So I've rooted my phone twice following the instructions on theunlockr.com without any problems. Tonight I tried rooting my dads phone and I got temp root using visionary and then was able to get S OFF but now once I rebooted and tried temp rooting it again I can't get it to work. Most of the apps just force close as well. The only thing I can think of is to factory restore by flashing the PD15IMG and then starting completely over. Anyone know why this is happening and what to do?

Try ./gfree. Never fails. This is happening because your dad probably has the bad emmc chip

Restore to stock... Warranty exchange... Then down grade to 2.2.1 then gfree method... Use the youtube video....

So gfree will work even if he has the bad emmc?
And also, if it has the bad emmc will I be able to flash the PD15IMG without any problems?

People tend to assume stuff without even thinking.
What happened to you, happened because of data corruption. Doesn't say anything about "good" or "bad" eMMC, definitely doesn't suggest that your phone is dead and needs replacement. But this thread suggests that reading, searching and thinking will get you better results than listening to people.
You already got S-OFF.
Most probably the only thing you need to do now is to get into bootloader, factory reset the phone, boot into the OS fresh (which will almost surely eliminate all the FCs you're seeing), install Visionary again, temp-root, and install custom recovery.
If you rooted with root.sh method (means, you have engineering bootloader installed) - you don't need even that. Get your fastboot operational, boot into bootloader, go to "fastboot" menu, connect the phone, download any CWM recovery image from the net, and "fastboot flash recovery your_downloaded_recovery_image.img".
And you might get it even simpler than both of the methods above:
Look in the Development forum for instructions on gaining root when already S-OFF. It's as simple as flashing PD15IMG with only a custom recovery image inside it. Since you're S-OFF, the image will be flashed by the bootloader without questions, and you'll get a custom recovery. The PD15IMG will be found in the right thread.
From there just boot into recovery and install a custom ROM of your choice.
Good luck.
Search before asking next time.

http://forum.xda-developers.com/showthread.php?t=944681
this happened to me as well. i went to ice glacier and then followed the instructions on the link
You cannot use 3.0.0.5 for stock. You must flash an older recovery or 3.0.0.6 and then it will work. This is covered extensively and has been asked over and over again.
Flash the correct recovery for the rom you want to use and your problem will be solved.

i have loaded other roms...i cant get a single one to make it past the splash screen...i cant flash another recovery from fastboot...i am baffled, you would think if i have fastboot...hboot...and clockwork it owuld be an EASY fix...anyone point me a lil further in the right direction, i dont mean to be dense i been up for a LONG time trying to figure this out, now im at work...slow day...and if i can fix this before i get home my wife will be SO HAPPY..so which version of ice glacier i just tried 1.1

If the computer recognizes your device in pretty sure you can set up adb. Thanks flash recovery through there, you should have been able to flash Glacier, you must be doing something wrong, try flashing and stock zip through recover maybe?

i'm not exactly sure of the commands but i know if you've got adb set up you should be able to do it. maybe adb shell?? i have no idea as i'm not a developer.
i'm confused why you couldn't get royal glacier flashed, it shouldn't matter which recovery you have with 2.2 roms. sounds like your phone just doesn't want to comply...
forgive me... as this is much a partial question as it is a suggestion... (coming from WinMo to Android myself) but can't Clockwork Mod be updated FROM Clockworkmod recovery??
I seem to recall that update.zip is the install for the clockworkmod install... and there's an option within clockwork to install update.zip... isn't that correct?
If so... wouldn't you just download the .zip for 3.0.0.6, copy it to the microSD and install it, then install the ROM of choice?

I followed multiple steps for installing perm root on a friends Mytouch and for some reason we could not get the s-off feature to work. I noticed too we do have SU access via adb or terminal but I still never could get s-off. After we got root we used rom manager to install clockwork and after the fact it installed 3.0.2 i believe it completely borked the recovery. I stepped it down to 3.0.0 and still had the same issue. The main reason i wanted to get s-off was to adb flash the recovery but I got a signature fail. Any ideas or suggestions to get this working correctly? Rom Manager will not let me install 2x cwm basically stating it can not install it period. I also tried the batch file root method same issue. Thanks for your help in advance.
Please look at the "True permaroot and S-OFF" thread in Development section, and execute step-by-step.
If your friend's MT4G is running Gingerbread (Android 2.3.3), you'll need to use Gingerbreak instead of Visionary+ to get root access.

If you are following the ./gfree method step by step it will root your phone.
If you are using the root.sh script and it's not working chances are you may have a bad eMMC chipset and it will not root properly.
I know HTC said they would be pushing a update to close the exploit hole that is used to root our phones but i don't think it's been pushed yet.

HTC isn't "becoming a Motorola". HTC, is completely different. We do have a locked bootloader. But it's easily unlock-able whereas Moto's aren't.

I don't know what to tell you. There are several threads out there regarding how to root your phone. Both in a .gfree form and a root.sh form. One of those 2 work for 99.9% of EVERYONE.
You might try finding someone who put together a .zip that can be flashed through the android recovery(white screen with boot options, etc.). Otherwise you can get the RUU and return to stock. After that follow the directions step by step and DO NOT mix methods. This should result in an S-OFF root, if you did your homework correctly.

As someone already explained before, when the security is on, no matter the amount of flashing you're going to try - it won't get you anywhere, since it won't be written to the actual ROM, and will be gone on restart.
Unless you pick a guide, follow it exactly, and post here everything that doesn't look proper - I don't think the forum can help you much.
Here's one:
http://forum.xda-developers.com/showthread.php?t=858996
Follow it and post anything that doesn't look good. Including your bootloader version.
The root is sticking on reboots but still no soff that's what i have been saying and i followed the guides to a t its not my first picnic rooting or roming a phone this one just has me stumped

What does your bootloader show when you boot into it?
For the root to stick after boot, you had to have S-OFF, otherwise the eMMC wouldn't have been written to.
Having no info from you, as I said, it's impossible to help. If all this thread is just a complaint - by all means, but in that case there's no point in helping you. If you want help - you need to provide enough details for people to help you. Obviously, nobody can figure out anything from "I can't get s-off".
I do a adb reboot bootloader and i get us fast boot up top it says s-on but if i reboot to the os i still have root access for instance in titanium backup or Rom manager of that helps any

I've had an Evo for about 9 months....but....
Work basically just gave me an Atrix and laptop dock to mess with, and even encouraged the "development" side of it (aka root it, without saying root it)
I've done a bit of research, and I have a few questions.
Right away I used the "One Click Root" at the top of this forum.
What I want to know is how to enter Clockword Mod Recovory, which I can on the Evo to backup my phone and flash a rom stored on the SD card.
It doesn't seem to have it unlocked that menu though, as I can't access it by holding those buttons down.
That would make my life alot better, as I can start to experiment for the Roms for this phone (currently running Mik2.3? on the Evo, didn't see a ROM for this phone though )
Anyway, thanks for helping out....I'm excited to learn this phone.
Also, the dock is rather new, any cool tricks I could exploit?
(Have you unlocked the bootloader..?)
Flash romracer's recovery, check in the development forum. Or download Rom Manager and install the recovery from there.

He shouldn't have to do that. If it's rooted and has an unlocked bootloader he should just be able to download either rom manager from the market and install the recovery that way or he could flash romracer's recovery via fastboot. That's all I really did when I went through this and it worked just fine for me.
On another note, to the thread poster, definitely read the "stuck" threads at the top of each forum and you'll be able to figure everything out without having to ask questions that are already answered. I went through it all and have gotten to the point where I can do a lot of this stuff on my own so I highly recommend doing so.

rooted and unlocked bootloader doesn't matter, if he's on the stock ROM and has that file it runs on boot every time. you can install recovery, but it just won't stick since install-recovery.sh runs at boot every time the file is present.
Need tons of help! I've been searching the forums for almost 24 hours now trying everything and still can't get it fixed. I rooted and got s-off on my wife's MyTouch 4g. I installed ROM Manager and flashed the latest CWMR 5.0.2.0. I did a full Nandroid backup. Then I tried flashing a Gingerbread ROM over the Froyo rom. It went to white MyTouch screen and never left it. I finally gave up and went back to recovery. When I try to restore using Nandroid, I get this:
Error while formatting /system!
E:Can't open /cache/recovery/log
E:Can't open /cache/recover/last_log
I can get into HBOOT, Fastboot and Recovery just fine.
I tried the RUU exe floating around and it always fails trying to reboot bootloader.
I tried letting it autoupdate the stock PD15IMG.zip floating around it always fails.
I've tried flashing different recoveries including older CWMRs and always get error about remote not enabled.
I'm at my wit's end. I'm leaving town tomorrow and need to at least get back to the nandroid backup before I leave. Any advice?

Hey Guys,
So im in a bit of a peculiar situation here, I downgraded my device from Gingerbread to 2.2.1 following the wiki (kudos to whoever did that, thanks a lot!). However, when I got to 2.2.1, it now only mounts my SD card as read only? I have ordered another one to see whether i just got a crappy sd card (Tmo refused to replace it under warranty)... However, I have pushed Visionary and Gfree, in preparation to root this sucker, and have my way with it...but heres the caveat, is that now, when I try to boot into recovery, I get that stinking little phone with a red triangle on it...what does this mean?
Regards
Trevor
That means you don't have a custom recovery installed
Download Rom manager from the market and install the clockworkmod recovery
Edit: until you root the phone my post is useless the custom recovery is for after you root the phone
