Just before starting, I searched all over the place a solution to my issue but none worked so far (changing USB cable, use WugFresh steps to reinstall drivers on Windows, activate MTP/PTP, USB debugging, etc....)
None the steps described in this thread work for me.
My Device :
- Nexus 4 (bootlader unlocked)
- At some point I used WugFresh root toolkit to root my phone, but then reverted to Stock
- Now it is using STOCK 5.1.1
Also :
- I always used Samsung micro USB cables (5V 1.0A instead of 5V 1.2A for the standard Nexus 4 charger) to connect/charge my phone and it always worked great. (2 at home and 1 at work)
- I have flashed several devices (P1000, I9100, Galaxy Nexus...) using those cables and never had issues...
My story :
- My Nexus 4 was shutting down randomly in the past weeks, sometimes it was a pain to start, but I always managed to restart it (holding power 30s, release, then hit power again)
- One day I found myself with the blinking red light of death (one time it was blinking red and green as well), and honestly I don't really know how I got myself out of it and phone is charging well now....
Where I am now :
- After all that, I figured, well, it's time to reflash the device completely, maybe with CM that time...
- But, since then, it has been impossible to connect my phone through USB both on Windows and Linux (Debian)
- I am also hit with that issues where randomly when my phone restarts it optimizes apps just like it does when restarting after a major update

- I booted in recovery, wiped everything (cache + data), but still have the same issue.
- On Windows, I get : "Unknow USB device" (even after reinstalling drivers using WugFresh instructions)
- On Linux, I get this :

When I plug my Nexus 5 (hehe always have a spair device) it is recognizes straight away on Linux using lsusb. (same cable)
So at this point, I am thinking that this is a hardware issue. (maybe battery is draining to much power which causes USB connection to fail ?)
I teared down this morning my Nexus 4, removed battery and checked USB port (a bit darker on both top corners), reassembled the Nexus 4.
But still, the USB connection issue remains....
I was so pissed that I ordered both a new battery and a new USB flex before writing this, so that I'm hoping to find the culprit... provided it's hardware related....
The weirdest thing is that in the past my Samsung I9100 ended up the same way... (I changed the USB Flex but didn't work... after that I gave up)

P.S. : I will never buy again a phone with no microSD card support... drives me crazy to be stuck like this.

Just wanted to confirm that the culprit was the USB flex !!!
I tried the new battery first, didn't work, then changed the USB flex, crossed my fingers... and right after boot got USB debugging/MTP working again !
So if you ran into that same issue, first check the cable, if you pretty sure it's not the cable, then it might be the USB flex...
Attaching a picture of my - bad - USB flex (the connector is a bit dark on the top, also noticed moisture inside...) for reference in case it can help...

Just starting yesterday, when I plug my phone into my computer all it does is charge. No option to mount, no "usb not recognized", nothing. I can get into download mode but Odin doesn't recognize it.
I've searched dozens of other threads, tried most suggestions and can't seem to get anything to work. Suggestions would be appreciated.
Maybe you don't have drivers or bad USB I have 2 one only charges the other does both

I have the drivers, I've had no problems connecting previously. I've rebooted the phone multiple times.
Out of 3 galaxy s type devices: 1 doesn't even charge anymore, 1 charges but wont do data and 1 works if the cable is held at an angle. I think samsung can't make usb ports
I'm just now about to start a post asking how to change roms with out my usb. I had the same problem start months ago. First things where normal. Then, every now and then I would have to re plug the usb cable into the port to get it to mount. Then finally one time it just all together stopped working. I can charge, the charging icon comes up but no mounting options.
On the bright side, this is the first phone I have ever owned that I can just sync over wifi. And at least its not an apple device, if charging stops I can get an external battery charger ; )

My vibe is over a year old and I never close the door on the usb port. When I had the same issue as the OP it was a dirty usb port. I carefully cleaned it, which took a few times and it finally worked. Do it with the battery pulled. The loose port is caused from the male end of the original cable not the phone so I bought a new cable and it fits snug.
Seems a bit odd but my Nexus 7 (1st Gen) will not charge when connected to a USB wall outlet. But it has no issues charging when connected to a desktop/laptop.
The following I have tried:
- I have rebooted, shutdown, booted into the boot loader and shutdown/rebooted from it into the OS
- I have tried several USB wall power adapter range in amps output from 0.7 to 2.0.
----Also tried 4 different USB cables with the USB wall power adapter.
-Enabled/disabled developer mode along with USB debugging mode
-Tried charging the unit when it is OFF and ON, only charges when connected to a USB port on a desktop/laptop
-Removed the back cover to see if the USB port could be loose, looks and feels securely in place
Not sure if it could be a software issue, unit is running 4.3 build number JWR66Y. I am hoping to avoid resetting the unit but if I have to I will.
Thoughts anyone?
Is your device rooted? If yes, then try to change the ROM (make a nandroid first).

I would say if your rooted and cutomed and all that try a different kernel before a whole new rom. Kangaroo kernel first page of dev page is good.
Mine gets stuck sometimes. I installed battery monitor widget to see the actual current flow when charging. Even though it shows ac connected the graph can remain flat with no charge. Replugging the cord a few times gets it going. A charge only USB cord might help, but I've gotten the best amperage with the included cable so far.
I have a serious issue with my Nexus 7 2012. I put CM11 on it and all was great. The power got down to 11% on me, so I plugged it into my computer. I noticed it wasn’t charging, MTP didn’t kick in, and I couldn't get into ADB. I figured I’d screwed up the USB port. I tried different cables all with the same results. I turned the tablet off and plugged it in and the battery started charging. I couldn’t get MTP or PTP to work nor could I get into ADB after it was charged. I did a factory reset, clearing everything, and reinstalled. Same issue.
I can use my OTG cable and TWRP to install software on the tablet. I tried AKOP ROM. It allows me to use my OTG cable but the tablet doesn’t charge. I try to toggle MTP to PTP or turn off USB debugging and the tablet locks and reboots. I try to turn off the developer options toggle and the tablet locks and restarts.
Help! I don’t know what to do with it. I've searched all day.
Check the basics. When you plug your N7 into your computer, are you using a USB 2.0 port? Is it directly mounted on the computer's motherboard (i.e., a rear port)? Are the proper drivers installed so your N7 is recognized as a Nexus 7?
USB specifications only provide .5A power, v. 2.0A power from the AC charger. Get your N7 fully charged,then get back with whether it locks and reboots.

Thanks for the reply. Yes. I've done all of that. Multiple computers, multiple cables, multiple chargers. The Nexus is at 100%.

Well. I'm going to say disregard. My searches show that a craptastic 12 volt car charger probably hosed the N7's mother board. I'm going to try one more fix but I'm sure it's toast.
I have a really weird problem running Windows 8.1 whilst using my Galaxy S 5.
The phone, when plugged into any USB port keeps disconnecting and reconnecting like 2 times a second...
Things I've tried:
1. Different USB cables including USB 2 and USB 3.
2. Different ports.
3. Disabling 'turn off device to save power' in all USB hubs/devices through Device Manager.
4. Using 'High Performance' power mode and ensuring all 'turn off USB devices to save power' options are set to 'No'.
5. Reinstalling the Kies 3 drivers and software.

Yep, happens with mine too, and happens with my nephews S3 too
Also disabled power saving for USB etc
Seems to help if you switch between USB debugging and back again a couple times while it's connected
Is there anything like a proper fix for this? Does the problem stop on Lollipop?

I put the S5 on my trusty 8A 5-port Anker, and immediately noticed this. If you leave the S5 alone, it will settle to constant charging at max amperage, according to Battery Monitor Widget's log.
Nevertheless, I haven't seen such before, not even with my Note 10.1 2014, which uses the same charge mode.

I've been fighting the exact same problem.
While I _believe_ there should be a software solution for this, since it wasn't always a problem, I believe it to be power related and have gone for a hardware solution.
The definitive cure for me (as of this morning) was to purchase an external *powered* USB hub.
(FWIW, an Ativa Mobil IT "Travel" 7-port USB 2.0 hub from Office Depot. And only because that was the only model where it could be tested before purchase without destroying the packaging).

The switched on phone does not see the connected charger, does not see the PC. (PC does not see my Phone too. No New devices)
If the phone is switched off and connected to the PC, then turn on my Phone, the PC and the phone see each other, until the cable is removed.
If the phone is switched off and connected to the charger, the phone is charging. If turn it on(connected to charge) Phone is charging, but only slow charge (no qc)

Very strange... it should not just stop working with usb port suddenly like that. You might have to try ordering a new USB board from Ali Express to actually make sure it's not the board. Some chip on it may have bricked that would be harder to ping.
mine does this too, only way to get it to charge is to power off, plug phone in, and then power back on. Tried clearing cashe/delvik and even wiped everything clean and did a fresh install of newest version of bootloader/twrp/rom
I am also experiencing this recently. about two weeks already not having quick charging and PC connection nor OTG function not working.
If I have time, I'm gonna open up this phone too.
