When I try to flash any custom rom via twrp recovery it's showing "patching system image unconditionally...". Then my phone bricked.
I've wiped everything. And tried many times. But same problem
Please help me, how to solve this problem...
I had this issue too. You have to flash newest recovery created by kumajaya.

Regardless of what recovery I used, (yash28, regalstreak, official twrp) or even starting from the scratch( flashing stock OxygenOS using the Oneplus tools) and then reflashing the recovery, my attempts to succesfully load into a ROM has been futile. As of now, I am pretty much unable to boot into any recovery, and I am stuck using the stock OxygenOS. I tried flashing modem, first attempt was clean flashing the new modem, then flashing it dirty after flashing my rom, I tried doing the same thing with the old modem, my attempts have been futile. If anyone could shed light in this situation, it would be great, I would hate to be stuck using the stock OxygenOS rom, and not try out the great roms that are developed. I can however flash oxygenOS stock rom.
i have the same problem and i cant find any solution i thought of starting fromt he scratch as it didnt help u it might not help me too
Have the same, I've done everything. I've made the phone full stock again and still won't boot.
I have finally found the solution to my own problem
Apparently what made this impossible was due to sensors, as I noticed some of my sensors werent working. That could have made me unable to flash anything but OOS. Anyway, the solution is to fastboot erase persist, and reflash persist.img (Instructions are here: http://forum.xda-developers.com/showpost.php?p=65340562&postcount=591). I hope this fixes your guys problem.

Finally someone found the answer! Unfortunately I wiped whole internal memory and forgot to take backup of whatsapp backup so i lost that. Oh well, all works good now, thank you!

That's exactly what I did.
I think i faced the same problem, im using h2os 6.0.1 close beta, with twrp 3.0, but without rooting. I can restore my rom tru twrp without problem. But my try to flash grarak cm13 and new mokee os 6.0 ended up with bootloops, both stuck at oneplus logo

Do you have to go back to the stock rom before flashing persist and then install cm13? or can i do it while i am on the boot logo screen...i tried going into fastboot while stuck on the boot logo (which i was able to) and flashed persist and rebooted but it is still stuck on the boot logo screen

You have to downgrade fastboot/bootloader to the Lollipop one (just flash the usual revert_to_lollipop.zip found in these forums as usual).
Hi, today i just played with file systems today and now i can't install any other roms excepting supermanrom. So i think i have a problem with file systems. Which file system does s7 edge using for default? Can any master help me? After im installing any rom it's stuck at bootloop
Which one should i use for example system and data file? ext4 or f2fs or any other?
Can you give us some more information?
if your stuck in boot loop? can you not boot into twrp?
Did you turn off OEM unlocking in developer options?
If you can load twrp? lots of info on returning to stock on here
just use search for installing to stock after bad flash.
Or other option would be flash another custom Rom.

I can boot into twrp but i cant flash stock frimware with odin. It's giving FAIL error to me immidietly when im trying to do. And yes, OEM was open before
Now i flashed stock rom via odin, but when i install twrp again on my device it stucking at boot loop. Why?
Then i realized something that whatever i flash something via odin or something else, its stucking at bootloop

Format all with ext4 (in twrp). Especially the system. Because all touchwiz cusrom can only run on ext4. After all normal, you can just format to f2fs for data and cache

all started with me wanting paranoid android. flashed 9.9.25 blobs too & hads some bugs. so i wiped everything like I would rooting any other androind phone and now phone only boots to fastboot.
my ARB is 1 if that matters, how can i fix this please?

sorry, been too busy with work and missed this. after 48 of brick, I followed farhan20 guide he posted here
the miflash software actually did not work at all for me, i let that run for 30 minutes (way too long) and just unplugged eventually and went thru the guide above. after that was able to get into TWRP and then flash my fav havoc OS. life is good again.
Hopefully this can help anyone stuck in fastboot with a soft brick on their hands

The problem started after i WIPED THE SYSTEM FILES, now im stuck on the bootloader and since i'm stuck on the bootloader i can't also boot to the TWRP recovery screen, when i tried re-flashing the recovery it says its ok but didn't really do the job i need this phone ASAP. i use this for work

You can use MiFlash tool to install firmware to your device before flashing other roms.
