Just this morning I somehow managed to turn on the very annoying "loading started, page loaded" voice when opening the browser. I have no idea how I did it. I read many other threads about this and they say turn off talk back or disable all options under accessibility settings. I have done that and it still shouts out the message every time I use the browser. Where else can I look to turn this off?
I have discovered that installing any of many different apps from the playstore can trigger this behavior on Samsung phones. Appears to be no solution but to listen to the voice every time you use the browser or any number of other things that trigger it. Here is a thread that discusses it:

Thanks, but they are both off. The link I wanted to post to the other developer forum explains pretty well what caused my problem I think since I had installed one of those apps. Nobody seems to know how to fix it though.
I had the same issue when installing "AOSP Browser"
Uninstalled it, restarted phone and issue was resolved
Eventually when I am able to post links here I can link to some other forums where this is discussed. Apparently it is a bug in the Samsung software on this phone, the Note 2 and both the S3 and S4. Certain types of apps, like Missed Message for example, monitor things via the accessibility part of the phone and that somehow makes the Samsung software think TalkBack is on even when it is not. Samsung has acknowledged the problem but has not been able to fix it. Uninstalling any apps that use Accessibility Services stops the issue. If you want to use an app like that anyway, then using Chrome instead of the default browser helps although it will still crop up in a few other places. It is sure interesting if you are in a quiet room and suddenly your phone shouts out "loading started, page loaded, loading started, page loaded"

I had this issue on my Samsung Note 3 with the latest available software update and TalkBack switched off.
In my case, I have a Pebble smartwatch which, for some reason, requires Accessibility settings to send notifications. With Pebble activated, I get the "Loading started" and "Page loaded" voiceovers in the stock Internet browser. With Pebble disabled, the voiceovers stop. So until this is fixed, I have to make a choice between giving up on the Pebble or putting up with annoying voiceovers.
As a temporary fix, I tried downloading another voice which had settings that allowed me to adjust the speed of the voice so that the voiceovers became merely a brief noise. Then I realised that the same voice is used for navigation instructions in Google Maps, making them utterly unintelligible.
Fingers crossed for an update.
As you can see from other forums, this has been a problem with a couple of generations of Galaxies now. Samsung seemed to indicate several months ago that they had fixed it, but that was not the case. Now there seems to be no communication from Samsung on the issue. So I would not hold your breath for a fix. It must not be an easy one.
I started using Chrome instead of the stock browser and at least in Chrome the annoying voice overs do not occur. I also do not get them in my gallery like some people do. Then I cleared the "default" setting in the Application Manager>Internet tab. Then I opened a link in an email and was asked which browser I wanted and I set it to Chrome, Always.
This has made the issue tolerable for me. The only real, complete, solution is not to use any apps that tag in to the Accessibility setting. And to not use the Pebble either, I guess. Not really good solutions at all but boy I sure love this phone anyway!
As Mopar said, you need to turn off Talkback, and the most important that you need to make sure all the language setting must be the same. ie all English, all spanish, all chinese etc.

I have the gear watch & it's doing the same thing.
I almost never use the stock browser however (I use xScope Pro) & the problem is only with the stock browser.

Mine just started yesterday. I downloaded LastPass. This morning I uninstalled LastPass and all is well.

If you go to settings > accessibility > services and turn off all of the services the problem goes away.
The chrome browser does not have this issue.

Could be knox if you set it up or did you turn on reactivation lock
Edit could be mal ware get lookout to scan the phone
There are numerous reports of this on the net. Nobody seems to have worked out exactly what causes it or how to stop it, AFAIK

Ja, actually after numerous search, I posted here! It's quite irritating. I wrote to Samsung and their reply is as usual.... Factory reset or bring it to service centre.
I also had this notification on my Note 3 three or four times, until it didnt come again. I dont know what causes it but have you tried to long press on it and see what the app-info says? On my GS 4 I, I had the Prevention Information as well, but removing the knox apks fixed it. But beware this requires root.
Reactivation lock is the function you can enable through the settings or when setting up the phone for the first time, which locks your phone with you samsung account and when somebody has wiped it, he has to enter your email and password to be able to use it.

I‘ve been getting this message a helluva lot. Started after i noticed an update notification for an app which didnt appear in the notification window afterwards. I have autoupdate turned off in the Play store.
Usually pops up after Llama changes (or checks) settings according to my setup. For some reason removing the Amazon appstore app helped to cut down on the frequency. I have autoupdate security policy on, could that be the cause of these troubles?
I seem to be having issues with Light Manager now but not sure if its related.

Like I mentioned in the OP, the message is pretty much appears whenever I connect/disconnect from the WiFi. I honestly not sure how to relate that to this message. Other than that I don't have the app you mention.
Just wanted to confirm that disabling the reactivation doesn't help either. As soon as connect to the wireless, I get the same annoying message .
I had the same problem as well..a simple fix I read online is..when the notification pops up- long press it -clear cache -force stop...seems to work for now..

Prevention Information new findings
This has been happening on my S4 4.3 and my wife Note 3 . I just updated policies on Knox on my S4 and the problem has disappeared. Now the message doesn' t pop up when i reconect to wifi or change to a different wifi! Will be waiting for any other updates on the forum before doing the same on my wife's phone.
Bringing my experiences from G3 to Nexus 6.
With the mods to Google play services on my G3 I was able to increase my battery longevity by a significant amount. This thread is to guide and discuss as to which "services" could be disabled inside the Google play services app using "3Ctoolbox". Disabling certain services prevents Ggoogle play service from running in the background, thus, saving battery.
As a reminder, some of the services can still run after disabling them and even cause a large amount of wakelocks. To solve this, please disable the corresponding tasks in Activities, Providers, and Receivers. For example: disabling update.SystemUpdateService in the Services tab will not disable the service completely, so you must navigate to Receivers tab and disable everything that is related to it there.
Lastly, think ahead while disabling services. If you play games and use "games" app to sync them, then you should not disable anything that deals with games. Same applies to other services.
Update 5/30/15: Google play services version 7.5... here is the list of what services can be safely disabled. Perform at your own risk. I am not responsible for any damages.

It's not quite a disabling of a service but all I've done with play services is disable location, keep awake, and wake up through app ops. No downsides that I've noticed yet.

well, if you are a ok google user, then there are downsides. if you disable location services, you wont get any info like "traffic home" or "traffic to work"(for example). if you dont use ok google, then disabling locations wont affect you to much.

The services that I have disabled are such things as ads, report services, and wearable services. There are other also, but I want to wait a few days before posting the final list so that other users don't have issues.

I didn't notice any. It could help by lowering the amount of RAM that is being used, but i think this would be in very extreme cases where all the RAM is used. With the way the phone is made, it has more than enough performance in stock form. Disabling services is primarily for battery longevity.

This wake lock is not on any of my lists. Looking at your signature, you are modding your lg g3. I'm pretty sure I had a similar post in the lg g3 forums when I had that phone.
Nice thread but how to get to the menu shown, requires root and full version? looked around the app cant find those settings..... cant make the changes if I dont know what to look for... but looks impressive...

You need full root. Download the app suggested in the first post. Scroll left once you open it, find "application manager", make sure at the bottom it says" system", click on the "apps" on the top bar, scroll to Google play service and click open then details, next scroll right and select services from the top bar.
After doing that, GPS and location stopped working. It cannot stablish my location. Even after enabling all Location services, it still not working.
Restart the phone. None of the things that are disabled in my list will affect the GPS
I have somehow disabled my gmail from notifying me once I have received a new email. I can sync it manually. I just don't get notifications for my gmail like I used to. Does anyone know what I need to reenable through the suggested app in the op? Thanks

Restart your phone and immediately go to the services tab
I suspect most of us have seen the issue with 'missing notifications' on Honor / Huawei devices. When you add an app to the protected apps list, make sure the various battery and startup optimisations are disabled, do everything you can... still the system seems to shut the app down and notifications don't happen.
Why? I don't think anyone knows yet.
The problem seems to be mitigated in some cases by moving the app manually to the /system partition, but this doesn't always work I don't think - for example Hangouts sometimes misses notifications for me and that's a system app. There's another nuance too - why do some apps not appear in the protected apps list (e.g. gmail)? That's not technically any different to Hangouts... is there a whitelist in the ROM somewhere?
Now I have my custom ROMs and deodexed ROMs, I'm going to be having a good poke around so hopefully we can fix this once and for all for all Honor / Huawei devices, but it would be super useful for anyone to contribute their findings so we can try and establish a pattern of behaviour while I look for clues.

The Google settings don't accept notifications generally, and the problems - which I've seen on all Huawei and Honor devices - aren't limited to Marshmallow.

I had a problem with Tasker continuously being killed in Lollipop even though I marked it as a protected app, but I found that the issue only occurred if I used the "Smart" Power Plan, since switching the "Performance" plan the issue appears to have been resolved. I now see the same issue with Whatsapp on MM and have again selected the same plan to see if that solves the issue also

So I'm making some good progress digging around with this. I've had some interesting feedback suggesting that this is less of a problem in 'Performance' mode than 'Smart', but it is still an issue. Interested to hear thoughts on that.
App closure behaviour seems to be managed in 3 places on device...
in Marshmallow doze (this obviously isn't the cause of our issues)
in HwPowerGenieEngine3 (com.huawei.powergenie)
In HwSystemManager (com.huawei.systemmanager), also known as the 'Phone Manager' app
Sure enough, disabling the latter 2 packages seems to make notifications work 100%, but there are other things hooked into the Phone Manager app, so disabling it completely is a bit of a blunt instrument, it'll be better to try and tweak the behaviour, which is what I'm looking at now.
If anyone has clear*notification issues they can replicate and there*use to help me test fixes, please post below.

google hangouts, almost always fail to receive notification.
Small question for everyone having notification problems. When you look into the "automatic launch records" menu in Phone Manager, are these Apps with problems listed as restricted or as allowed? Aquamail and WhatsApp are working fine for me even with smart Power plan

Which android version are you running?
I am now on the final version of Android 6 and my notifications are finally working flawless. (I got a EU Honor 7 running PLK-L01C432B330 - EMUI 4.0)
I'm still having reports of issues in B330. Especially in the 'Smart' power mode.

After 330 Update I got some problems, especially with WhatsApp. After checking auto start menu I recognized WhatsApp autostart was disabled and aquamail was not listed in this menu, I activated both before. Both were listed as restricted in the records menu. So they wanted to work but system blocked them. I turned it back on and aqua got the menu back after reinstall.

So I've made some changes on the Honor 7 to these power optimisations with a view to sorting this out. You can test in the latest release of MoDaCo Custom ROM (r4.1).

After installing PM PLUS and it's two plugins start the Phone Manager, swipe to the right, open APP AUTO LAUNCH and you can see the apps with their permissions. By clicking the clock symbol located right top you can see the past recorded launchs and if they were restricted or allowed.

I find that I keep notifications active on those particular apps by clicking on the square for the running apps, and then pulling down on the window of that app, which will highlight a lock above it, that way it is never closed. Survives a reboot as well. In other words the opposite of sliding up to close the app.

This did not work for me. Then today I was poking around in the settings and got it to work. I went into Applications in settings and then in the top right enabled system apps. I scrolled down to smart caller ID and clicked it. I went to permissions and enabled contacts.
Anyway an hour later I got a call from a family member from their home phone. I do not have this persons name saved in my phone. It said their first, last, and middle initial right on the screen. I was like wow it's working. So naturally I went into package disable pro and turned it off LOL. I tested it again with the same caller and this time it didn't show their caller ID, only their phone number. I tried to re enable the feature and have not been able to get it back since. If someone can try these steps and see how it works, maybe we can figure it out.
Kept messing with settings and it started working again. Rebooted and it went away.
My phone doesn't even have smart caller ID in the app manager area. I did follow the OP and enabled it so we'll see. I'm not rich hopeful but we'll see

I was like wow it's working. So naturally I went into package disable pro and turned it off LOL. I tested it again with the same caller and this time it didn't show their caller ID, only their phone number. I tried to re enable the feature and have not been able to get it back since. If someone can try these steps and see how it works, maybe we can figure it out.
Kept messing with settings and it started working again. Rebooted and it went away.

Mine doesn't have that in applications. Not even after enabling system apps. Must be third party installed app you have there.
Okay use the app from the OP and search for "smart" and smart caller ID will show up. Click it and then create. Then on the home screen click the newly created shortcut and it should enable permissions for smart caller ID. Then try.
I've tried it multiple ways and I don't see a "smart caller" anything either. Total crap Verizon! Any way someone can extract it?

Looks like they really stripped the Verizon variant down. I have a request out in the international forums to have someone give me their phone apk. Hopefully someone responds.
Does anyone else have this issue? Most of the time, when somebody calls, I get nothing that comes up on my screen. It's totally blank, no options to answer or reject the call.
I've factory reset a couple times, once when unlocking the bootloader to root it (this was after it happened with stock OS).
Any ideas on how to fix this? Some calls work, most don't...

I've found a temporary solution : I've installed a third phone app, set it as default phone app, and then set the OnePlus phone app as default again.
after reboot, the issue is gone for a few days.
but it come again...

I do have the should I answer app installed which is supposed to be set as the primary dialer, and does have its own UI for answering calls, but that doesn't come up either. Even when swapping from one to the other, rebooting or not.
I had a chat with OnePlus, they told me to reset my network settings which I did, and it began to work just fine, but today it's doing it again and I've done nothing to the phone. Changed no settings, installed no apps, etc...
My solution is, answering a call by holding the phone to the ear (in quick gestures, lift to answer or something alike (I'm Dutch))
It doesn't show the button but it works

I have had many phones... LG Gx, Galaxy Sx, Huawei PxxPro, and the OnePlus 6.
The OnePlus 6 was my best experience.
But I'm very disappointed about the amount of bugs on this 1000€ OnePlus 8 Pro...
I think I found the solution... I might be wrong, need to do more testing.
Settings > Battery > Battery Optimization > Tap the 3 dots at the top right corner and select Advanced optimization
On that screen, I disabled the "Sleep standby optimization" and so far it's worked perfectly.
I did this because a lot of my apps, with and without optimization would not give me notifications until I unlocked me phone, as though my phone was deep sleeping. Makes me wonder if this was preventing the phone app from displaying the UI because it was in too deep of a sleep.
This is already disabled on mine, and I have the issue [emoji6]
Do you guys use Android auto? I was having this issue after disconnecting from my car and car mode is still running.

Short term solution is, after you unplug from your car, in your notifications it says something like driving mode enabled. Click it and you should be able to answer calls again.

Can you tell me the name of the app who "send" this notification? So that I can re-enable it?
I did the same thing, that's why I factory reset. I could not find it anywhere to reenable.
I've tried to clear cache and data from android auto. I'll see if it works the next time I take my car

When the notification is displayed, you swipe it on the left, and click on the wheel : the name of the app I'll be displayed on top.

So, I've found the answer
It's true, it's related to the driving mode.
As I have chosen, a few weeks ago to hide the notification related to the "driving mode" still open when I leave my car, I was fxcked.
I've wipe cache of the phone app : no result
I've reset presets of the phone on android settings : no results.
I've wiped cache of the phone : victory : the problem is solved : I have the notification to close the "driving mode" still open.
So if you want to do the same thing :
Power off your OnePlus 8 Pro.
Push power and low volume at the same tame until the phone ask you your code .
You now select English language, wipe cache and data, wipe cache, ok and then reboot.
Don't wipe data, just cache, or you will loose all your datas [emoji6]
How has it been working for you? I still have the issue even after doing what your said.
