Which cable are you using to connect the Nexus 4? From my experience certain cables cause issues so I would try a different one to see if that helps.

You can get a OTG cable for as much as $2.18.
Check out amazon! They have tons of them. They're really cheap. Although it's going to be from Hong Kong.
