Hello to all , i have a very strange problems about my Samsung Galaxy Note 4 ‘ s ( you will understand why i said like this instead of note 4 ) and with my Sandisk 32 GB Class 10 U-I micro-SD card , first of all i want to tell that , I were using this card with my old galaxy Note II (GT-N7100) since 2 years without having an issue or problem ( with all my musics and pictures inside the sd card and also almost my all programs were installed on sd card , while sometimes my Galaxy N II was in stock or custom rom , there were no any problem) , anyway , then i decided to upgrade to note 4 so , i bought 2 Note 4’s from a company in Cyprus (Models: SM-N910H and never opened box ) , 1 is for me (black) and 1 is for my wife (white) , anyway , i just opened boxes and setup phones and restores our backups (as you know contacts, sms and some programs that i already used on Note2 throu Google play) for me and for my wife ,(beside,but before i just takeout my SD card from note 2 and make o quick format on PC with a exFAT file system which is my note 2 was working great already with exFAT before and also FAT32 as well, then put it to my new Note 4 as new Formatted ) after a while (few days later) My note 4 start to pops up sudenly an SD card error "SD card is blank or has unsupported file system “ and when i touch it Format disk pops up and when i click yes it says checking the disk... and trying to format but does not format the disk and same error keeps again like SD card Blank ...... bla .. bla .. so i take out SD card from My note 4 and put to PC but no any problem , all my files at there , even i checkdisk the SD card but has not found any problem , then i put SD card to my old Note II again , even i can access files and has no any problem again , then i just put the card to my wife note 4 and also it detected card and it was working , after , i just format the SD card on my wife note 4 (whats an interesting point is that , my note 4 does not accept to format the SD card but my wife Note 4 formatted and successfully mounted the SD card without a problem) and i put it back to my note 4 again , and it was mounted succesfully , so started to setup again buy copying musics , puctures on to SD and install programs again to sd by using the application manager as for some compitable programs with “move to SD card” options , so i used my note 4 2 days with no problem then , i shut down my N4 at 2nd day night and put it into wall charge while it was powered off and when i wake in the morning and, i switch on my N4 and again my programs short cuts get dark (programs lost , and my musics plays first 4 – 8 seconds and N4 music player try to play next track like this loop then when i try to format disk again on my N4 , it did not format the SD and give SD card error "SD card is blank or has unsupported file system “ again then i put to PC but no any problem again , so i think that my N4 card slot is defective and give back to company where i bought it and request a replace but they dont accept they said they will test it about 10 days , so they said they tested it and the problem is about on sd card , because of under the SD card there were a black sticker which i was removed it 2 years before (also after i removed the sticker it was already working with my old N2 without a problem), and they said they fixed it by sticking to the under of SD card a white sticker (which is shown on picture) , so i get my N4 and tested 2 days worked great with SD card but when i shut it down and put it to charge when i wake up next day (3rd day) the same problem happened again so , ı went the company back for an replace but they said they will not replace but they would refund me with in max 10 day , anyway i left the N4 and start to use my N2 again with the SD card and it did not occured any problem and at the end of 10 th day , i got my %100 refunded money back , while in this period i did some research and learn that SM-N910H model only 3G variant and will never support 4G , so due to my research i decide to buy SM-N910C or SM-N910CQ or SM-N910U , for an 8 cores with 4G support , by a luck , i found on an other small company just one SM-N910CQ model which is an Turkey product and the box never opened before , and i told the seller i want to open box and test it here before buying it , she said ok , and when i just open the box , i put out the sd card from my N2 and put to it and first run it and then BOOM again it gave the same error again , so i left the unit at there and did not bought it the seller said me that your SD might damaged , then i said her , ok put it you personal phone , which it was galaxy S3 , and she put and test it , the sd card were working and she agreed with me SM-N910CQ might be defective (please note that the SM-N910CQ came from the first company that I bought the SM-N910H unit , which was the company name were SUITEX LTD.) , and I started to think that all the series that , SUITEX LTD. NOTE4 S are defective or damaged , finally on last other company , I found my monster again SM-N910U (the most prefered model prefered by me cat6 4G and an upgraded Mali 760) nand it was last un-opened box , but when i opend box , and tested the card , it worked , and i immediately bought it , i were heppy i setup all the things and used it 4 days without any problems , and guess that what happend in 5 th , when i wake in the morning and turn the phone on again my programs short cuts get dark and then i click them they are not working (which i moved to SD card) and then i try to format the disk from my N4 it try to format but it can not , then gave the same error loop again like "SD card is blank or has unsupported file system “ but after 1 or 2 hour it fixs it self and , and all works as normally , so experts please help me , what might be the problem ?? , i am sure %100 that all the N4 were ok , i can not be unluky like that 3 Different model of N4 from different company can not be defective at the same tame , also i %100 sure that my sd card also ok , because its working with my Destop , Laptop , Note2 , and on my wife Note4 like a charm , also when i test the SD card , PC’s does not find any problem , i also Low level format My sd disk and make surface test , unfortunately not found any problem , I also formatted my SD card with SDFormatter4 which is provided by sandisk , and it has no errors again , also , i checked the compability of my 32 gb sandisk over the sandisk site , but it says %100 compitable with Samsung Galaxy Note 4 , i searched google and forums but not found any solution , so please help me what the problem might be ?? it can be related about note 4 software bug ?? maybe something wrong about android 4.4.4 ?? , any user like me ?? like this problem ?? please help , sorry for my bad english

There is many posts on xda about sandisk cards failing in samsung phones. I dont think anyone has definite answer what is the problem. For some cards work for years, for others failing quickly. It appears that samsung cards dont fail when used in samsung phones. Is that coincidence?

My suggestion would be to connect the SD card to your PC and copy all your files in a safe location. Then buy a new SD card, preferably a Samsung one, from a reputable seller (as to avoid fakes) and copy your files back to it. That way, even if it has issues, you'll be able to exchange the card or get a refund. It should be much easier than getting new phones.

Dear darekz ,thanks for your answer but , did you read my post completely ?? as i mentioned there , i used that disk with in many samsung phones without having a even simple problem , but this disk makes problem only with Note 4 's and i think that to say "this model of sandisk has an bug with Note 4 with the current software at least (android 4.4.4) " is more logical , beside i did not see any post that sandisk is not compitable or fail with on samsung phones do you have a link ?? , can u sahre with me a few ?? , and also in my opinion samsung note 4 software development team must fix this bug with software updates , even this problem may will be fixed on lolipop update ?? , what you think ,

thanks for your answer but , to say put it out and buy new one is easy , we must try fo find a solution , or, we must make N4 software team to fix that bug immidiately at least , i bet that lots of people that using N4 have problem with SD disk but they even unaware of it due to music or pictures only inside on sd card (not software installed) , so i advise them to test the sd card with N4 by shutting the note4 off (power off) and after a few minutes , let power on phone and enter the file manager of N4 and let explore and check the integrity of the files of the SD card that correct or not (if the all files on SD card or not) then please wrıte the results here , , i bet lots of people will say that my files are missing , because i belive it is a Note 4 software bug so i advise all please test your N4 with SD card and verify that if is working or not and write the model no as well , especially Exynos user
so , if we sum all of it , I think that this problem is a general problem and its a bug between with Note 4 s and SD cards , please all note 4 user test your SD card if you have and write the results here with the model of SD card and phone as well by including OS build Thanks for all

I understand what you're saying and I agree that a solution has to be given by Samsung, if possible.
However, even if they do acknowledge an issue -which is doubtful, since most SD cards work fine- they need to find out whether it is something they can issue a software fix for and, if so, integrate it in an update some time in the future.
All I am saying is that it is going to be a lengthy process, if done at all.
So while you are right to be frustrated with this issue, the only immediate solution to it is to get a new SD card.
Good luck whatever you choose to do!

Thanks for your quick replys but what is a interesting point is that , now i am using my 32 GB sandisk as an flash disk with an Usb adapter on pc without a problem , also i am using my old Sandisk which it is 8 GB and even its class 4 with my note 4 without having a problem as well but, unfortunately the 8 GB is not enough for me and now , i am affraid to buy an new SD card that if the problem happens again , because as i mentioned before i could not evaluate that what caused this problem with my note 4 , also i am not sure that the issue because of sandisk , because my old 8 gb is also an sandisk and works perfectly without problem since 3 days , may be the problem is about classes of SD cards , because the 32 gb one was a class 10 card , the other one is class 4 , so anyway , i want to buy a new card which one you advise ?? i want 64 GB also if you have an card that just you are using with yours N4 without a problem please advise me , thank you

I have a similar issue but mine is that some files are getting corrupt (mostly pics and videos) even after many formats through pc. I have a 128gb Samsung sd card so it can't be compatibility issues. Maybe something wrong with the sd card reader on the N4. Can't find a solution yet. :/

lol my friend , you are absolutely right if you read my thread from the begining to the end , the problem is neigther N4 's nor sd Cards , and its a compability issues with SD card and the N4 reader on Android 4.4.4,even i rooted my N4 and installed ExTSDcard fix application from google play but it did not fixed the problem, but i did not test on lolipop (i never updated to lolipop) , only the solution that i found , i am only running an 8 GB MicroSD card (class 2 and Sandisk) about 20 days without having a problem for now , i think samsung may apply a update soon to fix this issue

Nope. Nothing new on lollipop. Already updated and I'm having the same issue. :/ Gonna try the Samsung card on another phone and another card on the N4.
So.. You have no problems with a class 2 card? Maybe it's something with class 10 sd cards.

Yes , i confirm that I have no problem with my N4 and old 8 GB sandisk card , no more any corrupted files and missing aplications (which are installed and moved to External SD card) but it is such a slow card if you compared it with a class 10 card and sometimes applications that installed on ExSDcard, the application logos get dark (like missing shortcut) on first boot for an about 2-5 second , then it became normal logos after 5 sec (i think this is because of the speed of SD card that which is class 2 , and when you turn off the phone , and turn on the phone again or restart the phone , it takes some time to mount the slow SD card volume again for prepare a usuage ) so this is acceptable and data integrities are ok but the problem is that the class 2 too slow for a Note 4 and i never seen an SD card as class 2 with High capacity like 32-64- or 128 GB , and i am also agreed with you and sure that its a speed related (class speeds) issiues that samsung must fix it immidiately with a firmware or with an small patch update

Yep. Surely is something with class 10 and maybe more. Sadly there are no high capacity class 2-4 sd cards, and they are very slow compared to class 10. I have lots of data so I can't put it on a low-medium capadity card. For now I can only save all in my pc.
I have a 64 GB UHS-1 Sandisk that fail after 1 year of usage.
Many confirmation of defective batch of Sandisk microSD especially with large size (64GB & above).
Sandisk even confirm this problem & offers free replacement.
It may be out there but I never read about 32GB failing except for this one.
But I can recommend Samsung EVO 64GB UHS-1 as it works perfectly & I bought 2 of it
You can quickly solve this problem by buying Samsung EVO UHS-1 or you can wait for Samsung / Sandisk solve this which probably takes years or may be even ignored as the problems is minor to their statistics.
Something to remember regarding using microSD in Android:
If you install apps into extSdCard, it may take longer to initialized after Booting.
While waiting to mount your microSD, all installed apps that located in the extSdCard will be broken.
The waiting time varies depending how many apps is in the extSdCard.
On my Note 10.1 (GT-N8000), I have to wait for almost 2 minutes before everything is initialized properly (I have almost 500 apps & many of it is in extSdCard).
On my Note 4, only a few seconds (fewer apps & faster CPU).
Widget apps won't be working if you move it to extSdCard
KitKat (and may be Lollipop) has SD card restriction that can break many apps.
microSD is using SSD technology therefore there is Write limit.
If the Write Limit was reached, you can only read your data, but can't write/modify/create.
It means, you can open documents, listen music files or watching video files in it, but if the limit is reached you can't delete/edit any of it. You can't even create a new file/folder.
In my failing Sandisk UHS-1 64GB microSD it shows all symptoms.
Moving many apps into your microSD, MAY increase the WRITE activities in your microSD & resulting in decreasing it lifespan.

My friend , thanks for your quick reply and yours helpfull informations but, can you explain me some of the points that then WHY ??
1 - Even there is some a few users that they have Samsung SD card and they use it with Note4 and some of have even have same symtoms like mine ? (please note that N4 and Sd card are not fake and i have a few links if required , i can post it ) ?
2 - If my Sandisk UHS-1 32GB microSD damaged or not stable , then why it is working with my old Note 2 - S3 -S4-PC or even on a other Android devices even without having a single problem then ??
3 - If Sandisk cards is not suitable or not recommended by Note 4 s or for Samsung then why My old 8GB sandisk (class 2) worked about since 3 years on my note 2 whitout having a single problem , or it is still working on with my Note 4 ??
4 - is' nt it so dictatorial that Samsung N4 series wil allow only Samsung brand SD cards ?? , but i think that it is a Generic MicroSD card slot , but if not so , samsung might supply a user manual with the phone that like; this brand and this ..... is compatible with the SD card slot , were a better idea (like a some of PC motherboards that some of mension that which brands are verified or %100 compitable for RAM modules) , because there are most of other brands of SD cards that also in good quality lilke Patriot, A-DATA , Kingstone , and .... soo )
5 - Lets think about SSD technologies (that is the fastest and the best Hard disk or Storage solutions technology ever in now a days ) , they have some of IOPS high ratings (input/output per sec.) like 5 - 10 M and think about a windows OS pc that equipped with an SSD drive , then why it does not gives errors like a blue screen or fattal error like Disk I/O errors when you are using it on too many process at the same time ?? , even on my note 2 i remember that while i were listening mp3's i were playing a slot game and at the same time phone downloads an update for an application , and were installing it , and it was performed before too much times like this on before ?? (please note that all the operations were on SD card because , mp3 ' s were on SD , the game that i played were also on sd card , and the updated application were also on SD card , none of them were not on phone main memory )
finally , i still belive that it is an sd card compability bug with Note 4 , and samsung must solve it ASAP with a firmware update or patch

In statistical science there will be around 0.5% -5% margin of errors in any mass productions.
So if Samsung/Sandisk produce 10 millions of products there are around 50,000 - 500,000 defects.
Of course they will try to reduce the margin of errors as low as possible by Quality Control.
The thing is QC is only taking random samples, therefore the possibilities of defect out in the market is still there.
Thus either Sandisk/Samsung will have "a few" defects in their products.
By this, I clearly stating that I'm not saying that Sandisk is bad & Samsung is better.
It's just some of Sandisk product batch in "large size (64 GB & up...+ probably 32GB too) microSD" is bad.
And from my knowledge, Apple iPhone 6+ 128GB is also suffering a problem with their SSD chip, so it's not only Sandisk & Samsung.
And remember that XDHC SSD tech in microSD is relatively new tech, thus glitches is expected.
"Many" is relative, 100 complain is plenty if the the total products is 110.
10,000 is a few if the total products is 5,000,000 as it less than 1% & usually manufacturer "ignoring" this case as it's only a minor case.
They will focus more on creating a new & better product than fixing the "insignificant" defect.
The best thing a buyer like you & I can do is: claiming warranty & get a non-defect one (or another device).
Another question for you: is it working for opening files & deleting/creating files or only for opening files?
My failing 64GB Sandisk is still accessible but only for opening file.
If it is working for both opening & deleting files/folders then most likely the Note 4 product batch in your area store's is bad (in the 2% - 10% margin).

This question is OOT (Out Of Topic), but I'll give you an answer anyway.
SSD is not HDD.
In SSD tech. there is no moving parts = only Chip(s)
In HDD tech. there is moving parts = Disc platter + reading head.
SSD & HDD have a different lifespan.
SSD has a limited write cycle & HDD will wear out it's motor (for moving the Disc platter+reading head) over time.
BSoD/Blue Screen of Death in Ms Windows environment is caused by Hardware/Device driver malfunctions & not because doing multitasking.
The malfunction causes are varies widely from defect product(s) to end of lifespan.
The speed of SSD tech. is also varies depending on what tech. is use on the chip (YES SSD technologies is also varies).
Samsung is using OneNAND tech. & Sandisk is using mDOC tech. but not limited to only those 2 tech.
Android is basically a Linux OS & in Linux, running apps in RAM is the best practice.
So, it's likely that Android will try "pushing" running apps into RAM no matter where it's origin (either internal memory & microSD)
It only accessing Internal Memory/microSD if "needed".
I don't know how old are you or what is your knowledge background, but my best suggestion is try another microSD that uses different SSD tech. from Sandisk such as Samsung (or others).
Another suggestion is returning your Note 4 & try to buy from another Store in another area, because the nature of manufacturing that produce mass products in batch & distribute it also in batch thus if the batch is mostly a defect batch then it's highly chances most other unit is another defect one.
Hoping for a solution from the manufacturer for a problem is a way, but it's better doing something than waiting

"Many" is relative, 100 complain is plenty if the the total products is 110.
10,000 is a few if the total products is 5,000,000 as it less than 1% & usually manufacturer "ignoring" this case as it's only a minor case.
They will focus more on creating a new & better product than fixing the "insignificant" defect.
The best thing a buyer like you & I can do is: claiming warranty & get a non-defect one (or another device).
Another question for you: is it working for opening files & deleting/creating files or only for opening files?
My failing 64GB Sandisk is still accessible but only for opening file.
If it is working for both opening & deleting files/folders then most likely the Note 4 product batch in your area store's is bad (in the 2% - 10% margin).

SSD is not HDD.
In SSD tech. there is no moving parts = only Chip(s)
In HDD tech. there is moving parts = Disc platter + reading head.
SSD & HDD have a different lifespan.
SSD has a limited write cycle & HDD will wear out it's motor (for moving the Disc platter+reading head) over time.
BSoD/Blue Screen of Death in Ms Windows environment is caused by Hardware/Device driver malfunctions & not because doing multitasking.
The malfunction causes are varies widely from defect product(s) to end of lifespan.
The speed of SSD tech. is also varies depending on what tech. is use on the chip (YES SSD technologies is also varies).
Samsung is using OneNAND tech. & Sandisk is using mDOC tech. but not limited to only those 2 tech.
Android is basically a Linux OS & in Linux, running apps in RAM is the best practice.
So, it's likely that Android will try "pushing" running apps into RAM no matter where it's origin (either internal memory & microSD)
It only accessing Internal Memory/microSD if "needed".
I don't know how old are you or what is your knowledge background, but my best suggestion is try another microSD that uses different SSD tech. from Sandisk such as Samsung (or others).
Another suggestion is returning your Note 4 & try to buy from another Store in another area, because the nature of manufacturing that produce mass products in batch & distribute it also in batch thus if the batch is mostly a defect batch then it's highly chances most other unit is another defect one.
Hoping for a solution from the manufacturer for a problem is a way, but it's better doing something than waiting

Anyone else getting this with their S4? I bought my S4 two weeks ago and a Sandisk 64gb card to go with it and started seeing this error about twice a day. I returned it for a Samsung SD and didn't see the error for almost a week until today.
The card actually appears to be fine, all I have to do is remount it after seeing the message and all is good, it's just worrisome cause I store all my photos on there. I'm wondering if it could be the phone itself? I have one day left to exchange it, but it's rooted.
SD Errors are because the card is physically moving when its being written to. This cause bad sectors on the card. Try putting a piece of thin scotch tape and cut to shape of the face of the SD Card. See if it fits more snug....
Are you jogging with the S4 or something? Samsung SD's are class 10 which I think write faster. Maybe that is less likely to cause damage because its written so fast? Not sure....
Try the tape trick and see if it reduces the problem. Else its a physical problem with the slot.
i also have a class 10 64gb, some guy on the forum posted this problem too, he said that by formating the card with a PC and then formating it with the format utility in the phone settings has solved his problem. If not, might be a slot problem like slider mentionned

I had never had an issue with a Sandisk card until my S3 ate a 32GB. I then started readibg how the S3 ate non- Samsung cards. I have been using a Samsung card with no issues

I had tried 3 different San disk 64GBP class 10 cards and had this problem, the fourth one works so far...doing some searching I found lots of complaints from lots of different device forums about this card. Only fix I found was running chkdsk repair on the card in Windows but this only worked for a few people ( couldn't save my first three cards)
anyway, imo exchange the card or better yet buy a different brand

Posting here as doing a search revealed this is the closest to the problem that I'm having. I've had my AT&T Samsung Galaxy S4 for approximately 1 month now. I purchased a Sandisk 32GB SD Card for the phone. Two days ago I began experiencing issues where when unlocking the phone I would notice the card would unmount then remount (this was in the notification bar). Later that night, the card unmounted and there wasn't any way to get it recognized again. I posted in some other forums and someone said there were some good posts here.
I've contacted Sandisk regarding the card and they refused any involvement stating it's a problem with the phone, not the card. They determined this because once I was able to get the phone to recognize the card and plugged the phone into the PC, the card was able to be read. As long as it's readable, it's fine. After dealing with this I eventually found my MicroSD to SD reader and plugged it into my media reader in my printer. I had quite a few issues from time to time reading the card, however I can't 100% for sure say whether this is the card or the printer.
Anywho, I grabbed copies of what I needed. Formatted the card on the PC, then formatted it on the phone. I began to copy my entries however at one point instead of saying I had 29.* GB Available, it said something like 2.0 EV of 3.0 PE or something like that... these weren't the typical numbers I've seen nor the units of storage space. I apologize that I can't recall that information. I reformatted again and it worked. Left the card in the phone empty over night then tried to copy files later today. This worked.
Now, approximately 48 hours after the problems began, I noticed more issues. SD card stated that it had been damaged and that I should reformat it. While this message stayed in notifications, I confirmed I was still able to read files on the SD card. When googling that error hoping I'd find more resources, the card again unmounted.
One of the resources I found had me do a CHKDSK /F /X on the card. I did this and found no issues. I've replaced it back into the phone and it seems to be working for the moment.
1) Are there any confirmations if this is a bad card, or if perhaps any applications are causing the card to do this? I do have QuickPic and have enabled some of the hidden file features if that helps.
2) Do we know if there are any logs that can be pulled to help verify this?
3) Is it a specific card? (I've heard SanDisk 32gb, but also have heard non-Samsung SD will fail)
Looking to get this resolved ASAP. This card was purchased directly from AT&T when the phone was purchased. I am just looking for some confirmation that if I have to buy another card, I'm not going to gamble if that's going to have the problem as well.

Edit: Per the thread, this is a Class 4 card. Also the phone was not moving when it was writing data. I've been couch-ridden all day and unlocked it. I don't believe the phone was in deep-sleep mode as I had received a text not long before I began noticing these messages.

It's the 2nd time that my SDcard stops working. Overall using SDcards on my SGS3 and now SGS4 - I have lost 4 cards (in a period of 1.5 years).
I am making sure that I am buying a Class-10 cards and they are usuaully 16Gb or 32Gb (the one that I've just lost is a Polaroid 16Gb class-10 - not the cheapest one).
What I see right now is that some folders has files that seems to be corrupted. For example DCIM folder has a file that looks like uAMERA (instead of CAMERA) and its at a size of 0b (this is a folder obviously and I cannot modify that name anymore).
All the files under the Titanium backup has weird names now.
My questions are:
1. Is there any type of tool (something like an 'fsck', etc.) that can actually try to recover whatever got corrupted on this card?
2. Is there any recommendation, anyone who had a good experience using an SDcard over 6 months or so, What should I buy as the next SDcard?
3. Is there an application that will check the SDcard 'health' and will, hopefully alert, that something is starting to get wrong with the SDcard? (not sure if that is even possible).

Get Samsung microsd card. Been using it since I got my S4 and no problems so far.
After i had a few cheap memory cards fall apart, i now only buy Samsung as they are very durable, see below.
Samsung Memory cards can endure 24 hours submerged in water and survive being run over by a 1.6 ton vehicle (SDHC, SDXC), not to mention the perils of airport x-ray machines, magnetism and extreme temperatures.

I can recommend you an ADATA micro SD cards (as well as SD cards). It`s got great performance, stability and price. Now i`ve got my third one which is 32GB ADATA mSD card with 28Mb/s read and 18Mb/s write. (five or lifetime warranty depends on country)

I've had 2 issues with my SanDisk 32GB Class 10.
First time it stopped mounting and kept trying to "repair itself." Brought it to pc.. the card had all my files but it was super laggy. I couldn't access anything, even though the files were there. I ended up formatting. Worked afterwards.
Second issue was it un-mounting and showing blank or unsupported files.. I was watching an episode of The Simpsons, the next file I opened was a Family Guy episode.. that's when it decided to stopped working. Restarting phone didn't work so I shut down and removed the sd card.. worked after reinserting.
None of my files were corrupted/virus
But so far..its working. Good thing I sync with Dropbox.. would have lost all my pictures and video's..
I'm using Transcend MicroSDXC 64GB UHS-I. At first i got bad memory, the card stop working or got error after copying 31GB of data or if i format the memory (non-quick format) it will give error after 51%.
After replacing the card, it looks ok now.

From what I took of that TL;DR-filled post is that your Sandisk SD Card is failing. Which model specifically may I ask was it? I had my Sandisk 32gb Ultra stop working for my Note 2 a few weeks back. At first I thought it was my custom rom, but I learned that it is actually a wide-spread problem with Sandisk cards on higher-end devices. Though eventually it got to the point where a week ago, the Sandisk card just stopped working period on any and all devices. My advice would be to call support to get a free replacement via warranty or a refund if possible, and buy a Samsung card instead. Something like a Samsung Pro 64gb SDXC microsd card would only cost $60 and have more than double the speeds of the Sandisk, while actually not failing.
What is the exact model of your "high end SanDisk card"?
I've had 3 64gb SanDisk cards (bought them in different times over a year ago), the first two failed after few days, while I was trying reformat them to fat32 on computer (TWRP didn't recognize exFAT cards back then), they would just become unrecognizable by computer or phone. Very much possible my fault.
The third card I left alone and it was working "fine". However progressively quite a few files got corrupted. Many photos became 0 bytes (this is without removing the card from phone). Good thing I had Dropbox and G+ backing up photos.
3 weeks ago I bought Samsung card, twice as fast, for less money, and so far I have no issues with it whatsoever, in fact I think the phone has less lag, especially the gallery (granted I did remove most videos from it)
Although I should point out, that I did transferring files from old card to new by using usb adapter and copied everything to the computer first then on new card.

Mine also works without problems in my S5. I get another (Samsung brand } sdcard in tomorrow to test. If that fails too, I'm going to exchange my Note 4 for a replacement

Thanks will give it a test, I'm currently using a 32gb integral to see if it's the Note 4 that is the issue or the Samsung sd. Kinda annoying since I have 4 times this issue during weekend

I've just bought a Moto X Play ( UK ) which comes pre-installed with Lollipop from carphone warehouse, I have Samsung 64 GB SD Card Class 10 which is official SD card, been using it on my old S4 phone & computer without issue However when used on my Moto and reboot the phone I get a message saying the SD Card is damaged, and I can't access anything.
If I put the card into my computer or old S4, the card is fine and can be read no issues. i've been playing around with it and what i've noticed is that I think I can now replicate this problem at will and just wondered if any others are noticing this problem.
To replicate the problem:
1. Move an App from internal to SD Card.
2. Reboot the phone, the phone will say the "SD Card is damaged, try re-formatting it" ( don't do it! )
To resolve the corruption, I just took the SD Card out of my moto put it into either my Samsung S4 or a PC, go to the SD Card and clear down the contents of the "Android" folder sitting in the root of the card.
Put the card back into the Moto on startup, the phone is happy with the SD card!, no corruption and all files ( Music, Video, Images etc ) work without issue on the moto.
Repeat the steps again and bang the card goes corrupt again!, very annoying.
It appears to me that there is a bug in the moto SD card implementation which needs addressing as I can't put apps onto SD card and re-start my phone without this problem arising.
I've tried replicating this fault with another memory card and get the same problem
Can anyone else confirm if they are having the same problem?, if you aren't could you tell me what brand of memory card you are using, and the exact software version you are on?, when I do a software update it says I have the latest version.

I had the same problem with a 32gb Samsung Evo SD card, yesterday I searched for a solution on the internet and found some suggestions about to format the card from Windows with "quick format" unchecked.
I gave it a try, it took 10-15 minutes to format it but I had no more problems so far!

What I did was to buy a new memory card from Curry's, won't buy an SD card off places like amazon/ebay due to the number of fakes knocking around on there.
I got a 64 GB PNY Class 10 memory card ( £17.99 ) , plugged it into the phone ,it was instantly recognised, I decided to Erase card using the Menu option before moving apps and data onto it and everything is working fine so far, have re-booted the phone several times without any problems!
I think probably re-formatting the existing Samsung memory card in the computer would probably do the trick as well. however I've got a new use for that memory card as it's going to be used in the car sat nav instead.
It's a strange issue but i'm glad it's solved.

I found this good utility for SD Cards, download it on your PC and try scanning your SD Card with this tool, perhaps there is an fault with your sd card, it's called macrorit , I can't post a link as the forum rules won't let me.
There is a free version for download, just go to google and type "macrorit download"
This tool is also good if you need to format a 64GB SDCard as FAT32 instead of exFat, something my car sat nav wants and Windows 10, with diskpart can't do
I am sure there are bugs in both the Android 5 & 6 code but when it comes to SD cards my first suspicion is the card itself. You have to understand that these things barely work and are only marketable because they have internal processing that corrects problems on the fly. Even manufacturers with the best reputations produce flakey batches of SD cards. My experience has been that cards can have invisible latent problems which manifest only when usage changes. eg: a swath is bad but never exposed until a size boundary is crossed, or a storage allocation scheme changes with a new or updated filesystem.
Micro SD cards are vulnerable to physical handling problems including excess g force or static electrical damage.
FWIW, my experience:
1. If even one device reports a problem then the card likely is not 100%. Replace it.
2. Never store valuable and irreplaceable data on it without an active and complete backup process in place.
3. Buying an expensive high capacity device? Get the extended warranty or in store replacement extension. Keep the packaging with all of the batch coding on it. Make sure the replacement is from a different batch.
Also, there are a lot of counterfeits. Buying from a big store says nothing about the true nature of the device.
Hard drives: Very good, predictable service times
SSDs: Almost as predictable.
SD cards: Very unpredictable even if not counterfeit. Easily reprogrammed by anyone with some tech savvy.

One more thing, if the SD card is 100% ok then you should be able to completely rewrite the card every day for at least a couple years.
If your card is under warranty and giving an error in even just one situation then back it up and throw a heavy "burn in" program at it. Let it run until it dies and get it replaced. Better in a burn in program than in your phone.
When you get a new card, make sure you run a tester that writes/reads the complete capacity at least a couple of times. Make sure the data is random.
Again, my opinion only.
Never think of SD cards as being reliable like the phone internal storage. They just are not. They could be but then you would be paying a lot more for them.
I bought a brand new 32GB strontium class 10 card and only managed to use it for a day before the phone kept notifying me that the card was removed and I have to restart the phone to be able to use it again. This happened multiple times before I changed the card with my other one, bought 4 years ago, a class 4 16gb sandisk. no problem whatsoever with it. Now I'm getting a replacement for the new card.

I have the same problem here with my Samsung 64gb SD-card. Did you find a solution? Before I tried to format it as an internal device is was using it as an external sd card and didn't met any problems.

I used the suggested macrorit utility to check sd card integrity.
I erased, formatted the card on a pc. Then I did several cycles of write, read with macrorit, erase. All these cycles went good. The card is clearly ok (this is the second one, the first was returned but I suspect it was good).
I decided to return back the phone. Appraisal for amazon who agreed on a money back. Excellent.
This phone is awesome, but I'm tired to go around finding bugs and losing my pictures. I'll look for something else.

the issue is with the andriod / phone . i had the same issue as i twerk with electronics i had tried checking the sd card on my pc and all the data is present perfectly . I have then used a spare SD card in the phone and then removed it after config and then used the issue SD card . to my surprise it is working . this i have done multiple times . for some reason when we eject the sd card the system feels as corrupted SD card and ask for format . Hope this will save ur data if you dont format before checking the same in PC.

Have been using SanDisk SD card 64gb class 10 from the day when i bought moto x play ....using it as portable storage only didn't formatted it as internal storage....now suddenly while using the camera.... Camera was unable to click pictures anymore..... it forcestopped whenever tried to click any pics......due to this i restarted the phone once....after that SD card shows for a while after phone been started n the card disspears....
