How can I determine which app did this so I can uninstall it immediately?

No one is going to be able to tell you that unless the app is advertising itself or someone has seen the exact same popup ad. More than likely it's an app (or malware) that seeks to generate ad revenue.. the upshot of which is that what you see in the ad may have nothing to do with the app. Google image search does not appear to have a match to it.
Still if you have any battery or power apps installed, they would have to be at the top of your suspect list.
Rather than make us try to guess, post a list of all your third party apps that you have installed and we may be able to spot the culprit in that list. Or in the alternative, install an ad blocker program on your handset.

Search the play store for and ad detector, their are many. These will tell you what app has an advertising function built in.
You could try clean master also

Another suggestion:
Install Shortcut Master app, open Menu --> 'Capture Shortcut' and do a capture when this ad popup is shown.
It will tell you this ad belongs to which app.

Did you download anything from the market recently? The market will add shortcuts if you have Auto-add shortcuts checked in settings in the market.
Just checked market settings and it was not checked. The only thing ive download was a couple launchers like a week ago and a couple themes for them.

Some free apps that can't display ads because they don't open - like themes - will automatically put shortcuts on your homescreen to ads or a webpage. It's their way of making money. These are usually the types of apps that use Airpush as well. You can remove the shortcuts and the app will continue to work. The developers probably get paid by whatever company the shortcut is for. It was probably one of the themes you installed.
Just be glad it was that and not Airpush.
Edit: if you use an ad blocker, that's probably why you get the screen you do when launching the shortcut.

It was most likely placed there by one or another crap app you've installed.

Sounds like you installed, maybe unknowingly, an app using "Air Installs".
You should get an "air push detect" app from the market, that should detect these things.

Hello everyone..I have a galaxy note 2 for about a year now I didn't have any problems with my phone until today morning when I was surfing Internet I came across this page that tell me "Your phone is infected with (13) virus"
Now since that incident occurred I get adds on my home(main) screen(as shown in the picture below) and sometimes my phones touch doesn't respond especially when someone's calling I can't answer also my phone feels slow at some point..Please I need your as I don't know what to do it would be really helpful

Do not pay attention to this if you have stock samsung 4.3.
Some app you installed gives you really annoying ads. Think about what you installed lately and un install it.
Edit: install some free antivirus and check your phone.

This happened from today morning..I uninstalled all programs that I installed for past 2 days..and tried all kind of anti virus possibilities(I.e trust go, avast, lookout and avg pro) they all say my phone has no virus..even checked with lookout ad detector still no hope the ad shows every time I turn my phone on plus the bad thing is when I press my lock key the phone doesn't lock(as if I didn't set any security level)..well thanks for your response I formated my phone now everything seems to be going well

Did you try "ads push detector" or similar app?
Yes I tried Ad detector, Airpush Ad detector and Trustgo Ad detector non of them gave me a clue to the problem

I don't know if this is a hyperdrive setting I cannot find or some reject of rooted phones or what but whenever I am in any browser (stock, mozilla,. Puffin, dolphin) if I click on one of the ads at the top of a search list (bing, google, doesn't matter which search engine) I cannot connect to the ad. I get the message below. It's like some sort of ad blocker is engaged but I don't know where or how to shut it off. Please help if you can. Thanks!

I tweeted vzw asking about the new lte band coverage in my area. they tweeted back a link with the map. i couldn't open it because it is considered an ad. I did want to open it. When I specifically try to open a link I don't want it to be blocked. Blocking. pushed ads is great. Ads within apps I ignore anyway. No need to block an ad I want to open.

Help.. this app just pop up suddenly and I can't figure out how to remove it.. not showing on the apps store or on the application unless they call it some tricky name.. this app gets triggered by swiping from lower corner to the middle both side

This seems to be something similar to a "Pie Control". Do you have a custom ROM that might have this feature?
Also, if it's a third party app you might find it under "Accessibility" in Settings. Some apps (like Pie Control) require permissions in Accessibility so if you disable them from there, it might stop happening (and also you might find out what app is causing this pop up)
