Wbaw, I installed your latest KANG (cm-7.2.0-blade-KANG-wbaw-120225), and also have the same wifi problem, like here. (Can not post there, because I haven't got the 10 posts)
Wiped and format everything before install. It says "scanning...connecting...disconnected". I've tried everything from changing the encryption, to changing the SSID, nothing helped. I have a Linksys WAG54GS router, "G" network, channel 10 - 2.457GHZ, encryption is WPA2-PSK. Attached a logcat log from boot, if it helps recognizing what's the problem.
With other KANG and with older kernel & wifi driver, my phone connects to my home wireless network.

I'll probably have to make a couple of test kernels for you, it looks like the new wifi driver is failing. It's either the new driver & kernel source or wifi undervolting, but I don't think it's due to undervolting.
You can also try this kernel: http://forum.xda-developers.com/showpost.php?p=22887053&postcount=92 it's the same except I removed some useless looking files from the wifi driver for the kang, they're in that package.
I'll make a couple of packages tomorrow for testing.

Right now i have to get data back from my SD card, because i accidentally formatted it...
You probably know that by now, that the new wifi driver works only with 802.11b network, so if your router was set to "G-only", the phone could not connect. Of course mine was set to that. I set to "mixed", and it connects without problem.

Seems strange, it seems to connect at 'g' speeds for me, but my router is set to 11b/g/n auto.
It looks like the new wifi driver causing the problem rather than the kernel, unfortunately I think it's the only wifi driver that works with the new kernel source.

Just wanted to find out how many people are having the WiFi disconnection/reconnection issue?
I'm thinking of delving deep into the frameworks to fix it, but I'm not sure if it is just a few devices (including mine) or more of a systematic problem.

i had the same problem over the last few days. If factory reset didn't work it was a reflash. It works for me now but you never know when it's gonna crash out again.
currently running your 3.1 rooted rom and i'm in Sydney if that makes any difference.
had this issue with the leaked 3.1, Acer_A500_3.019.01_COM_GEN1. i'm using the new stock OTA Acer_A500_4.010.07_COM_GEN1 and haven't had that problem since i upgraded. i'm connecting my Iconia to a home router and a mobile hotspot and both work without problems.
A. if you're rooted, you can just delete dhcpcd-wlan0.lease located in /data/misc/dhcp and try reconnecting.
B. if not, you can try this:
1. Forget the connection
2. Reconnect to the hotspot, this time using Static IP instead of DHCP.
3. Assign a valid IP Address and connect.
i always do option B when i was having the issue.

Just delete this file in term or root explorer whenever the issue occurs.

I had the same issue initially, then choose the Forget network option, did a reboot, and then added the network manually (I had to anyway with my hidden network).
Then the network did its thing. Hope this helps

I cant find the wifi forget option though.
In my case it won even turn on the wifi and therefore it cant find any.
Was searching for the wifi forget option in the wifi settings -> advanced, but could not find it.
Really curious of what to do exactly to make it work again.

In my case it started to work after I installed tiamat kernel back after p12 installation wthout wipe. But since rotation and ksm in tiamat do not work yet, I decided to make wiping. So after fastboot -w, and clean install of p12 it works again.

Did a clean install and reflashed tiamats kernel and nothing worked.
Some people say that comming from p11 and straight away to p12 works just fine.

This worked:
1. Wipe everything 3 times - reformat sd card
2. Install p11 - start it up
3. Install p12 right over it, dont wipe anything.
4. Enjoy the wifi

KJS021: When you're in the WiFi settings, press & hold on a connected network & then you get the option to either 'Forget network', or 'Modify network'.
Other info:
I started fresh with p11 (ran it for a couple of weeks), then updated to p12 without wiping anything.
I had wifi enabled with the icon appearing, but my network couldn't connect.
Google stuff played up, so next reboot was with gapps reinstalled.
Then I tried the network again & with wifi enabled I couldn't even get the icon appearing. I tried to re-add the network but still no luck. That's when I chose to 'Forget network' & give it a reboot. Then I added the network manually & mysteriously the network connected and the rest is history..
Other weird things that have happened in p12:
Google Maps crashed. A pop-up every few seconds kept informing me of this. I uninstalled Maps (which gave me factory Maps, before the update that is), then I cleared data just because.. Reinstalled from the market & all was good again.
Gallery wouldn't launch from my shortcut. Thought there was an issue, but found the gallery working through another app. Removed the Gallery shortcut, re-added it & that did the trick.
1 random reboot early in the piece, as well as 1 screen lockage I couldn't break out of. It's been good all day today though.
Misc Rants:
Also running A2SD (really needed with this rom) with 1GB ext4. One of the biggest issues I've had to deal with. Formatted my damn card various ways and just wouldn't seem to mount. Still not sure how I exactly got it working, but it wasn't long after I upgraded to the CWM recovery in texasice's downloads (CWM 5.0.2.7), even though I formatted the card elsewhere.
Other Specs: Radio: 5.12.00.08, everything else was done as in the OP of the proper thread.
Loving the rom so far, most things have been running fairly slick for me.
Waiting on the camera/video stuff to be fixed, and also pinch zoom to be smoothed out.

I've searched a lot without having responses, to get Wifi to work
When I use WinMob, GB or Froyo Builds, everything works fine.
With every ICS, JB or CM ( GB too ) Build which I tested, Wifi won´t start.
In every Build when I activate Wifi, it's stuck on "Wi-Fi Scanning" and no networks on the list.
After this the Button for Wifi is grey.
I tested a lot of NativeSD haret and SD Builds and cannot find the problem.
This problem appears with or without SIM Card and Wifix Manager didn´t worked.
My Router has no MAC Filter and switching the Channels from AUTO to any Number didn´t solved the problem.
Please help!

Same to me, i have tried everything, flashed different builds even the latest, but wifi don't connect it's only sees the networks. Everything is Omni stock, also tried with latest cwm and latest twrp to flash build and all the time the same wifi don't connect to network even if i disable the encryption to not secured (no wep, wpa, or wpa2) without password won't connect.
I don't know what to do more, i'm stucked to CM10.2 while i cannot use wifi on Omnirom 4.4 and even CM11
Maybe @Entropy512 can help us, you know what could be?
It's working fine for others, which makes me guess that you guys might have a new wifi MAC address configuration that needs to be added to macloader...

I have checked with CM10.2 and the mac in Omni seems to be identically with the one from CM10.2, even on CM11 i have the same mac with CM10.2 and omni but only on CM11 and Omni wifi does not connect only saves the password.
Anyway, how this can be done with macloader, is anything we can do? can you be more/give specific/details?, or this has to be done from I9300 omni sources?

Can you post the first 3 bytes of your MAC (please don't post the last 3, that's a privacy issue) and also what is present in .cid.info when you're running stock?

So in /data/.cid.info i have: murata but this in CM10.2 in Omni this file is missing from /data, it's placed somwhere else?
The first three bytes of my mac are: 1c:99:4c

LE: i have tried to manually copy from CM10.2 .cid.info to Omni /data and set proper permission via es file explorer and rebooted, but after reboot file got removed by the system i think wasn't there anymore as system default on clean flash.

Macloader generates it on reboot, and if it doesn't recognize the MAC address, it will just nuke the file and not regenerate it (see the source code I linked)
1c:99:4c is not a recognized MAC in our macloader - so I'll add that tonight. If I manage to merge it before nightlies, your MAC address should be working tomorrow.
I have just check the link and now i understand what is really happening thanks to you.
I'm happy that finally someone heard me and found the solution for my problem and probably for others with these three bytes.

I installed the optimized CM12.1 20150602 with Xposed and Xprivacy and now I got a connectivity problem:
When I come back from flight mode or enter a known wifi, wlan0 connects to my local wifi but Internet still gets routed through mobile data (LTE).
ping rounter not possible
Wifi icon has a "!"
netcfg shows:

on turning off mobile data, wifi data starts working and keeps working when I turn mobile data back on
I tried with a clean system: no problem, added Xposed/Xprivacy: no problem. So I guess one of my apps with root access must have caused that bug.
Possible candidates: AFWall, AdAway, droid VNC, OpenVPN, OS Monitor, SSHDroid, Tasker, TB.
Anyone knows how to fix this? I guess it must be a problem of how and what gets prioritized.
The setup took me quite some time and it would be great if I could fix this without starting from scratch. Plus I'd like to know the source of the problem.

I run none of the apps you mention and have the same issue and it happens on all wireless networks. I know of two workarounds, if they can be called that.
First is to cycle WiFi from within settings. When I do that it normally works right for a few days after.
Second is to power cycle my wireless router. That normally only works for a day or so as well.
I personally feel this is a flaw in CM as I have also seen the exact same issue on CM and any ROM built from it.
FIX
I found a fix for my problem by trial and error:
First I dirtyflashed the system again which didn't help, so I thought the problem must be in /data... thats where wpa_supplicant.conf is located so I had a look at it and strangest thing was:
The priority numbers for high eventhough it had only three entries. I changed them to 1/2/3 and rebooted. After that automatic connection to wifi worked again. BUT:
I rechecked the

/data/misc/wifi/wpa_supplicant.conf
again and the priority was back to the old state: 41/47/38 - in this order for the only three networks.
So the fix for me was: Change priority and save. Reboot afterwards.
Can someone elaborate on how this could fix the problem?
Oh and I must revoke my solution. Didn't work on the other network. :/
OK next trial and fail: deleting wpa_supplicant so it gets renewed. No difference.
Maybe I just try to install a fresh install and just restore /data.
I tried deleting the whole /data/misc/wifi folder. Didnt fix it!
And now I even installed from scratch and restored /data only from backup. Didn't help either!
What do I have to remove to fix this???
Rule number 1 of the Optimized ROM: do not use Xposed if you want support. So, uninstall Xposed, make a logcat of your issue if uninstalling Xposed doesn't fix it, then send that to AntaresOne.
He doesn't read the threads in Q&A, and wouldn't reply to this one anyway because of Xposed.

Yes, thank you, I know that.
I didn't report a bug, I'm just trying to understand the problem, so I won't run into it again.
I tried tried a clean system and a clean system with Xposed and Xprivacy installed and did not have that problem, so I ruled that out.
Lucky for me: I did mention that in my OP.
My point is that you're presenting your information to the wrong audience. You need to post the logcat, without Xposed, on the Optimized CM12.1 thread, if you haven't already done so. That way the people who can actually do something about it can attempt to fix it.
I do apologize however for not reading the thread properly. It was late at night for me and I was half asleep. *sigh*
SOLUTION
So I hope it is fixed now.
My last resort was to flash a new modem/non-hlos.
I was on a 4.4.2 modems (DTM-I9505XXUGNG8_I9505DTMGNG3_I9505XXUGNG8_HOME.tar) before and updated to the latest modems for T-Mobile Germany: DTM-I9505XXUHOD5_I9505DTMHOD1_I9505XXUHOD5_HOME.tar.
This seemed to have solved my Wifi problem.
Step-by-step (no data loss but still: make a backup):
Check which is the latest ROM for your country/carrier on sammobile.com
Download ROM on sammobile.com or samsung-updates.com (no account necessary).
Extract ZIP > rename extracted *.tar.md5 to *.tar > extract modem and non-hlos file from *.tar with WinRAR or the likes.
Shutdown mobile and start into Download mode (Vol-Down)
Run Odin, disable Auto-restart and flash modem and reboot back into Download mode (I did that twice because people were hinting that but I doubt it's necessary).
Flash non-hlos in case you have a S4-LTE version.
Reboot and test.

I think I finally found the problem: AFWall v2.0-alpha2 seems to cause the connectivity problem.
As soon as I deactivate the Firewall connection won't have any problems. I can't believe I needed so much time to find the bad boy.
Guess I didn't want to reduce security on my system, so I tried to avoid disabling/deleting my firewall.
