Do you accidentally have them diverted to another phone or something? Really strange... Can you ring him?
The exact same thing happened to me a couple of days ago. The missus wanted to know why I hadn't rung her back, but I had no missed calls. I got my brother to ring me, stood next to me and it was showing it was ringing on his phone, bit my phone was oblivious to it! A reboot fixed it, but there's no way of knowing if it had happened. I could still send and receive texts.
A similar thing happened to me 2 days ago. The phone wouldn't ring, even when I called it from a different phone, but I did get a missed call notification. I had to restart and then re-choose a ring tone and then it worked again. I have no idea how this all of a sudden happened as I didn't change anything in the phone.
Were you using GPRS connection at the time? Because when connected to GPRS, and data is actively being sent/received, the phone will not respond and the caller will get an engaged tone.
This is ONLY when data is being sent/received, and only on GPRS, not 3G/HSDPA (Edge i'm not sure about)
Edit - this is based on quite old information from my early days with GPRS, you might want to check it.
I don't know what connection I was on, but I wasn't sending an engaged tone. Anyone ringing me had the ring tone, just no answer (obviously really as I couldn't answer even if I knew it was ringing!)

Is the "phone/ ring sound" switched of?
This may be associated with the CM6 that I'm running, not sure. But today it just started happening. When someone tries to call me it rings about 2-3 times then BEEP BEEP, dropped call for them. I can make calls outbound, but when someone tries calling me they cannot. Any ideas?!

Looks like I can receive calls from land lines, but not AT&T phones. This is driving me nuts..

I know I must be missing something easy here.
When I am on a call and I get a second call I can answer the call. I can transfer between the two calls, but I can't hang up on one of the calls. The second call seems to stay connected, even if the other party hung up the line.
How can I hang up on one of the calls without disconnecting both calls?

Yeah, it sucks. That's CDMA for ya. I always worry about it using my minutes because it's usually a landline that I stop talking to & switch back to a cell call.
I just drag the notifications bar down and click the end call button to the right of the person's name.
Hello, i've got a strange issue with my Nexus 4. Sometimes, when i make or recieve a call, the signal goes down, but the call doesn't end. And i think that the person on the other side of the phone can still hear me, but i don't hear him. I must close the call and make a new one to keep talking. It happens a couple of times every day and it's very annoying. Does someone have the same issue?
I'm not really sure if this is an isolated incident to a specific person or not.. but here is the situation:
I dial a number and my phone rings and rings and rings.. Eventually I hang up as there is no connection or voicemail. However, the other person picks up on their end and can hear EVERYTHING unbeknownst to me.
Am I alone on this or has anyone told you they can hear you even if the phone doesn't connect? This happened at least twice that I know of (just now and yesterday).
