So this is what happened.
> Was successfully running RC3, have done wipes and re-installs multiple times with no hitches.
> Upgraded to new Clockwork mod recovery
> Did "fastboot erase system -w"
> Booted into recovery and installed RC4
> Rebooted phone, stuck at boot logo
> Re-did steps and re-installed, same problem.
Any ideas what could be wrong guys?

So I have tried flashing 3.0.0.6 via fastboot and installing the stable for glacier with no luck, it keeps rebooting at the cm boot logo. Then I installed RC3 and it got stuck on the T-Mobile logo. COULD SOMEBODY PLEASE HELP!!

Yup, just updated to the newest one before I flashed RC4, im downloading the stable for glacier now. Hopefully the problem lays in the copy of RC4 i downloaded.

If you flashed the recovery via fastboot, I'm sure it's good. To my mind, the only variable left here is a bad SD card. Beyond that, I have no other ideas.
Is there a way I could flash the ROM directly from my computer erasing the element of the SD card?
Good question, but I myself can't answer that definitively. Looking at the list of fastboot commands, it seems possible. If it can be done, I might do that from now on rather than using recovery.

As far as im concerned you should do a factory reset after the fastboot erase system. That worked for me
If gou want flash the ROM via fasfboot it is a bit more complicated as the. Zip not only contains the ROM but also the boot imagw and some other components.
You can try extracting the .zip and tell me what files you get so maybe we can figure some commanss out. I would recommwnd the factory reset first though

I would do exactly that. If after formatting the SD the ROM still fails to boot, I think then you can say for sure the SD is the problem.

At that point the phone is wiped clean (except for recovery of course). In fact if you haven't already, wipe all 3 items in recovery before flashing anything.

Looping Issue
I have the looping issue too. I formated the mem card and I went into HBoot to factory reset. Whats different here is when i do the factory reset, it loads me into CWM Recovery and that does nothing. I hit any option and all i get i get is a black screen with the hat. Is it my Recovery version? (v3.0.2.4)

lolz im lost, Why would you go into HBOOT to do the factory reset? You need to do that via recovery. A black screen with the hat? :S Ok, try to reboot to recovery, do a factory reset from there (to scroll around you use the vol down and vol up, to selct you use the power key), then hit install zip from SD card, install the ROM you wish to use and reboot. that should do the trick
I tried to install a new rom after updating TWRP 2.3.0.0. after all was complete it stuck on the boot screen. I then tried with the Nexus Root Toolkit to use a restore from backup. Same problem. I then tried to flash stock and unroot. Same problem. I then tried a number of things through TWRP from backups I had their. Same problem. At present I have no OS and I can only boot into TWRP, turn if off or look at the GOOGLE screen. I have run out of ideas and would appreciate any help offered.
If you have tried everything else (flashing a ROM zip etc.) Then you should flash the factory image from Google.
Don't use the script/batch file as that will wipe your internal storage.
Just flash system and boot.

Sometimes a rom takes a long time to boot for the first time, please be very patient. Also, try wiping everything from /system, and then re install a custom rom from your /sdcard partition. also make sure your roms are for the nexus 7 not galaxy nexus or other similar devices. ONE MORE THING 4.1.2 wont flash with 4.1.1 bootloader.

There was a bug in TWRP V2.3.0.0 use fastboot to flash V2.3.1.0 and then reflash the rom

I know many people hate the talk about toolkits but I was in the same situation and I used Wug's toolkit and it saved me. I downloaded 4.1.2 image directly from Google, as the toolkit only has 4.1.1.
I tried to return to stock on my own and it just went from bad to worse. I would of spent more time trying to fix it on my own but I was on a very tight time constraint. It will unroot your device but it is easy to root again.

THANK YOU the "ONE MORE THING" is all I needed to know for the same problem!
So I rooted, then put CM10.1 nightly and gapps on my sdcard, then went to flash, and I spaced out and accidentally formatted SDCARD when i know that I shouldnt have. crap.
i tried using the "sideload" feature but it wouldnt work on my adb-mac version, i also tried adb pushing ROM files to SDCARD, it says transfered but when i go to flash it says no files in SDCARD, I also tried pushing to SDCARD/0 but i get an error that it isnt a directory since it is formatted.
SOME ONE PLEASE give me instructions on what to do. Do I have to go back to stock or can I just get this to go straight to CM10.1? Id prefer straight to CM10.1 obviously.

Hindsight 20/20, but it never hurts to do an adb backup, it will b/u your sdcard as well.

So you've successfully flashed a recovery img via fasboot? Did you also push a copy of the ROM zip back to your phone?
Boot to recovery and then adb push the ROM to the sd card.
Code:

The above code assumes you've placed the rom zip in the adb or platform-tools folder on your PC.
Can't you just boot into fastboot, flash a recovery, boot into said recovery and mount the internal storage and drag and drop the files from your computer?

yes i did repush the rom file. this isnt working because i dont believe that flashing a recovery adds folder/partition information to the sdcard. maybe im wrong, but it didnt work.

EDIT:: This is giving me some trouble as I cant turn on USB DEBUGGING (because i have no OS installed) to install the drivers and such.....

Yeah but doesn't the Nexus 4 lump all the internal storage together, I know other devices like the OXL have like 6GB internal storage and then a 'simulated' mSD card of about 10GB, making 16GB total. But the Nexus 4 uses the whole 16GB (or 8GB) as internal storage.

although ive always wanted to try one, and it worked! if this were to happen again (if i dont shoot myself) then i will be doing it manually.
Thanks so much to everyone for all the help! I am now running Official CM10.1 Nightly (newest) and running beautifully so far. This is my second N4 tho, so I will be using my 1st stock one for today since its already setup, have to go to work now and will setup my new CM10.1 N4 later, hopefully wont find any crazy bugs...
I had the same problem and after reading this thread I tried the Fastboot method first:
after I finished it didnt boot so I continue with the inst', then it booted but only with 8GB, so I continue with the inst' and from that stage it just didnt boot.
At the end I tried the root toolkit V1.6.2 and the phone back to life with stock rom and 16GB.
I'm back to stock now. But I've used the sideload successfully. After you get your phone restored, you'll probably want to do a full factory reset because some of your pushes have probably gone through and are now eating up space. You can't delete them that I know of unless you FULLY format.
ANYWAY
When I got sideload to work, I had to push it to /sdcard/0/, /sdcard0/, or something like that. Wish I could be more specific, but I don't want to change from stock to install cwm (the ROMS have too many bugs so far, and we might be getting 4.2.2)
The key is finding the right directory to push it to. Experiment with slashes, and it SHOULD work.
I recently tried to flash PA on my nexus.
Installed cwm on my phone. Then downloaded the PA ROM and moved to the device storage.
Rebooted into CWM and performed all the wipes.
I accidentally wiped the system and the internel partitions too.
when i tried to install ROM from sdcard. it says "NO FILES FOUND"
Iam stuck here. Phone wont boot into Android... any suggestions? please help!
How did you wipe system within CWM?
But since you say you wiped internal partitions too, it seems to me that you wiped your internal emulated sdcard... so I think your only option now is to flash stock images again... Read this: http://forum.xda-developers.com/showthread.php?t=2010312
If you don't want to flash the stock images you can also push a rom to your phone with adb while booted in a custom recovery
adb push nameofrom.zip /sdcard/

Flashed TWRP and sideloaded the ROM file. And flashed it.
Earlier the same process with CWM was not successful. Now the phone is loaded with PA ROM!

wiped /system? have you tried simple re-flashing your ROM from the .zip ?
no worry... download the nexus 4 twrp latest img from their site, and flash it to your N4
what you needed:
twrp img file
some n4 rom
adb and fastboot
instructions:
boot to fastboot, vol key down + powerbutton when n4 is off.
on your comp, , dos/terminal - fastboot flash recovery twrp.img
reboot in recovery
select the adb side load from advanced tab in the twrp.
on your comp - adb sideload rom.zip
everything done....reboot phone.

did you wipe using fastboot erase command? if so, you need to re-format to the android supported file system to access the partitions!
try installing the twrp recovery and perform 'FORMAT DATA' from wipe. then install ROM.
N4 no OS.

I need help, having the same problem you had with my N4. Cant seem to fix it. I had PA 3.69 for 3 weeks when I rebooted my N4 i had no OS on the phone. Just my Bootloader and Twrp 2500 was recently 2600 but cant do anything else. ADB works seems to fail when writing to N4. Im trying to get it back to stock. Any Ideas

can u get into recovery? did u check changing your USB Cable? The original nexus usb cable sucks, try another one. If you can access the fastboot/adb then it will be pretty simple..

Hopefully this helps people in my situation.
So on the first day of having my nexus 7 i found a way to have no OS and 0MB internal storage showing in TWRP. Yah i suck.
I tried tons of things but knew a system image was needed.
HOW: I was about to flash the superuser root for 4.3 newly released and forgot to put it on nexus so i then proceeded to exit TWRP, however upon exiting TWRP the recovery detected i havent installed supersu and asked do i want to install superuser? I though why the hell not!
I confirmed to install superuser and then tried to reboot but for 20 minutes device could not boot. TWRP showed 0MB internal storage and pushing adb files in this state yielded nothing as there was no directory
This is how i managed to have a working tablet again
1. FORMAT DATA in TWRP - after doing this your internal storage should be back and not show 0MB
2. reboot to the bootloader
3. On your computer download this file kindly given to us by phuduong and uploaded by Nburnes

4. Once downloaded move that system.img file to your adb directory
5. With you nexus 7 in the bootloader, adb open, and file downloaded copy and paste this command into adb terminal
fastboot flash system system.img
6. Now you have an OS but it still would loop and wouldnt start
7. Boot into recovery and Format Data again, because for me 0MB interal storage was back again
8. Then Factory Reset
FINALLY YOUR DEVICE SHOULD BE BACK TO ITS ORIGINAL STATE!!!!!
Is that the factory image? Have you tried going from unlocked bootloader & rooted back to factory using that image?

Google has yet to release the official factory image for the new N7. However, a kind user was gracious enough to make their own. It is likely to be almost identical to what Google will eventually release and should work the same way.

I think deleting boot screwed me up, because even though I can go into recovery now after reinstalling twrp and I can restore my system using system.img, I still can't boot (get stuck on the google logo). Any way to fix that?

This worked perfectly! I had a wiped system folder (no ROM), and obviously no USB debugging enabled. And with the lack of a factory image, I've been counting seconds to seeing somebody post this!

You can flash old and new image through fastboot. I did 2 times ^_^

I can't, get an error with that too

OP and nburnes saved my butt!
I did this and now it seems my device is booting and it shows up as N7 when I attach it to my PC. But the display shows nothing at all. It just goes blank after the Google screen. Any ideas?
After flashing everything I could think of and banging my head and the device against the wall for two hours, I finally got it to work. Not sure how exactly but the last thing I did was reflashing bootloader. So if anyone has the same problem, try that.
