and at this point wifi and cellular signal bars are now gray. Reboot and the phone works again.
Does anyone have any ideas????? I may need to send the phone back. Do you think ATT has something provisioned wrong???
update: I did a factory restore, I still have the problem. I have a bad feeling it's something to do with ATT / Nexus 4 / fake IMEI ??? and this happens using either popular Nexus4 ATT APNs.
OH and another update... today when this happened I noticed my Ford's Sync system flashed "In Privacy..." the rest was cut off... then silence!!!! WTF ?!?!??!?!?!? In Privacy Mode?!?!?!

Moved from an iPhone to the N4 on AT&T about a month ago. Until today I did not realize that the voicemail feature does not work (might be a blessing in disguise ). I do not use any app for visual voicemail though that's how the iPhone worked. I tried dialing 1 for voicemail and AT&T prompts me to enter my voicemail number, I enter my mobile phone (10 digits) and it says "invalid mailbox number" or something to that effect. I suspect it's because it used to work differently on the iPhone. Also if I call my cell from a different phone it just rings and rings, it never goes to voicemail, so I guess I have no voicemail service.
Anyone know how to get it to work or should I just get the AT&T visual voicemail app from the play store and hope that works? I know, last resort is to call AT&T, which I try to avoid if possible.

You need to call att and tell them you are no longer using an iPhone. Voicemail is set up in a special way specifically for the iPhone, at least on att it is. I had to do the same thing a couple years ago when I moved to the first galaxy s. It's stupid.

Ah, thanks. People in the community know more than AT&T of course When I called them to get the data going on my N4, they first put me on some Blackberry plan that stopped working after a day. It took another call to get provisioned for the "phone" APN. You'd think that they should have noticed I no longer have the iPhone and they would have fixed my voicemail too...

You should just install the Google Voice app and follow the setup instructions, it's much better than visual voice mail

The at&t VVM app won't work on the N4 as it's not one of the phones that the app supports. Just get Google voice and let that be your VVM. It's much better than the crappy at&t app (I used it on my vivid...)

Thanks everyone for your help. I called AT&T and got the VM going, old school long press "1". Good enough for now. Not a big VM user, took me almost a month to notice the voicemail was not working on my N4

I'm on Tmobile's $30/mo prepaid plan. I want to use google voice to ring my nexus as I've been using a GV number for the last year or so. So in other words, I don't want to use my carrier number, I want my GV number to ring the phone. I had succeeded in doing this with my GS3 and Galaxy Nexus before without any issues. When I call my GV number from another phone, it won't ring my nexus, and eventually goes to GV voicemail.
What makes it stranger is that I was able to get it to ring once or twice by calling my GV #, but trying the call again would result in the Nexus not ringing anymore.
I have tried doing the **004*XXXXXXXXXXX# codes a dozen times getting a message like "Call forwarding Connection problem or invalid MMI code."
I've tried deleting the phone from my computer on the GV page and adding it back again. No success there.
I'm short on ideas. Any help would be much appreciated.

I'm not interested in using VOIP. I want to use the cellular network because the call quality is better than any VOIP solution. I just want my GV number to be in front of my tmobile number.
If I understand correctly you want people to call your GV number, and GV to forward/ call your device on the device's number?
If so, all you should have to do is log into GV on the computer, put your device's number into the settings, verify you have the phone, and it should be set up.
I have it set up that way with T-Mobile prepaid, and never had any issues. If you are doing those things and it is still not working, please elaborate more.

I'm having a similar issue, I have a Nexus 4, I'm on the T-Mobile $30 plan, I go to verify my T-Mobile # on GV and after I hit "Connect" it says 'Verifying your phone...' for 30 seconds or so, then says "We could not verify your phone. Please try again.".
I've tried calling from at least a dozen computers, nothing, my phone never rings, no matter how much signal I have or what, it's INFURIATING.
I just got setup on this same tmo plan, and I'm having similar issues with GV. I managed to get GV to forward incoming calls to my tmo phone number, but If I call out, it still uses my tmo number regardless of whether I have GV set to make all calls. The GV voice mail also is screwy. If someone calls my phone and I let it ring and don't pick up, it will go to GV, but if I tell the phone to ignore the call, it goes to tmo voice mail. This 30$ plan is just what I've been looking for but honestly its really aggravating that it all worked flawless on Straightalk with ATT sim. I don't want to have to use GrooveIP or some other app to call constantly through GV, when it worked properly on ATT's network. Tmo, definitely has to work to do in this area.
Here's how I got it to work.
Went to google voice and put in my work deskphone number.
Verified it, then boom, I have a verified number.
Used that to set up a google voice number that's 3 numbers off from my old one (Same area code, first three different, last 4 the same) and paid $4.99 for GrooVe IP, logged into my Google Voice account and now I can make calls to and from my google voice number. Be sure to download an app called Contact Cleanup or something like that, I think GrooVe IP needs you to have your phone numbers in international format to dial. Also, my only real complaint about this is occasionally someone will call my old number because my new one wont ring, but signging out and back into GrooVe IP seems to fix this.
Every few days I try to verify my cell number with no luck, I was reading and the best I can come up with is it's either a fault, or something on purpose, on T-Mobile's end.
And FYI my phone shows up with my GV number on caller ID, and text messages, and all that jazz, I sent out a few dozen text messages using GV online to everyone in my contacts list know to only use my old number for emergencies.
I haven't using a single minute on my T-Mobile plan since I did this like 2 weeks ago, I love it.
Two things to keep in mind.
1) "Forward calls to" phone numbers can only be used in one GV account at a time. Otherwise you could create 100 Gmail/GV accounts and forward them to one line. I had this issue trying to add my home phone to both my GV and my spouse's GV account. Many free SIP DID's have this as a known issue. Google can remove the number, but it's a painful process.
2) The Nexus 4 has a ARP issue on WiFi. Ping your phone LAN IP after you lock the screen to see what I mean. This has caused some issues with some VOIP apps.

Are you using the same gmail on your phone as the account you originally created the GV number on?
I was having this problem and I transferred the # to my new gmail, problem solved.

I know this has been touched on before but I have not seen anything recent about it and I just got this phone today and I have no incoming SMS. I have everything that uses data working fine (email, etc), and outgoing SMS, and incoming and outgoing calls are fine, but I am not getting ANY INCOMING SMS.
Used the SIM from the Google Pixel 2 XL that I had before this phone, went to the Verizon store, they put the OP 8 on my account with the correct IMEI from SIM slot 1 and it was able to pull my number over and everything else was working including 4g, but incoming SMS was/is not working.
Does anybody know why this is still going on, and how to fix it. Do I need to get a new SIM from a VZW corporate store? Do I need to tell them anything special like CDMA-less provisioning, and if so, do I tell the store employee that, or do I have to call tech support?
Also just so you know I also tried another sim from another phone number on my account and it also does not receive incoming SMS, everything else works fine on that sim and number also.
Thanks guys, really don't want to have to take this phone back!
I had the exact same problem with my one+ pro on Verizon as well. I ended up going back to a corporate store and they just gave me a new sim card with a 5g logo on it and all seems to be well again. I can finally receive text messages again.
You need them to put CDMA-LESS on your account. It basically makes all your SMS and not just voice, etc to go across LTE instead of defaulting back to CDMA for incoming txt messages. Once you do that, BOOM, it'll work. I've had more success chatting with support than going to the stores to get it done. If the person you're talking to doesn't understand CDMA less, disconnect and try again.
Hey everybody, just wanted to let you know that I got it figured out by calling support and getting someone who knew what CDMAless was and they provisioned that and it works now. Although what is weird is that it only works with googles messaging app, not the default app.

I spent at least an hour on the phone with two levels of Verizon support trying to resolve the problem of not receiving texts. Verizon supports has to add the "CDMA-less" tag to your account and device. After this is done, the phone and texts work flawlessly.

it works for the default messaging app as of yesterday
I just went through all of this with Verizon. Because of the pandemic getting through to customer support is quite difficult. The only way I can get through was with Facebook messenger, and the link that they sent so I could verify my account was broken. Finally they called me and it took a whole 10 seconds to fix this. Too bad it took 2 days to get ahold of somebody so I can get it fixed..........

I just tried this, and the person claimed they had added it, but it definitely did NOT work, at least not while I had the 4G SIM installed. However, my OnePlus 8 5G UW had come with a new Verizon 5G SIM in the box, so we had them activate that new 5G SIM, at which point yes, everything started working. So Verizon really wants you to switch to a 5G SIM.

Big red must have changed tactics. I had this problem last September I think it was, it took a couple of phone calls to Verizon but they added CDMA less to my account and then they said they had to send some data to my phone or reactivated or something like that, but anyway I have not had this problem since then.
