Since updating to Android lollipop 5.0 then to 5.0.2, I always get a message saying that Sim Card got removed even though I didn't so there is no 3G connection and also there is no signal.

first of all there is not a stock 5.0.2 for nexus 4.
so assuming you are on a custom rom that is probably your problem. either change rom or go to your rom thread and ask there for help
I am on on stock 5.0.1. I wrote 5.0.2 by mistake

test another sim. if it is rejected i cannot help you. if not it might be something wrong with your sim.
What if the problem is in the software? Since I didn't face this type of issue when my phone was on KitKat

go back to kit kat and see if the problem exist. you need to try things to see what is the problem. also you could try to reinstall lollipop
This happened to me last week, no sim card showing even after several reboots and even a backup of another rom installed. Nothing helped until I was able to remove the sim card (not always something lying around I can use to eject it). I pulled it out, checked it, blew on it and reinstalled it and rebooted and all was fine for another week. That was on 5.0.2 xenonHD rom.
The other day I flashed 5.1 factory image. Today the same thing happened. Removing the sim card fixed it for ten minutes this time.
I'm starting to think it's a hardware problem.

I suggest try a factory Reset first before trying to flash Modem
Same here. As soon as I flash SuperSU, VoLTE and Wi-Fi calling stop working.
Flashing stock ROM gets it working again.

VoLTE doesn't work for a lot of the roms. Which ROM are you on?
IIRC, RR, Omni, and AICP should all work on VoLTE.

I have been crack flashing all of the rom. I thought it was rom related, I even reflash the modem including Turbo's modem zip he has in his rom section. I just couldn't figure how to fix this issue. I went as far as formatting the phone

It seems like only TURBO'S ROM are able use volt for my calls. Every other rom I am unable to get volt on calls. Not sure if I didn't anything wrong when I formatted the phone.
