Hi Folks,
I'll try to be as detailed as possible..days ago, I flashed a custom ROM on my ICS 7.7 stock Tab, however it kept on a boot loop. I made a backup via CWM so I tried to restore back to ICS...can't restore it ("Error while restoring /system") OK, no worries....
I tried to flash the HC and ICS stock back using ODIN....can't do it (in both times, It keeps getting stuck at "factoryfs.img" at the beginning)...starting to get worried..
Tried to use adb shell and e2fsck -cy /dev/block/mmcblk0p9.... no luck (kept getting the "error reading block *numbers increasing* (Attempt to read block from filesystem resulted in short read) while reading inode and block bitmaps. Ignore error? yes Force rewrite? yes" and then nothing happened...tried to wipe/format data...CWM crashed
Tried to flash a P6810 PIT file to repartition the tab...no luck, ODIN still stuck at "factoryfs"
Tried to flash Android Stock Recovery...finally got wipe/format data to work...tried to flash "SkyHigh ROM" based on stock ICS (Remember that ODIN method doesn't work?) it installs, BUT when rebooting, it just stays looping at the Samsung Logo... now oficially Freaked out..!!:crying::crying:
Finally everytime I went to recovery mode, I get the following:
"E:failed to mount /preload (Invalid argument)
E:install_application_for_customer:can't mount /preload
your storage not prepared yet. please use UI menu for format and reboot options
copy aplication failed"
Like I said, I can wipe/format data and cache on stock recovery (can't do it on CWM) and also can flash ROM .zips (can't do it in ODIN)
So, to sum it up:
- Can't flash anything using ODIN (stuck at factoryfs)
- Can't flash using CWM (getting Samsung logo bootloop)
- Can't wipe/format data on CWM, it crashes (althought I can do it with stock recovery)
- VERY BASIC skills in adb shell (don't know to use it very much)
- I know (hope) it's not hard bricked because I can enter Download and Recovery modes
- Basically at this point, yeah I'm pretty sure to start banging my head on the wall
Can anyone help me out on this one..??? It's been 4 days trying everything

have you wiped using the stock recovery? this happened to me before (there's a thread here entitled stuck at bootscreen/boot loop) some let the device drain it's battery, what I did was wipe it using the factory wipe the connect via ODIN to install the stock HC ROM
I wiped using the stock recovery...however, everytime I try to flash HC or ICS stock ROM using ODIN, It gets stuck at the very beginning of the "FACTORYFS" phase....
Last night tried to adb shell but now it gives me the "/system/bin/sh failed: no such file or directory"
I've been reading every single thread about how to revive it, but somehow I can't....

i got stuck at the 'FACTORYFS' in ODIN no matter what i tried. i had flashed CWM using ODIN, but no ROM would flash.
turns out my usb cable being plugged into a USB hub was the culprit. it still flashed CWM like that, but the first time i tried flashing the ROM with the USB plugged right into my desktop, it worked.
just throwing that out there.
Yes. Seems like you have emmc bricked your tab. There is a fix for it. Search for emmc bug

to add to this. while i was experiencing this, i too could not flash any zip rom via CWM, it would bootloop also.
however i could wipe cache using CWM, which you can't.
Finally got it working... !!! However I could only installed CM9 "successfully" since it keeps freezing and crashing...Is there any stable custom ROM that I can install via CWM..?? (Odin keeps hanging at "factory's")

I have been having issues with CWM on my Fascinate for about 2 weeks now. Whenever I try to flash anything via CWM I get this message:
Samsung Recovery Utils

wipe data/factory reset

The "reboot system now" is highlighted and I cannot do anything to select apply update.zip
On the bottom part of the screen it says:

E:signature verification failed
Installation aborted
Now I have flashed back to D101 and did the ota update to DL09, and flashed the CWM via odin posted on a thread from the android central forum, it is basically a version of CWM for DL05 and DL09. (can't post outside links so I havent posted enough in this forum).
I have tried to apply the update.zip but whenever I do that, after about 25% of the way through it goes back to the error.
I have completely run out of ideas and the people at the android central forums have as well. I have tried everything that I can think of but obviously I am not doing something right.
Can anyone give me some suggestions as to what I can do to try to resolve this issue? Or if there is a way to flash a rom and kernel without CWM. I really do not want to be stuck with stock Fascinate so anything would be appreciated.

You have to go into CWM immediately after flashing in Odin, before booting up, and flash a new kernel. DL09 breaks CWM on the stock kernel

I had that same issue until a user on another forum informed me that right after flashing cwm, replace battery and go immediately into recovery mode, do not let the phone boot normally.
Now I am able to add kernels and roms to my hearts content.
I'm in a similar situation except my error comes while trying to back up my ROM.
I did root via pushing rage.bin via ADB
Then I used ODIN to put CWMrecovery.tar on the phone.
Flashed CWM via ROM Manager.
At this point I did a full backup with TiBu and did a ROM Backup via ROM Manager.
After that I froze bloatware, 'upgraded' to the Sywpe Beta and De-binged.
At about this point I realized that I had lost Haptic so I flashed the DL09 stock Kernel and that problem was solved. I thought I was good to go until I tried to do another backup via ROM Manager.
When I ran the backup I got the same error as the OP. I ended up in 2e recovery. Since I'm new to this I just chose reboot phone. I did some searching last night but all the posts I found dealt with people flashing custom ROMs and I was getting tired I called it a night with the plans to do more searching today but then I came across this post.
I reviewed my notes and saw the other thread about CWM Recover mode and I checked and I can't reboot into CWM Recovery (via ROM Man) either. I end up in Android (2e) recovery.
I saw the post above me and I need to search on these instructions so I understand this step better.

I don't seem to be able to boot (up and down key = Power buttong) into any recovery.

After de binging yesterday I went todo a back up via ROM Manager and got the same error as the OP. I ended up in 2e recovery. I didn't want to really screw up my phone guessing so I just chose reboot phone.
I was going to do more searching tonight but came across this post first.
I have done two successful back ups via ROM Manager, so after reading this post I assume it's flashing the kernel that got me to this error.
I'm new to rooting phones so some help with this error would be appreciated.

After flashing CWM you will need to put your phone into recovery right away instead of letting the normal rom load. You will then flash over another kernel using CWM and that is it.

I have never put a new kernel or rom on my phone but from what i've read this is the jist of it.
You shouldn't have to flash back to stock...
1. Download all necessary files and place on root of your SD card (nonstock kernel, update.zip)
Edit: also add in the superuser and busybox zips, and flash those after kernel (step 5) and this will give you root back if you lost it updating
2. Flash modified CWM .tar with Odin
3. Replace battery and boot directly into recovery by holding both volume buttons down while powering on
4. Once in recovery, apply update.zip to get into CWM
5. Flash new kernel
6. Reboot, enjoy

I followed the instructions to the letter over at AndroidCentral (including flashing busybox_su) and I'm back up and running with Haptic feedback and I'm watching a ROM backup via ROM Manager as I type this. I have not done a lot of testing yet but everything seems to be working fine.

Just did that and after I select Install zip from SD car, the screen goes black. When I press the home button it goes back to the green list but when I select anything from there, the screen goes black again.
I have tried flashing back to stock, updating to DL09 and flashing cwm again, but same issue.
OK I am getting really tired of dealing with this. Nothing I try works. I am just going to give up on the idea of installing roms and kernels on my fascinate. I have no idea what the problem is and nobody knows either.

Shh, I had no idea about that. I assumed since in order to get to cement recovery you had to press home key that that is what you use while in recovery. Everything works now, thank you.
ok, I'm ready to sell this damn Fascinate on Craigslist for a peanut butter sandwich. I'm miffed with getting CWM working. I have pushed the updated version onto the phone via ODIN, immediately booted into recovery (got the red recovery), and it won't mount my SD card. I had a similar problem in green recovery - it could see my SD card, but wouldn't show me all the files - including the download folder. So I couldn't flash anything. I've tried this 100 different ways. There's nothing wrong with the SD card. It reads fine in the phone, files working other than CWM. Please help. I'm stuck on stock. Have mercy.

OK, just to make sure I'm not crazy, I tried again. First I downloaded the SuperClean 2.2 update to my SD card. I re-downloaded the updated update.zip file and put it at the root of my SD card. I flashed DL09.tar.md5 via Odin to get back to stock. That completed with no problem. I flashed the EB01 radio, no problem. Flashed the updated CWM. Immediately booted into recovery (red). Attempted to Apply update.zip. Errored out. Attempted to apply update from SD card. Here's the error I'm getting:

I think I see what your issue is. The red CWM that you loaded via ODIN installs onto the phone itself. You no longer need an update.zip file containing CWM on your sdcard. As long as you have successfully booted into red CWM, your next step is to do a full wipe, and then choose the "install zip from sdcard" option. You should then be able browse your sdcard and select the zip file containing the rom.
DO NOT rename the zip file containing the rom to update.zip and then select the "apply update.zip" option, as that will definitely not work. That option expects to find the CWM recovery app in the update.zip file, and errors out if it's anything else.

Oh, and make sure you have the most recent new CWM file from jt1134. The post from him that had the first fixed CWM I think may have required an update.zip file, but the current, updated files in it do not. If you're not sure, download the latest one from that post for Odin, and flash it from Odin again. You will them no longer need update.zip for sure.

I had a 2.2.1 Froyo version of Darkky's ROM (9.1 or so) installed.
Everything worked fine until I tried a "fix" for call recording. For this I copied 3 files to the /LIB folder but it didn't work, the call recording software couldn't record but did create some broken files. After I uninstalled the software (but didn't delete the files from /LIB) I restarted the phone after few days because my media players couldn't open .flv files anymore. After restart, the phone would stay in an endless loop on the boot-up animation. Tried all options in the Recovery console (the one with red text, can't remember the name) but nothing fixed it. At this time I could enter both recovery and download mode.
Than I tried to flash a bootloader but the only SBL file I found, doesn't seem to fix it, I only see the icon with a phone and computer w/o link on my screen. I tried flashing other versions of stock ROMs but nothing changes.
I think there is something wrong with the partitions or the boot loader but I wasn't able to find other bootloaders and Heimdall (I used it for flashing as Odin can't see my phone) doesn't let me use .PIT files to re-partition.
Please help me
P.S. the phone can still enter download mode, but not recovery console
Have you tried flashing in PDA just a kernel like voodoo. No other files just kernel and no repartition. Auto reboot checked.

I have just flashed the "stock+Voodoo-latest XXJPY" from
here and it didn't work . It displays the GALAXY S GT-I9000 logo and that's all.

I think the call recorder program messed up my file system/ memory or something...I'm a noob though, so I don't really know what happened. Maybe the primary boot loader wold fix it?

So your phone can get into download but Odin does not connect? Can you add just the kernel file on your program then? If Odin does connect, just add a Froyo speedmod or voodoo kernal. No code, modem or csc.
I had a similar problem once, nothing worked until I tried just a kernal by itself and all was good.

Odin doesn't connect but I use Haimdall and it works perfectly .
So I already tried to flash new ROMs with CSC, Modem and CODE. It didn't fix...
I tried flashing a SBL, no luck.
I tried flashing just a kernel, it at least fixed it so I can go into recovery console but phone still doesn't boot.

I'm not really sure (as I started to play I9000 for some weeks and came across similar case few days ago) but it might be some problem with kernel that can't boot up the device.
The only way I can "rescue" my device from "death" is to flash stock ROM. It now works perfect.
Since you can get into recovery, you can mount on the sdcard and backup all the important stuff before you flash stock ROM.
I tried flashing stock firmwares already...a few versions actually.
As I can't use the .PIT files and re-format it, my sd-card is untouched.

I just "fixed" it . I flashed the XXJP1 stock ROM which came with all the files (Haimdall allows you to select 9 files, so for each "function").
I also figured out why Odin didn't work. Odin and Haimdall use different USB drivers, u have to install the proper driver.
So I tried Odin but didn't fix it and than I tried Haimdall and put in all 8 files from the .tar archive (the 9th would be "Recovery")

I have looked and done searches for this problem with no matches. Phone stuck in bootloop after battery died. Pulled battery and went to recovery, tried to restore after wiping showed error message for data wipe

Try installing another rom. If that doesn't work, try flashing a new clockworkmod version.
Yes, I have tried three different roms and flashing a different recovery with the same type of results. The problem seems to be with data. Everything I try to do including wiping data ends up in an error message.
Hmm. Then you might try going back to stock from hboot and rerooting

Id start with the newest version of clockworkmod and an new SD card.
Unrooted using the RUU with reverted HTC drivers. Process seemed to go fine but when it reboots it gets stuck in a bootloop with original droid bootscreen. I still have s-off and am wary of removing for fear of being stuck.
You could try flashing super vr wipe and do it that way.
sounds like the sdcard isn't mounting. i would back up the sdcard then reformat it and try again. or try another sdcard if you have one handy i usually keep a spare around just in case.

This kind of thing frightens me to death about flashing a new ROM. I'm dying to get that 2.3 ROM that just posed but I'm afraid of bootloops, whitescreens and error messages etc.
It's really not that bad, as long as you dont touch it while its doing it's thing it works great. I just installed Ovo Community Mod Test RC5 and it is SICK!
You need to update your hboot to .92 first though.

Sounds like something isn't mounting, but I'm not sure if it's the SDcard. Those directories are on eMMC, aren't they? Not sure if this would help, but I'd check to see if it's mounted as read/write. Looks like it's read only.

I followed those directions and flashed the new 2.3.4 without any issues. I was originally on 2.2 rooted with unrevoked3
Much bigger problem. Now that I have run the RUU I no longer have clockwork recovery and cannot flash anything. When I reboot I get the droid bootscreen in bootloop. Frustration!
Have you tried just booting with the sd card removed? Issues with the sd card can stop it from booting
Also if you flashed the RUU your unrooted therefore have no cm recovery. You will need to root again for that.
Yes recovery was gone, attempted to flash clockwork again. When booting into recovery strange bar appeared and screen went black. After several tries with same results flashed Amon ra style recovery and now have this recovery but am still getting bootloops when flashing and having same E: can't mount /dev/block/mmcblk0p1 (file exisits) error messages. Confident there is a solution but have no idea how to get there. Determined though! Any help is greatly appreciated.

Ok, I've said this a few times on here, but I don't see were you've mentioned that you've tried it. Remove your SD card and see if it boots!
Recently my Galaxy S4 was having boot problems. It wasn't able to get past the "Samsung: Custom" logo. I was using TWRP 2.6 and the CM 10.2 8-19 nightly. I tried to do wipe all data and cache then re-flash the rom, no difference I tried to flash CWM Touch, no difference. At this point I figured something just went wrong when I originally rooted and flashed TWRP. I decided that it would be best to start from scratch and flash to stock in Odin.
I download a file from stockroms.net (can't post the full link as I'm a new member, but its a pretty easy website to navigate)
When the file is unzipped it has a .tar md5 file in it. I factory reset, wipe cache, and go into download mode. I boot up Odin 1.85 and load the .tar file into the PDA line. I make sure that the phone is connected fine, and I hit start. Everything goes fine, it says it passes and on the phone it appears everything went fine. After it reboots though, It wont make it past the "Samsung: Custom" logo again. It wont even hang on the logo, it will just shut off after a few seconds of showing it. I'm thinking that it's trying to boot, but cant. So I boot into the stock recovery, and it gives me this text at the bottom:

-- Applying Multi-CSC...
successfully applied multi-CSC
E: failed to mount /data (invalid argument)
can't mount '/data' (invalid argument)
E: failed to mount /data (invalid argument)
So it looks to me that the phone is just unable to access the /data folder to boot into, and it gets stuck. How can I fix this?
EDIT: Also want to say that "repartition" was not checked in odin.
EDIT2: It seems that I fixed the problem, thanks for the help guys!

You'll want to flash a different recovery until you find one that can fix your /data issue.
I have great luck with TWRP 2.5.0.2, though some others have said it causes problems when formatting /data on TW ROM's.
I use AOSP and have not had a problem since I last used TWRP 2.6 and subsequently went back to 2.5.0.2.
2.6 and some other recoveries just kinda suck.
It's amazing that a phone as popular as the Galaxy S4 has gotten such slow development process from CWM and TWRP. S3 recoveries were all rock solid within a month or so of release. We're not what, FIVE+ months into release and TWRP still hasn't gotten it 100% right. And I don't remember what the deal was with stock CWM (not Phillz which I have no experience with).

I would just flash the .img recovery file straight through Odin correct?

You should not get any files from stockroms.net. I have seen many people including myself try their stuff and fail. Always get you stuff from XDA.

I'll try to remember that in the future. I actually just retried what I did with the stockroms.net file and the one that's in the sticky post here and it worked. I'm now back into TW and I'll be flashing a ROM in a bit. So thanks for the help guys, I guess I just needed to get my stuff from the right sources.

Hi,
I'm at my wits end. I had SlimRom installed for a week and all was fine, but I was curious about what new ROMs were out there so I backed up my phone using ROM Manager and did all the usual stuff to get a new ROM. I installed Gummy, but didn't like it; I installed one other ROM that I can't remember its name, but also didn't like that. I realized my comfy home of SlimRom was where I wanted to be, so I tried to flash the backup.zip that I had created but it kept saying installation aborted (Status 0).
After hours of countless attempts and just weird behaviour (sometimes boot loops, sometimes not booting into recovery, etc.) I finally got my phone back to stock.
I downloaded the latest SlimRom file 3 times to make sure it wasn't a corrupt zip (twice from my PC and once from my phone directly). Both the zip directly from Slim as well as the backup.zip that I made with ROM Manager give me the same installation aborted. The other error it gives is "E: Error in /data/media/0/slim....zip". I'm wondering if somehow my partition is screwed up. I tried using Odin's repartition, which just erased my recovery partition, but I got that back thankfully! When I wipe data/factory reset it says wipe complete (and the ROM does reset to factory) but it also says "E:unknown volume '/sdcard/.android_secure' "...I'm wondering if that relates to why the backup.zip file won't read properly. Maybe the partition got screwed up somehow?
I can flash a SU zip so it's not a flashing problem, per se. And thankfully Odin always works!
I think I may need the stock PIT file for the Telus S4 l337m, which I found but my phone says "secure check fail: pit"

Rom manager is your problem. That software I'd waaaaay out of date and rarely ever works on newer phones.
Get you goo manager. Allow SU. Then hit the menu button and then hit install openrecoveryscript. You will get TWRP recovery. Then you follow the OP of your ROM of choice to install.

Thanks for the suggestion. I used Goo but it was no help. I can flash other ROMs fine. It seems to be just SlimBean that I can't flash :s. I've used TWRP recovery as well as CW and neither work.
Any other ideas?
This is a pickle I am completely unsure about, then... I am sorry I can't be of more assistance. It does seem like the partition is borked.
