I downloaded Ricardo's repair.zip file and took the liberty of mirroring it on xda-developers, since my download went at a whopping 200 bytes per second.
We've seen this recipe before, and it does appear to fix at least some of the problems caused by RSU upgrades not finishing.

Just try to do following: start 3.23 ROM update to extract RSUpgrade.sa.CAB and place it to SD card instead of file found in repair.zip.
The situation you describe looks like your device died while flashing new 3.23 version. So you may try to continue process with 3.23 version instead of old one. This is because 3.23 don't allow to "downgrade".

If your device died while trying to upgrade to 3.23 radio, the CAB-file should be found in "C:\Program Files\Radio Stack Upgrade Utility".
My device died while trying to upgrade to radio 3.19 but I trying to copy to SD all Radio upgrade, but don't work. Still stop on 1% and error message is "Flash failed. Press OK to restart device".

i have the same pb the alnasser and sebi; i have tried everything; what we need is a new rom version for the radio and not an upgrade; also while using boot loader and if i try to copy the GSM rom from a working phone i got a message saying: connect to RS232 and run PC monitor; i have USB and i do not know what is PC monitor...any ideas???

The soloution above worked for me after a little trial and error.
I was trying to upgrade to new UK Radio Stack but flash failed in the middle of upgrade (probable because the keyboard popped up onto the screen for no apparant reason, then locked up).
However, after much wailing and gnashing of teeth, I found your website!!
I installed the Files onto an SD card as described above, and although the XDA was going painfully slowly (about 3 mins to warm reset). managed to get to the today screen. I ended up pusing the "record button" repeatedly to try and get some response out of the machine, finally it auto ran the program on the SD card, only to be told that flash failed
Undeterred I repeated the exerscise with the other RSUpgrade.sa.cab
file, this gave the same result!!
However I soon realised that this cab file is kept within the .exe file of the original upgrade I tried to do, so... Using winzip I extracted the relevent RSUpgrade file for my XDA and put that file on the SD card instead.
It Worked!!!
Consequently I conclude that if the file fails or stops at 1% then it must be because it is NOT the correct file for your XDA, you need to use the correct upgrade cab file for you particular XDA.
Hope it helps someone else.

hi i have a dead xda and i'm trying to fix it but it stops on 85% sometimes on 83%,i'm using the copy of cab file to sd card and runnig it from xda anyone can tel me how to get over this?

i have removed it from charger and put the file on the sd card,the progress bar went to 99% then flash failed,now it gives me that message on 1%,how to fix it??!!

Hi,
seems to be the same case as I had: I found version, which did not freeze in 1%, but in 85% all stopped.
I unfortunatelly don't exactly remember complete list of my steps, but THIS MAY BE IMPORTANT:
After I made cold restart, I saw radio upgrade progress bar ( If you can't see this, try to follow instruction about repair.zip solution, mentioned hereinbefore ). Radio upgrade progress window only flashed for a second or two, then disappeared and WIN CE ( trying to boot up ) painted its splash screen. After that WinCE froze with this screen.
BUT:
If you have a luck, radio upgrade may continue on background. After 10-15 mins upgrade was done, and after restart ... I saw normal radio version in bootloader, and it worked well !!!
Note 1:
I killed phone with latest radio upgrade ( maybe lack of space, maybe radio switched on ), so there was no way to use lower versions ( it leads to "1% problem" with downgrade )
Note 2:
When my steps was still unsuccessfull, I continued [up|down]grade WIN CE and Bootloader. Maybe it was solution, maybe not, I dont know. I did it because I thought it would solve some problems with space for radio installation. My second reason was, that it should stop neverending ( 1% freezing) radio upgrade window after each boot.
Note 3:
During these installation steps it seems to me that battery si getting weaker, and does not recharge. So I tried to revive WIN CE first to be able to recharge.
Some steps may be unimportant, based on my feeling how it may work.
Anyway,

Very strange thing happened to my QTEK 2020:
I had installed the Widcomm Bluetooth Stack the 'Bluetooth.cab' floating around and I was not pleased with it nor did I truly need it and tried uninstalling and NO GOOD. Hard Reset and I in turn started from scratch with reinstalls. Here is my problem:
Upon the Hard reset there is no longer a camera.exe nor album.exe in the \windows directory - and the button of course is not assigned to the Camera because it IS NOT THERE!
This is a HARD reset peeps! It is inherent in the ROM 1.60.00WWE, no? How is this possible? Did this villainous bluetooth.cab do this? I have tried again hard resetting to see if there was anything and EVERY time there is no Camera nor Album installed!
I have installed the Camera/Album 2.5 which came from HTC inherent in the 1.72 ROM and it works great until I soft reset and then it locks the device so there is not a solution there.
Very strange indeed, any comments or remedies?

i've had the same problem with at times a hardreset would not load my extented rom programs another hardreset or opening the extented rom manualy and running the cab files solves it
Looks like part of the extROM was not installed.
Check the config.txt if its still complete. If not: repair it.
Check is the cabs are still in the extrom. If not: didnt you forgot to lock the extrom when you hard-reset the device? You have to get new cabs then.
I think the old versions are lying around somewhere.
OR: Upgrade to 1.72.00! Good occasion now.

Thanks for the help... I didnt even know all of the possibilities of unlocking the ROM and seeing what is in there!!! This little machine is COOL!!
I suppose backing up the External ROM is 100% important now that I know it is there!!!

I installed the ExtROM unlocker and re-installed the camera 1.5 cab that was there, rebooted and tried out the camera and it is giving me the same problems as the 2.5 version of the camera did... I will leave it alone for the time being, I didnt really use the camera that much any way

what problem ?
I dont know what happened, but I replaced the older version of the Camera CAB with the 2.5 and changed the config file and then locked the ExtRom then hard reset and the camera works fine as it should...
Before ithe device was locking up - just a black screen when the camera was called... not any more.

Device: Qtek 9000 - straight from Dangaard with no operator personalisation.
Started out with the USB connection problem. Solved that according to descriptions in previous threads.
Flashing stopped due to it not being an O2 device.
Extracted files from the Xda_Exec_Upgrade_v113188.exe.
Tried to download /Uploads/Universal/MaUpgradeUt_noID.exe, but exactly that file could not be downloaded (I can download the other files - ????). Found another file called MaUpgradeUt_noID.exe linked from this forum. It is from 16/3-2005 and illustrates an S100.
Flashing seems to go OK. PC and Qtek says 100% complete after the 3 stages.
Hard reset gives the option of 0 for Factory reset or X for reset
Hard reset results (either 0 or X) in dark screen saying "Serial", "v1.00" (Bootloader?). I have now flashed 4 times with the same results :x
Can the cause of my problems be the "MaUpgradeUt_noID.exe" from March? If so, can someone give a working link for the right version of the file - please :wink:
If not - please help - I am in deep troubles

You have to upgrade the Radio ROM separately. Look for posts in here or Universal Upgrading for "unbricking your Universal" or something to that effect.
I have been looking and searching, but I’m unable to sort out of the abundance of information what should be done in my situation.
The installation information is blank for all the 3 groups displayed under “From version”, so I guess the device is totally blank – under these circumstances I can’t see why the radio Radio ROM should be updated separately.

I found out only a few days after I installed the 1.64.08.21 radio rom that my camera was not working anymore :
black screen, hyper slow, "Unable to capture photo. Unable to get frame data from camera device.", cannot switch from rear to front camera, forced to kill the process to exit.
I saw a few threads related to Kayser saying that camera problems were linked to radio rom.
I tried many combinations to revert :
1.64.08.21
1.58.25.14
1.58.21.23
1.70 OliNex
nothing changed : camera still not working (although it worked well with Olinex and 1.58 before I switched to 1.64)
I even tried different main rom sets and SPLs to come back to the original config : HardSPL 2.20, SPL Original, WM6.1 bepe 67, WM6.1 Aserg, SFR 1.25.406.1, SFR original... camera still not working.
The only positive thing is the Nadavi_HTC_Camera_5_0_4_2915_00_NEW_HTC_FIX.cab released on Kayser that makes the camera application usable again (not slow anymore, but screen is completely scrambled, multicolor and showing front camera image when using the rear camera, and switching between rear and front cameras shows that the front camera only works, keeps saying "unable to capture photo" when using the main rear camera (and sometimes with a strange sound like some diaphragm trying to autofocus).
The phone did not receive any shock, so I am a bit skeptical about a hardware issue suddenly rising, but I am concerned by the fact that reverting to any rom does not make the camera working anymore after 1.64.08.21.
My latest working config was
WM6.1 Bepe 0.67 Standard
1.70 OliNex
Please share and if someone found a way to make the camera work again (if this issue is software only and can be fixed...) that would be great !

I flashed all roms including the original one (raramomo_ROM_SFR_Original) and still not working.
(one strange thing, I thought the splash boot screen was part of the original rom, but it stays even after flashing the original rom)
would it be possible that software broke the hardware ?
Personally I think your hardware had issues before you flashed this Radio. I am running the same radio and have been for awhile now without issues.
Camera and Viddeo works fine....
I had the same problem - I flashed my Polaris and after it "Unable to initialize camera" I tried use other ROM without good result. I had to flashed to original WWE ROM and sent to servis - of course by warranty ;o) Next week I will have my Polaris after service

mmh... camera has worked flawlessly since the beginning and after many flashes, until I flashed that specific radio rom (once again no physical shock so no reason that it does not work suddenly)

that is my concern : how possible a soft can break the hardware ? (and apparently not for everybody)
I hope a soft solution exists (I want to avoid sending the phone to service and waiting for a new one, but that seems to be the only issue)

Solution is unbelievably simple. Take out the microSD card. Take out the battery and leave it outside the phone for at least 15 minutes (found this hint in other forum). Then put the battery back into the phone and start the phone without microSD card. Et voila camera is working. Don't ask me how it is possible.
//some comments says that it didn't help them either (don't know if they took out even the microSD card)
UPDATED: After few days the same problem "unable to initialize..." nothing helps (battery, hard reset, anything)

no luck : did not work for me, even with SD out, even with a hard reset after 15 minutes off with battery and SD out... camera still sluggish and black.
still need to intall the Nadavi fix for Kaiser to make the camera tool reactive and be able to switch to front camera but the rear one is still not available...
Try to find the file "Camera.exe" in windows directory, and run it from there.
If the camera is working, than just create a new link to camera in programs folder.

yes, the camera application is working, but not possible to capture photo/get frame data from camera device.

you know what ? thanks to your idea, I completely deleted all camera.xxx files in /windows, and decided to install the Nadavi_OLD_HTC_Camera_5_0_4_2915_00.cab camera tool (found in the Kaiser section) :
I received a message telling me that it could not properly remove the old camera version, proposed to install whatever, which I did, and then restart.
And tada !! After weeks of flashing differents Roms or radios (which proved to be useless) it is working back again !
This is a completely crazy software bug not at all related to the hardware !
Thanks to all and especially you Shayd for sharing your ideas, that finally helped me !

I have installed the Nadavi_HTC_Camera_5_0_4_2915_00_NEW_HTC_FIX.cab and now I cannot launch the camera.exe.
Any idea to solve it without hard reseting my Polaris?

(For info my Cruise was in Repair center last week : I could't hear my caller (maybe a solder issue) so they change the motherboard, reflash the phone and now the camera is working again).

I had exactly the same problem with HTC Tytn. Unfortunately I never solved the problem (camera never worked well again: at times yes at times no).
Now I am with a new Polaris and I am scared of flashing anything into it.
I got my htc cruise about ten days ago. Camera was working fine.
But four days ago, i got the message the camera could not be initialized. I read on this forum that it sometimes helped to remove the battery for about 15 minutes. So that's what i did. And guess what, it worked for about 1 day
Then i kept getting the message the camera couldn't be initialized. Removing the battery didn't help anymore.
Then i hoped a hard-reset would do the thrick. Windows reinstalled and everything went fine. But now the camera icon is missing from "Programs". I tried to start it by going to the photo album and pressing the camera icon. Nothing happened. Then i use file explorer to go to the exe file. I found the camera.exe file, i tapped it but nothing happens.
Anybody have a solution?
Return it asap and get a new one

I got this problem 5 days ago too... Very strange! Tried most of the procedures said here in the forum and none worked. The weirdest thing is that the camera just stopped working from one day to another, i clearly remember of not installing anything during those days.
I am very pissed about this issue! My problem is bigger: I bought it at UK while travelling and currently I live in Brazil (no support center around here).
Do you guys really think that its a hardware problem? How can that be if some other people already said here in the forum that a few days or weeks later, the camera went back on?
Anyone else can help us with this issue?

Thats quite strange! My Storage Card is 40% free...
My problem is bigger now... After 2 weeks of "could not initialize" issue, my camera simply doesnt start anymore... I tried to lauch the application directly from the camera.exe but it doesn´t work either...
Don´t know what else to do...

did you upgrade your radio rom to 1.64.08.21 ?
if your answer is "yes" you should downgrade to Murata radio rom (1.58.21.23) ..
don't forget to backup your data, because Murata Radio does hardreset after downgrade..

I tried to uninstall them (the original ones) or delete them in the File Explorer, even in safe mode, and windows mobile wouldnt let me.
Kenhas, looks like Im in the same point as you (exe is not working either). Did you happen to make yours work?

have you tried to delete the /DCIM directory from your storage card and soft reset ?
if your exe is broken you probably need to hard reset.
I formatted my storage card (8 gb) and soft reset... no deal
My exe is broken now. Do you really think the hard reset now is going to fix it? Before I had the "unable to initialize device", now I don´t have anything
I installed both Microsoft Portrait and Coolcamera to try the camera, they get quite the same error.. something like "failed to get camera"...

I don't know if those tools share the same camera dlls, but if that is the case, a hard reset might fix things, otherwise, it means it is a hardware failure, and you will need to send your phone back to service.
FYI, when I faced camera issues, I installed the NADAVI WITH FIX cab for Kaiser , and that version only made the camera tool work again (although for me the rear camera was not working due to the storage card issue)
Maybe try installing that one first before hard resetting
