This might have been touched on before but i have read so many pages i cant seem to find it. i am still a noob and usually pretty good about figuring my problems outby reading thru the forums.
I have a mytouch4g/glacier and am stuck in recovery. When i try to flash a rom all i get is assert failed: write_raw_image("/tmp/boot.img"."boot") (status 7) installation aborted. E:cant mount /cache/recovery/log
E:cant open /cache/recovery/log
E:cant mount /cache/recovery/last_log
I have installed several roms before and no problems. Wanted to try cyanogen 7 rc4 and it kept rebooting so i decided to go back to what i had and this is all i get... Please Help.
What recovery version are you using? What rom are you trying to go back to? Did you download CM using the phone it your computer.
Sounds like a bad download. Hard to say without more info though.

Actually I had to end up unrooting my phone to get it to work again... any Rom I flashed wouldn't work... so im gonna stick with stock for now until I can root again.. I still have s off dono if that meens I have to do anything else

that's what I'm thinking too. I used to be able to switch ROMs and restore nandroid backups, but after updating to the latest clockwork I got the "can't mount /cache" errors and can't fix it now.

If your recovery image is corrupt, you don't have to un-root an re-root again. Just download a new copy from here and flash it via terminal emulator.
I'll post instructions for anyone wondering how to flash via terminal.
First rename the downloaded file to "recovery.img", then follow this:

start the terminal app
su(press enter)
flash_image recovery /sdcard/recovery.img(press enter)
reboot recovery(press enter)

that could work if the phone booted up, but as of right now it doesn't boot and it won't since I had just wiped the phone when clockwork started acting up with the "can't mount /cache" errors. thanks for the suggestion though.
I was pretty desperate to get my phone working again so I jus dl PD15IMG.zip put it on the root of my SD card n went into hboot n let the phone do the rest. Back to stock byt hey I'm phone is working again

you lucky dog.... Mine does not want to restore with PD15IMG.zip, it fails on items 1-6, skips one more and freezes at updating the radio. Even tried with a freshly formatted sdcard.

Ok I am hoping I didn't brick my phone, but seeing as I can get into clockwork, I am thinking I just messed something up.
I get the following error in clockwork:

Can you get the ROM up and running? you just cannot access the recovery image is that correct?
No, unfortunately, I try to flash a rom and it won't boot past the boot screen.

have you tried flashing a recovery image via the hboot? it sounds as if something is boogered with the recovery image.
yeah that's what I am going to try to do. Seems as though it's an issue with clockwork we'll see. I guess if I unroot it works again. Hopefully a fix will be discovered soon.

What it looks like is our part7-xxxxx.bin file is being removed, so when you try to restore using pd15img it doesn't work....

I think you may be on to something there. the combination of RoyalGinger and the latest clockwork seem to have bricked my phone, I'm getting the "can't mount /cache" errors on clockwork.

I don't think that's what it is, I'm using the part7xxxx.bin file on a freshly formatted sdcard along with pd15img and it's not working. Seems more like the internal partition got corrupted by either RoyalGinger or clockwork, or the combination of both.

Can it be that royalginger requires to format boot & the new clockwork doesnt have that option anymore,im on royalginger & im now afraid of flashin to any other rom.......

i'm using clockwork recovery 2.5.1
try making a folder on your sd card, clockworkmod.
tried that, and it started working for me, dunno if it was a coincidence though.

I had the same problem. I think its due to the original JRDNEM_U3_2.59.0 sbf.
SO... Use this:

When choosing your firmware select the Nordic 2.2.2 (Froyo) version [JRDNEM_U3_3.4.2_177-005]. Then re-root using : http://forum.xda-developers.com/showthread.php?t=913709
Then you should be able to mount sd card in CWM.

I've been trying to install a custom ROM (CyanogenMod) all day on my phone but now I'm pulling my hair out by the clumps. This seems to be one stubborn device to play with. My desire didn't give me any trouble at all.
Please help with the following queries I have:
I have installed 2nd init boot and then was accessing clockworkmod through that.
But when I try backing up my ROM I get the error "cannot mount /sdcard".
On the defy if I press up and power on I get the default bootloader with battery check. I can only get into clockworkmod from the recovery app.
On my desire, I can directly boot into ClockworkMod Recovery by pressing up and power button. I don't remember now how I got t to do that, but I would like that in my Defy too. I suppose this will solve the sdcard problem?

This is hell annoying but simple solution worked on LGO2X. Reinsert SDCARD while in Recovery. It should work in all devices. Nothing stupid to try. Tell me...
It is because you formated your sd card with some other file system. Just go to settings / sdcard and storage . Unmount your sdcard and format it. It will get mounted in cwm from now on .

USB Debugging is NOT on when I reboot.
I formatted the card, remounted it in normal operation, then tried going into the Android Bootmenu and mounting the sdcard, formatting it, backup but none of them worked b/c it couldn't mount the sdcard.
I also tried doing the same thing, but not remounting it in normal operation and shutting it off to no avail.
I also tried adding the folder: /sdcard/clockworkmod but with no luck.
Specifically, when do Custom Recovery, then in ClockworkMod Recovery, do backup and restore and do backup, I get the error:

This particular version of the OS makes rooting very difficult. z4root doesn't work as it exploited some security feature that was figured in the firmware update. Same issue on Galaxy S Plus
Have to same situation here with a Galaxy S Plus (9001). Everything works great mounted on the stock rom, but not in clockworkmod. Card is formated as FAT32 with clustersize at 1024 bytes. It's another phone but it might be the same issue ... Reinsert card while in recovery doesn't work, it reboots the recovery I think. Putting a folder 'clockworkmod' on the sdcard also doesn't work.

Is there any advice for this problem? I have the same problem. I realized the last time i rooted and flashed a moto defy (different one than now) i was on T-Mobile USA 2.2. Hi !
today I decided i should upgrade from CM7.0.2 to nb 118. But I first installed latest radio. I did a backup at this point. The radio flash went ok, the phone booted and everything worked fine.
I installed nightly build 118 from ROM manager without google apps, and with a dalvik wipe (no data/cache wipe). The phone booted up ok, but some widgets had disapeared, some shortcuts didn't work and some apps had disapeared too, the autorotation of screen at 0° had been unticked too... I rebooted, and finally decided I should revert to my backup.
That's where the real problem happens : after the backup restoration, the phone is stuck in a boot loop.
I can start with fastboot mode, but I can't access recovery mode, the red exclamation mark is there...
I'm not sure this tutorial : http://forum.xda-developers.com/showthread.php?t=751265 is adapted to my situation, should I follow it ?

This should be @ the Q&A section but anyway, here we go:
I just had that bootloop situation myself and yes, you should follow that usb unbrick tutorial. That'll help you. Also, you should try if you can get to fakeflash by pressing VOLUP + POWER while @ the red exclamation window. If you can and you should be able to, search XDA for "fakeflash clockworkmod" or something similar, you should find a CWM image in a ZIP form. Then put that image into your SDCard, rename it to update.zip, go into the fakeflash menu and apply update.zip, it should fly you to the real recovery menu where you can do a full wipe and flash your ROM again.
Hopefully I'm not forgetting anything =D At least that's how I solved my situation and my phone is working fully again!

Do you have the update.zip from recovery clock in the root of your sdcard, if so just press volume up and the power down at the same time, or the other way around ;-) when it change picture choose option, install update.zip and you should be in recovery mode

it fails to verify the file.
and when i do get in to recovery I get E:Can't open /cache/recovery/command
i was trying to root it and it is not going so well I just want to get it back to 2.2

That "Can't open /cache etc." error message indicates that you are in the fake-flash menu. Do you have a working recovery image in the root of your SDcard renamed to "update.zip"? If not, try searching the forum for "clockworkmod fakeflash" or something similar and it should show some topics that will help you.
I recently got my Legend working again from bootloop and usb brick so I'm ready to help you with your further problems that may occur =)
EDIT: And please, make sure that your recovery zip is just "update.zip" and not a ZIP file named "update.zip" cause then it will be a file called "update.zip.zip" =D

I hope you read this and if it isn't fixed yet then take a rom YOU KNOW that works.
go into recovery Go to "mounts and storage" and format SYSTEM. NOTHING ELSE.
Do a wipe and THEN flash your zip.
Remember to NOT go out of recovery when you have formatted the system.

Hello to everyone, well i was new to the whole rooting and flashing roms thing, but i did it anyways and it seemed to work just fine , i will explain everything that i did:
i rooted my my touch 4g using the gfree method , and it worked then i flashed the rom royal ginger 3 and it booted just fine , then i just decided to turn off my phone and when i rebooted it it got stuck on the boot loop that i mention, i tried going back to recovery erase all data and re-installing royal ginger but it wont let me erase a thing not data not cache data, nor install the rom, its says the following:

E: can't open /cache/recovery/log
E:can't mount /cache/recovery/last_log
E:can't open /cache/recovery/last_log
and it goes on like that on the bottom of the recovery page
i would really appreciate the help as i am very frustrated

your phone might either be dead or you have on chance to save it. hapened to mine yesterday the same thing and now doesnt turn on and i already called tmobile for a replacement. Search on how to get the pd15img on there using hboot if you have it if not your phones dead and theres nothing you can do

sounds like its dead or a bad root. once you get it back running i would unroot and use the visionary method to root and flash using the clockwork.
if u made a stock backup like all threads strongly recommend before u flash a new rom, then just boot into recovery and install your stock backup. You do not need to wipe anything when u restore a backup. If u were attentive enough to make a stock backup then restore it and Then start over by unrooting and rerooting

Your emmc chip fried,I had the same issue. Try to use adb to push the stock recovery, then go to Tmobile an get it replaced.

The op says nothing about not being able to restore a backup. When it says ” I tried to reinstall the rom” it sounded like he was trying to reinstall the zip file that booted once and then went kaput. I was only offering an option to get the fone working again. I've rooted over 100 mt4g's and ran into this issue myself and was trying to help out with what worked for me.

hey, i'm not sure if you just can't read or you're blind or something... but the OP clear states

if he can't even erase anything, then he won't be able to install anything either.
Mr. Highway is right, your phone's eMMC chip probably died, like my old one did.

no it does not and my emmc-samsung says 2151 mb 4407295sectors
So I downloaded new kernels and CM10 for my newly rooted N4. I did a backup before flashing the kernel and didn't pay attention to the error message saying cache was not found or whatever. But the kernel flashed fine and it booted up, nothing out of the whack. When I went back to flash CM10 it said the MD5 sum didn't match so the flashing processes didn't finish. Since my backup was ****ed up and the MD5 didn't match I couldn't restore the system either. Device will boot and will go into fastboot and recovery. Currently using CWM. If I turn on the device normally it will stay on the GOOGLE screen and it will not go into the boot screen. What do?
Usually when it hangs on the google screen it is kernel related.
You can try booting into recovery, clearing cache and flashing the kernel.
If that doesn't work, re-flash the ROM with signature verification turned off. (in the update from .zip menu)
OR You can push a new rom with ADB to your sd card. adb push /sdcard/

The CM10 zip itself is bad. How can I get another one on the phone? Or can I flash the CM zip without the file being on the phone? Thanks
Also, the error message popped up when flashing the CM zip was set_perm: some changes failed

I would just push a new cm ROM to your phone. Command is.
adb push nameofzip.zip /sdcard/
Then flash it.

3 years of rooting and flashing stuff on 3 of my previous phones, there wasn't a time when I had to do this, or had to ask for help. Damn it.
Could you be a little more detailed in how do I go about doing so? As you can see my post count I am a bit of a noob Despite those 3 years. I just don't wanna mess up again, you know?

Sorry I was out and about the past few hours.
You want to be in recovery. Custom recoveries allow you to use adb.
Rename the ROM something easy like CMrom.zip
Then put the CMrom.zip in the same location as adb on your computer.
Open the command window from that directory. (hold shift and right click; then choose open command window here)
Type the command: adb push CMrom.zip /sdcard/
It will push the file to the root of /sdcard/
Then you can flash it.
If that fails, use toolkit to download factory image and flash it through fastboot.
Oh man you guys saved my life. I cannot thank you guys enough. You won't believe how happy I was when I saw the bootscreen. Glad that was dealt with. Again thanks guys
Hey everyone I don't know if this is the right place to be posting this but I had the same problem as OP a few hours ago. I flashed LiquidROM and it got stuck on a boot loop. Needless to say, I panicked and used the adb push method since I decided to delete my AOKP ROM from my folder. Anyway, while panicking what I did was is before flashing, I backed up my entire SD card onto my laptop as ADB backup wasn't working and I pushed the entire folder using "adb push sdcard /sdcard/. Next thing I'm in recovery (CWM) and when I go through the flash from sdcard option, usually there were 3 options name 0/ and 2 others, however since I pushed the sdcard folder to /sdcard/ I get a 0/ at the top of the list which is where I would usually go but then underneath that I get all the folders I pushed! Is there any way to sort this out? I could attach pictures at some point for those of you who find it hard to picture what I'm describing Thanks in advance for any help!
Edit: just to add a couple of other things, my NANdroid had a checksum error so I cannot revert to stock without having to manually flash it which I cannot do at the moment due to a lack of WiFi, is there any way to sort this out? And the other thing was that for some reason sdcard cannot be unmounted at all in cwm, is it meant to be like that or am I missing something? Afaik its supposed to toggle.
