Hmmm, does the app at least work on a rooted note 4 ?

When you start the app you open the first top menu that says install package tools see first pic then select the top right tap see pic 2 then select find dialer codes and wait a bit and a bunch of system apps pop up see pic 3 then click on each app and the related dialer codes drop down see pic 4 hopefully this helps unless the app is just not compatible with 4.4.4 but there is a new version 14.5 out so make sure to update it hope that this helps.

Where should I go in this menu? Selected the first [1] menu, now the phone starts "Blocking mode" what's this???
First hit menu then back. Then I hit menu-select-4 and and then hit menu then back and it gives me a list of options. Never messed around in there but I just looked around to see what was there. I tried 1,2,3 and phone just rebooted but nothing really happened.

Hm...
I do *#9090# and see:
DIAG CONFIG
[1]DM/DUM over HS-USB[]
[2]DM/DUM over HSIC[]
[3]DM/DUN/RmNET over HSUSB[]
[4]DM over UART(*)
[5]DBG MSG ON(*)
[6]DBG MSG OFF(*)
Could someone check what the defaults are?
Ok did the sequence and found it
What is "blocking mode enabled"? Did I push something wrong?

Tried it and it didn't work after put in my tmobile sim.
Tried it and verified that Perso SSH 256 was off and when i popped another carrier sim, it still said sim locked. Caved in and purchased the unlock code form cellunlocker.
p.S: Don't forget to switch Perso SSH 256 back to ON if you want to unlock it via cellunlocker code
Tried it as well and didn't work. So used unlock code.
Thanks @mikron15 for letting me know to turn SSH 256 back on as I was getting unlock unsuccessful.
tried it as well with no success... any other suggestions?

How to SIM unlock Samsung Galaxy S4 SGH-M919 (tested - works)
Hi Everyone,
I'm a long time lurker and benefited greatly from the postings here at xda.
So I'd like to contribute something back.
After trying many methods and reading all the posts in the various S4 forums, I finally found a way for SIM unlock that works for me.
I have tested it, and it works on my T-Mobile S4.

chavxxx's post pointing to the right final step
Here are the steps:
Goto phone dailer, and enter *#27663368378# for the debug service menu.

In "UMTS MAIN MENU", select [1] DEBUG SCREEN

In "PHONE CONTROL", select [6] NETWORK LOCK
In "NETWORK LOCK", select [3] PERSO SHA256 OFF
It should show:

SHA256_OFF => SHA256_ON
Now touch the first line SHA256_ENABLED_FLAG[1]
It should show:
MENU NOT EXIST
PRESS BACK KEY
Current Command is 116631
Now press the menu button and select "Back"
It should show:
SHA256_ENABLED_FLAG[0]
SHA256_OFF => Not Change
Again, press the menu button and select "Back", and you're back in the "NETWORK LOCK" screen
Press the menu button and select "Back", and you're in the "PHONE CONTROL" screen
Press the menu button and select "Back", and you're in the "DEBUG SCREEN" screen
Finally, press the menu button and select "Back", and you're back in the "UMTS MAIN MENU" screen

In "COMMON" screen, select [6] NV REBUILD
Now you are in the "NV REBUILD" screen, you should see the following text:
------------------------------
Golden-Backup exist
You can Restore Cal/NV
------------------------------
Now for the final step.
In "NV REBUILD" screen, select [4] Restore Back-up
The button lights for menu and back should come on, and the phone will auto reboot in a few seconds.
Note: chavxxx's post of selecting "[1] Load default NV" DID NOT work for me.
Congratulations, your phone is now unlocked.
Stick in a sim card from a different carrier and try it out.
Please post followups of your experience with this unlock method.

Make sure you are not on the MJA update. You will not be able to revert to earlier firmware to complete the original sim unlock procedure. You will have to have Sprint Worldwide unlock the sim for you before doing the US gsm unlock.

'Hidden menu" on our note 3 is not enabled...
you need to be rooted...
And you'll need Root Explorer...
Go to file EFS/Carrier/hiddenmenu
Open with root explorer text editor
You will see the word "OFF"
Delete and type in "ON"
Use capital letters
Hit the back button "you have made changes to this file do you wish to save?"
Click yes
Reboot
You now have your hidden menu but I don't know if all the codes are the same as previous Samsung devices... I don't mess with it to much... I just used it to turn off the wifi power saving mode.

Not anything that I know of...
If you go to a Best Buy store when the Samsung specialist is working... they are supposed to be able to plug in and enable that menu to do their tests before giving you a warranty claim... maybe if you found one of those dudes and bought him a beer?

No need to enable the HiddenMenu anymore.
You can use 'Phone INFO ★Samsung★' app --> go to 'Secret Codes' menu, or use 'Shortcut Master' app --> go to 'Secret Code Explorer' menu to launch the secret codes (ROOT is required).

If you have root, just open Phone INFO ★Samsung★ app > Menu > 'Secret Codes' > scroll down to *#27663368378# for Service Main Menu or *#197328640# for Debug Screen. Tap and select "Launch 0' to launch.
If you don't have root, open Dialer, enter *#0011#, Menu > Back item (not Back key), Menu > Key Input > Q0, wait some seconds...
maybe
download this app scroll to the bottom click additional info then phone information then scroll to the middle of the page on drop down box and try one of the settings im not on a gsm carrier so im not sure which one

*#0011#
Menu>Back
Menu>Input>Q
Menu>Input>0000 wait some seconds and Hello service menu
Make sure you write the letter "q" with capslock on

If that doesn't help, you may need root and use Phone INFO ★Samsung★ app > Menu > 'Secret Codes' to launch the Hidden Service menu.

Before hand i am very sorry that I bump an old thread however, this pertains to this and i can't find any info on my current issue and following you has got me the farthest along with my problem.
My galaxy note 4 reads no service, so I tried the *#0011# approach (no root) could not find the menu, with the input method
Then I rooted it and used your app and was now able to access the menu.
However I still can not find the run ping test option.
My firmware is version 5.1.1
Model: SM-N910T

"run ping test option", what do you mean?
I have never mentioned it before so I have no idea on it.

I am trying to fix it so my phone no longer reads No service, It seems like the fix is to go into service menu and run a ping test from basic info, but thats just what i have read in other forums.

Phone Nightmare, i dont know what i have done :(

i just bought the phone yesterday screwed up and ive been working on it for over 15 hours and im hoping someone of expertise can get me out of this issue.
I have a N9005 , SN: RF1D963X6PD
I live in Canada and use Wind Mobile (AWS BANDS) same as tmobile. I bought the phone and insterted my sim and noticed that i would only connect roaming. I then figured out that i had bought a international version and it didnt have the bands i needed to connect to AWS bands. So stupid me... not backing up anything... at all, thought it would be fine to use QPST and restore it with a AWS file that i always used to enable aws bands on a S4. Didnt work so i came across this thread and noticed someone mentioned somthing about using the note 2 qpst file , that also didnt work.I then noticed that my phone showed no bars or roaming. I go into mobile networks settings to search for a network and it just searches and never stops or sometimes it will say error. My imei still shows.
Im on Sammobile.com trying differant firmwares for the n9005 with odin and no luck, they all boot up and show no bars, no networks nothing, i feeling im looking in the wrong area of where the problem is. Im not sure what i have done. All help is greatly appreciated. Thanks

I don't know where I should start from, but here are some suggestions:
1- Try to flash the original stock firmware for your device. You can find your original CSC using Phone INFO ★Samsung★ app, look for the 'Original CSC' in the 'CSC INFO' section. Then open the menu 'Firmware Browser', enter that CSC into the CSC textbox to search and download the firmware.
2- Open the dialer > *#0011# (capture screenshot #1) > Menu - Back > Menu - Key Input > Q0 > wait... > DEBUG SCREEN > NAS INFORMATION > MM INFORMATION (capture screenshot #2) > Menu - Back > MM REJECT CAUSE (capture screenshot #3). And post those 3 screenshots for more investigation.
3- If you want to add some bands to the phone, you may need the Qualcomm NV Calculator app.

Thanks for responding vndnguyen,
1, i did as you said and coincidently the firmware that it suggested was the same one i had already installed (which seems to the be most stable one of the ones ive tried) so im not sure if the csc code was just reading what i had already flashed or if it was a coincidence i flashed the one i was supposed to. But i did a fresh flash, wiped everything, clean cache and flashed and samething no bars.
2, when i try to go to service mode using *#0011# , its all blank, so no luck in screenshoting anything.
3, i installed nv calculator and wrote the code for my requested on NV 1877, but no change.
I screenshoted some info in phone info samsung app, maybe that can somewhat help you with my issue. Check the attachment below. Thanks again

a- As we all know the Galaxy Note 3 was launched on September 2013, but your manufacturing date is 04.2013? This sounds weird.
b- Your manufacturing date does not match with the S/N, so the phone is refurbished. You can check this by open the menu 'Refurbishment Check'.
c- Actually your phone may be N900A, but it has been flashed with N9005 firmware. Can you show a hi-res picture of the back sticker, behind the battery?
d- As your Knox flag is 0x1, you can root your phone (if it has not been rooted), then open the Phone INFO ★Samsung★ app > Menu > 'Secret Codes' > 0011 > 'Launch 0', and follow the steps #2 above to take 3 screenshots for further checking.

yes you are correct, it said it was refurbished
ive attached a pic of the back sticker, im not sure if this may hint to anything, but the sticker on the back looks as if its on a slant (maybe someone changed it and put it on)
i tried what you said and once i click the launch 0 , the servicemode screen still shows up blank, i read in another thread that Samsung disabled it in N9005 ROMs. could that be why it shows up blank

The info on the back sticker are matched with the IMEI and S/N in the app. That means the back ticker are original. But the phone's hardware (mainboard) has been changed, it may be due to repairing or warranty etc...

Nope, Samsung only disable the service menu on some models (e.g Verizon). But with this tool (Phone INFO ★Samsung★ app > Menu > 'Secret Codes' > 0011 > 'Launch 0') you can launch any secret codes that are available in your device.
Can you show me that blank screen? And also show me the screenshot of Phone INFO ★Samsung★ app > Menu > 'Secret Codes'?

Your device has been modified in unappropriate way so all service menu do not work as expected.
I dont know what I have to suggest now. Try to flash the stock rom to get it back to the original state.

I appreciate your time and effort , one more thing before you go.
I loaded on VISION X ROM, from there i was able to capture the screen shots you requested, they are in the attachments below.
Also i looked through your app phone info samsung and it appeared all my info was quite differant from when i first showed you it, now its saying my phone is a n900t with the vision ROM, I will also attach pics below, please let me know what you think. thanks
Try flashing the stock firmware and capture screenshots again.
update
after flashin with vision x and using blaze kernel i was able to look around the service mode menue and found NV REBUILD. i just went through every menu until i found it and bam, got roaming signals back, so i was happy, now i just have to figure out how to get the aws signal working, thanks for all your help and time
So the key solution is NV REBUILD.

try *#197328640# .. if that takes you to the service mode, then open the menu bar and click "key" and type "Q" , then open menu again click "key" and type "0000" (zeros) ....
if that first code doesnt get you in, use *#9090#
For some reason, that code doesn't work on many phones. You can use this way instead:
Use 'Phone INFO ☆Samsung☆' app > Menu > 'Secret Codes' > 197328640 > 'Launch 0'.
Note: Root is required for it to work.
solved

I do but nothing works. I have signal and when i place a call the signal drops and i have the message not registered on network. I compare the efs file with my wifes 900w8 and its ok no files missing, Also did a hardware test putting the board on the other phone and nothing happen. its definetily a software issue.
