for server u can use wap.voicestream.com if mms is not working also...

I have attempted using the default that came with your new modded darkstone froyostone build (the same one you listed here), however i could not get mms. Are these perhaps region dependent? As a Telfort employee I must tell you Telfort does not have a username and password, those 2 should be left blank.
Most important is that you check the roaming feature, without roaming on, you can't use data.
If this isn't working for you, go to forum.telfort.nl and pm a administrator, they can check your network registration and give you some help if needed

You can also replace the apns-conf.xml in the \system\etc folder. Then reset the APN.
First off, I have a Desire Z (Bell Version) that I have rooted with stable version of CM 7.03.
I have been problems sending MMSes for a while now. I've looked up the various settings listed all over the Internet, but nothing has worked. I've tinkered and I rebooted the phone everytime I changed the APN settings.
Here's what's on my phone currently (this is after doing much tinkering from the various settings I've found on the Internet):
