let's go to the basics
first make sure to flash your back up based on the cwm used to back it up in the first place
second check if your back up is placed in the clockworkmod folder and has no spaces
if the above fails then flash the stock firmware of your back up then restore your back up
ex:
your back up was based on DXKL2, so to match the md5 flash DXKL2 and restore your back up
ps: worked for me
Try using Cwm by madteam..

can u open .md5 file n tell wats in it?
the most posible reason behind these problem is because the backup was not succes or there was problem with the sdcard. try to reformat your sdcard in your computer without using quick format option. after that try to make a backup and restore.
good way

Hi all
My IMEI is totally gone. Fortunately, I found the backup in 'sdcard/backup/efs'.
I also found the procedure to restore it from a backup in another thread. However, I am having trouble:
According to the procedure, I have to delete the old efs folder - the one that is corrupt. I am using Root Explorer. I have mounted as writable. Everytime I try to delete '/efs', it says 'Delete Failed' ??
Solved. Make sure you backup your /efs!
And you did....?
to solve the problem.
Please share your experience or delete this (so far) useless thread.
:banghead:
I flashed a stock ROM using Odin. I was then able to delete the efs folder successfully, and subsequently restore the backup.
The guide to recovering your IMEI has been posted here: http://forum.xda-developers.com/showthread.php?t=1264021
Remind me just why did you opened this tread?

Hi,
I came from Galaxy S2, on which I learned that it is crutial to backup the /efs folder with nv_data.bin file immediately before messing around with the phone.
unfortunately I can't find the nv_data.bin file on the S4 in the /efs folder or subfolders. Where can I find it?
I have an unlocked rooted i9505
How to backup /efs on this phone?

Hi,
I am using root explorer and see the contents of /efs, but there is no nv_data.bin file in there. Is this file located somewhere else on s4?
