Do one thing... Try rebooting in safe mode ( by long pressing power button> Touch and hold shutdown option> You will get a popup of safe mode rebot ) .
Then later on check if recents key is functioning properly. Now reboot normally as u do.
I had got the same problem after 1st FOTA.

Completely locked out. How do I boot to safe mode or last known good configuration ?
There's a Blue-Box that appears after depressing F10 which offers a HDD:
Toshiba Mk4009GAL. At the bottom of this box offers up and down arrows and 'ENTER to select boot device' then ESC to boot using defaults. Either of
these Options returns me back to the 'Missing/Corrupt File Display. That error is there even with regular start up .. I can't remember how you get to Safe Mode ..
Thanks

Hello Diane,
Entering Safe Mode;
1) Restart HTC Shift
2) HTC logo and F10:Boot menu will appear.
3) Press Fn+F8 key quickly two or three times when HTC logo disappear.
4) Select Safe Mode in Advanced Boot Options.
If you cannot start Safe Mode, would you try Fn+F3 for recovery ?
Be carefully, all data in hdd will be lost.

I have just recently flashed mt ATT SGT to JJ4>JK5 , rooted unlocked and voice enabled with Bell modem. Everything seem to be working just fine, but there's one thing that does not seem right : If I put the tab in recovery mode, I get in the background of the recovery window the little android and the yellow triangle. Whatever recovery command I select ( reboot the system for example ) all I get is the android and the yellow triangle in the foreground and the phone stay stuck on it. I have to press and hold the power button to reboot in normal mode.
Any suggestion ?

i have exactly the same issue and been trying to find a solution
anyone kind enough to show us how to fix this?
I found a solution to it , I do not why it's working but it is !
1) select the command that you want , let's say "reboot system now" and hit the Home button . You will now get the Triangle and the android
2) Wait a good 30 seconds
3) hit the Home button again ............... Voila' now the command it's executed !!!
You can now hit the "thanks" button on the bottom of this post
Today, i didn't remember what did i do in CWM 5.0.27 on my Atrix, after reboot it, the MOTO log gone and show all commands in screen(eg. fastboot, android recovery ...),
but my phone pass through it and into the OS. However, when i click 4 button in the bottom of device, all of them working abnormally, the phone not open the Configuration, Home, Back & Search by desgin, seem they points to the bottom of screen.
For example, if there is OK button show in right bottom of screen, when i click Search button, phone won't pop up Search web page, it will click this OK button.
Who can help me to fix this problem? It is very urgent & critical for me, in fact, my phone is not can be used for me, . Thank you so much.
I'm not sure to clearly undersdand well the problem !!!
But before the OS is mounted, the 4 buttons are not activated.
So in CWM, you should use only the vol + / - and Power button.
Vol + / - for selection
Power button for "enter"
Edit : My reply is totally un-usefull !!! Sorry, I can't help...
you probably formatted the pds partition in CWM. This has info about your touch screen, wifi etc...follow this link to flash the pds image and have your 4 buttons work again.

Actually first try to reset and wipe by recovery mode.... Volume up+ home + power..... Its much simpler.....
Only if this method not work than you should try downloading mode.... Its complicated method.... You need to get expert advice for that...
Maybe OP is busy . Let me update the thread.

Thanks. It works for me.
i saw this issue in couple of threads. To all --> go to recovery and make wipe! several times fixes this issue. i guess you have to keep doing 3 or 4 times unless the issue is fixed, other method is very long

Not sure if it would help but I decided to restore to factory settings. I lost all my data on the phone that wasn't backed up but the phone is working again. In fact, the restore didn't work all the way as it threw an error, but when I rebooted, it came back to life with Android 9!
This is how I did it....
1. Make sure the phone is powered off. If it isn't, press and hold the volume up button and the power button until the screen closes. then release your hold.
2. Connect the phone to the charger and wait till you see the charging screen (a big battery icon with a percentage).
3. Press and hold the volume up button and the power button for exactly 10 seconds and release. After 2-5 seconds you should see a new screen with an android saying "no command". If you don't see the new screen you either released too early or too late. repeat steps 1-3.
4. Hold the power button and press the volume up button once. and release. This should take you to recovery mode.
5. Scroll using the volume buttons and select factory reset using the power button and confirm.
This should reset your phone.
