Samsung support suggested a soft reset and also to clear cache. This seems to have solved it for now. Will keep an eye on it.
For anyone interested, this did reoccur though I've figured out the cause.
I've set up Bixby routines to change certain apps to QHD 60hz, this is caused if I lock the screen while leaving one of these apps open. It seems to be the transition back to HD 120hz for the lock screen / AOD.
Disabling Bixby Routines this no longer occurs.

It may be trying to force the screen to QHD 120Hz which it cannot support, likely why Samsung never gave us that option

I used the update.zip from the Samsung site to update to EB13, everything went off just fine except my Epic now takes an average of 2 seconds to wake from sleep after pressing the power button, sometimes going up to 4 or 5. Not the worst thing in the world, but pretty annoying.
I narrowed it down to my non-usage of the lock screen. I typically run without using a lock screen at all (disabled via SwitchPro Widget). If I turn the lock screen back ON, my Epic wakes from sleep either instantly or within 1/2 a second. With the lock screen disabled (no pattern lock either), it's super slow to wake.
Anyone have any ideas on what I could try?
Mine works just fine, but I'm using the lock screen, try uninstalling that widget and see if it corrects the problem when you use the lock screen.
Small update to this. I had an idea to move all the apps that I had moved to the SD card back to the phone to see if that helped. It does. With the lock screen on, my Epic comes back from sleep instantly every time. With lock screen off it varies between 1/2 and 1 second.
The apps I moved to SD were ones I hardly used.. I don't know why that would have an effect on the wake time?
Mine seems a bit slow too. But it's not the 4 or 5 seconds you described. It seems like it's about one second, when it used to be anywhere between instantly and half a second.
It's kind of annoying because the delay is just barely too long to where I start second guessing myself on whether or not I pushed the button in all the way or not.
Go to your display setting and reset your horizantal calibration accelerometer... that fixed my phone waking up issue...

I use the same program as you do and I have the exact same problem So your not alone.
Mine is slow to wake also, about a second. When its plugged into the charger its instant, weird.
I'm glad we aren't alone though. Hopefully if this is a big enough issue, people will look into it. And also, they've pulled the EB13 update from Sprint and Samsung's website.
It's causing problems with reading media from the SD card and also has data issues. So hopefully in the new one, they will fix this problem too?
Looks like it's No Lock, from what I can tell. If you disable it, it comes on instantly. If it's enabled, it takes it a moment to wake up.
My screen randomly turns on and sits at the lock screen for a few seconds for no apparent reason. There are no pending notifications, etc. I assume there might be a rogue app causing this, but I don't have that many installed and even less apps doing any background tasks. Any idea on how to find the culprit?

On the Galaxy Nexus this issue was sometimes caused by a flaw in the micro-USB connector. I sincerely hope that is not the case with the Nexus 4.
I noticed that the phone wakes up whenever it reconnects to Wi-Fi. I was changing some router settings last night and I constantly saw the phone waking up shortly after the router rebooted. However, this is not the only scenario where it does it.
Mine has done the same, no idea why, only seen it the once, but it certainly happened, sitting three feet away from me, and suddenly Bingo! its on.
Mine wakes every few minutes. Certain apps (tasker, facebook etc) and syncing seem to cause it to happen more often but even if I disable most apps it still happens. I've been unable to identify the source with better battery stats. It happens bone stock and on CM10.1. I'm still getting better battery life than my GNexus but it's maddening.
Mine too. It will do it a few times in succession then be still for a while before starting again. No apps open and google sync off.

So my Nexus 4 doesn't want to go to sleep.
- I have sleep set to 15 seconds
- Dream is off (just in case)
It just stays on. It doesn't matter if it's on the the lock screen or at the home screen. I'm guessing some app is keeping it awake. It just started last night, but I don't think I have installed any new apps. Maybe it from an update.

My phone's screen also used to wake up randomly. it was very annoying, especially at midnight
finally, I found out the reason by using "Wakelock detector" app which is freely available in market.

the cause of problem was ad Notification, it was holding "full wakelock" (you can see it in screenshot)

Dear all,
I was experiencing random screen wake issue in my XOLO PLAY smartphone.
The problem was with the AppLock software. After upate is this issue appreard. Earlier AppLock was there and no issue was experienced.

Had the BEST VR experience to date last night using the bundled Gear VR and playing Gunjack/Lands End until phone ran out of juice...
However that experience was marred when I fired the phone back up this morning and found that all the applications have suddenly starting rotating into landscape and despite rebooting twice, I cannot change the behaviour (it was fine yesterday). Ran some sensor checks which appear to be OK. I don't even seem to have a rotation lock button available as I did on my Note 3 as a workaround.
Adding to this, I have also noticed that the fingerprint unlock feature seems to have disabled itself, even though it's still enabled in settings together with stored fingerprints. I know this is a new device, but this is pretty basic stuff that should be working... Not impressed!
Any others having similar issues?

Fixed... kinda
I since discovered that the rotate lock had become enabled and is still available on the settings icons (didn't recognise the icon in the locked portrait state). It actually did it again this morning and appears to be defaulting to this state after a cold boot - weird)...
The fingerprint issue appears to have been caused by loss of the screen timeout/instant lock with power button settings; however I hadn't changed these values, so there do seem to be some software glitches that need ironing out...

Has anyone else had this happen? Happens quite often for me. I'll be playing a game at work, get up to do some work while leaving game open. Come back and unlock the phone and it's on home screen. Does the same thing while listening to Pandora.
I've never noticed that happening to me, I just tried to reproduce it with not luck. How are you unlocking the phone?
Either fingerprint or iris. It does it with both. I don't see it happening when turning phone off and on right away. Phone usually sits a few min before it does it.

Battery optimization setting probably. Try disabling optimization for an app that currently does it and see what happens

Couldn't replicate issue either. Let Rayman running and screen went off for about 5 mins. Unlocked with fingerprint and went directly to the game. I had power saving off and performance mode set to optimized.
OP do you also lose your place when it directs you to the home page? Or does it resume when you reopen the app?
It resumes when reopened.

The one app where it does it almost every time is Star Wars: Galaxy of Hero's. Only other app I notice it once and while is Pandora.
Are you sure you aren't iris unlocking it without knowing you are? Sometimes mine unlocks almost instantly when i press the unlock button. Or when I turn my screen on to check the time.
Is "iris unlock when screen turns on" enabled?

I'm sure. Most of the time my phone is face down and I grab it by the side and put finger on scanner. Turned iris scanner off just to be sure and it's still doing it.

I have this exact problem as well. Seems to happen rather randomly.
I sometimes see the app actually minimizing to the home screen when I unlock the phone, so it's probably not some background task killing it.
The app is only minimized, not killed.

I think I found the problem. Uninstalled bxActions and as of the past 2 hours the problem is gone.

Are any of you happening to use the long press home option, the one that bypasses the lock screen? Even though you may not be using the home button maybe the option is still affecting the apps that are open and thinks for some reason you're pushing on the home button.
It might be an app causing it. I still haven't had any issues since I uninstalled bxActions. If you don't have that installed it could be a different app causing it.
I was also having this issue. Stopped bxactions and the issue is gone. Started bxActions back up and issue came back.
Same problem here. On Verizon s8 and no bxaction installed. Very irritating!
Unrelated but another problem is when I change to aggressive wifi/cell handover in developer options it allways defaults back off after a reboot? Any ideas??
Anyone else having a bug where the screen does'nt turn off after the designated time out is reached? I have set mine to 15 seconds but it only dims down after 7 seconds and remains dimmed forever and ever.
Only way to get phone to work normally is to restart it. But the bug keeps coming back every day, sometimes serveral times a day.
Now I have to turn off screen manually by pressing the powerbutton.
Update Solution:
I actually had locked(in recents) Edge browser into the memory so it always would be available lightning fast. I just unlocked and closed it from recents app, and now the screen shuts off normally. Apparantly there was probably a hidden ads-page active somewhere keeping the screen from shutting off properly.

Not much, only the Force all apps to be resize:able.
The screen awake while charging is off. USB-debbuging also.

I have encountered this bug occasionally, a few times this happened some web page was open (No video was playing) not sure if this was the case with you sometimes some ads or something on the web page can keep the browser active. I did not think much of it nor explored it much but will keep an eye out if it reoccurs.
Turn off Developer options for now.
Does it happen in Safe Mode?
Make sure a 3rd party app isn't the cause.
Clear system cache on the boot menu.

Big Thanks! I actually had locked Edge browser, Telegram and File Manager+ into the memory so they would never be killed off. I just unlocked and removed them all, and now the screen shuts off normally. What a relief [emoji846]!
Using Adguard adblock and Edge content blocker so it might have been a hidden ad of sorts keeping phone screen active.
