I was on 4.1.1 (rooted and CWM installed, no custom rom), I heard that the update was out so got the 31 MB update and installed it through CWM. At the time of rebooting system by CWM, I disabled the recovery flash. It rebooted normally.
But after I tried to reboot it into recovery, it just get into a double loop of google logo and then reboot in ADB. I flashed the recovery again using Google Nexus Toolkit 2.0.0 but had no luck on rebooting in recovery.
I flashed the recovery with clean update of 4.1.2 full new but no luck on recovery flashing after that too. Can anybody help or is there any other way to install recovery on 4.1.2??
Have you tried installing CWM with ROM Manager? That worked for me.

There are quite a few users experiencing the same problem...myself as well. There doesn't seem to be a fix for this issue. There are a couple of threads in the Q&A sub forum on the issue. Last I check no one has found a solution.
I have this exact issue as well.
This is a temporary fix: http://forum.xda-developers.com/showpost.php?p=32680976&postcount=10
Hi all,
Few problems with my TF101. I was running Cyanogen mod rom (not sure witch version, the boot screen was an alien in a vortex). Anyway i was getting some random reboots and the tablet started booting into CWM every time. I have tried several versions of CWM and other recoveries but none will install. In CWM i select the zip install-success, hard reboot and the pad again boots to CWM recovery v5.8.3.4. (also i cant access my external SD with this version of CWM)
While this is a nuisance the latest issue is than no custom will boot. They stick on the boot animation (i have waited up to 4 hours). The only rom that works is Revolver 4.2.1 but wifi does not work.
When i try to install stock roms in CWM it aborts with no error.
When I install the recovery installer APK it reboots to CWM v5.8.3.4 and does nothing
My ideal end state is a functioning CM rom and CWM that boots correctly.
Any ideas folks?
fixed my CWM issue with this http://forum.xda-developers.com/showpost.php?p=29632231&postcount=3
I then installed the recovery apk and wsa able to install CM V6
Hi,
I previously had purity rom on my nexus 10. I decided to back to stock and use the xposed framework. So with 4.4.3 I flashed the stock images (manualy so it didn't wipe my sdcard partition), wiped data, cache and dalvick.
Stock 4.4.3 works fine, no issues here, but in acquiring root by flashing supersu update (both 93 and 99r3) root doesn't seem to stick. I can adb in and su from terminal, but when booting android supersu days it couldn't acquire root. I have tried other clockworkmod recovery and team win recovery so see if it was a bug with recovery, but no dice. Clockwork did occasionally say it lost root and press Yes to put it back, it did this on my nexus 5 and I still kept root.
I have even tried the universal nexus toolkit but that didn't work either. All methods I have tried have run the zip output fine, saying success.
Any ideas what's wrong. I assume I have done something wrong but can't find much information about root not working like this.
Did you try chainfire 2.00?

Hi,
Some time ago I've faced the same issue as you, what worked for me was using an older supersu flashable zip and after flashing it I flashed the latest. Don't know if it was placebo or something, but always worked for me .

I did the exact same couple days ago after running Omnirom. I used this odex stock image http://forum.xda-developers.com/showthread.php?t=2529492
After the initial factory wipe I wiped dalvik, cache, data, and flashed SU after flashing the ROM but before reboot. Using latest TWRP. Hope this helps.
So I have installed the 6.0 CWRecovery in my Note 3 (hlr) in order to install the newest CWM12(7.8), I managed to install the CWRecovery touch 6.0 using the app known as Flashfy, it worked fine and so I managed to install CM mod 12 (I have some problem with the stock Music app, which crashes all the time). I saw a new update (8.1) and I wanted to update it again, but the boot does not load the recovery mod and thus, it just keep on a recovery boot loop. The only way to get out of such loop is to load the download mode, and then it boots the CW12. I have tried reinstalling the recovery mode using the flashfy and rom manager, but it does not work at all, even when using the said programs to flash the ROM itself. What should I do? Thanks!

Try downloading the TWRP...tar file for your phone (http://teamw.in/project/twrp2 - click top right for your phone) and flash it using Odin - it will give you TWRP Recovery.
Also go to Chainfire's site and get SuperSU.zip (http://download.chainfire.eu/695/SuperSU/UPDATE-SuperSU-v2.45.zip) so you can flash it in Recovery.
I flash the ROM, then GApps (I use PA version) then SuperSU in one flash session, then I Restore my Data partition from my nandroid backup and always good to go after that.

Hello there.. I normally don't have a problem figuring out what to do but I'm totally stuck here. I have a Sprint Galaxy S4 SPHL720T. I was rooted and running a custom ROM. I was having issues with the wifi so I decided to flash back to GPE 5.0.1 here from the xda forums. I am running twrp v2.8.4.0. I made a backup and wiped system/data/cache and dalvik. I went to install JFLTE-GPE-20150303 (LRX22C) from ultimarom.com via here at XDA. It said zip is corrupt. So i redownloaded on my PC and transferred it to my sd card. Still said corrupt. So I downloaded a different ROM on my PC and moved to SD. Still said corrupt. I tried to flash a GAPPS package and it worked but no ROMS I tried would. I tried to install my latest backup through recovery. It said MD5 doesn't match. If I hit reboot in twrp it prompts to install superuser because the phone doesn't appear to be rooted. If I hit that it runs the script for about 2 seconds then reboots. It won't boot into the OS as there is no OS and if I reboot into recovery it still gives me the "install su" prompt at the reboot screen. So then I hooked up into ODIN and download mode and flashed "CF-Auto-Root-jflterefreshspr-jflterefreshspr-sphl720t.tar.md5" and "openrecovery-twrp-2.8.4.0-jfltespr.tar". Still having the same issues. I have no idea what to do at this point. Any help would be GREATLY appreciated as I need my phone for work and play .
Thanks,
Aaron
