Phone defaults to speaker phone - AT&T Samsung Galaxy S 4 Q&A, Help & Troubleshootin

So every time i get a call my phone defaults to the speaker phone. The speaker icon wont show it's on speaker phone but taping it will set the indicator on and speaker stays on the taping again will kill the speaker.
Has anyone had a similar problem? I did some searching similar threads i found had no resolution.
I was running Gummy Kitkat and now on KangaKat so its not a rom specific problem, maybe a Kitkat issue as i only noticed it when I went to Kitkat. I've tried a couple apps to kill car/dock mode with no luck. Tried to find anything running that might be causing it but nothing stood out.
See screenshot for for phone specifics.
Any help would be greatly appreciated.

Disregard. One of my profiles in Profile Scheduler switched the speaker mode on while the others had no change. Never happened before may have happened on an update...
Dumb.

X7500 Lock ups

Hello all
I've been having problems with my X7500 locking up randomly. This doesn't happen often but happens at least once a day. It just lucks up for no reason and i have to soft reset it. The screen freezes and displays weird squares in the right corner as you can see on the picture. I noticed this happens while the device is not being used (ex talking on the phone or internt). Never happened while I was using it. Always happens while it is connected either to a charger or a USB, I couldn't see it locked up while not plugged in to anything, but i'm not sure.
Any ideas what may be causing it ? Maybe some software ?
Thanks for your help....

Do you have Batti installed?
That causes an identical problem on mine.
Yes I do, do you think that is related to this problem ?
My friend has the exact same phone, he also has Bati installed, I remember he had this problem too but it went away.

Help! My "Answer" and "Hang-Up" buttons are the wrong way around!

As per image. The last thing I did was flash an "update.zip" that contained the ringtones from the Nexus S, but I have no idea why it would cause this to happen.
Running CyanogenMod 6.1 on a US T-Mobile G2.
Any ideas, anyone?
*facepalm*
It fixed itself upon a reboot. Never mind. I'm an idiot.
you missed a call lol
That happens to me with a bone-stock (but usually temp-rooted) OS when I rotate the phone as it's ringing. The slider still works as it should, but sliding the broken graphic over.
Same here, randomly... rebooting fixes it
wonder whats causing it
I think a bug with the auto-rotate causes it.

[Q] New owner with strange issue

I bought this phone yesterday new from verizon and after the update was applied this morning OTA I have an issue where everytime I place a call it has a toast pop up telling me that "The called party wants to switch to a video call. Accept?"

I can't find an option or setting anywhere to disable this and it actually pops up the second the call is placed. I have tried googling for help and searching these forums as well.
Any help would be appreciated.
same happens to me, I thought it to be because of in call recording after rooting.... maybe not
I have not experienced that issue, but my phone has been sluggish since the update.
for me it is definitely something to do with the toolkit and call recording. i can restore to stock, be rooted, and not get the message. once i add call recording it comes up. maybe thats the way it is.....

Strange Problem: i9505 Shows Warning Screen for no reason

My cousin's i9505 works fine, but this is the second time in 3 weeks it boots up into the Warning screen of Odin mode for no reason. The solution to this seems to be to pull out the battery for an hour or so and put it back in and phone works normally again. Is anyone else experiencing this problem? Any permanent solution to it?
P.S The second time this happened, the earpiece stopped working and later the phone booted back to Odin Warning screen. Possible water damage causing the buttons to malfunction?

sounds like you have something like this http://forum.xda-developers.com/showthread.php?t=2429585
maybe look for a disassembly video on YouTube to see if you can resolve it yourself or pay to get the button mechanism fixed.

Couple ongoing issues that I doubt is rom related.

I first tried paranoid android then tried evolution x and havoc os. I experienced the same issues on all of them. Could it be firmware related? Any help would be appreciated, been dealing with for months. I'm hoping someone has experienced this.
Phone info pic attached.
1. BT.... When I connect to my car or any other speaker /earbuds my phone seems to enter a phone call which in turns mites my music. On my car it either shows my own number or a random 100000000 number. I have to hang it up then I get music back. It doesn't happen every single time but way too often.
2. After a few days my phone will stop waking up. When I double tap the screen or push the power button nothing happens. I have to force reboot the phone. I know it's not "frozen" as I'm still active on wifi.
