Does anyone have any ideas why my rooted S5 would lose the stored WiFi passwords if I manually turn off WiFi? Once I turn WiFi back on I have to re-enter the password for WiFi networks and that doesn't seem right.

what are the steps you "manually" turn it off? are you removing or forgetting the wifi?
Do you get any fcs?
Are you using a rom slot?

what rom are you on?
i was on ricks rom and had the same issue.
i did a clean wipe and flashed stock rooted 4.4.4 and all was fine.
i would try that

After updating my google play unlocked Nexus 4 to Kit Kat 4.4, I've been constantly having problems with my WiFi being stuck in off mode.
Turning on WiFi would get the toggle stuck, while it's Grey too, and it stays stuck. Solution was to turn on and off airplane mode. Then wifi works again.
I've now realized this is triggered every time I try to enable the mobile WiFi Hotspot. It shows Error next to the check box and no WiFi network is broadcasted. I tried barnacle tether and FoxFi, and neither work. Has kit Kat broken any WiFi broadcasting? Tethering works just fine for me over USB. Something is wrong with the WiFi. Has anyone else been having this problem?
Here's some screenhots.
I did a backup in CWM, then I did a factory reset, and then I restored the backup. Everything started working just fine again.

A factory reset and cache clearing was the only sure way of fixing it. Did you by any chance install the moto x dalvik libraries?

Have you tried using Googles factory restore image? Or the OTA?

I used the OTA... tried the suggestion in your OP and it did not work... Trying again with the full Google image...
Full Google image worked.

Alternate way is, if you rooted, delete directory

I'm having the same problem with my nexus 4 when I turn on wifi hotspot.. I get the "error" message..
I don't completely understand everything in order to fix this.. Please help.
Hi all, I lost all wifi passwords after reboot. All the passwords i had in my s4 for wifi networks are gone! I've updated Adfree and reboot and… gone, everything's gone! Everything else seems fine, settings etc, any ideas?
I've recently had this problem this weekend, can you shed any light on this how did you fix it? or did you have to factory restart?

just to add, I didn't reboot or anything, mine just disappeared
Install root browser n open the file wpa_supplicant.conf there u ill get network name n its password...this only works on rooted devices

I didnt fix it, I added the passwords again and since then I havent had any problem. So its still a mystery but im sure that has to do with Adfree?
Is there a way to unroot without factory reset? Basically the only reason I want to root is getting the Hotspot enabled but now my company is not allowing me to connect to exchange server with rooted device. I would hate to do a factory reset and loose my hotspot feature. Any way around this? I've tried using xposed rootcloak but unsuccessful so far. TIA

Do you have SuperSU app on your phone? if yes, open the app, go to settings, scroll down and you'll see Full Unroot
Thanks zlebors, I didn't know that option existed. I got it now, working as I wanted with unroot and hotspot enabled on my unlimited data plan. Tks.

Typically when you full unroot through supersu it still shows system custom. I don't know if your exchange server sees that or not.
Have you tried fake system status in Wanam exposed along with root cloak?
Beware of the incoming ota! Be sure you have sdm and fota froze so you don't take ota and loose tether.
If you take the 4.4.4 update you will lose root.

I have tried rootcloak from xposed with many com & app, nothing seems to work. I ended up installed nonroot rom, connecting to exchange server, turned off exchange auto sync, root, installed xposed, mototether from xposed, reboot, uninstalled xposed & mototether, reboot & to ensure hotspot continues to work, unroot using simply unroot from app store & reboot. Hotspot & email remain working.
I managed to root my device and successfully installed xposed framework, and now I can't get my wifi to work at all.
Does anyone have any idea how to fix this other than going all the way back to stock?
I have tried flashing the mm modem. It still doesn't work.
I did notice when I looked in the build prop the WiFi channels are blank?
Is anyone who has wifi working able to see what they normally say?

This happened to me as well. I made sure to have the "uninstall xposed" zip on my phone and flashed that, and after I did so, wifi was OK again. Here are the xposed modules I installed:
flashed: xposed-v82-sdk23-arm64.zip
I never got as far as installing the xposed installer apk, as when I noticed wifi was gone, I flashed the uninstaller:
xposed-uninstaller-20150831-arm64.zip
I personally used xposed-v81-sdk23-arm64.zip and it seemed to work well without breaking wifi. For what its worth, rooting with the latest SuperSU broke my wifi, but flashing BETA-SuperSU-v2.62-3-20151211162651.zip worked out alright.
As far as reverting the changes you've made so far, if you are sure it was xposed that broke your wifi and not SuperSU, then flash the xposed uninstaller. If it may have been something else, just reflash boot and system from a TWRP backup (hopefully you backed up). If for any reason you dont have a backup, download the stock image for whatever version you are currently on, put it on your SD card, then in TWRP, backup your data partition, then flash the firmware zip over, and then restore the data partition backup, and in theory things should work out, no guarantees though.
OK, just flashed the xposed-v81-sdk23-arm64.zip and the wifi is fine. I am getting "unfortunately messenger has stopped" messages now. I've installed Gravitybox for MM. The tweaks seem to be working.
I was working on my dads phone to get it rooted. Downgraded to 4.4.2 to root it then odind back to ok2 firmware still rooted but his Bluetooth doesn't work. Any ideas why it doesn't? I disabled some apps that I thought were bloatware but is it possible an app I disabled was a BT helper app of some sort?
