Your last rom is a stock. Have you tried to update by OTA before ?
Try this guide, the part for flashing a Factory Image : [GUIDE] Flashing a Factory Image with fastboot / return to stock
I have the same problem. I think it could be an issue with being rooted or a custom recovery,

Hi, my phone is rooted, but still on stock rom with twrp recovery. My problem is that whenever I try adb commands or even check devices, it says it offline but when in fastloader and try fastboot devices, my device is seen and communicating well. Why is this so? What did I miss to do?
here is what shows when I try adb devices:

"lightbox_download": "Download",

"lightbox_zoom": "Zoom",
"lightbox_new_window": "New window",
"lightbox_toggle_sidebar": "Toggle sidebar"
}
Pls advice what to do.. thanks!
P.S., after unlocking my bootloader and installing twrp, i locked it again. Is this the reason i cant connect via adb or this issue is not related at all?
Thanks!
Do you have the latest adb? With 4.2.2 a security feature was added to allow for white listing computers connected via adb. Older adb versions don't work with this.

Hello friends,
at first - sorry for my bad English.
Yesterday I bought my new Asus Nexus 7 3G (32GB) and i rooted him via Nexus Root Toolkit.
Version of Android was 4.2.1, now is 4.2.2 (JDQ39), but...
This is screen of bootloader (fastboot) - version of radio is unknown:

"lightbox_share": "Share",
"lightbox_zoom": "Zoom",
"lightbox_new_window": "New window",
"lightbox_toggle_sidebar": "Toggle sidebar"
}
// bootloader.jpg
Here we see that the version of radio is 1231_0.10.0_1020.
// android-version.png
If i want to upgrade radio version to 1231_0.10.0_1021 or 1231_0.17._1025 - there is next error:
// flash-radio.jpg
I believe that is only small problem for you, but i dont know how to upgrade/fix it. Although GSM/3G works now.
Thanks for any advice :angel:

Sideload build fingerprint is incorrect. This is the message I get when I try and flash the OTA lollipop (Ando 5) zip via sideload in CWM.
I'm rooted and unlocked.
I'm running 4.4.4 as per phone and about, however when I try and flash the lolli OTA update it tells me that the version is 4.4.2.
I even tried flash in 4.4.4 OTA cleared the cache, but the issue persists.
Is there a way to edit the signature?
EDIT: Also to note that build.prop in the System folder is showing the right version.

You set it to refurbish, it needs to be set to upgrade

I did this upgrade and refurbish each, but same error.
But I already solved this problem! It was very very simple, when I changed USB-C line into another one, there were no error more!
