Ive tried installing the "PD15IMG file" but I get "failed pu" on every step.
When I try to reboot into recovery, this is what I get:
E: Cant mount /cache/recovery/command
E: Cant mount /cache/recovery/log
E: Cant open /cache/recovery/log
E: Cant mount /cache/recovery/last_log
E: Cant open /cache/recovery/last_log
Everything was fine until I got a boot loop from a newly installed ROM... I pulled out the battery and thats when all hell broke loose.
From what Ive read... Looks like i bricked it. Any and all help is greatly appreciated....

hmm usually pd15 is flawless. What phone do you have and what recovery/version try backing up sd to computer and wipe data and all caches (dalvik and main) then re flashing a rom (CM7, Royal ginger 3.0, or AOSP preferred) if doesn't work or you have continuous bootloop go here and scroll down to my comment

No, you did not brick your phone, but your phone is most likely bricked. Do a search on eMMC chip...yours failed.
I have a mytouch4g CWR 3.2.2.4
But there in lies the problem: I cant wipe anything because it cant mount anything!
I get a whole bunch of "error mounting cache, data, & sdcard" and a whole bunch of "error formating system, data, cache... I cant do anything in recovery

And since I have S-off... If I ship to HTC for warranty I surely wont get replaced right? Thanx by the way...
There have been alot of people that sent their phones back rooted and had no issues. I actually don't recall hearing about anyone that had to pay for the phone..

May I ask why?
I checked out that file, and it looks like it has a ridiculously long file name. Did you rename it to "PD15IMG.ZIP" when you copied it to your SD card?
edit: Oh wait, of course you did. That's the only way the HBOOT will load the image.
Guess this isn't very helpful now, but from now on I'd be very leery of downloading and flashing any PD15IMG.ZIP files that don't give you an MD5 sum to check against to make sure they're not corrupted or anything.

Looks like my eMMc chip failed... Yup! its bricked...
Try sending it back to T-Mobile or HTC. I've read reports of people sending back their broken rooted phones and still being able to get a free replacement on warranty.

I searched a lot (hours, really) but i haven’t found a solution at all… (only more ??)
I was running the IncertCoin_Vision_v1.5 rom, and no problems at all. Only when i downloaded a app from the market my Phone hangs.. So, restart.. booted but after a few seconds (when asking for my PIN) it hangs again!
So i restarted to hboot > recovery and with Clockworkmod recovery (v3.0.2.4) i installed the rom again.. but now it hangs on the green HTC logo and won’t go any further.
So, again to Clockworkmod.. only now it is giving errors:
E:Can’t mount /cache/recovery/log
E:Can’t open /cache/recovery/log
E:Can’t mount /cache/recovery/last_log
E:Can’t open /cache/recovery/last_log
When selecting install zip from sd>choose zip it starts with:
-formatting system and cache..
-Clearing dalvink and boot cache..
-Installing system files..
E:error in /sdcard/InsertCoin_Vision_v1.5.zip (status 0) installation aborted.
Ok, ok. I made a backup with CWM, so after putting it on my (formatted fat32) card and starting CWM to restore it.. it gives an error: Error while formatting /system!
So I searched for a solution for this too.. but a answer? Nope. So is there anyone how can help me? Or point me to a thread that gives a solution? Is it a hardware problem (cache??) or software?
Sorry if there IS a thread about this.. i really searched.. and im so tired now..

I tried to reinstall PD15IMG.zip and found all types of that image. The PD15 (stock) the system stopped, I get bootloader - Failed - PU. All the time it stops in the same moment! I found another file (312 mb) that worked a lot of times, but it didn´t work for me.
Please help me.
First, wrong section.
As for the issue, sorry to be the bearer of bad news, but that phone is 100% bricked. Cache errors in recovery followed by the Fail-PU message when attempting to flash back to stock indicate that it has a failed eMMC with no chance of being fixed.

Supply more info please. Things like how you were flashing rom are a good place to start.
i booted into recovery from rom manager then i did my wipes i needed to do then installed the rom, and the phone rebooted started up to the boot screen of the rom waited for a while then it went to black screen and nothing happen so i pulled the battery then booted into hboot then into recovery and now i get that error
Did you run a nand backup? Or is your recovery giving you an error as well?
On my evo fresh 3.5
i did an nand back up n when i try to restore in recovery i get

I got the same problem as this. Any way of fixing this issue? I cant write anything on the system either.
Check your eMMc chip in the general forum.

Try starting over flashing stock pd15iimg.zip via hboot. It looks like you have the stock radio still so it should be a clean flash. Reroot and reflash cwm recovery

I know there's already dozens of threads with this problem but I've been going through them for three days now and I've pretty much given up trying to fix this. I'm posting here as a last resort.
All of sudden my nexus started shutting off or restarting and then it froze at the google screen, not the X logo like most people. I went into fastboot and then to recovery then tried the wipe data/factory reset option, doesn't do anything. Left it on that screen overnight and all that happened was the battery drained.
Now I'm stuck with in the android recovery screen because I kept trying to wipe it so much

I've downloaded sdk files, images and toolkits. I've done commands but I can only get up to unlock bootloader because it fails. I don't know what else I can do at this point, nothing seems to work. I've watched a video of a guy doing the wipe data/factory reset and it reset in seconds so I'm very frustrated with this thing. It's only 10 months old and I can't remember if I got a warranty or not and I don't even know how long a warranty is on these anyway.
Forgot to mention a couple things. When I try to wipe it, the android is frozen unless I press the power button and volume up again. Then the blue line and the little shape in front of the android start moving. There's no text here and the text freezes on -- Wiping data... Formatting /data and I never rooted it.

The reason there are no replies is probably because there is no solution if people can't help they won't comment and clutter up the board with useless posts. Tell me everything you can about your situation is your bootloader unlocked? Rooted? What ROM are you on? Can you access fastboot mode? Ect and I will see if I can help

I was on cyanogenmod 11 4.4. I had many random reboots and was going to do a factory reset through recovery, it was freezing too much to reset through Android. I used Nexus root toolkit when I rooted/ installed CM11. Now when I try to use the toolkit I get fails and trying though recovery gets me the E: failed to mount errors.

I tried to flash via fastboot and was unsuccessful.

What error message do you get when you use wugfresh NRT back to stock and unroot with the option for soft bricked
Edit: It just occurred to me the most common reason for flashing failing or "failure to mount" is a corrupted nand memory, a bit like bad sectors in a hard drive.
Nand going bad could produce the spontaneous reboots and would explain why you are unable to reflash it.
The only option if that is the case is RMA or a new motherboard.

So here's the thing. I got myself into a sticky situation . My Nexus 4 got soft-bricked from the lollipop update. Im stuck at bootloop(The small colorful dots that you see when you start your nexus).
Now heres the sticky part , The Bootloader wont stay unlocked(It goes back to being locked after a reboot, even after I did it manually with Fastboot oem unlock )
And I cant do a Factory OR a Cache wipe aswell
Im getting errors like

Hmm, something might be wrong with the cache partition.
So you can't unlock the bootloader, wiping cache from recovery doesn't work, fastboot obv doesn't work, and you're in a bootloop. So, is your warranty still available?
Edit: Thought of something
Try unlocking the bootloader, then, without rebooting, try flashing the factory images:
