Disclaimer: I searched THIS forum over the last 8 months and I did not find any mention of this issue.
I have this issue in all custom ROMs I tried (CM11, LiquidSmooth and OmniRom) and not in the stock rom and its derivatives (like Negalite and ACHERON).
I have Pioneer DEH-4500BT stereo. Bluetooth connection gets set up properly, but the default audio player (Smart Audiobook Player) app does not get automatically started the next time I start the car. This happens with ALL the custom ROMs but with none of the stock derived ROMs. (I mean: in the stock derived ROMs, the Smart Audiobook Player restarts when I start the car with stereo in BT-AUDIO position. Not so with any of the custom ROMs. To confound the matters, If I cycle through the inputs - BT-AUDIO -> AUX -> TUNER -> BT-AUDIO, Smart Audiobook player restarts properly once I reach BT-AUDIO again).
Also, CM11 and LiquidSmooth restart Smart Audiobook Player appropriately on Samsung Galaxy Nexus (which was my default device till last week).
Much as I like Negalite And ACHERON implementations, I like LiquidSmooth a lot and I would like it to be my daily driver.
The only reason I am asking this question here, instead of LiquidSmooth thread is that this issue is common to all the custom ROMs.

I just tried installing the LAST cm11 for jfltespr image before unified stream (cm-11-20140217-NIGHTLY-jfltespr.zip) and this issue (no bluetooth autoplay) is not there in that image. This rom (old as it is) is going to be my DD for now.
One more piece of puzzle: I updated to latest stable (M8) of the unified via the cyanogenmod updater. Exact same issue of no autoplay on Bluetooth. I reverted to my backup quickly.
Can anybody tell me how to debug this issue (bluetooth autoplay not reaching the concerned media player) in unified roms?

Any HDCR Kernel Based ROM
It seems that any HDCR Kernel based rom causes the Nkia car-kit to reboot when downloading contacts.
I was originally running WildPuzzleRom v8.0.11 FRF91 2.2 (kernel 2.6.32.21-g4b3e397 htc-kernal) which worked as expected and I could download the contacts to my Car-Kit.
I them swapped to CM7 RC1 (great work by the way) and contact download starts, but then causes the carkit to reboot. I tried the nightlies for a while but same issue and then tried RC2 but same issue.
I have tried different versions of the Kernel 4, 4.1 and 4.2 but that all cause the car kit to reboot when downloading contacts. I have tried a second Nokia CK-15w Car Kit and the same problem occurs.
I put CM6 on the phone, but same problem and have since tried a few HDCR kernel based roms but they all have the same problem. So I can only think something in the kernel is causing the issue.
Bluetooth connectivity works well, but contact download is the issue.
I quite like CM7 and would be very happy if I could resolve this issue, but so far I have been unable to. I also posted on the CM forum but have had no response.
So thought (i am new to this) I would ask here if anyone has any ideas? or if there is a Stock (non hdcr) rom i could use with either cm6 or cm7?
There is a new kernel in the latest nightlies (nFinity tuxbox by Arco), and, you can check that out. But, remember, some people are having other issues with that, like 3G not working, call drops when screen shuts off etc. There are fixes available in the CM thread here that solves it,but you will have to hunt a bit.
Either way, a Nandroid backup before trying it is essential.

its actually just nfinity.
Tuxbox is the name of Arco's host computer
Cheers, just downloaded CM7 nightly 30 so will at least be able to check to see if the contact downloads works. If its all good then will have something to look forward to once the other issues are resolved. I always take a nandroid backup lol Better safe than sorry.

Media volume is locked to 0 in my gt n7000 after flashing Omni rom 4.4.3 (latest nightly build). I am not able to increase media volume by any mean though other volume controls are working fine. It worked fine for 1 day but after that it got stuck at 0 volume. The same scenario happened twice.
Any suggestions on resolving this problem?
Known bug with hotword feature of launcher3. Right now you can only disable it until this gets fixed. Or use a different launcher.

I have the same issue, but did as you advised and disabled hotwords, that did not work.
Any other options other than installing another launcher?

I'm facing a very strange problem as i'm unable to switch on my bluetooth... last week only i rooted my phone and installed CWM recovery...then i installed Custom Rom's like Hassan ROM, Omega ROM, Cyanogenmod 11 Rom, Pacman ROM etc etc just to check various Rom's and their features out.....At first i installed only one ROM at a time and den also dual booted two ROM's by romswitcher...While i was checking my phone once i discovered that my phone's Bluetooth is not turning ON...The toggle is only becoming light green and also from the settings its only showing turning Bluetooth On but it's unable to turn bluetooth On.....Now i googled my problem and read across various forums for solution...I tried various things also flashed my firmware to stock samsung but still bluetooth does not turn On...also i flashed with stock samsung firmware of feb and march 2014 and updated my phone over OTA but still the problem persists....i even installed latest perseus kernel while rooted but still could not turn my bluetooth ON.
I would be very grateful if someone can help me solve my problem and help me Switch ON my bluetooth as it is very needed aspect for my daily use.
My Version of S4 is i9500 with android kitkat 4.4.2. Also all my other features are working normally on custom ROM's too.

I too installed this Omega rom at some point and once I did that these problems started.
A user in that thread posted a reply in which he uploaded a file which he claims fixed his problem.
I really need my bluetooth to work again and would love to flash this file using Odin if it would fix the problem
BUT
I have no clue what implications flashing this file can have, can it mess other things up? I've spent the last four hours setting things up again in my new ROM and I would hate to have to do it again or even worse, cause some unsolvable problem with my phone.
I assume some of the developers here could help me out - should I run it?

I flashed stock oreo roms on my g5 plus using fastboot and twrp methods. I also tried different versions of it (soak test 1 and october patch).
Each time I flashed the stock oreo rom, I encountered audio issues on music player apps like play music, spotify etc.
When a song played, I could hear a slight buzz as the song was playing, making it seem like the audio file has problems. Happens when I plug in headphones/use speakers.
I tried uninstalling and reinstalling those apps and factory reset but nothing seems to fix this.
This does not happen with apps like youtube or the phone app, where the audio is absolutely normal. (with headphones/speakers/earpiece).
I have used custom nougat roms (stock and custom) and custom oreo roms like RR, PE and never faced such an issue with audio quality before.
Can anyone help me identify what the problem is?

There's most likely no way to figure out why some apps sound normal for you and others don't. A clean flash is always a good advice, if that doesn't help it's most likely a hardware issue (of your device or your audio hardware). I flashed stock Oreo a few days ago, rooted with magisk and flashed viper4android and ainur sauron as modules. The sound is great using Poweramp, YouTube or any other source.
I'm facing an issue with custom ROM, there non audio on my bluetooth device, its connected but has no sound.
is there any custom ROM with working bluetooth audio in here?
Derpfest on Android 9 works OK, I think. There's also a Magisk file, GSIBtFix that solves the issue as well. Worked on Android 9 roms, never tried it on 10.
