The sd card in my Moto X Play had a corrupt file system. I formatted the card (by Android, as external storage), and discovered that the mount point changed.
Some applications now can no longer find their (restored) data.
I put the sd card in a card reader, connected to my pc (running Linux). Using exfatlabel I changed the card's label, so it was the same as the previous mount point (which was 0123-4567). When I mount the card on my pc, it is mounted as 0123-4567.
Putting it back into my phone, it is still mounted as /storage/FE51-5E4C, the label it had after formatting.
When I look in Settings - Storage and USB, the sd card is shown as 0123-4567.
What algorithm is used to determine the mount point?
How can I get the card to be mounted as /storage/0123-4567?

Ok so I tried accessing my gallery and my folders are missing - rebooted the phone, still missing.... file manger >> sd >> all there is is a friendly name.txt
where has my content gone!?
Also when plugging in my phone to my PC using media player mode only the phone directory displays, no storage card
Has it wiped itself? - How could I check this?

now reports External SD card as total space 'Unavailable', and the internal SD card as 13.43GB I've tried unmounting and re mounting - no change
if you put it back on a PC, does it read fine your SD card?
make sure you don't have it mounted
when mounted, you can't access it via the phone
I know this is a stupid question, but is it physically present and on the right place? If you try to remove it and put it in another card reader does it read itself? Do you have some other phone you can try this with?
Sorry its the internal SD card that seems to have formatted itself, I had many images etc. on home\sd - now its empty

Your SD card might be corrupted, can you write files on it?
My galaxy tab fail to detect the internal sd and external sd card today. Don't know why.
The case is that:
if I use the application "my files" I have the following message
No SD card is inserted in your phone
in the the menu card and SD memory:
I have
Internal SD card
Total space not available
Space available not available
any possible action on:
Disassemble the SD card
Mount Sd Card
Format SD card
menus are dimmed
External SD card
Total space not available
Space available not available

phone's internal storage can be shown as usual.
The above problem happened after backup all the apps (about 90 apps) into sd card (I am not sure which one) and then I shut down galaxy tab and put out the external sd card for copying some files. I returned the external sd card to the galaxy tab and switched it on. Then I found that it cannot detect the internal sd card and external sd card.
I had tried to hard reset it and still fail to solve the problem.
Help!
Anyone can help me?

Weird thing, I got that same message today on my tab, but it was for a sec and disappeared.
However when i went inside the my files, all content and everything was ok.
Don't know why it gave that message.
Also i got the message for external sd card - not internal. (have a 32 gb one added).
I have exactly the same issue. I just got my today and have being going crazy on internet all day to try and find a solution to it...anybody has any idea on how to fix this...this is driving me crazy...

how to reformat internal sd card ?
Im also facing the same issue. Cant mount both my Internal Sd and External SD.
I had a related problem. My tab originally had no external SD card, but there was a folder in storage called "external_sd". This is where I've been putting my music etc.
Today I went to add and actual external SD card. Upon doing so, the file explorer on the device now shows the new external card as "external_sd", and its icon portrays an SD card rather than merely a folder.
Problem was, my original "external_sd" folder was now nowhere to be found.
However, upon mounting via USB, my PC detects both storage devices - the internal memory containing the "external_sd" folder, and the actual external SD card.
So on a hunch I renamed the "external_sd" folder on my internal memory to "internal_sd", unmounted, and low-and-behold, I can now see my "external_sd" card, and my "internal_sd" folder.
It seems like Samsung mistakenly named a folder "external_sd" and this name collides with the name of an actual external SD. I never questioned the name, I just assumed they included memory that was logically external, but physically internal. Seems instead they just had a dumb name for a folder.
...or something else I don't understand.

Much help would be appreciated. Total new experience for me.
I have cm7 stable on sd of my nook color. I have everything up and running great. trying to overclock and when I take my sd card out of nook and put into laptop, it asks me to format it. It is not allowing me to copy the kernal to my sd card. Is there another way of doing this?
While inserted in my laptop, I look at the properties of the sd card and it is showing to be in raw format.

Are you unmounting the SD card before you eject it? Also, are you sure the FS on your SD is FAT32?
When I originally formatted the card before I burned the installer and the cm file on the card it was formatted to a fat16.
I have not unmounted the card prior to ejecting. I will give that a try.
okay I unmounted before ejecting... put into laptop and the laptop is still wanting to format the card.
when I look at the properties it says file system is raw, and storage space is 0mb.
Maybe your card reader couldn't read microSDHC card?
With CM7 running off a uSD which I am doing as well, some built in laptop readers will not read the uSD but it may be read with a USB reader. But, it will typically only read the 1st partition which is the Boot partition (unless you use a program to set the 4th partition as primary and active).
To access the 4th partition for storage, you can try to connect the Nook by USB cable to your computer and enable USB storage mode. That should connect you to the 4th partition and your internal mmc storage partition too.
If not, download and use the following app:

The app is a little deceiving, you have to scroll in the app to see the "Confirm" button when you are mounting, and unmounting.

If you still have stock on eMMC, boot with the card out, then insert it before the stock OS fully loads (like on "n" or "Nook Color" splash screens). The NC will mount the boot partition as storage; there should be an SD-icon notification in the lower left, and if you tap it, it should say "SD card inserted." You can then connect via USB, and hopefully your card and PC will kiss and make up.
ETA: I've had no luck inserting the card after the OS is fully loaded--it just reads "SD safe to remove."

I have accidentally formatted my SD card in ext3 file system mode and now my phone cannot read it.Even when i connect the phone to the computer in usb memory card mode...the sd card is not shown in the computer. So i am not being able to format it again.Please tell me how to reformat it back to fat32 ....
Use EASEUS Partition Master, it will read your SD card in EXT3 format and will allow you to manage / format it!
open disk management (start->run->diskmgmt.msc) and erase your ext3 partition (doable in windows)
If you have any trouble doing this while the card is in your phone, you can also remove it and use an USB Card Reader.
But as said before, you just have to change the unit type to get it working again.

You can use a USB Card reader and this software: "hp disk storage format tool".
I installed App2SD (https://play.google.com/store/apps/details?id=in.co.pricealert.apps2sd&hl=en) on my head unit in hopes to create symlinks between my internal and external storage to free up my internal storage to install more apps. In process of doing so it needed to partition my external SD card. I selected FAT32 & ext2. After it rebooted I found that my SDCard, USB Flash Drives, and External SD card were no longer mounted. Infact they dont even appear at all in ES File explorer as if they didnt exist. Looking in storage, I do not get an option to format my SD card or USB drives. I took them to my PC and confirmed they are still FAT32. I reformatted my Micro SD Card from my PC and although it was successful it did not bring any of my external devices back.
I cannot mount the SD card.

The above is the original mounting point of my External SD Card, taken from External 2 Internal - it is no longer mounting and External 2 Internal no longer even sees it when I scan for additional devices.
This is what appears to be currently mounted.
Additional troubleshooting I took.
If anyone could help I really need it because I cannot reinstall redmod without the device being able to read the external SD card, I also cannot listen to any of my music or access any of my apps that were moved to the SD Card. Also can anyone show me how to properly create symlinks using that app without losing all my mounts?
