SO long story short I followed the rooting guide below:

What I did:
I Unlocked Bootloader & Installed Custom Recovery, but after
I clicked Reboot, TWRP prompted me to "Fix Root Permission"
and install Root, which I made the grave mistake of swiping to
accept..
Now everytime the phone boots I see a "Google" Logo with a padlock
underneath, for a good few minutes, then a never-ending bootloop!!!
I can get into Recovery, but I can't see the Phone connected to any of my
computers, So I am unable to add in a file to flash and possibly fix my Nexus...
(I also can't mount, nor can I wipe anything except cache)
Please please please, is there anything I can do to save this phone???

When you're in recovery mode you need to use adb to transfer files to your Nexus. Look for a howto that tells you how to use it.
Go into recovery and factory reset. If it still bootloops go back to recovery then on your PC go to device manager. Plug your phone up and see if it starts trying to update the drivers if it does stop it and manually direct it to the driver folder and let it install. Download super SU and use adb to push it to the phone
adb push su.zip /sdcard/
Flash it in recovery then reboot

There is a sweet yummy feature in recovery called "adb sideload". Boot into TWRP recovery -> Advanced -> ADB Sideload.
Typing "adb sideload path-to-your-flashable-zip" in the command line will send the zip to your phone and flash.
Nothing seemed to work however I got it working using one of the tool kits that was able to pull me out of the boot loop in the bootloader thanks guys

Hold your power button for 15-ish seconds (even hold it for a while when the screen is black)
After that, hold the volume up+down+power button this will boot you into the bootloader.
After this you can just simply push a rom through adb on your phone, or try the factory reset in the recovery mentioned above.
so finaly how you decide your problem? cause i have the same problem

So I successfully downgraded back to Froyo. I followed the Rooting process exactly, mdr #'s 2 and 3 matched. I entered the reboot command, and now my phone only vibrates 3-4 times when you turn it on. the led keeps flashing. and nothing is happening with the screen. I cant get it to the bootloader screen either
is it being recognized through adb?

Ok , so I open up adb. enter in adb reboot recovery, then it should be ok to work with after that? Sorry for being so inexperienced at this. I'm just a car guy who has been geeking on his android lol.

Ok, so i tried adb reboot recovery... Same thing, just 4-5 vibrations and flashing amber LED. What is my next option?
I can now get into the bootloader and Clockwork. but now it just gets stuck on the Clockwork mod loading screen when i update .zip file from sd. I tried the factory reset, nothing. It shows S-Off though lol. Is there any info that I can give you guys from the menus I have available in order to help decode what I did wrong/what went wrong?
Try this...Take the original 2.3.4 update and put it in your sd card.(assuming your pc detects the sd card). Take the battery out if the phone for about 10 min. try to boot into hboot and reflash the phone back to stock. Then redowngrade and root again.
Sorry if I'm not too much help. I'm learning from what I read here and there.

Hey Mike, I am having the same issue with my phone I tried the adb reboot recovery command in adb but the phone just reboots. I wanna know how did you boot into clockworkmod recovery or the bootloader. Please help me out I am trying to fix my phone. thanks
clockwork is "adb reboot recovery"
bootloader is "adb reboot bootloader"
you can only do this stuff if the device is recognized. If it just reboots when you do reboot recovery, you probably don't have it flashed correctly or don't have the right bootloader

Download PD15IMG.ZIP here, copy and paste on the root of your SD card. Don't forget to rename the files to PD15IMG.zip before you copy them to the SD Card, otherwise it won't work
Turn off your phone.(you should have enough battery charge)
Press POWER and VOL DOWN simultaneously (this will bring your boot loader)
It will automatically install the PD15IMG.ZIP, need to confirm some actions.
Wait till the process is done....
If you did not encountered any error such as FAIL- PU, you can open your phone normally and start setting it up...

Here's an update:
I think I've made it worse ...
Nothing will boot now and it's currently locked. Any time I hit "Start" it says "Booting Failed"
My CMD now says; Error: Cannot Load "Bootloader Grouper-3.34.img"
I'm seriously lost. I've been working on this for about 6 hours now ....
Can you get into bootloader and then recovery by holding the power and volume down buttons? Was ADB actually working before all of this??

Nope. Nothing is allowing me to get into recovery. At first it was just stuck on the google screen now it says "boot failed"

No I can't get into the recovery at all. When I turn on the device I don't even need to use the rocker keys to get into bootloader either. Actually my bootloader is a bit different now too. I have the 'Google' logo and at the top the regular options for Start, Recovery, Power Off, etc.
In simple terms: I can't even get to the animated splash screen anymore
Open the command prompt IN the directly WITH fastboot
Fastboot oem unlock
Then wipe / flash factory images. Fastboot -w update image whatever the image name is.
Then uninstall / delete that stupid toolkit and never use them stupid craps again.
Are you still able to communicate with the device via fastboot? If you issue the command 'fastboot devices' does it return a serial number?
Edit: I'm going to bed so if you are able to use fastboot, download the factory image pack from https://dl.google.com/dl/android/aosp/nakasi-jro03d-factory-e102ba72.tgz
Extract the archive. Inside are there will be an install script, manually issue the commands contained in the script in fastboot. The only thing you need to add is the location of the .img and .zip files from the archive you extracted e.g. 'fastboot flash recovery c:/Users/name/desktop/recovery.IMG'
There is no reason this will not get you up and running again.

Thanks for your response.
I've followed the instructions in the file. From previous research, I've had these files. My device does communicate with Fastboot. When I give the command for Devices it gives me my serial. Also, when I ask it to reboot-bootloader in CMD it does.
My issue is when I get to "Fastboot flash bootloader-grouper-3.34.img". CMD states "Error: Cannot load bootloader-grouper-3.34.img"
I've followed the commands previous to that. (Erase boot, cache, recovery, system, userdata)

Yes my device is unlocked again, but I still can't get into recovery mode. It just tells me "Recovery Failed" in small text in the upper left hand corner of my N7.
UPDATE:
After unlocking the device again. I used the Nexus Root Toolkit to restore to factory settings and it worked
The Problem: My device was no responding correctly to the toolkit when locked.
Cheers for the help. I'll attempt to root again, hopefully no problems this time If there are I'll be sure to post again!

For the love of God..stop using the toolkit from here on out.. learn how to use ADB and Fastboot so you can avoid these issues.

So you erased the recovery then wonder why you could not boot into recovery....
You flashed or tried to flash the bootloader. You already had the same bootloader installed and there was no need to do so to try and fix your issue.
You still don't know what you're doing and are back using the tool that lead you to this issues. A toolkit. Learn what you're doing and you wont face this type of silly issue again.

Don't use toolkits. Most people don't have a clue what the toolkit is actually doing so if something goes wrong they aren't in a good position

I followed ToolKit 2.0 and did the one step for unlocking boot, rooting, flashing CWM and all option and successfully rooted. But however i am having trouble going into CWM mode.. Can someone please help me. I tried going into the power button and volume down button mode and with the volume keys went to 'Recovery Mode' but then the device just goes blank and then Google comes up written and nothing else happens. I then installed this app called ROM Manager via which i am able to boot into CWM. So how do i do it without the app. Please help me out guys.. Thanks a lot

You need to plug the device into a powered USB port on the computer and manually entering recovery will work. It's a glitch in the nexus bootloader for the time being.
Also, the Trinity kernel toolbox app and maybe goo manager work for going to recovery, and the roms advanced power menus with the reboot to recovery option will work. The app quick boot may work too. But for using the volume rockers you need to be connected to USB
You could also use adb "adb reboot recovery"
If you don't have a pc near you, download the Android Terminal Emulator and type
su (hit enter and grand the superuser permission)
reboot recovery (enter)
Me too sometimes my device wouldn't want to go into recovery from the bootloader. I would have to pass from an app such as Rom Manager to reboot in recovery or it would get stock on Google logo.
What I did to solve is to :
./fastboot erase boot
./fastboot format system
./fastboot format cache
./fastboot format userdata
then from there I could boot into recovery and flash any rom I wanted to.
I'm pretty pissed because clockwork has been nothing but trouble on the Vision the Ville and now the Grouper and I keep forking pocket change to koush for his little touch recovery and it is still buggy. I can't believe this problem of not getting f to recovery from fastboot without needing anything else.

Went to boot into recovery and the damn thing is stuck. I can move around up and down with the volume buttons but i cannot press enter with the power button. shutting it off only reboots the tablet and goes back into recovery. any ideas?

Can you get to the bootloader screen so you can use fastboot? If so try flashing the TWRP recovery, it's a great recovery.

I can press the power button and both volumes and it gets me to download mode... from there, every option i try just gets me back to recovery. when i press start, it sends me to recovery. when i press shut down and then turn it on - recovery. when i press restart bootloader, it still takes me to recovery afterwards. wtf happened here man? never seen this before.

If you can at least get to download/fastboot mode, either get the stock recovery.img file or extract the recovery.img file from a custom_recover.zip, such as CMW or TWRP and place it in the directory where you have the fastboot util on your Windows PC, plug in the device into your Windows PC. Then type the folowing in a Windows Command prompt(without the quotes): "fastboot flash recovery recovery.img". Once that finishes then try rebooting back to recovery.
If you that doesn't work you may have a damaged bootloader, you can try extracting the bootloader(bootloader-manta-mantalj12.img) from the stock ROM and flashing it with the following command: "fastboot flash bootloader bootloader-manta-mantalj12.img" then you can type: "fastboot reboot-bootloader" to reboot directly to the bootloader.

This happened to me yesterday.
Step 1 - hold down the power+volume up+volume down
Step 2 - plug up USB cable and use fastboot to flash the "touch" recovery version. Usebthe method as before, you might need to point your machine to the "naked driver" in dev section to get windows to see the fastboot device driver.
Step 3 - once you are in touch recovery you can just use the touchscreen instead of power button

You can just "adb reboot" from any computer with working adb and drivers.
This needs to not be flashed by default from Rom Manager until they get it taken care of. Twrp and cwm touch work just fine.

Seriously. Can we please sticky a thread or something that says CWM isn't stable on the nexus 10 yet. It would save a lot of people a lot of trouble.
I had the same issue get rid of crappy cwm and flash twrp, flash it by downloading the 1.3 nexus 10 rootkit selecting option 6 i believe with your n10 connected to your pc, it will fix your issue

For reasons that are not clear to me the machine restarted and is stuck in those 4 rotating circles, and doesnt go beyond it. Tried multiple things such rebooting, rebooting into recovery and booting again, tried adb to see if it can see the machine in recovery mode, but to no avail.

Did you do anything, flash anything to get the boot loop? Boot loop can usually be cleared by doing a factory reset. But you'll have to reset up your phone afterwards

This is a stock android and have no changes whatsoever except automatic update to 4.4.4. Only thing that is different is it is unlocked.
For last one week the phone kept complaining that it was unable to back up photos and videos. So I kept the wifi on over the night, and next morning I saw what i thought was it was trying to reboot but unable to do.
Is it possible to connect to machine while it is in recovery mode? My ADB can't discover the device either in recovery mode or while it is trying to boot. Is there a way to press some button and hard reset it?

Can you get into the bootloader? If so you can flash a custom recovery. ADB won't work in the stock recovery.

Yes, that's the bootloader. Google 15 second adb install xda and install that. Then download the latest version of this http://techerrata.com/browse/twrp2/mako
Rename it recovery and put it in the same directory as fastboot.exe which will be installed with the 15 second adb install
Open a Windows command prompt in the same directory as fastboot and then with your phone connected to the pc and in the bootloader type the command in the command window
fastboot flash recovery recovery.img
This will instal twrp to your phone

My Biggest concern right now is not TWRP. I need to somehow get the machine to boot. It is stuck in those 4 circling things, and I need to somehow get the OS to work (or re-install if needed).
I am able to get to recovery where I see options to 'apply aupdate from ADB,' 'wipe data' and 'wipe partition'. I have wiped off everything, now I need to somehow get OS to install, and I don't know how. I tried using 'adb sideload 'image-occam-ktu84l.zip', but it fails.That file was extracted from occam-ktu84l-factory-0d3fd624.tgz from Android site.
Update: Great, now TWRP is installed. What next? I don't want CM at this point. Just stock Os. How do I get on to the machine?

Now that I have TWRP, how do I install CM? I would eventually like to return to stock, but at least for now, I need to get the phone going.
UpdateOk, the TWRP wasn't functioning well (swipe function wouldn't work), so got CWM recovery, and installed CM. CM too goes into infinite loop. Now I am completely stuck, I have no idea what else I can do.

I installed both TWRP and CWM alternatively, installed factory images using flash-all.bat, install CM11. But in both cases, 1. factory image : The system get stuck in boot mode where those 4 circles keep rotating around each other, 2. In CM11, the arrow keep rotating indefinitely.
I don't know what else I can do.
Any pointers? Thanks in advance.
I am beginning to suspect hardware malfunction. In which case, is there a log somehow I can see?
As an Aside: 1. Adb refuses to see the device in fastboot mode. Not sure if this is an expected behavior. Fastboot can.
2. Stock Android USB Drivers never work. I had to get the WugFresh Development drivers.
This is Windows 8 environment.

As I said earlier, both factory image and CM go into infinite loop at bootup. I have no idea what else to do.
Question now is to see if something is wrong with hardware. I don't know how to check that. Is there a way to do some diagnostics

You probably have a failure of the emmc. What happens if you try to lock the bootloader and then reboot? Does it stay locked?

Yes, after locking (fastboot oem lock), bootloader the status says locked and I no longer see that unlocked icon when booting.
- At the moment I have CWM, and I see advanced options to format different folders which I have already done. But what I am not able to do is to use adb to copy the image-occam-ktu84l.zip to sdcard. Only way I have been able to install any zip is using sideload. Is that normal? I wonder if I am following right steps to begin with.
- Also, are there tools that can run and check if emmc has no bad sectors etc?
- In CWM I formatted whole log of folders, and now when CWM boots, it can't find /cache/recovery/* files. Which I take as a good sign - since old stuff is now gone. From here I don't know how to proceed. Any help - very thanful.

If you can side load you should be able to adb push a rom. When in recovery type adb devices. Does it show that your device is connected? Do you get an error?

I can sideload from adb. (adb sideload <filename>.zip while CMW is waiting for abd sideload). But can't push since adb doesn't recognize the device while in fastboot - the way fastboot can.

You have to be in recovery to run adb commands. The bootloader is only for fastboot commands. Try pushing while you're in recovery, not the bootloader.

Here 's what I see when I to sideload,
In CWM, I choose 'install from zip, which takes me to next page where i chose to select from sideload. I then start sideload from 'adb sideload occam-ktu84l-factory-0d3fd624.tgz' (sometime I use the extracted zip from that tgz),
Following is what I see on CMW,
'Restarting adb
Finding update package...
Opening update package...
E: Can't open /tmp/update.zip
(bad)
installation aborted'

Ok, you can't use that file (tgz) in cwm, adb or fastboot. What you can do is extract it and then use the zip file. But, you'll have to boot into the bootloader and then use the command
fastboot update update.zip
