Everytime I attempt to exit my camera, either before or after taking pictures, the entire phone locks up and I have to soft reset. I have tried both the 4.* camera, as well as the 5.* camera, and they both have the exact same problem. Anyone have any ideas? Using DCD's 4.1.2 ROM
using the same dcd ROM and i DO NOT have the same issue.
in your case i would start by checking the camera right after a fresh installation of ROM

I've got a Rogers HTC Magic, and whenever I've tried a ROM with Android 1.6 or greater (I've tried 3 or 4) the accessing the camera will cause the phone to reboot itself.
It doesn't happen the first time I use the camera, but it does the second time and consistantly.
Does anyone else have this problem?
1.5 ROMs are fine. I'm currently using cursorsense and it's not an issue but at some point I will want to use a newer version of Android.

Relash the rom. If you are using any of the legal roms (cyanogen/superd) flash the base before the rom. remember to wipe before starting.
I've tried cyanogen 2.1 test2 and Smoki both causes reboot when using camera apps (barcode scanner, google goggles, fxcamera, ...).
It doesn't reboot always tho like 1/3 times i use one of these apps i get a black screen and then a reboot.
Im using CursorSense now and all the camera apps work just fine :s.
Always did a wipe before flashing the roms.

Happened to me once a couple of weeks ago [stock rom]. tried again and it worked.
I keep getting this after installing the latest o2 stock rom.
It is happening to me with or without stock ROM...
Have you got opera 10 installed? if so uninstall it and the problem should go away. I had the same problem and since removing this version of opera, the problem hasn't re appeared.
I have the stock rom, and this had happened before I installed opera 10.

I use Close Process to kill camera.exe or opera.exe when I get this problem. Now when I close Opera, I can use Camera after 5 secs.
I have no idea why.....
I recently flashed Myns Rom to get the latest radio, prl, etc. Afterward I re-installed CM6.1.1 and noticed my camera didn't work. Just a black screen when launched, then force close. I tried a MIUI Rom too, same story.

After being frustrated and only being able to use camera with a Sense rom, I decided to revert back to the previous radio.

Once i did this, I was able to use the camera now with CM6. haven't tried a MIUI rom yet, but I assume the same results.
I hope this is clear, with caffeine involved, my thought to type ratio is 11.5:1
Edit: Do a battery pull after your first boot into ROM. This seems to help too.
More information now
Install a sense ROM, update PRL. According to Sprint Customer Service, they have had people call in with this camera problem and have walked people through the PRL update. This then also makes your camera work again. After the update, you can flash an AOSP rom.
I have had issues with some of the latest releases of MIUI and the camera. The camera issue I described above happened again after flashing MIUI. There are atleast 3 instances where this issue occurred. Not pointing blame, just describing what I have done to recreate and attempt to solve problem.
I have the same problem. CM 6.1.3 would be my daily driver if I could figure out how to get the camera to work. When do you flash this? before installing cm or after?
I flashed and still no luck. I have the 003 hardware with the 2.02 hboot not sure if I have the new camera

You want to flash the radio after you have installed the ROM. It will reboot back into recovery and complete install. After doing all of of this, let phone reboot, then power off, pull battery and put back in. Then power on.
I hope this helps. I have been able to use my camera with no problems since the battery pull. I believe the new camera came with hardware 004.

If wifi is not on, the pic is not saved, camera app closes and it says camera failure.

Go to Settings> Location and Security and Uncheck Use Wireless Connection. It will work fine then

Same issue on JPM, if wifi is on, all pictures are correctly taken. If wifi is off mostly the first picture succeeds but second cause the camera failure.
Does somebody know of another type of fix, patched Camera.apk or something? Wireless location is extremely handy for google maps etc.
I have been having this problem since JPF and I was hoping JPM fixed it..but no. And these were the updates the KIES gave me. Now I can't even connect to KIES anymore. Samsung has really messed up this whole thing... man...
JPM fixed this problem for me (flashed with Odin), you guys should try a factory reset.
Factory Reset did not fix that problem for me...
Going to reflash JPM with factory reset and wipe cache (both from recovery), will report back....
Ok, somehow the following steps at first seemed to fix my problem with the camera:
1. Before reflashing JPM firmware I did the following from recovery:
1a. wipe data/factory reset (scroll down to Yes -- delete all user)
1b. wipe cache
1c. reboot system now
2. Turn off phone
3. Turn on phone in download mode (volume down+home+power)
4. Flash JPM with odin
5. After flash and reboot again back into recovery (thru adb reboot recovery or volume up+volume down+home+power)
6. Again wiped data/factory reset & wipe cache & reboot (exact same steps as 1a-1c)
After these steps no camera failures...but after manually setting the apn of my provider ..... camera failures even with 3g turned off
Deleted the apn and no camera failures anymore :S So I think my choice is: mobile 3g internet OR fully functioning camera.
3g settings: mmc 204, mnc 16, verification pap, apn type internet (T-Mobile NL)

I don't know exactly what I did but after a few hours of playing around with settings and APN widgets the camera is now working correctly, no camera failures to date.
First I downloaded an apns-conf.xml from cyanogen firmware where the APN settings for my provider were available (since I had to manually enter them and that was the moment the camer failure started to occur). I copied the apns-conf.xml from my phone (/etc) to my computer and added the lines for my provider and pushed them back to my phone. Onfortunately this did not fix the problem.
Then I tried numerous APN widgets to disable the apn (with prefix) or switch to an empty APN, both also unsuccesfull. Uninstalled these apps and widgets again.
After all these tries, I dove into the application settings and for some reason I thought it was a good idea to clean the data of the, in dutch "Netwerklocatie" app (in english i guess "Networklocation"). After that no more problems.....could be some corrupted data in this application...
Hopefully this works for all of you!

i hav camscanner and taking one or two pics from camscanner fails the camera...
i usually wait for sometime and then camera works as normal....

Damn,problem keeps reocurring:-( cleaning networklocation application data only helps for a small amount of time...
Only option now is disabeling networklocation service or using different camera application,does anybody recommend a good basic camera app?(stocklike and fast startup)

Im running the stock JPM I got via KIES. I've done several wipes/hard resets lately cuz Im currenly having problems with installing a theme. Or at the moment, installing the Contacts.apk correctly... Yet the camera fails. I just tried.
Maybe it is the ROM, I don't know. Am not yet brave enough to flash... Want to fix this theme issue first.
I'm on JPM too, flashed twice thru Odin and wiped everything. It has something to do with APN/Networklocation but I don't know how to fix it permanently

Blame Geo-Tagging
The reason why our phone can't (normally) take phtos is because of geo-tagging.
Note that when you use the camera.apk, there's a live notification at the top, a sattelite getting sinal. The camera is trying to associate your photo (it happens with any app that uses camera) with the place where you took it.
When I flashed the Apollo 1.0 rom (deodexed custom rom), this problem was solved. The camera.apk don't even show this notification, and I was able to take as many photos as I wanted to with no crashes. I tryed to use his apk in others roms... but no success. I don't know if are some dependencies, libraries... But I know that it worked!
What I do is to turn off networks (fligth mode), and take photos. Works perfect!
Bought an S4 last week. Everything looked good, I tested the sound, vibrate, gps etc and it all checked out. However, stupid me didn't check the camera (I know I know).
So the whole phone seems to run fine, but the camera (front and back) can't be accessed by stock or 3rd party applications. (Usually "warning Camera Failed" or something similar). I've factory reset, cleared cache and everything. I even installed the nandroid backup from a working phone, but the camera gives the same issue.
Because it's both cameras I thought it had to be software, but after all the resets and cache clears I'm starting to lean toward hardware.
Can someone give me an idea where to start? Would it be camera connectors or motherboard? Is there a sensor that may be causing this? I'd like to get an idea before I start gutting my phone.

If that does not help either..flash stock rom via oding
If that does not help..turn the phone off..charge till 100%..turn the phone on..clear the ram..and start camera..

Had this same issue a while back. What solved it for me was to re-flash the stock firmware. I too tried a nandroid backup, it didn't work but the flash did.
Turns out it was software related.
I installed a few stock firmwares that didn't fix anything. I didn't want to wait the 2 hours to download for free from Sammobile.com, but eventually i bit the bullet and got the appropriate (latest) version of my phone. After the reboot it was solved.
Granted, I tried many different firmwares, and some had the same name as the official firmware that i downloaded from sammobile but it did the trick.
