Do you have an Ext SD Card installed?
I had similar problem after I transferred some apps to Ext SD.
If you have Ext SD installed try to remove it and see if it works fine.
Of course apps from Ext SD will not be functioning.

I've same problem with my Galaxy Note too. It was happening on the device before and O2 replaced with a new one and still I have same problem. (2-3 times a week) So it may not be device specific. It's unrooted and there is no SD card. Is it better rooted? On stock UK O2 Rom (updated).

My phone reset itself for no reason. It was laggy so I restarted. When it rebooted, all the apps I had installed wod force close (samsung default apps worked fine) and all my personal info was gone (phone nos, contacts, settings, etc).
When this happened first two times, i replaced my phone. Now it happened on the replacemwnt SGS.
I am thinking it may be the sd card (I am using an 8gb external sd I have from my previous Moto Mestone). I've been running for about a week with no reset since I took the card out from the phone. I would however like to use the sd card for the additional storage.
Anybody know if the sd card could have been the problem and maybe how to fix it?

Yes, it sounds like this
Did you already try to boot the phone without the external-SD-Card being plugged in? Or just tried booting with another external SD-Card?
If not, copy over your data form ext. SD-Card to your PC and reformat the external SD-Cardwithin the system settings.
Maybe there are some system files from your Milestone left which SGS doesn't like
Installed Darky's rom. Do not seem to have that problem anymore.

I need some help please
I have installed loads of apps recently on my phone (excess of about 80-100)
everything was going fine and i was loving the way it was
this morning, my phone was running really slow so i shut down and booted it up again, now all my shortcuts are little windows with a gear on the front, so i thought the sd card had not mounted properly, so i unmounted and mounted again, then i released all the apps that had this shortcut are actually installed on my phone not the sd card.
I moved over a couple of videos to my sd card this morning, but i dont think that would have affected the apps that are installed on the phone
i cant restore from a back up as i haven't made one yet (only put wildpuzzle 0.8 on last night)
can anyone help or point me in the right direction,

plus it dosnt seem to be the sd card thats the problem...just the apps installed on the phone
Dare i say try ruu at ur own risk. It will update u 2 froyo though. or make a nandroid and try a safe froyo file

I bought a brand new class 10 SD card and I tried it with the s3 and worked fine. When I got my new galaxy S4, I format the SD card and put it in the St and again I format it from the phone itself. I put my picture and song. Then when I turn the device off and back on again it will tell me the SD card damaged need format. But when I go to setting, Storage. And mount the SD card it will work just fine.
Can anyone tell me what is the problem? because it is really frustrated

Try formatting on your pc with exfat and then reinsert the card in your s4.
Have been getting that on mine also. I reboot and then it works fine. Not sure I actually thought I had damaged mind by maybe not

This happens a lot on my S3 and the only long-term (but not permanent) fix I could discover was formatting in the computer like the guy above stated. I always keep a backup of my phone ext. sdcard on my laptop to make sure that if it happens and can't get it to work I still have my data, I recomend people to do the same.

I also have been getting this problem with a 64 gb card. It must be a bug. So far, I just reboot and it seems to read it fine. I noticed it also throws me into safe mode on the phone if I reboot too often. I'm not sure exactly how it is triggered.
I had the same issue, it happened when I removed my SD card from my S3 without "safely ejecting" it from the Storage menu. I plugged it into a computer, and went to a command prompt and ran "chkdsk d: /f" where "d:" is the letter of the SD card and it fixed the filesystem and all of my files were back and my card was perfectly usable again.
I got a S4 i-9500 in Taiwan and everything was fine until a couple days ago when it updated the firmware to XXUBMF8.
Now whenever I connect my phone via USB to my windows laptop, the MTP driver fails to load and sometimes makes my sd card unusable so I have to restart it to try again.
I've tried updating the samsung usb drivers for mobile phones but that didn't work. I tried using regedit and looking for the string 'upperfilters' that other guides mentioned but there isn't a string for that on my computer. What else can I do???
EDIT: If I take out the sd card, the MTP driver installs correctly. I remember reading that maybe some files have unrecognized characters or caches in the sd card are messing up the mtp install process.
Anyone else have the problem?

My sd-card has been reduced to unworkable even just in the phone so I had to take it out.

I had similar problem. I was using Transcend Micro SDXC 64GB. After meddling for several days i found that the card was broken. Same thing happened even after i get card replacement!
Now i'm using SanDisk 64GB and i have better experience with this card.... However i only use this Sandisk card for 5 days... So far so good. Let's see how it goes.
Btw, I'm using XXUBMEA firmware + rooted.

I ended up formatting the sd-card and everything seems fine now when I connect to the computer. It actually recognizes my phone and sd-card but I've lost all my pictures and music which kind of sucks but at least it works now. I hope I don't have to do this each update
edit: NO it can't copy files from my computer. It just freezes in the copying process. What can I do??? Get a new sd card? I've had it for 2.5 years and it worked fine on my htc desire.

When i try to unmount my sd card from settings it takes 30-60 secs and booom!my n9005 restarted itself and as if nothing has happen..my sd card still mounted. I did try to unmount it again and still same thing happens. At first i noticed that when i try to browse my files from the sd card using es file manager .it shows "invalid path" but after i updated the app it works fine..but still the problem unmounting occurs.. i search all over the web but seems nothing was facing this problem before.here r the other infos of my device:

Micro sd sandisk class 4 16gb
Hope somebody can suggest how can i fix this without requiring me to format my sd card coz i do have a lot of files there and i do not have other device that i can use for backup..

I dont know about this because I'm on 4.3 did you tried unmounting your card when u were on 4.3?

Yes,thats what i was doin before removing my sd card.,unmounting thru settings first but i didnt have this problem.after updating to kitkat it seems that this was due to some one of the numerous bug to report..

Haven't u tried formatting ur sd card yet?the safest thing i can do for now is switch off my fone and unmount the sd card, i really dont want to format my sd now until i have my backup.and i also having a thought that this wasnt causing a bad sd card as you were saying.

Well when I updated my phone to 4.4.2 I did a full factory reset then formatted the SD card. I like to move what apps I can to the SD card to free up internal memory.

I just cleanup uninstall my recent downloaded app one by one and try to unmount again my sd.and finally it was fixed..yeay!
