I downloaded Ricardo's repair.zip file and took the liberty of mirroring it on xda-developers, since my download went at a whopping 200 bytes per second.
We've seen this recipe before, and it does appear to fix at least some of the problems caused by RSU upgrades not finishing.

Just try to do following: start 3.23 ROM update to extract RSUpgrade.sa.CAB and place it to SD card instead of file found in repair.zip.
The situation you describe looks like your device died while flashing new 3.23 version. So you may try to continue process with 3.23 version instead of old one. This is because 3.23 don't allow to "downgrade".

My device died while trying to upgrade to radio 3.19 but I trying to copy to SD all Radio upgrade, but don't work. Still stop on 1% and error message is "Flash failed. Press OK to restart device".

i have the same pb the alnasser and sebi; i have tried everything; what we need is a new rom version for the radio and not an upgrade; also while using boot loader and if i try to copy the GSM rom from a working phone i got a message saying: connect to RS232 and run PC monitor; i have USB and i do not know what is PC monitor...any ideas???

The soloution above worked for me after a little trial and error.
I was trying to upgrade to new UK Radio Stack but flash failed in the middle of upgrade (probable because the keyboard popped up onto the screen for no apparant reason, then locked up).
However, after much wailing and gnashing of teeth, I found your website!!
I installed the Files onto an SD card as described above, and although the XDA was going painfully slowly (about 3 mins to warm reset). managed to get to the today screen. I ended up pusing the "record button" repeatedly to try and get some response out of the machine, finally it auto ran the program on the SD card, only to be told that flash failed
Undeterred I repeated the exerscise with the other RSUpgrade.sa.cab
file, this gave the same result!!
However I soon realised that this cab file is kept within the .exe file of the original upgrade I tried to do, so... Using winzip I extracted the relevent RSUpgrade file for my XDA and put that file on the SD card instead.
It Worked!!!
Consequently I conclude that if the file fails or stops at 1% then it must be because it is NOT the correct file for your XDA, you need to use the correct upgrade cab file for you particular XDA.
Hope it helps someone else.

i have removed it from charger and put the file on the sd card,the progress bar went to 99% then flash failed,now it gives me that message on 1%,how to fix it??!!

seems to be the same case as I had: I found version, which did not freeze in 1%, but in 85% all stopped.
I unfortunatelly don't exactly remember complete list of my steps, but THIS MAY BE IMPORTANT:
After I made cold restart, I saw radio upgrade progress bar ( If you can't see this, try to follow instruction about repair.zip solution, mentioned hereinbefore ). Radio upgrade progress window only flashed for a second or two, then disappeared and WIN CE ( trying to boot up ) painted its splash screen. After that WinCE froze with this screen.
BUT:
If you have a luck, radio upgrade may continue on background. After 10-15 mins upgrade was done, and after restart ... I saw normal radio version in bootloader, and it worked well !!!
Note 1:
I killed phone with latest radio upgrade ( maybe lack of space, maybe radio switched on ), so there was no way to use lower versions ( it leads to "1% problem" with downgrade )
Note 2:
When my steps was still unsuccessfull, I continued [up|down]grade WIN CE and Bootloader. Maybe it was solution, maybe not, I dont know. I did it because I thought it would solve some problems with space for radio installation. My second reason was, that it should stop neverending ( 1% freezing) radio upgrade window after each boot.
Note 3:
During these installation steps it seems to me that battery si getting weaker, and does not recharge. So I tried to revive WIN CE first to be able to recharge.
Some steps may be unimportant, based on my feeling how it may work.

Thanks for all of the good work happening her in these forums. It’s really amazing how much information is here. I have the following situation and wonder if anyone can help me sort it out.
I have a running qtek with an un-working radio. I have the developers SE ROM 1.2 installed and it’s running but very slow. The system can’t detect the battery and its charge. The radio is off and will not turn on. I tried to install the O2 4.21 radio update using the SD card using the repair file downloaded from your site but no luck. It stops after it reboots and start the upgrade process with the dreadful 1% can’t flash error. From what I read so far the problem hover around the radio not being able to detect the battery power so it stops the flashing process. I tried changing the upgrade file and to do it many times but still always stops at 1%.
One way I didn’t try is copying the upgrade file to the windows root directory and tries to upgrade from there instead from the card. I don’t know where to find the 2 monitors files needed to be installed there before the upgrade. Anyone knows where to get them from?
Is there any other way to flash the radio using the SD card method (I am traveling and don’t have the cradle with me.)

I have exactly the same problem as ahmedateeq. Runnung the repair.zip files as suggested by beeheij just results in Radio Stack Upgrade failure at 1% every time the XDA soft resets (the only way to break this loop is a hard reset). I have tried two different RSU upgrade versions, but to no avail.
What else can be done?

Did you try to turn on your GSM using the bootloader, by Holding power button then Soft reset the unit, then if you see the logo "Bootloader 5.15" press the calendar button, then you will see the GSM menu, try to select GSM ON then press the action button( the middle of navigation key), when it go back to the menu then soft reset again, then check if it is working or not..Please telme the version of your unit RSU and RUU... thank i hope this will help you.

Did what you said - I was able to turn on GSM in Bootloader (though there was no message to confirm) - but this hasn't improved the situation. I still get the upgrade failure at 1%.
My RUU is SER1.2 (based on O2 3.17.03. RSU is (WAS!) O2 4.21
Any more ideas? Is it worth trying to copy the upgrade files to the Windows folder on teh XDA and running it from there?

Yeap they're included in the RSUpgrade.sa.CAB!
What I would do is copy the RSUpgrade.sa.CAB to my device and run it from there and if the upgrade fails do a search for the needed files and relocate them manually.

I was curious. I've noticed people said that if you put the 4.00.16 ROM on, the only way you can downgrade it, is if you reprogram the ROM from the SD card and not by the EXE method.
I was curious as to WHY? What does that .16 ROM do to the XDA to prevent you from doing an EXE rom reprogram?
So far, we lacked time to look into this one. Tell us, and we'll see if we can fix it.
Downgrade unseccesful using SD method from 4.00.16 ver
Sorry if this is OOT: I'm a non techie person so please somebody explain me how to use the SD card reflash metod to downgrade from 4.00.16 bact to 4.00.11 version. If write the new rom to sd using XDAtools it's done. after it I put it back the SD card back to the XDA. I enter the wallaby 5.17 screen by pressing power+ soft reset button. It said wait and then GSM ok after that freezze.
What I'm missing?
The why, I can't say as I have not investigated in detail, I just said to heck with it and bought a SD writer.
What happens after you load 4.00.05 and 4.00.16 is when you try to run and upgrade/downgrade/re-install from Prgramme-A or the newer programmer that came with the 4.00.x installs it fails saying it can't connect to the device, please check to make sure it's plugged in (even though there is a active connection with Activesync).
Using the SD card there is no issue with running the upgrade.

but what is it that .16 has that .11 doesn't that stops you from changing ROMs via the EXE method?
Also, after putting on .16 and you go back to say .11 via the SD boot method, are you able to use the EXE method to change ROMs after that?
Currently I use the AT&T A20 radio stack and was thinking about trying out the Tmobile 6.24 to see how it works out. Haven't decided yet.
Investigating the no reflash .EXE method
Ok I have found a roundabout method to resolve this only using the .EXE method.
I used the rom kitchen to generate me a 4.00.11 ENG WM2003. After flashing my phone and wanting to make sure i could revert I tried my origonal o2 provided flash upgrader "Radio_Unit_Upgrade_v31413.exe" and the latest downloaded one from the o2 website "Radio_Unit_Upgrade_v31703.exe" neither of which would let me flash them - i get the error "this rom is not for this device" (or some such).
I went back to the rom kitchen and built myself a 3.17.03 ENG O2-Euro rom image and flashed that onto the phone. this suceeded where the o2 stock .EXE failed and I was nearly back where I started.
Now that I had my rom kitchen 3.17.03 back on I tried to install the latest "Radio_Unit_Upgrade_v31703.exe" file again and was greeted with sucess once more!
The question this begs is the same as the origonal however, ok we can get the stock provider rom back on - but why will it let me flash the rom kitchen 3.17.03 back on and not my o2 downloaded rom? Hopefully the developers will be able to spot whats going on and sort it out in the kitchen (hopefully removing what ever right protection the new rom installs).

I think the "official" rom checks for versions, whereas the roms from the kitchen use a modified bit of code called "adaptrom" to fool the software. I wish I had a tenth of the capabilities of the guys on here that work out all this stuff.

I'm not sure about this, but i will give you some hint, ok :?
The T-Mobile Rom has a protection to it that you can not upgrade any ROM except T-Mobile ROM's other than that if you have a version of rom that is not T-Mobile ROM you can use the adaptrom to normally update any kind of ROM because adaptrom ignores the Version like PW10A1:007, the explanation of this versioning is in this forum(some kind of country and GSM band 1800/900, 1900/900).
Now why we have to use SD card after we update T-mobile ROMs, if we want to change to another kind of ROM's, because if we use "exe" or sometimes we called it "Program A" it checks the version of the ROM's, since the adaptrom can ignore only the version of the ROM's it can ignore the Type of ROM's, if it is T-Mobile or other ROM's, now for SD card it doesn't check for any version or type, and it goes directly to updating the ROM's in the Bootloader way or we can call it forcely updating the ROM'

I've downloaded the 4.00.16 ROM by mistake, it really didn't tell the ROM number, so now I want to revert to a earlier one and I can't.
I've got the following message: "ERROR: GetDeviceData - An established connection was aborted by the software in your host machine.

and then
"ERROR 011: EXECUTE REMOTE COMMUNICATION PROGRAM ERROR. Cannot execute the remote communication program. Please make sure that the USB/Serial cable is properly connected."

No matter which ROM I cook, when I try to install it using the .EXE it gives me the message I posted before.
Would you explain to me, idiots guide, how to flash not using a .EXE?

i have been searching the forum for the solution of this problem, some guys suggested solutions that involve a file that is no longer available, upgrade.zip which is aparently a radio upgrade(or downgrade) made for the SD card (a startup.exe in a startup directory).
Well, having said that, i had to solve this problem last night, and here it goes.
after trying original ROM upgrades, all versions, 3.14,3.17,3.18~4.
nothing worked.
finally, i grabbed a copy of xda-developers's 1.1 ROM
and guess what!!!
the phone started up, gave me the screen allignment and the whole menu!
sure enough, ran file commander(that version did not have file manager) copied the RSU files over to the windows directory, and the exe file to the windows\startup and rebooted the phone.
it stopped at 1% again... NOW REMEMBER, i had an EMPTY BATTERY...
the 1% ERROR is a battery issue, that turnes into this issue when the modem upgrade FAILS.
so here is what i did. and you be the judge.
i ran active sync from the phone, and it did connect(while the upgrade program is still running) and left the room/
in 10 minutes i came back and the upgrade was completed, so here is what i think
i think that the RSU will still run if it was in the background!!! even with an empty battery(connected to the charger ofcourse.
i did not have much time to do anymore testing, but i am sure that someone with the same problem will make the time. the key from my experience is that the proccess will complete if it is in the background.

i forgot to add that the upgrade i used was taken from yorch's page and it was the latest Tmobile, i had to downgrade later to an older version that i have to solve the network issue, but it was done(the downgrade) like a charm, i had a full battery then.

No success with SE ROM 1.2. Where could I download 1.1 from? Found it nowhere.
One more thing: maybe the version of active sync counts? Mine was 3.8.

your problem is in the network selection, search the forum for the proggy that changes it from 1900 to 900 and you should be OK.
i hate telling anyone to search i know how hard it is, but i do not have a clue where the proggy is, meanwhile, if i run across it, i will point it out,
if not, go to the bootloader, select GSM and change it from there, you will loose all data so backup first,

I just picked up a XDA from a guy who screwed up the radio stack upgrade (it cosy me @$80 USD ) and im now on a mission to fix it...
using your advice I used the XDA developers rom 1.1, this got the phone to start up with the working today screen and menus etc....
I install/copy over the new RSU upgrade files over and reboot
but here is where i get a bit stuck...
I cant get into active sync fast enough before I get the 1% RSU upgrade error, so the upgrade wont run in the background, it just sits there....
how do you get into active sync before the error comes up?

Tried the latest Android Froyo build on my phone with some interesting results.
I downloaded Froyo, copied the Android folder to my SD card, hit CLRCAD and then Haret. Froyo started booting, but after 4-5 lines all stopped. There was nothing happening after 15-20 min, so i pulled the battery, and started the phone again.
Now i get the message (in Norwegian)
"The file <filename> cannot be opened. Either it is not signed with a trusted certificate, or one of it's components cannot be found. If the problem persists try reinstalling or restorig the file"
The problem is that none of the programs installed on the SD-card runs. They all get the above error.
I can not install any programs (reg editors) because i again get the above message. My registry editor that is already installed wont work, and i can not install anything.
I have tried Googling the problem and i've also searched the forums here in XDA. But i have not found anything that actually might work, because they all wants to install something. I can not install anything, and all of my 3'rd party apps doesn't work.
How do i disable the certificate verification without having to install anything or run any 3'rd part apps??
Is there maybe any program that i can run from my Win7 computer??

I'm also having this problem, seems to affect .exe files regardless of whether they are on the SD Card or on the device itself. Same cause as well.
2 reasons may be:
Either you run a stock rom and you need to install certinstaller search here on xda or run a custom rom
Or the sdcard is corrupt so you have to reformat
When you post issues pls give details as well like what hspl, rom, radio you use so people could help you out

Hmm, yeah, it's a stock ROM (for now). I cant't install anything so the certinstaller can't help me.... Now i have to find out how to run a custom ROM.
I have also reformatted the SD-cart with no luck.
My (for now) stock phone:
Manila: 2.5.19211619.0
ROM: 1.66.409.1 (76641) NOR
Radio: 2.06.51.07
Using a custom ROM as a base for Android will heed much better results.
Full instructions for flashing a custom ROM are found here (and here for TMOUS HD2).
^^^ Also I notice you have an old radio (2.06). I'm quite sure you need a 2.10 or 2.12 radio for Android...most people seem to have had the best results from 2.12. You will learn how to do that in the link above I just gave you.
As well, there are programs that allow you to edit the registry while connected via activesync that don't require installing anything on the phone.

Well, it seemed like a good time to reset the phone now...
While having this problem, i upgraded the SPL with no problem.
Downloaded the newest Dutty-rom, and tried to upgrade, but because no 3'rd part progs will run, this didn't work. I had to hard reset the phone, and then upgrade the ROM and then the Radio.
Everything seems OK so far...
