I am getting the message in ODIN when I try to power my phone on:
Firmware upgrade encountered an issue. Please use Software Repair Assistant & try again.
It happened after I tried to "reroot" my phone after the OTA update from yesterday. I was using Root De La Vega. I must have done something wrong. After about 20 seconds of the recovery process it gave me an error: secure check fail : cache at the last line under the paragraph in the upper left hand corner. Now my phone just sits in ODIN with the above error message. Is there a way to recover?

What the guy was saying is that the OTA breaks root...that's why you're getting the message you're getting. You'll have to go back to the previous build to root as stated above. Next time, do a small amount of research before accepting an OTA.

OTA doesn't break root, you just have to use caution - go in SuperSU, select Survival Mode, deselect Enable Superuser. Then take the OTA update - when it's all finished, go back into SuperSU, select Enable Superuser, then uncheck Survival Mode and you're done. Root on latest OTA update.
I believe you need to have the pay/pro version of SuperSU for the Survival Mode option.

Correct if I am wrong....wouldn't it be easier just to revert to stock using Odin and then take the update on Kies or OTA then root again? I think this would save a lot of folks the trouble. You can root the OTA and safe strap still works....

Yeah I saw this after i softbricked. I'm relatively new to the Android scene. I did a lot of modding on Winmo and a good bit of jailbreaking, but it's been a while. Admittedly, this was a bit of a careless mistake, but it didn't warrant all of the douschebag remarks. I did do some searching before I asked....

That's how I got myself into this mess.. used that after my OTA update. I have tried it several times since but if fails almost immediately with the following error. Not sure exactly what it means but I assume it's telling me it can't update the BL because i already have a newer version (after OTA MJ5 update)
Code:
SW REV CHECK FAIL : [sbl1]Fused 2 > Binary 1
thanks for the suggestion
P.S. no luck at sammobile either

Try going back into Odin mode and re-flash the new update. When you tried the root Vega method you were trying to flash the prior firmware, but that is blocked, can't downgrade. So have to try and redo with the new firmware.

I reverted to stock, took the OTA update, then tried Kingo but it failed to root. Tried to flash back to stock again using droidstyle's thread, but the process halted after a couple of seconds. I pulled the battery, tried to boot normally and I got the same "firmware upgrade encountered an issue..." message, and almost crapped my pants. I saw this thread and tried again, flashing just the .pit image, and got my phone back, but no root.
Hey fellas. I have to take my hat off to all of those who have done some great work for the VZW Note 3.. Well to my issue now.
I tried to step back to the original release from the MJ7 (I havent read you cant do that yet but I think I figured out why you shouldn't). I wanted to deox, root and safestrap the MJ7 release and I thought this would be the only way how.
Anyways now I am stuck at the "firmware upgrade encountered an issue. Please use Software Repair Assistant and try again" screen. I have tried to download the stock release from six different sources, all corrupt (one site, six different mirrors) and obviously failed.
Any suggestions? I need to go to work soon and my phone is very useful at work....
Thanks

Not sure of your exact situation, but you cannot flash MI9 back after going to MJ7. You will need the full MJ7 Odin file and most probably the pit file too.

Verizon - Samsung Galaxy Note 3 - White
I have been trying to update the the latest OTA firmware update, but I am having troubles. I am sorry if this is a repeat post or this has already been solved, I did do a pretty extensive search for my problem before I made this post.
MI9 to MJ7
Status: Update failed
Code: 404
I have included 2 screenshots below with the problem I am having. I am not currently rooted, I did not trigger Knox, and it is still not working after the third factory data reset.
Does anyone out there have any ideas as to why this is broken and what I can do to fix it?
When you plug your phone into a computer, a folder pops up with some folders and some files. One of the files is a program from Verizon. I do not recall the name at the moment. But if you double click on it, it will install a program on your computer that will help you install firmware through your computer. Try updating with that. Just follow the prompts. And let us know if updates you to the current firmware, or if you're still having problems.
♢NoteIII♢
Since you are not rooted download Kies 3. It's a Samsung utility. Download it and install it on your computer and that should get you on the latest and greatest. I did it on my windows pc with both updates. Current being MJE.
Then if you want to root later you can with Kingo app.

Thanks and Update
I tried forcing the update through the pc using the Verizon Wireless software, and had no luck. It fails at 66% exactly when my phone switches to ODIN Mode and says Downloading... - Did not trigger KNOX - 0x0 in both rows.
My next step is to try Samsung Kies3 and see my luck with it. I will post an update with the results.
I want to thank everyone for the information. I had not heard of Kies did see Odin mentioned but it looks a little involved.
Credentials and Thoughts:
I have some experience with root and custom roms from Droid 1, Droid 3. I rooted my Note 3 in October 2013, so I may have missed the first OTA update that was so minor, it didn't get a build number. I have a feeling that my problem is that I am trying to go from update 0 to update 2 while skipping minor update 1. With no build number, I don't know how to tell if that is the problem. Is there a stock rom I could install, that has the newest update? I already backed up everything and wiped so, it doesn't hurt to go one step further.
Thanks again.

OP, were you able to update through Kies? If that doesn't work, I would recommend backing up what you can (search for a no-root backup solution, or use the one built into Kies), and factory reset, then attempt the update. If you back up your stuff through Kies, the only thing you'll lose is apps, I believe, but Kies might even back that up for you as well.
I'm having similar problems myself. Update fails through the OTA, Kies, as well as the Verizon Update Assistant... Any ideas on how to update otherwise? I've flashed phones with ODIN before but i haven't found a good walk through with the Note3 or what order to flash everything in...

hey im test for root an device equal to you.
root fine (kingroot apk)
but when reboot there is an error "kernel fail" & dont work any more
some sugerence?
Hello!
Need a stock 5.1.1. recovery.img if anyone knows how to upload one? I have made a noob move and tried to root via kingroot, then installed supersu and it said it worked, but it needed a reboot so I reboot and now it says kernel fail security error flashed unknown firmware and won't boot at all. I can get into recovery mode and odin mode, but everything I've tried to flash fails. I believe this is due to a reactivation lock which I (stupidly) set to active when I first received the phone. I have the Cricket version of the Samsung Galaxy Prime SM-G530AZ.
If anyone knows anything to help, please let me know. I have been working on this for about 24 hours and have made no progress thus far. Kies even says unknown error occurred, upgrade not finished, whether the phone is plugged in or not.

Are you on LP or KK?
I have an unactivated (not even turned on) SM-G530AZ (for Cricket) that I'm happy to pull an image from. I assume it has KK since it's stock from Cricket. Y'all just tell me what you want me to do and how to do it
Just got November ota update in 60mb i am just curious to know if it is gonna lock the bootloader.
UPDATE-/ BOOTLOADER STILL UNLOCKED I TESTED & DOWNGRADED TO 5.1.1 OK1 .
I just received it as well. I use my phone for work so it's encrypted but could you try to downgrade and let's us know what happened
Yes, you can still downgrade to 4.4.4 with the new update
This update should give me enough time to read the details of the update before it reboots my phone in the 25 seconds. At that point, we don't have the ability to "abort" or decline the agreement anyway.
Note 4 tmo, downloads the 60mb update, phone enters recovery mode then i get "update failed phone will recover the original configuration" .
I'm full stock, no root, no custom recovery no nothing. Anyone could help me?
I'd recommend you use the desktop version of ODIN with the latest FULL ROM to restore your device (without wiping) to the version before the OTA update. You can then try the update process again.

I encountered the same error last night. I held POWER + VOLUME DOWN to boot into Recovery mode, then I rebooted to System. My phone went through the app optimization process (you know, the screen that you'd see after wiping the Cache/Dalvik Cache).
I assumed that the update failed, but when I checked "About Device," I noticed that it now has the November 1, 2016 security patch (Baseband Version N910TUVS2EPK2).

I assumed that the update failed, but when I checked "About Device," I noticed that it now has the November 1, 2016 security patch (Baseband Version N910TUVS2EPK2).

Sadly mine didn't, as it gave the update failed it reboot normally without app optimization, checked about phone, no changes.
I gave up and updated through Smart Switch... althought the "dm-verification " error still present.

How do u update firmware through smart switch
Sent from my SM-N910T using XDA-Developers mobile app

This update should give me enough time to read the details of the update before it reboots my phone in the 25 seconds. At that point, we don't have the ability to "abort" or decline the agreement anyway.

I noticed it said something about gear3 and RCS messaging system. Didn't have time to read the rest of the change log

I'd recommend you use the desktop version of ODIN with the latest FULL ROM to restore your device (without wiping) to the version before the OTA update. You can then try the update process again.

I flashed the rom through odin, all good.
Still, dm-verification error hunts my phone when entering recovery.
Still no one has a solution to the dm error?

Well, I also used odin until now. The fact that you tell me there's no "working method " assures me that you made your searches like I did and found nothing.

I have custom ROM and Custom Recovery TWRP.
Will updating the Modem and Bootloader cause me to have to reinstall the ROMS?
Thanks in advance
