I have been fighting this for days now and I can't figure out what is going on. I have an i337 32g ATT S4 and I'm on T-mobile. I already did the AWS mod and have 4glte working great. Did this back in May.
I now want to install the T-mobile rom to get full functioning but I can't get my phone to sync with Windows 7pro. I can't get keis to see the phone. Here is a more detailed breakdown. I have phoneutil.apk running. If I set UART to PDA and USB to RNDIS+DM+MODEM and the USB debugger off, windows will show the phone in device manager and I can see it mostly in QPST. It shows up as unknown phone, 0 ESN, 0's for the phone, etc but you can do a backup so it is at least talking in some form I think. Latest version of Kies will not recognize the phone, says unsupported device.
If I activate the USB debugger, the phone disappears from device manager and nothing can see or connect to it, although it does charge. If I take the USB developer switch back off, device manager and QPST can see it again (at least somewhat).
I've read a ton and can't find any help.

I have unistalled all drivers, reinstalled drivers only, no Kies, etc. rebooting, etc. nothing works.
I'm going to try casual from a naked boot (no drivers but with USB debugging) but I don't think it's gonna work.
Worst case, factory reset unless I get some help.
Take your phone off the RNDIS+DM+Modem mode... put it on MTP + ADB mode. To sum it up reverse the steps you followed to enable aws mod. You no longer RNDIS/DM/modem mode enable, that was only to get qpst to detect your phone.

Oddly, when I fiddle with the lte band selections, it relocks my phone. Very strange.

Just ran into a problem which took me some time to solve. Thought I would share it to save other people some frustration.
After an adb reboot the i5800 got stuck in "modem" mode. i.e. android booted fine but gave a modem and a serial port over usb rather than adb. Reboots did not solve the problem.
The solution is to enter recovery screen:
reboot recovery
OR
press proper buttons to get into recovery mode
And then perform a reboot from there
Is this the same as *#7284# and selecting "PDA" on UART and USB, or just on USB? Or is this a completely separate thing?

Thanks so much for this!!!! After helping hack a friends build.prop to enable Apparatus compatibility in the Market, I found USB wasn't working properly anymore. Resetting changes in the phone didn't help (I had to do this via the phone terminal) although I at least managed to buy Apparatus before the sale ended.
After initially getting confused due to a lack of drivers and the occasional 'nH' showing up devices without drivers which made me think something was borked, I worked out from the VID & PID it was just the lack of drivers making (well I'm not sure why it did the 'nH' sometimes it got serial something instead), once I installed the drivers all was fine.
Except still mass storage wasn't showing and the Samsung connect manager didn't show up on my phone when I connected USB, nor did ADB work at all. In fact, it didn't even show up in the phone menu as debugging mode if I didn't start phone with it connected (or something like that). Funnily enough when I did get it the debugging to show up I did get the USB mass storage to show up in the phone, but it didn't help (it just unmounted the card which I think is all it ever does).
Anyway after a bit more fooling around I eventuall came across http://forum.xda-developers.com/showthread.php?t=1215103 and the stuff about modem and mkasick's post which sounded a lot like it could be my problem. The code for that Samsung phone didn't work so I searched for the 5800 and came across this giving me the right code. Sure enough UART (but not USB) was set to modem instead of PDA. I was unsure whether that was wrong but changed it anyway and yep that fixed it!
I still have no idea what went wrong. I had thought may be the phone didn't like me rebooting via ADB but I've now done that several times. In fact most of the stuff except install a superuser update has been more or less repeated. I guess it was just some strange rare occurence that it somehow got changed. I did have to reconnect the USB while working the first time, not sure why but ADB lost connection then I found couldn't connect. Perhaps that's somehow related. Now that I know what to do, it's trivial but very annoying when I didn't. I still don't get why Samsung decided to make the connect manager (my phone doesn't have that) only to have some hidden option where it can be set to modem or USB.
Partially because of this, I never got to get Tilestorm HD become compatible in time for the sale although my friend wasn't so interested in that. (Eventually I worked out using the THC Tattoo modded market which reports a fake resolution enables it. Well, I also modified the build.prop but I don't know if that was necessary. I wasn't able to do it solely by the build.prop. But it was way too late by then to buy Tilestorm HD cheap.)

This may sound like crazy idea, but is your USB port/connector clean?
(The USB port allows for dozens of different phone functions when using very specific resistance between data pins.)

My phone is running DL30 based Blackhole 4.01 with the DL09 modem. When I try to plug my phone into a computer, the phone detects the computer, but the computer does not detect the phone.
- The phone starts charging
- The phone says "USB Debugging Connected"
- The computer does nothing
- adb devices lists no devices
- same thing in download mode, the computer doesn't see the phone
I have tried on OS X and booted into Windows XP.
I have no idea how to handle this. The one thing I tried was installing the Stock/rooted DL30 rom instead of Blackhole, but that didn't seem to fix and I went back.
To make matters worse, assuming that everything in ROM Manager is basically safe, I accidentally installed orange clockwork today and I cannot figure out how to get back to a working clockwork. The All ClockworkMod Recoveries option in ROM Manager does nothing - they all say download error.

Edit: Also did you try to Odin, the title indicates no odin, but your explanation doesnt say so. (Open Odin, remove battery, hold down volume and insert usb cable)

Yeah, odin doesn't work because the computer never detects that the phone is attached.

What do you mean odin stopped working, your explanation doesnt say anything about odin as my edit in above post.
What I mean is that I used odin to install DL09. After that, I don't recall every attaching to my computer, but when I tried recently to install EB01, it no longer works. Sometime after flashing DL09, my phone is no longer detected by computer.
Im saying does odin detect the phone, not the computer.

Are you sure Odin doesn't work?
Check USB settings on your phone. My rom gives me media player mode and mass storage mode. Don't have my cable with me to test if media player mode stays hidden from the pc.

I was IRL with OP trying to solve this a few days ago. It's not that improper/no drivers are installed, the computer literally does not detect that there is a device connected to that USB port. lsusb on Linux turns up just the controllers and root hubs, and devmgmt.msc on XP does not show anything attached at all, even after a manual hardware scan. adb shows absolutely nothing. My Captivate does show up on all the same scans, regardless of the USB mode (Kies/mass storage/media player/what have you.)
He has tried this on multiple computers, so individual ports/USB controllers/drivers/OS bugaboos have been effectively ruled out.
My thought is that between your last flash, OP, and now, your physical microUSB port on the phone is damaged. If you no can haz insurance, I'd say you're SOL.
FYI: The phone does not say "debugging connected" when plugged into the wall. I think the my USB connector is damaged or something. I may consider seeing if I can get it repaired, or I may just wait until I can get a different phone that's better supported by manufacturer (and therefore by the mod community)
I'm not sure when this problem first appeared. For whatever reason, my phone doesn't show up at all when connected to either of my two PCs. It must have when I first rooted the phone (to load ChainSU) but I'm not sure if the problem is recent or occurred immediately thereafter.
USB debugging is disabled. When I plug my phone into a computer, it attempts to install the MTP Driver and fails. When I uninstall my device, it disappears and I'm unable to manually install the driver's I've downloaded from other threads. When I load into the Bootloader, my device is recognized by Fastboot.
I'm hesitant to factory reset as this will leave me unable to root if the problem persists, which is important to me as I can get rid of the yellow tint on this screen.
Any thoughts?

Are you definitely sure usb debugging is disabled? The phone should install as a portable device on your pc. Also ensure to enter your password onto your phone when connected otherwise the phone will not show up its contents.
Make sure for windows media player to be updated to the latest, for the windows mtp driver.

There are a few things you can try. Have you tried uninstalling the drivers completely then reinstalling from the toolkit? In the past, I've had issues where the phone wouldn't be accessible in some way on one computer but would be fine on another, so maybe try a different computer to try to narrow down the problem as to whether it's an issue with the phone or the computer? Also, this may sound a bit ridiculous, but simply switching USB cables and / or ports has fixed detection issues for me in the past as well--something to consider.

Remove any existing drivers (while phone is connected), do the same for other android devices if you have any, then reboot and install Universal Naked Drivers. Have always had success with this driver pack.

100% sure on both counts.
I have this same issue with my rooted nexus 4, I've tried 3 different computers and several cables and it never connects to the PC and only charges the phone. It doesn't even give me option to choose media files.
I've had this issue before, my Nexus/Computer was acting in complete stupidity. When connecting the phone to your computer you will see a notification saying "Connected as a media device" on your phone, just touch it, and change it to "Camera (PTP)". It might access the camera for you, but It actually accessed the storage for me.

Thats not the problem, it doesn't give any option. It doesn't show up on the computer at all and only charges. It's the same for every computer.

I'm thinking we've screwed something up with the phone's side of the MTP protocol - mine is totally independent of the cord/computer used. All the correct drivers installed.
Have you tried Factory Resetting? I'm really hoping this is a workable option.
If you have a recovery back up the current Rom and flash stock and see if it's working

I had same problem on my pc yesterday. Even if the drivers installer showed up that installation was successful, somehow for a mtp mode system was using some other Microsoft driver. Correct one was only used in ptp mode. So when I connected n4 in mtp mode storage wasn't available, when I switched to ptp mode everything was fine. Turn on your usb debug, switch on mtp and connect. Go to device manager in system, find your nexus there and rmb on it and manually reinstall driver. After this my n4 is visible in system in mtp mode again.

The problem is that even if I switch between MTP and PTP it still does nothing, I have tried multiple cables, computers and have reflashed the ROM and wiped the device but it still does not connect with the PC. This is frustrating.

Is yours able to connect during Bootloader/Recovery? Fastboot/ADB have no trouble recognizing my device.
It's really odd that if these things work, meaning your USB connection is fine, replacing the all of software fixed nothing... unless MTP occurs at a much lower level of the OS (maybe kernel is corrupted?)
I've got a similar problem that others have posted - PCs not detecting my N4. I got it last Friday (2/1) from Google and after the registering process and updating to 4.2.1, I connected it to my Win7 laptop and it showed on the PC as a drive. But since I couldn't copy/paste Office docs to that drive and had to do that through Dropbox, I didn't bother trying to reconnect until Monday, at which time the PC didn't detect it. Neither will one other Win7 nor two XP (both with WMP10) PCs. Have tried multiple cables, all of which work with other Android devices (pre 3.0). I've seen several suggestions on this and other threads, but one major issue I'm having is that some of the fixes require enabling USB Debugging. For the life of me I can't find that option in the N4's Settings. Unlike my other phones, there is no Applications subcategory under Settings on my N4 (4.2.1, Build JOP40D). Can anyone help me find that option? TIA.
Edit - Found answer. Click on Build in About Phone setting 7 times. That activated Developer options. Now to try the fixes which require USB Debugging mode turned on.
For some reason, I can no longer get any computer to recognize my Nexus 4 when connected by USB. I can charge but cannot get the phone recognized.
A couple weeks ago, everything was working fine. I was running Cataclysm (November build based on 4.3). I upgraded to the Nexus 5 and decided to give the Nexus 4 to my daughter. I was able to connect the Nexus 4 to the computer and move all my photos and music etc off of the phone. After moving my files off and doing a Titanium backup and Nandroid just in case, I did a complete wipe on CMW (6.0.3.5). Everything fired up fine and I had no reason at the time to test the USB.
However, last week, my daughter was complaining about bad battery life and I decided to take a look and go ahead and update the phone to the latest (Dec. 12) Cataclysm build based on 4.4.2. Before flashing the new build, I connected the phone to the computer by USB but could not get the computer to recognize the phone. The phone charges, but is not recognized in device manager and does not connect.
I've done everything I can think of to troubleshoot the issue - all without luck - including:
Turned USB debugging on and off. No luck.
Changed the USB storage mode from MTP to PTP and back. No luck.
Confirmed fast charging isn't somehow enabled. It isn't.
Tried several different USB cables. No luck.
Tried a different Nexus 4 and my Nexus 5 on the same computer. They work.
Tried the subject phone on a different computer running Windows. No luck.
Tried the subject phone on a computer running Mac OS. No luck.
I did go ahead and flash the latest Cataclsym build (I did a full wipe again) but sill no change. I don't see any evidence of physical damage to the micro USB port but I took a small toothrush and gently cleaned the contacts with rubbing alcohol.
At this point, I believe the problem is either the a) the port is bad or b) somehow, the phone software/ROM is screwed up.
Is there any way to determine whether the physical port is bad? Any other ideas. I'm kind of at the end of my capability here.

I don't think so. I'll have to check tonight when I get home. I tried to get Wug's toolkit to recognize the phone with no luck but haven't tried that command on its own.

No luck. I attached the Nexus 4 to the computer, entered a command prompt to navigate to the platform tools director that has the adb files and entered the "adb devices" command. The response was "List of devices attached" with no phone (or anything else) listed.
I have that issue too. I´m on windows 8 and to root and being able to recognize by the pc I changed it to PTP. It only works in PTP and I have to pass all the data to the camera folder to send info

I don't think I've got the same issue. I cannot get any computer to recognize the phone under any mode (storage, photo, debugging on, debugging off). Nada. At this point, I'm pretty sure it's the port gone bad. I just wiped the phone again and clean flashed the stock 4.4.2 ROM, radio and bootloader. Still no connection.

can you try going to recovery, then type adb devices?

I had this problem and i found away to fix it. Plugin your phone to a computer. Start > right click on my computer > properties > device manager. Look for Samsung > expanded it you will find Nexus 4. Right click on it and choose uninstall and check uninstall driver. Unplug your phone and restart the computer. Re-plug the phone and wait for the drivers to install.

I think their is a problem with something on the phone it isn't a pc or cable problem. I also had a new flex port recently installed. My phone will charge when connected to computer but it will not sync nor even recognized by device manager. I cant find any info anywhere with this issue. I try to enable mtp through hidden menu using *#0808# but it wont stay enabled resets after reboot. I also cant find menu under settings for usb. There use to be a menu in storage but it seems like its not there anymore. im on beans 4.4.2 nc4 with nc2 bootloader. can't find a solution nowhere.

Try dl'g the Sammy drivers directly from here and just click on the "See all Downloads", click "Send" and save the .exe file.
Run it as administrator, and once complete, plug in phone and that should correct the pc to phone issue.

Enable developer mode (tap on settings, about and then tap on the build number 7 times to enable developer mode).
Then back to settings, developer options and turn on usb debugging.

Do you mean through hidden menu or should there be an app on the phone? I didn't disable it i tried to enable it through hidden menu because it wasn't working but it just resets after rebooting the phone. Also there use to be a menu in storage under settings i could get to but it no longer exists. If you go into settings- storage- it should be a button in the top right corner of your screen which leads you into settings for mtp but it's not there.

I too am having this issue... it is quite frustrating. I rooted using towel root and that worked fine up until the beginning of September then USB was gone gone gone!. I was thinking of just doing a factory-restore of my phone, but I am not sure that is possible with having Towel rooted my phone. I just don't know what steps to take. Any help would be greatly appreciated!
i am going to the local verizon store today and if they can not fix this, then i see no reason for me to continue to pay for my phone. there obviously is something wrong with all these phones that people are complaining about. i have been on the phone with verizon and samsung for HOURS. i now am a pro at knowing what's wrong but no one can help me. It does not stay in MTP mode or PTP and goes back to charging. this is a phone problem that should not even exsist. plug your phone in, upload pics. has anyone gotten verizon to replace the phone????that's my goal today

Older versions of stock software actually put the phone in Airplane Mode when you were using USB 3.0, and had an inactivity timer or something that would turn the USB link off after a while. That didn't happen if you were operating on a USB 2.0 link.
So, if your PC has any non-USB 3.0 ports (the connector will not be blue) you could try using a USB 2.0 port.
Sometimes intermittent behavior is caused by bad cabling or a damaged connector, so a second thing to try is using a replacement cable and/or alternate ports. (Don't buy bargain basement eBay $2 cables btw - you will get what you pay for)

Is this a phone that you flashed a custom rom, rooted, or unlocked the bootloader?
