I've recently just started having this problem where all my audio is playing back in mono.
At first I thought it was my headphones, but I've tried 4 different sets of headphones.
Then I thought it was my headphone jack as I've read some people have had headphone jack problems on other brands and models of phones.
So decided to try a stereo bluetooth headset, but it still plays everything back in mono.
I then thought that maybe it's Google Play Music that's streaming and/or downloading all my stereo music tracks in mono as that was the app I was using to test my theories.
Then my last test was to download a Stereo Left-Right Test.mp3 file. It's just some guy with a French accent saying Left-Right-Left, etc out of the corresponding channels.
I uploaded it to Google Play Music and I also transferred it to my phone directly to test in other music playing apps.
UUGGGGHHHH!!!!
No matter what, that file also plays back in mono even when trying different headphones and the stereo bluetooth headset.
What the heck?
Now here comes my stupid question. Is there some setting somewhere in my phone that I cannot find that switches between stereo and mono?
I've searched my S4 and I searched the web and cannot find anything on this.
Please help!!!

No custom Rom. Rooted though.
Running stock Baseband I337UCUAMF3 / JDQ39.I337UCUAMF3
I did have Xposed Framework and Wanum Xposed installed, but I uninstalled them when I was trying to troubleshoot the mono thing.
Anybody out there have any solutions?

There's an option in accessibility to turn on/off mono audio. Perhaps you accidentally turned that on.

Thank you, thank you, thank you.
I don't remember ever changing that setting, but maybe my kids did it.
Again thank you.

There seems to be no way to listen to music or the soundtrack of a video via Bluetooth a2dp ...
Tried with both the music player and vplayer but each time the sound only comes from the phone speaker and not the Bluetooth device.
Anyone managed this?
It works with the stock rom but I haven't seen any custom roms with it fully operational for media, just for calls.

Thanks, it seems there are a couple of widgets in the market that add the functionality. At least it's not a hardware limitation.
What apps are those? I want to try them please.

I have no problem with the stock ROM but i do use this...
A2DP Volume, you can get it on the market, it's Free.
The one I tried was called bluetooth audio widget, free trial. Will try a2dp volume though as the one I tried seemed to interfere with dsp manager so I installed.
Just tried a2dp volume and it doesn't work for me. That's with leedroid v1.2. Maybe this only works on stock?
Got a2dp volume to work. Seems to do the job well for audio only but has anyone tried watching a video using Bluetooth a2dp for sound? Everything is completely out of sync. I don't there is anyway round this but thought I'd put it out there?
Same with stock player and vplayer.
Just tried Bluetooth audio widget again from the market and this keeps the audio in sync nicely, the only problem being that the quality is pathetic.
Anyone know of a better alternative that keeps audio and video sync?

Yes bro. I have ticked mediq settings in BT.
I've found that rom / kernel combo's make a big difference. I'm currently using wanam on Franco kernel. I've had nothing but issues with cm10/aosp/aokp roms even though their my preference.

You mean with Franco kernel u can play songs through BT well?
Make sure your bt device is stereo not mono. As much as I read about it, mono bt devices can't play audio in Android..

Thanks mate, I am glad you like it and no, it wont work if its mono.. There are few apps to do the task on google play, you may want to check your luck there although none worked for me.. Search for btmono or something like that.

Does anyone how I can get a mono audio option for sound? I use spotify at work and would like to be able to use only one earphone...but then I miss out on half the sound. I have no issues with PowerAmp, since you can downmix to mono but have trouble everywhere else (TuneIn, Spotify, etc). So anyone know of system wide APK or function that allows for mono audio?
I do this every single day at work with podcasts.
Option 1
A2dp Bluetooth
Option 2
Super mono froyo or BT mono in play store
These "hack" your audio to a non a2dp bluetooth
Edit: I re-read your post and it seems you want better quality rather than the wireless capability. Nevertheless a2dp may still be the easier way to go.

Thanks for the response. I actually got a stereo -> mono 3.5 plug but it doesn't like to fit in the N4...works on just about everything but the N4. I think I'll just look for another stereo -> mono adapter that actually fits as BT wasn't the solution I was looking for.
I can connect to BT wireless speakers however the sound ALWAYS comes from the phones speaker, and NOT the wireless speaker. I know I'm syncd and everything is checked for Media Audio Device in the BT settings.
SOOOO.... What gives?? I am stumped. I have tried the Sammy music player and clicking "via bluetooth" - nothing. I have tried poweramp which has an A2DP setting - nothing. Same speakerphone BS.
I feel REAL stupid. I have flashed, rooted, un-bootlocked my Note 2, use TWRP, etc so it's weird for me not to figure this out.
Please help community!! I am on stock rom roooted via the "just twrp + stock MDL" mod (credit would be given but I forget whom on this forum typed that one up - sorry).
and I have searched google and this forum high and low to no results.
Thanks!

Have you tried deleting and repairing the device? Sometimes this works when BT problems arise.
I have done a factory and data reset, But I'm not too sure what you mean by repairing the device. If you could explain an elaborate more on that I appreciate it! Thanks
Re-pairing the device after you have unpaired. But you already covered that by doing a full reset.
Do the speakers work with a different BT source?

ill try it out but that is not acceptable to me. The gs4 shouldn't need a third party app (and I haven't tried it yet but it says mono so I'm assuming its not even stereo quality).
Obviously other ppl with this phone don't have this issue or it would be mayhem. There would be questions asked in all the forums and review ssites would have mentioned a bt bug. I think I will go to the store and get an exchange.
FWIW my note 2 connected/paired with anything I threw at it! Can't wait for the note 3
I stream Bluetooth audio via my car stereo and Bluetooth speakers at work everyday and have never had an issue.
That being said try a different music player and see because I have heard of that being an issue.

The scenario:
I am streaming music to my bluetooth enabled stereo and I want to check out a video on YouTube or play a game at the same time.
What happens is that music stops playing and the sound goes out to the bluetooth.
What I want is to be able to stream music to bluetooth in the background, while the sound from youtube or games/ notifications goes to the phone speaker.
Is there a way to choose what sound goes where?
I know this is possible in windows, but surly this powerful device must be able to do the same?
Any advice is greatly appreciated!

That's what I am using for music, and I want music to go through the bluetooth, so that part is all good.
But I want everything else to go through the phone speaker, and still let the music continue to play through bluetooth.

That is definitely "by design" in all Samsung phones (so as to not miss notifications). In my previous phone - i9300 - the fix was to use a custom kernel with that option.
