I'm in over my head and hoping the kindness of a stranger can bail me out! It all started when I attempted to root my wife's Razr M from Verizon. I used a utility (the DROID_RAZR_Utility_Jellybean_XT912_WinMacLin.zip) to attempt an update and then a root. Well the update failed and now I see this when the phone starts up.
AP Fastboot Flash Mode (S)
Device is LOCKED. Status Code: 0
Battery OK
Transfer Mode:
USB Connected
Fastboot Reason: Flash Failure
usb connected
So I figured out that I can get to BP Tools and this somehow bypasses the problem and boots the phone normally. So as long as my wife doesn't turn the phone off, she'll be fine but I cannot leave it this way. I am open to suggestions and at your mercy. Thanks in advance!

What caused the update to fail? Also, you're lucky btw - Normally a failed update == a brick.
If you didn't root, you can probably take it to Verizon. If you were flashing the OTA and it went bad, that shouldn't be a problem at all...

I watched the script run and it kept saying "fail" on several line items and I knew I was in trouble. Unfortunately this was self-induced and I have no desire to go to Verizon for help. I'm not that much of a noob. Anyhow, I'd just like to get back to square one and start over. What I did to get to BP Tools is to hold down volume up, volume down, and power button. Screen shuts off..let those buttons up and press them again which brings me to a second menu. From there I can choose recovery, fastboot, BP Tools, factory reset. The only one that helped was BP Tools which basically sent the phone to a regular boot and it acts normal. If I choose recovery, I get the android dude on his back with a explanantion or question mark, don't recall. If I press volume down, volume up and then release volume up, I can get to the menus where it allows you to run install from disk or something like that. I can't get to the point where I can see my phone on the computer so that I could place a flash file on the phone and re-flash it or something. If any of this sounds remotely familiar to anyone out there, please feel free to add your 2 cents. Thanks!
Try to wipe all user data after you get to the screen where you can install zips in recovery. See if that works. Pm me if you get stuck and we can possibly fix it through adb

I did try that..sorry should have mentioned it. No dice. Same screen and I got the pleasure of setting the wife's phone up again.
I'm open to suggestions. adb?

The M is an XT907...

This looks really straightforward. I think I can handle it. Are you certain it will work? Just plug in the phone and run the bat file? Right now the phone works so I guess will give it a shot and hopefully I'm on my way. Thanks everyone for the assist.

First I tried to root it with the help of geohot (for razr M), but my phone stuck on the reboot, what I saw on the screen was:

You need to flash the stock firmware using RSD or House of Moto. This should be posted over at the HD forums, but the process is essentially the same for any Moto device. There's tons of threads about it. Download the software, download the firmware, connect the phone and flash.

that hasn't been updated in a long time, likely wouldn't work with the bootloader on your device

there is no error on the screen
that is normal fastboot screen for the xt907
device has the latest bootloader only 183.46.15 firmware will work

I tried to flash a 0.54 modem.img , and incidently used the command "fastboot flash modem modem.img" instead of "fastboot flash radio modem.img"
the device is still in fastboot mode, is there a way to fix what i did ? please help I'm afraid to restart and not be able to start fastboot mode again!
Did that command actually work, or did it give you an error saying unrecognized command?
AFAIK there is no fastboot flash modem command. Just reflash the radio using the right command.

Pretty sure there is no modem partition on the Nexus 4 so it's probably done nothing...next time, don't call the file modem.img!

just to be extra sure , if i do 'fastboot erase modem' and reflash using correct command, nothing bad will happen right?
If you want to be sure, get the 422 Google ROM and run the run.bat.. it should flash every partition with the correct image.. Wugs tool kit can do this too

He's right, I prefer to input the commands manually so I know what the hell I'm doing, and not rely on someone else's script, you can never know what they put in these

Weird... In the screenshot it actually looks like it did something. Is it possible that you can flash to non existent partitions?
Cause I don't think there's a modem partition.
Someone should try fastboot flash hello recovery.img and see what happens

Well if you think you can handle the very complicated command run.bat, knock your self out.
I mentioned wugs kit just as a method of convinience, because it downloads the files for you and checks the md5 sums before running the very complicated run.bat command for you.
For example, if you used it to flash the radio in the first place, you wouldn't be in the predicament you are in now.
I used a restore to stock tool and now my cellular radio doesn't work. I'm pretty decent at trouble shooting. I have tried every thing I know to fix it and it is still broken. I have tried rsd lite to restore it fails when it flashes modem (IMG to large). I have tried flashing stock firmware line for line in fast Boot to no avail. I have tried several custom ROMs I am out of answers so I'm turning it over to you XDA.

I have tried that also when I do it will not boot up. Everything works but cellular data when I try to make a call it says turn airplane mode off and they deffinetly aren't on and in the back ground where the numbers are it says radio off it will not let me turn them on. I even tried in adb with " adb shell put settings global airplane_mode_on 0" it still says turn airplane mode off to make a call.

And it won't let me turn the radios on in*#*#info#*#* everything is there like normal when I try to turn them on it does nothing
Reflash the stock radios from the correct image for your device, or reflash the complete stock image via fastboot.
AIO or "one-click" tools are bad news on Moto devices... Avoid them and do it manually, my best guess is something didn't restore properly.

Ok update i flashed original firmware from xda and the one provided by motorola still nothing. But i have pin pointed sv fuse read failure anyone have any ideas?
Nobody knows? Come on xda I know someone has the knowledge i would be willing to fork up some cash for help to fix it
Well I sent my phone in and within a week I had a replacement within a week. Moto left a 32 and came back 64 gigs.
Unfortunate you couldn't get the help you needed here....

Xda has helped me a lot this I don't think was fixable. I would still recommend someone to seek advice here.
Had the same thing happen to me after flashing back to stock, Baseband Unknown, IMEI Unknown. Found solution in a different Motorola device thread that worked for me.
Flash the modem twice.
fastboot flash modem NON-HLOS.bin
fastboot erase modemst1
fastboot erase modemst2
fastboot flash modem NON-HLOS.bin
fastboot flash fsg fsg.bin
fastboot erase cache
fastboot reboot
Happened to me a few times. I Just flashed the modem zip floating around around here via twrp. Great score on the 64gb replacement. :highfive:

Here we go. This is for T818VVRU1AQB1 firmware, but probably works on any revision 1 firmware.
Instructions
1. Enable OEM UNLOCK
In Settings>About>Software info tap the build number 7 times to enable developer options. Once you've enabled developer options, developer options will appear in settings just above the About device option. Open developer options, the 4th or 5th option should be “OEM Unlock”. Turn on.
2. Download the eng boot image.
This is a special boot image normally used by engineers, it allows selinux to be booted in permissive mode with relaxed root restrictions.
Do not replace this image with the stock image after rooting your device or you will possibly soft brick your device.

3. Install boot image:
Download Odin v3.11.1
Reboot Phone in Download Mode (press and hold Home + Power + Volume Down buttons)
Connect phone and wait until you get a blue sign in Odin
Add the SM-T818V_eng_boot.tar to AP / PDA
Make sure re-partition is NOT ticked
Click the start button, sit back and wait few minutes
Reboot
Check in Settings>About>Software, your device should be in Selinux permissive mode.
4. Install and run Kingroot:
Make sure you have an internet connection.
You may need to run Kingroot several times to gain successful root.
5. Install and run SuperSU me to remove Kingroot and convert to SuperSU (not required) :

6. Wifi fix:
modify the line below in /system/build.prop:
ro.securestorage.support=true to false

The boot image flashed OK with odin? Which splash screen?
This could mean you need to wipe the data partition first for it boot.
I should be able to get the stock firmware for you sometime tomorrow.

Flashed fine. It's the screen that says Android very big and had the tab model, right before it goes into the boot animation.
I'll wipe data partition and report back.
Please don't feel responsible for it, I'm about trying anything for rooting... I'm sure we'll get fixed

Sure, we'll get it fixed no worries.
Anyway it seems like you need the accompanying botloader if its getting stuck on the bootloader animation.

Are you saying that the full firmware didn't flash either? At what point does it fail?
Anyway don't worry I'll upload the stock AQA3 firmware later.
EDIT: I think this is all you'll need to get booting again. Flash via AP in ODIN.

It remove the splash screen. Possible that your screen will be blank foe several seconds. You may try.

Nope. I'm happy with stock Oreo with locked bootloader.
I haven't tried that, but since I unlocked the bootloader, I don't get that " Bootloader is locked" screen. Nor do I get unlocked text on splash with Mi logo

I used the command you said in the forum "fastboot erase splash" in my Xiaomi Mi A2. He had unlocked the bootloader and wanted to get that warning message. I ended up taking the fastboot mode. Would you have the command back to normal? It would help me a lot

The splash image stays in /system. So if you erase or remove it, it will temper the system partition which means OTA update wont work anymore.

CosmicDan has already made a tool that can replace the current splash image with your preferred image. When I had Mi A1, I made my own splash image by the help of tool. It used to show my name instead of that crapy warning message.
