I flashed Frank N4 ROM and my device has been crashing since then. It freezes randomly, becomes totally unresponsive for 5-10 seconds, then reboots. And even when not in use, the device keeps on crashing and rebooting randomly.
I have tried both BaNks as well as Txuki Gapps. Same issue persists.
I have reflashed stock rom, flashed latest twrp, wiped everything except Internal Storage, flashed rom and gapps, wipe cache and dalvik. Done this like 3-4 times. Still no luck.
Any help would be greatly appreciated.
Have you tried another ROM, to see if the issue is there as well? If not I would suggest that.

This was my first Marshmallow ROM. I used to be on Paranoid Android(the latest stable one) and before that on stock rooted 5.1.1. Never had this kind of issue before.
I'll try another Marshmallow ROM and report back.
EDIT: I tried another Marshmallow ROM. I'm experiencing the same issue. I guess all Marshmallow ROMs will make my phone crash. Any help?

I installed stock via fastboot. Erased data, system, Internal Storage, Dalvik, Cache via twrp. When that didn't work i installed stock 5.1.1 via fastboot.

I'm not a total scrub at flashing roms and there's still a bunch of things that I don't understand. I'm pretty positive that I flashed Slimrom correctly but here's the situation.
Started on 4.2.2 Slimrom
Reboot to TWRP recovery
Format factory reset/format data, system, cache, dalvik cache
Flash Slimrom 4.3 stable build 1
Successfully reboots into 4.3
Okay now here's the weird part.

Are you flashing the right Gapps package? Be sure you are flashing the latest 4.3 one.

Yep I'm positive. I redownloaded both the 4.3 rom and gapps package multiple times just to make sure. Also it doesn't reboot even if I don't flash gapps. Doesn't boot at all if I flash both the rom then gapps at the same time.

It works for everyone else though. It's Slimrom 4.3 stable build 1.
Rom isn't the problem.Slim rom beta 1(4.3) is super stable. Use any version of twrp recovery other than 2.6.1.0 and wipe system+data,flash slim rom,flash slim gapps,reboot.
Note: if you flashed franco kernel then this may be the issue for your bootloop also you may want to upgrade your bootloader and radio to 4.3 versions(bootloader more important)
On flashing a Custom Rom, my phone gets stucked with continous FC ("Unfortunately Phone Crashed","Press OK") just after the phone boots.
I had faced the same problem earlier so someone on XDA suggested to use twrp recovery. Then i flashed the purity rom and it worked fine, but then as you may know now, as PA is out, so flashed it and again i'm getting continuous fc error....
currently i'm using purity rom, stock kernel and twrp recovery (2.6.3.3)
I would flash back to a stock 4.2.2 and start over. I find that this is the only way I can install certain roms.

Strange. Are you updating the bootloader and radio before flashing a 4.4 rom?
Are you doing a factory reset???? And flashing a proper gapps after ROM? You should always do this when going from one ROM to another. Sometimes a manual format of /system is necessary also.
no i am not updating my bootloader and radio before flashing (rather didn't know if i had to update them,fyi i bought my nexus 4 this september)....and yes i'm doing a factory reset (using the wipe option in twrp recovery which warns clearing of data, cache and dalvik memory)
I officially upgraded to 3.0.2, unlocked and rooted my phone.
It was fine for the first few times I flashed multiple ROMs. But ever since I flashed Exodus, I keep having issues switching to different ROM.
A week ago, I flashed "Switch to OOS3_H2OSMMv7" zip and was able to flash Resurrection Remix, but today when I tried switching to CM13 or PA it gets stuck at flashing the ROM zip. I tried flashing the firmware zip again and flash the ROM, but to no avail. It won't flash any ROMs now, just stuck at recovery while flashing the zip.
I have a working fastboot and recovery mode though, so I'm trying to avoid flashing the entire factory image but that seems to be only option left. Does anyone have any clue as to why this is happening and how I can fix it?
Edit: Resurrection Remix gets installed fine, CM13 and PA still giving me issues.
Edit: I downloaded the latest version of CM13 and it works. Earlier I was using June 30 build which seems to have issues with the new firmware. I'll try out the new PA build to see if it fixes that too.
What error does your recovery gives?
Do you have Data Backup?

It does not show any error as it just looping while installing the zip. I have to force shut down the phone to exit it.

Yes, I am able to access my phone in both ADB and in fastboot modes.
After flashing Custom ROM, Wipe data, cache, d-cache and then reboot.
If still doesnt work try flashing another custom ROM. You can use adb-sideload to flash another zip in recovery mod.
I bought this phone used and the person who had it before me it rooted and installed some of different ROMs on it. After getting my used moto x pure 2015 running android 6.0. I booted it up and tested all the functions (including downloading a few apps) Wi-Fi, camera, audio/charging jacks ext. Everything worked fine. So did normal factory reset (not using TWRP) just to make sure everything was clean before using it as my daily. Then, (BUM BUM BUM!) when I tried to start downloading all my apps. None of them would download even though the Wi-Fi was working. Error 492 I think.
I started trying custom ROMs because I was having two big issues (apps not downloading and incoming call not bringing up accept/decline screen) with my stock rom. and wanted to fix those issues with a fresh stock image.
This is all new to me and has not been as easy as anticipated. If anyone knows a clean factory image I could flash or a fail proof fix to error 492 that would be helpful. I really just want to return to fully working Motorola stock android, but the files I have used haven’t fixed the issue. I have searched for a factory image without finding one…

Install stock firmware via fastboot... if that dont fix the issue, install TWRP make a full wipe, FULL, and install the stock firmware again, eventualy that sould fix the issue.

I'm having a similar issue. Tried the TruPure rom but I want to switch back to stock. If I do a full wipe, would I be able to use the files that Motorola provides on their website and flash those throught fastboot? Or through TWRP? Any chance you have a link to a guide on how to do the latter option (installing through TWRP)? I'm just trying to get my device back to 100% stock so that I can receive OTA.

Odexed one, install with TWRP, full wipe before doing that, no GApps, just ROM, them, install stock recovery vía fastboot. After that you Will be stock with OTAs.

Thank you for sharing that link. I think I get it but it seems too easy considering what I've tried so far. Just to make sure I get it:
I should download the odexed rom, go to twrp and wipe data, cache and dalvik then flash the odexed rom through the install function? And thats it? I've literally tried a ton of stuff so if this takes care of it, this is great!

Yes, also wipe system, make a backup before If you want, but remember, for the OTAs support you need yo flash the stock recovery via fastboot. Good luck.

This got me back to stock! You are the real MVP.
So I remember seeing a recovery.img file in the factory image that motorola posted on its site. Does it have to be a specific stock recovery or do I need a specific one to match with the file I just installed?

So I'm on stock build 49-18-8 and flashed the recovery from 49-18-3. I went into fastboot and selected recovery. I got the android character with an !
I then did the power button and volume up and got into stock android recovery. Am I supposed to see the android character with ! each time I try to reach the recovery menu? Just wondering if I did this correctly.

I tried to factory reset a custom ROM, got sent to TWRP then I can't reboot to system from there. Tried a clean install but to no avail. I've looked around on the internet and I think I have to reinstall stock ROM and then reinstall custom recovery and ROM, except if I were to install the stock ROM once again (with sp tools) I need my phone to be turned off, not in recovery mode. EDIT: Managed to get rid of the TWRP bootloop by restoring from a stock nandroid I've found on the forums, coupled with flashing a stock recovery image using TWRP. Reunlocked bootloader, flashed TWRP all over again and then flashed custom ROM and Gapps. Everything running fine now.
Mods feel free to delete this post since it's solved itself, or leave it in case anyone needed help concerning the same issue, up to you.
