"something" keeps turning my sound back on after I turn it off/vibrate. Is there an easy way to find the culprit?

I have a stock Verizon XV6900 running WinMobile 6.1. I have an issue with the sound profile changing almost every time I turn my bluetooth on or off.
The 6900 comes with only three sound profiles: on, vibrate, and off. I keep mine on vibrate. Whenever - again, _almost_ every time - I turn my bluetooth on or off, the sound profile will change from vibrate to either on or off. I've used different bluetooth devices - Jabra, Motorola, and Plantronics - and get the same results. I've spent time on the phone with Verizon "service techs" (both the non-help screen-readers and the ones who really knew something about the device), and they even forwarded me over the HTC technical support. Everyone has the same response: that's just a glitch in this device.
It's infuriating to miss calls because I took my bluetooth off and turned it off, and my phone went to sounds off! Is there a way to prevent this? Maybe a registry hack to keep the sound profile permanently set to either vibrate or on? Or maybe a script to poll the phone when the blurtooh connection changes? Something???

So I'm noticing my phone will continually change my sound profile when I open various apps.
Example... If my sound is on and I go into phone settings, the phone turns sound to mute. Leave settings and it goes back to what it was.
Right now, phone was on vibrate. Opened tapatalk, sound is on. Wtf.
It's been awhile since I've owned a Samsung.. Am I missing some type of setting?
Think I may have figured it out. I have an app installed to silence the shutter. I turned it off and it seems like my sound profiles now stick.
Anyone know how to turn this off? I have no idea how this switched on. I'm looking under notifications and sound and see nothing about the flash turning on when incoming calls sound.
For T-Mobile, Settings, General, Accessibility, Hearing, Flash alerts.

My phone is the unlocked version of the S7 running
I don't hear the following sounds any more:
Touch sounds
Screen lock sounds
Charging sound
Under Sounds and vibration these are all turned on. All volume levels are on all the way up.
Restarting the phones does not fix the issue as well as booting in safe mode.
Is there another setting I need to set to get these sounds back on?

I have a strange issue.
Firstly i turned off all system sound, the lockscreen sounds is OFF!!
But i always hear a clicking sound when i unlock the device. Than I thought maybe it is the NFC sound. I turned off, but no change!
Of course If the change the phone profil to the vibration the sound is disappear. maybe it is a bug or feature?
I have IN2020 with 10.5.12.IN11DA rom.
Bug report to OP? However it's really strange since I have no unlock sound with media sound fairly high. Which version are you on? Sure it's not the sound of the vibration motor which actually sounds like a deep "click" on this device.
Are you using any app that generates a lock screen overlay? Maybe to display more information or something like that?

I am not stupid, i have been two different sound after unlocking. The first was disabled, the second remained.
I have made a small progression.
If I turned off the media sound in the volume panel, the clicking sound is disappeared. Of couse i turned the sound on it will be reappear.
