The above method stated by the original poster did not work for me but the following method did:
( I have my sim card in for this)
1) enter the MFG app (dial ##634#)
2) Input the password 277634#*# and press enter
3) Choose No. 7 engineer menu
4) in the engineer sub-menu choose other setting.
5) Choose Set Network Profiles
Let the list populate
6) Scroll down the list and find your appropriate carrier and select the appropriate xml, so for me in the
7) Exit the mfg app, restart phone
8) confirm that data connection is active in settings>mobile network

5) Use Typhoon-NBF-tool to open the nbf-file located on your PC around here:
C:\Documents and Settings\(your username)\Local Settings\Temp\pft(a number).tmp\
Notice! If you can't find the library specified above, it may be because your Windows Explorer is not set to show hidden files and folders! Open Explore (Windowskey + E), go into "Tools", choose "Folder Options", then "View". Make a checkmark in "show hidden files and folders".
Notice! The library specified above may have a slightly different name. Look a bit for it, there's a .nbf-file in the correct library. icon_smile.gif
6) In NBFTool, Choose Tools > Edit Header Data
7) Delete everything in the Operator field (where it says something weird - just delete it)
8 ) Press OK
Choose Tools > Recalculate Header CRC32
9) Choose File > Save As and overwrite the nbf-file you just edited...
9.5) Back to the QTek installation screen. icon_smile.gif Check the field where you agree to that your phone may blow up your entire house (or whatever it says) and press carry on with the installation as usual.
10) As the phone updates, please be patient at 99% as this may take quite some time!!
11) After succesfull installation, the phone will take pretty long to boot. You can just play with the scroll stick if you're bored... the phone will update as this is first boot (Cold boot), then it'll restart.
12) The Qtek ROM boots normally, and TADAAAA - you're done! icon_smile.gif

Now, setting up MMS and GPRS has changed in this ROM-version.
I'm not into describing how to set up MMS and GPRS for all teleoperators in the entire world - but here is a rough description that you, combined with the setup details your service provider can give you, enables you to setup MMS and GPRS (hopefully). icon_smile.gif

1) Choose Start > Settings > More > Data Connections
2) Choose Menu > Edit Connections > GPRS Connections

Thanks for many tips. I have a question on this upgrade network bands. I just want to upgrade 850 to 900 so that I can use in Asia. If I use only English, can I skip the below section of Alternative T9? This will be a lot of work for my family to work on via oversea phone support from me. Please kindly advise

There is hardware difference in C500 and STM5600 which runs on 850. Difference is in switchplexer which in European models filters 900 and 1800/1900 MHz (Murata HA 165) and in STM 5600 (Audiovox) filters 850 and 1800/1900 MHz (Murata HA 216AW). You have to change switchplexer on the board if you want 900 MHz on Audiovox
Or HA 358 (EPCOS D1028) from Sony Ericsson K700... -> 900 Mhz

You must be rooted and on Android 4.4.2 with T-Mobile for this to apply for you. This method worked on my Moto X, but may not work on other devices and software versions.
An explanation of how this works and why it's needed:
T-Mobile has had a second APN (Access Point Name) built into Android 4.4.2 called "DUN" and it is meant to be used only when tethering. It's a way for T-Mobile to check and see if you are using your phone, or if another device is using your connection. If it detects another device, it will only load up a webpage on that device that asks you to buy a Tethering option or switch to a Simple Choice plan that includes tethering. What we will be doing is removing the need for built in Wifi tethering to use or create a DUN APN, and then removing the DUN apn for T-Mobile.
Let's begin.
1. Be rooted. If you have an unlocked bootloader, install CWM or TWRP and then flash Superuser.zip or SuperSU.zip in your recovery of choice. If you have a locked bootloader and you're on 4.4.2, you're currently out of luck. You have to unlock.
2. Open SQLite Editor, grant root for it, then under the APPS list, scroll down and tap on "Settings Storage".
3. It will search for databases and find "settings.db", tap on it.
4. While in "settings.db", select "global" from the list.
5. You will see a table with "id, name and value" columns. Tap on the + sign on the top right to add a new entry.
In the name field, name it: tether_dun_required
In the value field, write the number zero: 0
Tap on Save to save the entry, then exit the app.
6. Go to your APN settings (Settings, More > Mobile Networks > Access Point Names) and you'll notice two APN's there. One labeled "DUN" and one labeled "fast.t-mobile.com." Click on the name of "DUN" and you'll see all of its settings. Hit the menu key in the top right, and choose Delete APN.
7. The "fast.t-mobile.com" APN should be set to IPv4 by default on the Moto X, but on other devices it may be set to IPv6. You need to change this to make tethering work if it is IPv6.
8. Reboot and enjoy!
Your traffic will still be monitored for tethering! You should be using a VPN, proxy, User Agent Switcher, etc. to hide your tethering more effectively.

Did u try a nv data restore or you can unlock frequency bands with qpst.

Hi Guys,
i have a problem with my Note 4. I attached a screenshot and you can see that there is no Preferred Network Mode option in it. I also try the code (*#*#4636#*#*) for hidden menu to change it but nothing happens coz when i changed the setting from GSM/LTE/Auto to GSM/CDMA/LTE/Auto and go back from the menu it will be revert automatically.
Can someone fixed it or have any solution???

I believe it requires a build prop or csc code edit that requires root for the settings menu but the service menu method should work as long as you do it correctly...and I say correctly not assuming your doing it wrong but to encinuate that there is a certain way you have to enter the keys..... Google it and you should be able to find a video
Anyone having issues using freedopop global sim card with their unlocked Samsung Note 4, follow the steps below.
First make sure your phone is Domestic Sim Unlocked (DSU) and not International Sim Unlocked.
1. Create a FreedomPop APN (https://support.freedompop.com/app/answers/detail/a_id/2670/~/setting-up-your-phone-gsm-sim-card) for mcc use 234 for mcn use 20
2. Choose this APN and back out to your home screen
3. Install Phone Info (Samsung) or dial the secrete code to open GCF settings on the phone (*#4238378#)

4. Tap on GSM/(E)GPRS/WCDMA GCF ON
5. It will show Success
6. Now power off the phone using the power button, not a battery pull.
* If you applied GCF Setting and want to revert it in order to use a CDMA SIM (to return back to Sprint for example)
just follow the same steps but chose "GCF OFF" instead in the latest selection.
