My phone keeps crashing then getting caught in a boot loop it usually requires a factory reset and being connected to a charger/computer to get it to full boot again.
If not connected it'll get to the OS splashscreen then crash and begin booting again.
Any ideas on how to fix this?

Looks like you've only done a factory reset every time. In addition you also need to wipe cache and in some cases dalvik cache.

I'm using SM-G900f with Fusion ROM.
I decided to reboot my device which normally worked without any problems. But this time it got stuck on black screen and I couldn't turn it on/off. I tried like 3 time and the same thing kept happening. Even when I just turn off the device it's still stuck on a black screen and doesn't turn off unless I remove the battery. I would really appreciate if anyone knows a solution for this or has information on what I'm dealing with. I tried wiping cache and dalvik cache but didn't help.

Got no answers here so I tested wiping cache/dalvik cache, system and install the same ROM on top with fusions TRWP.
I can now reboot to recovery atleast which lets me reboot/shutdown but I still can't reboot or shut it down normally.

I searched all the threads related to boot loop but did not solve my problem. I was in stock lollipop and my phone was working without any issues. One fine day I felt that the phone is hot and tried rebooting. The phone was struck in the initial graphic screen. Tried restarting again but the same result. After multiple failed attempts, tried doing a factory reset from the recovery with no luck. Thought that something got corrupted, installed twrp and did a fresh install of the factory image. That also did not help. Then installed Chroma ROM and then back to Kitkat factory image and all the attempts failed. Then tried installing Chroma ROM again and after a long wait, it booted up. I inserted the sim card and the network was not detected and hence had to reboot again. The reboot resulted in the bootloop again. Then installed CM 12.1(without sim) and it booted after considerable amount of wait time. Then inserted the sim card and it was not detected and had to reboot again. Now struck again in the boot animation. Is this a hardware issue? Also does the SIM card insertion requires a restart in Nexus 4.
Tried clearing cache/ dalvik cache and did factory reset, file system check but nothing worked..
Can some one help me on this..

I also have same issue,Then I installed clean os with Nexus Root Toolkit.But issue is not solved.some time when plugin into charger or pc that want happen.

Have you tried out the Nexus Root Toolkit? I know most are against toolkits but it really can't hurt in your situation! Try following the guide while having the SIM installed. And yes, SIM card insertion requires a reboot.

So I'm on my 2nd day of owning my 6P, and it rebooted without prompting at some point when it was in my pocket. After entering my PIN it just sits there on the animated Google logo, getting pretty hot but never making it past the boot screen.
I'm planning on trying a factory reset and seeing if it happens again since I was going to unlock the bootloader anyway, but I fear I've hit a hardware issue. Anyone seen this before?
i've had the same thing happen to me although this was after i restarted the phone. Been stuck on the animation part of the boot screen for a while now, wiping cache hasn't helped. Don't really want to do a factory wipe as I have a lot of stuff on the phone I need to backup first.
So my phone has been acting up the last few days, randomly freezing and unresponsive, so I decided to take cyanogemod off of it and go back to stock ROM.
So I have now flashed it twice to see if I could fix it but it keeps booting up showing my network logo, then gets to the initial screen and then goes back to the boot loop, any idea how to fix this?
I got the ROM from SAM mobile for my device and network.
Go to Recovery then wipe cache and data/factory reset

I did the factory reset and wiped the cache but still it keeps being on the roos screen.

This is something usual and nothing to be afraid of, since removing cache requires the phone more time to boot.
