So about 2 or 3 weeks ago, i rooted my N4 and installed TWRP. I used Nexus Root Toolkit. Everything worked perfectly until today.
I was using the phone and suddently it powered off. When I booted it up, it would stay forever in the loading screen.
I tried to go into recovery mode but it wouldn't load. After a few attempts, it finally loaded. I tried to do a factory reset, which failed with this log:

I tried to flash stock + unroot using NRT, but it also failed:

The flash memory might be corrupted, or have some bad blocks. In this case, I know no other solution except replacing the flash chip. But I'm not all knowing. Wait for other replies
Seems like it could be a hardware fault, but I would definitely try just flashing the stock rom the normal way without the toolkit first.
I sent it to RMA and even though it was unlocked and without any ROM at all, they sent me a new phone

Hello guys, i have a problem with my N7 2012 WIFI Grouper, i flashed a F2FS Rom with F2FS TWRP 2.7.0.0 Rec. and every worked fine for 2 days, till this morning, i powered it this moring up and it stuck at the Google Logo. Access to Recovery is not possible, but i can boot it via Fastboot ... all Data is Lost even on the Internal SDCARD but i dont find any Mount SDCARD option, that i can copy the Data on it.

It looks like you have a NAND failure. There seems to be an epidemic of these NAND and corrupted bootloader failures in the first generation Nexus 7s as they approach two years of use. I found a used $50 32GB motherboard (or $35 16GB motherboard) was far more cost effective than Asus' $213 repair estimate.

I got a serious problem on my OPT.
When i get into recovery mode i can't flash anything anymore because my /system partition is gone...
Everything i get this error:

I can't do anything with my device now...
WHat to do.. What to do
Try doing a factory reset.
So right to it. I'm trying to backup my phone now using TWRP after having to reset the whole phone.
But when I try I'll get this error with

Looked around on solutions which would be manually formating the sdcard through the terminal but I have no idea how
you need to format data (with TWRP) before TWRP is able to do it's job
this will factory reset your device though, so make sure you have a backup of your data!

Thanks for the advice. So it will completely reset my phone again? I have a backup with TitaniumBackup but does that really do anything if it's a factory reset? ;/
I was talking on a phone and suddenly my OP2 got restarted and what I see is "Encryption Unsuccessful".
I don't know what has gone wrong, maybe when on call it just got mistakenly browsed into encryption option or something?
I am on Oxygen OS 3.5.6 and rooted with TWRP 3.0.2-2.
Now whenever I boot my phone it shows
"Encryption unsuccessful
Your phone storage has been damaged, please download the firmware package to brush".
It appears that some part of OS is loaded because when I change the volume the android styled indicator bar and the settings gear appears but nothing happens when I tap it.
I entered TWRP to erase data (again because I'm sure had checked it) and it says (in red):

When I go to Wipe / Repair File System selecting Data partition it shows me:
Present: Yes
Removable: No
Size 0MB
Used: 0MB
Free: 0MB
Backup Size: 0MB
When I select Internal Storage and tap Repair or Change File System it says Invalid partition selection (in red).

but I am stuck at recovery tool. After clicking the square button it shows ok and then pressing start it starts some procedure and then stuck on something called FirehoseCheckRSPE and some Chinese thing written ahead of it.

Tried fastboot comands to check whether it works and it does work.
Like when entered comand fastboot devices, it does shows the device.
I tried to flash stock recovery by --> fastboot flash recovery recovery.img
It succeeds like this

But when i restart to recovery it still shows TWRP.
What should I do?

I cant't enter the system, it just into TWRP again and again.
And now, my computer concider my device is Moto Z2 Play, it doesn't show the internal storage.
Can I get some help in there, please?
I suggest to reboot from TWRP to bootloader, then fastboot flash the 3.1.1-0 version from the TWRP thread which shouldn't have this problem. Try not to use older outdated guides/videos/etc and read/search the current forum threads first to help avoid problems.
You can also flash the image from within TWRP as well.
Just ran into this problem earlier. Go into fastboot/bootloader and enter "fastboot continue". It'll reboot into system afterwards
thanks to everyone, i fixed this problem. I just flash NPN25.137-35 Stock Full Firmware in bootloader.
