[Q] Google account action required - Nexus 4 Q&A, Help & Troubleshooting

I've been getting a notification for months now that says "Google account action required." I've tried clicking it, and every time I do my phone reboots. I've installed several factory rom updates since, and every time I do I always wipe my phone, religiously. I contacted Google and they said "boot to safe mode and wipe the cache"...well herp derp, guess what a factory restore does?
Google basically threw their arms in the air and told me they have no idea, then passed me along to LG. LG told me they want to RMA the device...but as you can probably guess, I'm 100% against having no phone for a month while they install a factory rom, exactly like I've done several times.
I've done a little research, and I've never found anything that helps. Most people are running devices from other OEMs and they say unlinking an account from the OEMs app works, and it only happened after updating to 4.4.x. Well guess what? We don't have that sort of app, and it's been happening since 4.2.x, and has happened to me immediately after reinstalling a factory image before I restored any data at all, and after signing in to my Google account. So it's narrowed down to the core android firmware or my account. Since I have thousands of apps, gmail, G+, photos, contacts, etc, etc, etc, wiping my account is NOT an option.
Anyone have any intelligent suggestions, unlike those I got from LG and Google? I'm so tired of my phone rebooting for absolutely no reason!!

xposed (but it happened before xposed even was a thing, I'm just listing it for completeness)
NO mods, additions, add-ons, etc. I'm running a 100% pure factory image, with the exception of xposed...and that runs on top.
Just for the fun of it I just did a nandroid backup, did a FULL wipe (including userdata partition) and reflashed a fresh factory 4.4.2 image, signed into google, rebooted, and 10 minutes later the notification came back. When I clicked it my phone rebooted.

I had this a few times when I had a wifi connection but no actual connection to the Internet. (orange connection status) when my network became a little more stable it went away I think.

Honestly no idea, normally that notification is from an account issue. Either the password changed or account frozen due to incorrect logins. I take it you can log in fine on a PC? Would be interesting to see what happens if you sign in on another mobile device. Pretty much impossible that this is caused by a hardware fault and if it persists through flash or even a factory reset it indicates it's to do with the account. I'd try disabling backup and restore for your phone, maybe the restore process is bringing back your error.

I've started scanning all my installed apps, but I have a bunch, so I haven't finished. so far nothing.

Do you use Google my tracks? If so open it. And accept the Google account you want associated. That's the only time I've seen it happen.

This just started happening on my phone, PA 4.6 beta rom. Signed then about 30 mins later asking to sign in again.

rebooted itself at home screen

I have researched this and could not find anything that applies. A few minutes ago after moving through several home screens but not selecting anything and no apps selected, I set the device down and lo and behold it had apparently rebooted without having been turned off and without the usual tmobile etc. happening on screen and was in the final stages of reboot with the Galaxy logo. This had to have happened in a matter of 1 or 2 seconds or less. I am stock but have rooted with the one click root. I have removed some bloat ware but nothing else has been done and only removed what was considered safe on other threads such as the bootup sound and telenav, amazon mp3 etc. This was all done about a week ago with no apparent issues and the device was operating in its usual smooth fashion. Prior to this I have had no issues except for the GPS. Is this something I should be concerned about and does anyone know what could have caused this? Any and all comments will be appreciated.
Delete all personal data, cache, after that it should turn on completely, but market may not work, you may need to download google apps, the same thing happen to me when I open google maps, the whole thing frooze on me

Can't seem to wipe this Spring Galaxy Tab

I've been given a Galaxy Tab by my office to play with for a while for some various development projects we have coming up and to evaluate its use. Initially another employee had it, but he quickly lost interest.
Problem is, his Gmail account has been tied in as the primary account for the system, so I can't use my own Market apps, etc. No biggy, I'll just wipe it, right? First I tried a data wipe from recovery, but despite claiming to be successful after a reboot it's still got his account in there. So then I did Privacy/Factory data reset, but that mostly just reboots into Recovery where I do the same thing again and still fail to wipe his data off the device. I then pulled the memory card and formatted it and also reset with it out, but no improvement. I finally downloaded the stock image and flashed it with ODIN and STILL his account remains.
So... any thoughts on what I could be missing here? I just want this thing back to pure stock so I can put my own account in and work with it as though it were new.

Agreed. I thought I was the only one.. It just doesn't make sense.. I flashed mine using heimdall and still the same issue. The only problem is that mine force closes when you do anything.. So if I can't restore the software on this it's absolutely no good to me.
Yeah, same here. Nearly none of the apps functions now after the reset. This is absurd.
I'm in the same boat. It's very frustrating. Sprint was not able to help me. All he could do was walk me through the factor reset. I downloaded a Market app that bugged out something and now I'm getting quite a few force closes.
I just notice that if I merely restart this thing is loses all changes I made since I last restarted it, adding back any apps I uninstalled, removing accounts and WiFi settings I installed, etc. This is screwy. Wondering if there's a hardware problem with the flash memory.

Listen to this, even more crazy than that... After my last unsuccessful wipe, I decided to just uninstall all of the applications. I just did another factory restore for the heck of it. Now ALL of the applications I uninstalled are back!.. AFTER the factory restore!
I think the only sure way to fix the problem is to get a zip file of the stock firmware and load it through the recovery screen.
Edit: since it put the applications back on there, I decided to try your situation. I uninstalled all of them, turned off the Tab and then back on. All applications are back.

It seems incapable of now making permanent changes to the flash memory. This is really screwy and annoying.
I am having the exact same issues. We are all in the same boat.. I have a $30 reward if anyone can fix this..
Any updates from the others with this problem?
I received 2 more Tabs today for my sales reps. They both work fine as far as factory restore. I noticed that the bad one goes to the boot loader when I initiate the restore through the Android software. The new ones do not. They format and reboot back into the operating system. Sprint is going to exchange the bad one.

Thanks for the comparisons. Mine also goes to Recovery mode when I initiate a data wipe, so clearly it's the same situation. I'll have to see how mine was acquired and whether an exchange is an option.

Anyone else having LOTS of crashing and bugs lately?

I'm on the latest non-rooted stock. Lately, I've been having lots of serious bugginess and slowdown, crashes, all sorts of issues. I've had this tab since 3 week after it came out, so it's been a while. I've though about wiping it and starting over, but I don't know if that will fix my problems, and doing so will force me to have to reconfigure EVERYTHING, since Google still allows us no local data backup system and their supposed cloud data backup seems to only remember minor settings, and often doesn't work at that.
(seriously, you'd think they would at least offer some sort of explanation about how it works, what it backs up, or even just let you log into their services and see what's been backed up. very odd for a cloud computing company to be inferior to the iPad's backup system right now....)
I honestly don't want to root, because I keep seeing posts about glithces and serious malfunctions resulting, and honestly I don't have time to deal with it, I've rooted 3 phones and it's just a pain to unroot for updates, hope it gets rooted agains, without bugs, etc.
So, does anyone know what the cause of these problems may be, if it would be fixed by wiping and restarting, any other ways to fix it, etc.?
Making a backup now will only backup your issue
Sounds like a rogue app is causing your issues.. Chances are if you did a backup. then reset your device. If you restore a backup you will put the problem back..
Its not that much to allow google to reinstall your purchased apps back from market after a data wipe. I say go for a tablet reset.
Then consider a backup before you have a Issue.

You loose all settings, apps, but at least you come back on a fresh tablet without anything making trouble.
I got a weird problem a week ago after updating the leaked version of ICS my tablet was starting, then after unlock screen, between 1s to 1mn of playing (not game, just using the tablet like usual) the tablet was rebooting...
tried many times... to see what happenned..
so I go fast as flash and reach RESET TO FACTORY
and VOILA... problem solved.
I'm in similar boat with you, I'm on the latest stock, and have some problems. I did a hard reset, as soon as I setup my google account, google push some settings back, even the wifi settings. But nothing really got fixed. I have map crash like crazy, and I got a new problem that the gps logo in info tab at bottom kept blinking, while I had nothing running. Really got tired with all these problems. I thought I didn't install too many apps other than some basic google's apps. Somebody pointed to the Latitude for the gps logo, but I didn't have that on at all, not logged in. I found enter/exit navigation sometime will help.

So, if you already have that message after the card is removed, it's possible you have malware
It's also possible that will the latest android update, something unexpected happened with your phone

That's a little odd. That might happen if you have a hardware problem but irrelevant to the current issue
Let's try this..... can you get it to turn off, perhaps by taking out battery? Wait a minute (or a few if you want to be safer) and then immediately upon Power up, go into safe mode by hitting home + volume up + power at the same time, holding for maybe 10 seconds
That may throw it out of the loop and into actually recovering itself

We want to break the cycle, get out of a loop and begin to recover
You could alternatively do a factory reset, but you lose EVERYTHING that way, so I prefer to exhaust all possible options before recommending that
The main idea is that you have a soft brick, which is recoverable

If you place the card into a computer (presumably with an adapter, if it's microsd), you run possible risk of infection. I would run a virus scan on the card itself, but outside the phone
If you can save the pictures, that's ideal. In the future, consider google backups to the cloud or amazon prime photos
Well, I tok out the card. Then I did a factory reset.... it gave me a FEW of the annoying "this has stopped" or that has stopped, but I don't remember what, now.... It started installing stuff, but some programs it refused to. Clean Master, Pokemon Go.. it gave me error code -504. Then I forced some and it seemed like they were working ok and installing, so I figured maybe I was just DLing too much all at once. So I went back and put in my mem card in, and it froze. rebooted, and **** went to pot again.
I had Google Drive, but it kept filling up my phone's hard drive and prevented me from doing stuff, DLing more.. I had to clear all the data.

Basically, one aplication said "I want to be king of the playground" the firmware said the same thing and they shoved themselves off the playground and both broke their arms

Yeah. It does three boot screens. 1, the Galaxy Grand Prime screen. Then the Pretty Samsung one with the musical tone. Then the Tracfone screen, then it boots. Since these problems started, it will often freeze on the Tracfone one.. cause after a few seconds the screen will dim. And then it boots.. but like, before I took my husband to work.. it was still froze there. I had to pull the battery, cause it wouldn't move. Though, just now it did finally
If it is incapable of factory reset, it's the kernel. That really would have to be handled by the dealer who sold the phone (if under warranty and they feel like replacing it) or doing things that void the warranty anyway

If settings are failing to load during language selection and you can't get to the home screen, you can try takin out battery for 10 minutes and then try rebooting and trying again
If it still fails to work after 24 hours (sometimes if you leave it alone long enough it'll work) I'd go to where you bought it with proof of purchase and hope they'll help get a new one
Ok, battery removed. Is it possible it's an overheat issue? Cause when we were playing Pokemo Go Friday night, my phone froze.. and when I booted it the screen went black, and lines went through it, like it lost connection to the MB or something
I've had issue like that with like, DS lites when the ribbon cable gets crunched.. but...
It's possible the hardware overheated, but because it boots, it's not necessarily a gonner
I've taken very good care of this phone, no hard drops or water...

If there is a malfunction and it overheats from processing too much at once, that is possible

I soft bricked my phone (and recovered it) because of pokemon having an issue during a specific sequence in game play. It definitely has bugs
I warned friends about it and that other phones may not be recoverable. It really depends on many factors
I've noticed it allowed me to install it on both my tabley (really crappy NextBook from WalMart) and my husband's phone (LG Fuel) and it won't run on either.. I figured, 200 dollar phone, it should be able to handle it....

On an unofficial capacity, I think your phone may have been google account locked. This commonly happens when you forget to remove your google account before factory resetting the phone. You will know if your phone is google account locked by holding Volume up button + Power Button + Home Key, then when you get blue screen press volume up button, If the FAP lock: ON, then your account is google locked (Google locked phones tend to not allow you to go past the setup screen as you cannot click the "next" button). You can send it in to your phone dealer for them to send back to Samsung for repairs, however you seem to live in the U.S so I don't know their policies. I live in Canada and work as a part-time Samsung repair technician and our policy is to unlock the google lock and factory reset then return to phone company. However, that is not covered under warranty so we do it as a sort of "customer service" capacity.

It is FRP, not FAP, it means Factory Reset Protection.
Well, if that is the case, on the setup you will get a window telling you to choose the WI-FI you want to connect to. Do it, then try to log in with your gmail account (This "gmail" account must be the same one you used last time otherwise it will not work.

Phone automatically installing unwanted apps!

I just traded my sprint lg v20 for this verizon lg g6 vs988 yesterday because I needed a gsm unlocked phone. Now I finally got a phone my son card works in but I've been up all night trying to fix this issue.
Seemingly at random the phone just starts installing "random" apps that I've never used before. Some of them include
Go90
Wish
Yelp
Yahoo mail
And several others I can't remember at the moment but I'm sure ill be reminded soon. I keep uninstalling them and they keep coming back. By the manner which they are installed on sure they are not normal bloatware. They download silently in the background even while I have an active foreground download going.
I've googled the issue and found no resolution. I also noticed the phone charges ridiculously slowly. The phone was also on an outdated firmware with a security patch from 2017. The built in system update would not detect any update for the phone. I had manually download and install the latest update on the computer last night.
After doing this with a completely fresh factory reset these apps were still installing themselves. I then used verizons pc upgrade assistant again and did a firmware repair but skipped the phone backup step so I again would be starting fresh. Again these app's keep installing themselves.
It seems to be on some kind of interval. After so much time that the phone is running it suddenly install another random app or apps I've never heard of before, then after so long all of a sudden some more different apps suddenly appear.
I also tried malwarebytes and of course it says nothing is wrong. Can someone please help me get this sorted?
Which firmware you have?, maybe it would be the best to flash clean stock firmware with LG UP.

16a download via verizon upgrade assistant. Would reflashing the kdz via lg up do anything different than verizon upgrade assistant?

Suspicious. What I do to prevent problems like this is to disable all apps possible, including googles apps and the Play store crap. Then go in to each app within settings and deny data access to them, clear the data and deny all hardware access that makes sense. For example, Groupon would not need microphone access so that access is removed. I think only the Contacts app has the phone permissions. It takes some time, but worth it. Reboot after you are done and re-check because sometimes the settings don't seem to stick. Maps will still work without Google Services. Using Android_x86 in a VM, trusted apps are downloaded in a virtual machine and then put on the sd card and then installed on the phone. I'm not a very trusting person Its suspect that the "Emergency Alert" app has access to everything and I can't deny microphone or any other access for it.
I think it may have been the app named "app flash" that was doing it. Its apparently the app verizon added that by default sits on the left side of the home screen. I disabled it from my home screen first while setting up my phone after reset but left app flash active. Its supposed to be some kind of assistant app that recommends apps to install and acts as a hub for all your other apps.
So far I haven't had anything else auto install(nothing visible anyways). After uninstalling all the 10 or so apps that were installed they haven't come back yet. Hopefully it stays that way and nothing malicious going on in the background.

Both my wife and I have had that happen. The apps we got were Big Farm, TMZ, and some other crappy game.
Our phones are unlocked and rooted, so I immediately did a bare-metal refresh - wiped everything including system and internal storage, and did a complete stock flash from recovery. Since then we have kept "Install from unknown sources" and "Auto-update" both set OFF, and have had no further issue.
