My phone keeps vibrating when the screen turns on when I'm in a call. It'll vibrate twice, and this just happened after I changed SIM cards. Is this common?

I just started experiencing a problem with the first incoming call after a restart. The lock screen is blank, and the touchscreen and hard buttons are unresponsive. When the call ends the lock screen finally appears with a message that I missed the call. On subsequent calls the lockscreen appears and the phone works fine. I've had the phone for 5 months and haven't installed anything recently. The phone is rooted so I restored from a nandroid backup but it still occurs. Anyone else have this problem? Any ideas?
My phone vibrates every time I take my face off of it during a call. the sensor assumes my face is off of it so it turns on the screen and the phone vibrates. How do I get rid of this?

NOTE: The vibrations during calls are not due to messages or emails. It is just vibrating to practically let me know that my face is off of the phone.
try messaging>settings>notifications> vibrate when on call and uncheck it.

Can anyone help? I've tried a lot of techniques and the only way to stop it is to cut off vibrating from the sound option altogether.

The phone is no longer vibrating when I receive texts and emails. The phone does vibrate when I receive calls and when hard buttons are pressed.
Recently, I installed and uninstalled a Vibrate widget which I thought it interefered the vibrating.
And, the vibration did not work after I removed the battery.
Anyone got any ideas?

I had the same issue awhile back.
I saw some very strange behavior from my Verizon Galaxy S5 (running OptimalROM) this morning. I was watching a video at my computer with the phone (screen off) sitting on my thigh, when I start hearing something about once a second -- I pause the video, and figure out it is my phone -- I pick up my phone and discover the noise is actually the phone vibrating. It is vibrating for a full second, then nothing for a second, then it vibrates again for a full second, and this kept repeating. I turned the screen on, thinking there was an incoming call or an emergency alert, or something, but there's no phone call and no notifications of any sort. After the vibrating continued for about a minute, I finally decided to reboot the phone to see if that would stop it -- which it did, and it has yet to reoccur.
I've never seen this behavior from any Android phone, so I thought I would ask the community if they've seen it and know why it was doing it.
