What exactly is a boot.img file and when should I backup/install it?
Does it have the stock recovery? It's the same as we do on windows with adb and fastboot flash recovery img command or did I confuse them?
Should I take backup of boot.img when I install a custom rom or kernel?

No, you will have to flash one of many revert zip-files back to OOS 2.2.1 to get the old bootloader. Then you can revert TWRP to the one for that bootloader and then format data and reflash CM13.

flash it via modified twrp ans ASAP
yes flash via modified twrp.... then flash the official twrp img...
official twrp can be flashed with modified twrp or fastboot

Yes you can download OOS 3.5.6 flash able zip from OnePlus official site ,and flash it through twrp...
Make sure you download twrp/recovery zip,not fastboot zip

I've make custom kernel my zenfone 3 with gcc aarch4.9 from google and with defconfig ze520kl-perf_defconfig , kernel from Asus latest version. The compile is success, but when i flash it with twrp use flashable zip it not work. Anyone can help me ?

maybe you can try to flash SuperSU or Magisk directly after flash the boot.img (without reboot)

Thanks , i've flash the supersu after flash a kernel, my phone booting normal, but with old kernel not with new kernel, do you know why ?

If you mean if you can use two kernels simultaneously the answer is no.
Dirty flash your ROM and flash the other kernel.
It's possible to flash another kernel over the actual one but it's not recommended.

Dirty flash your current ROM, after that you can flash your new kernel via fastboot (kernel has to be in your ADB & Fastboot folder)
fastboot flash boot nameofkernel.img
Or flash it via TWRP (kernel has to be on your internal or external SD)
Install - chose .img instead of .zip - locate your kernel - swipe - chose boot.
Reboot.
