Nexus 4 TWRP encryption with password - I think the phone is gone

So, I have read a lot about TWRP sometimes bugging out and randomly encrypting phones and adding a password to which the user either never set it up or that TWRP doesn't accept it. This has recently happened to me and I cannot use my phone and I don't know what to do.
I have had the bootloader of my phone unlocked since day 1 when I bought my phone (release date last year) and have been running PA ever since. I have never had any problems with it, until about 5 days ago, when I grabbed my phone and everything started crashing and the camera was apparently not available. I didn't know what was the deal with it, so I rebooted the phone and it entered a bootloop (it hangs on the Google logo for about 5 minutes and then it hangs on the PA logo screen forever). I went on to the bootloader and started TWRP to see what was the deal. I originally wanted to grab certain files off the phone in case I had to do a system format, but when I got to TWRP, it asked me for a password which I have never set up and I tried every combination I could think of but it always failed. Everything was encrypted and without the password I couldn't get to it. I tried wiping everything including a factory reset before resorting to a system format (which I read about it and apparently is meant to be the solution). Everything failed because it couldn't access the /cache, /data, nothing. I went on to do a system format and although that is meant to wipe the phone and remove encryptions, it also failed to wipe the /cache and although TWRP says that the wipe is successful, the phone is still encrypted, the password is still around and it bootloops. I have tried the system format like 50 times and always the same.
Can anyone help me out with this? I am currently travelling around but am at a hostel where there is great internet and pcs, so I can install drivers, toolkits and such. I actually started looking around for new phones, since I cannot for the life of me get the phone to start again.
Note: The phone was working perfectly fine, I put it down to have dinner and once I finished, I tried to use it and it started to bug out. I also read around about this, and everyone's issue gets fixed after a system format.
Any help, would be greatly appreciated, that phone is the only net device I have on this trip and it sucks to not have any way to contact anyone.

Don't blame TWRP for something it doesn't do.
Looks like your device have an EMMC failure, sorry.
You can try installing factory image, or last resort restoring by download mode, but I think the flash memory in your phone is gone.

I didn't mean to blame TWRP. It is just that it was when I am on it that I get asked for the password and I was under the impression that it was TWRP that did the encrypting (I am in no way a developer or even very knowledgeable when it comes to the under the hood stuff, I usually only follow the tutorials). But thanks for the reply and the link, I will try that and hopefully get the phone back working.
Also, I did read in a thread that apparently this can occur due to hardware failure, I was just hoping this wasn't the case.

This happened to me on an old phone. Not sure why. Factory image fixed it. Could be a different issue but its worth a shot.

Did it suddenly happen or did you change ROMs?

I tried a new rom for the first time and then went back to wicked 9.1, I'm thinking that triggered something.

Galaxy S4 stops working

Hey guys, i've got a Galaxy s4 running 4.4.2 android version and all of a sudden my device started freezing and stopped working, I don't even know how to describe the problem, i haven't installed new apps or downloaded anything since a month and this problem just started yesterday.
Basically, my phone just stops working all of a sudden, the "X app isn't responding. Do you want to close it?" with whatever app I have on the moment, and after that, the screen just freezes, I can lock my phone and unlock it, but nothing happens, just the same screen frozen, and I have to reset my phone, but the problem is that I have that problem happen to me like 5 minutes after I restart my phone, so basically, my phone is unusable because of this problem, and I don't know what to do to fix this. Any suggestion guys?

I would definitely try and back up any data you need from the phone. Then, I would perform a factory reset from recovery to see it that solves the issue.

I would if I could, but actually I don't think I have enough time to backup all my data before the device freezes, but in the end, if there is no other possible solution, I will just factory reset it even without backup

Try pressing the menu button when powering up your phone. that way you will get into safe mode. Maybe then you can make backups.
or
You can flash a custom recovery. Like TWRP and then boot into the recovery. There you have Aroma filemanager and then you can transfer the files to an external sd card.

Phone restarts at startup.

Well this is my 1st post so first of all hey everyone
I just spent a week in Amsterdam and my phone restarted twice, different times different days and I dont remember what I was doing with it back then.
But as I landed back home I put the phone back from "Airplane Mode" and went on to toggle "Cellular Data" on.
The second I pressed the cellular data shortcut on my the phone restarted itself but this time whenever it would finish the screen would be on but fully black and sometimes it'll show battery precentage at the top but then will restart again in an infinite loop.
Sometimes it'll pop up the "Optimizing Apps" window and will go through all 24, upon finishing it will restart once more and do the same.
I tried using the bootloader and it worked (the bootloader that is) but the phone would do the same thing even after Rebooting from there or shutting down from there and then turning back on.
I contacted tech support on chat but they couldnt help past telling me to go to bootloader and reboot, said I should factory wipe when it failed.
Also tried wiping cache but to no avail.
I have no backup of my phone (wanted to do one before the flight but got lazy and I suck for not doing one sooner) since probably October and google doesnt backup my contacts properly for some unknown reason.
I already downloaded Android Studio with everything included for me to try using adb to sideload stock without wiping but its my first time and I couldnt get it to do anything at all...
I already got the official stock .gz file from the motorola website.
Anyway here are the specs to my phone:
XT1575
Everything is stock. didn't root or change recovery once in my life.
Was fully updated and had plenty of drive space (I have almost 128GB with the 64GB version + 64GB SD)
I'm really lost here so help will be very appreciated.

Start with a full wipe and if that doesn't work flash the stock image. Not sure what your question is though as you never really say. Once fixed I would unlock and make a backup so you don't have to deal with this again.

The thing I pretty much want to know is if theres a way to either backup the phone now with it connected to the computer while in bootloader or recovery. or a way to flash the stock rom without wiping which I read someone did with his tablet. I cant post source normally because I'm a new user so here it is but change spaces to dots: schnouki net/posts/2014/08/13/flashing-a-stock-android-image-without-wiping-user-data/
Also while in recovery I noticed 2 options under wipe: 1. userdata only. 2. userdata+(insert something I dont remember here) and I was wondering what gets deleted in both? if I delete only userdata what data will remain? something helpful?

Sure you can try flashing without wiping data but being locked I personally wouldn't risk it. No idea about backing up in your state... somewhere I remember a thread on backing up without root try searching in q&a.

Out of curiosity why shouldnt I try flashing with no wipe if ill lose the data anyway? worst case wont I just have to wipe after it fails and flash again?

Well I tried turning the phone on and it took a couple of minutes before loading to a fresh start. It apparently wiped itself on its own. Managed to restore almost everything.
Anyhow the phone takes too long to boot now. Was even stuck at boot and I had to restart it.
Anyway to fix my software? Or at least check for corruption?

Normal (rooted) phone, went full retard all of a sudden. Need help extracting data

Hello all,
I have a Nexus 6P, stock, rooted, with Xposed installed. The phone is completely functional for the better part of its life time since I bought it at release -probably first or second batch- (except for random reboots once daily on average, deemed by my investigation and exhausting all options to be a hardware issue or Xposed related).
Today, out of nowhere, the phone randomly rebooted itself and surprisingly it launched the "Android is starting..." dialogue which, if I'm not mistaken, optimize apps for ART. The battery was at 19% before it randomly rebooted, so when it started that optimize apps it went down to 0 (I assume) and it turned off. The charger wasn't connected correctly so that happened. Now the phone keeps restarting itself up to before it gives me the boot up decryption phase of the Data partition and reboots again. I assumed it can't decrypt Data for some reason, so I launched TWRP recovery to try and decrypt data and take a backup copy of my Titanium Backup folder on storage... Unfortunately, TWRP reports "failed to decrypt data" even though I'm 100% positive that the pattern is correct because I'm literally inputting it almost 10-20 times a day for the past two years...
Note: there seems to be a red blinking LED in the top left corner of the the phone. I never even knew this existed.
WHAT I WILL DO
I'll reflash everything system-wise.. I'm ok with that. I'll reflash the factory images, root it, install Xposed, no problem.
WHAT I NEED HELP WITH
I need to extract some folders from my phone storage, namely the Titanium Backup folder. I know the pattern protection of the encryption.
How do I go about doing this?
UPDATE:
Help is urgently needed now indeed: The phone doesn't even turn on to bootloader mode anymore now. It was able to, just half an hour ago. Now all I see it the red blinking LED.

I was thinking, if I boot into bootloader mode, can I use adb to pull any folder? If so, will that work in my case with decryption fails or Data partition doesn't seem to mount correctly?

Bootloader: UNLOCKED
I did NOT upgrade to Android 7.0 nor was I planning to, until Xposed is available.
I did NOT flash anything **new** recently. However, Installed an Xposed module yesterday which caused a bootloop. I rebooted into TWRP, uninstalled Xposed, rebooted into system (which looped once, out of the ordinary) and I uninstalled that module, back to TWRP installed Xposed, and rebooted back into system. It worked for 12 hours and then what I wrote this post for happened as stated above.
UPDATE3:
15 minutes into charging... NOT EVEN A RED BLINKING LED ANYMORE.
Last? UPDATE:
Almost 1 and a half hour into charging and there is no longer ANYTHING. Is it hard-bricked now?
How the **** is this possible? I'm very much capable of solving any sof-brick but not a hard-brick...

Thats a really crappy scenario. Im a little busy right now (not that I can get you out of this regardless) but I sort of skimmed through your post. Are you able to connect adb? If so you might be able to use "adb pull" to get that data.
On my 6p TB is located at:
/storage/emulated/0/TitaniumBackup/
Itll probably be the same location on your phone. Good Luck

I was indeed able to connect to a PC after I saw the problem (the bootloop essentially)... But I didn't at the time as I didn't have a plan of action as to how to get my data, so I turned it off. After sometime, I couldn't do anything with the phone... Not able to boot into bootloader, not recovery, the phone is essentially not turning on to anything, not even the splash screen; like a hard-brick. I'm not sure if this indeed is a hard brick as I've never seen any.. and don't currently know what to try next.

Is the LED still showing? if so, do the bootloader boot combo, and see if fastboot devices gets a response in a command prompt - or terminal if you're a linux guy.
if so, you can boot to twrp, decrypt data - try default_password -, and browse the phone's files from your computer's file browser, the phone will show up as Nexus 6P. That's my suggestion, wish you luck.

I honestly declared it hard-bricked.
Will Fastboot show any devices if the phone didn't show bootloader screen?
I'll try again today.

S7/Verizon - Updated to Nougat, now no Mobile Network/Data

Just updated my Galaxy S7 to Nougat this morning, now I can't get a mobile data connection or send/receive text messages.
I've tried restarting, removing the SIM, turning data on/off, and airplane mode. Any help would be greatly appreciated.

Have you gone into recovery and wiped cache?

Worse comes to worse, factory reset if clearing cache doesn't do it.
As long as you set everything to back up to the cloud, you should be good. But make sure you have done this before you do a FDR. I usually copy stuff down to my PC just in case though before doing an FDR.
I've had no issues with the Nougat update. I'm really pleased with it. Phone is just as smooth and snappy as it was under MM. Could be the euphoria of having Nougat, but it seems as if it's even smoother than before.

Trying to do this, just getting a bluescreen with a yellow triangle and dead android icon. Says "No Command" or "Error"

From that screen press PWR and Volume up

If you can still boot into the desktop, I'd copy down whatever you can to your PC or some external drive and then do a factory data reset. That might be your only option now if you're getting the Dead Andy.
BTW, you weren't rooted or had any other customizations done to the phone's image? Usually, OTAs won't work with modified/rooted phones, but I've seen some cases where the OTA goes ahead and applies itself and then the phone is rendered useless until they either flash back to a stock image (when they've rooted or customized) or factory reset if they didn't customize it.
In this post (https://forum.xda-developers.com/showpost.php?p=71321950&postcount=15), the user I believe had disabled some stock apps that were required for the update and that caused some issues. He used the Verizon software repair tool and it apparently either re-enables those apps, or if you removed some, it puts them back on. Once he did that, he was able to accept the update and it worked for him. So, if you had disabled some apps, that might be part of your issue, especially if it's an app that is required by the stock system.

Trying to avoid having to do this, but if I have to I will. Was able to get past the dead andy screen though.

What build were you on previously?

Try downloading the Samsung repair assistant and go into download mode. This will wipe your device so you might want to back it up using smartswitch.
